1.Analysis of the Transmission Route of Human Papillomavirus in Heterosexual Couples.

Carcinoma of the uterine cervix has been considered to be a sexually transmitted disease(STD) and at present time, particullary human papillomavirus (HPV) is considered as the most likely infectious causative agents of uterine cervical cancer. But less is known about the sexual transmission of HPV and the status of HPV infection of male partner. Therefore, screenng of couples for HPV is very important for understanding HPV infection as a sexually transmitted disease and prevention of cervical carcinoma. The polymerase chain reaction(PCR) was employed to detect HPV 16 and 18 in cytological samples from the uterine cervix of the patients with cervical carcinoma(4 CIS and 34 invasive cervical carcinoma) and from urethral metatus and glans sulcus of their male consorts. The results are as follows; 1. HPV 16 or 18 were detected in 31(81.6%) of 38 patients with cervical cancer(HPV 16; 78.9%(30/38), HPV 1S; 28.9%(11/38), HPV 16 and 18; 26.3%(10/38)), 2. HPV 16 was detected in 27(90,0%) of 30 males whose wives were positive for HPV 16. But HPV 18 was detected in only 3(27.3%) of 11 male consorts whose wives were positive for HPV 18. And HPV 1S was detected in all male consorts whose wives were positive for HPV 16. In addition, HPV 16 or 18 were positive in 3 of 7(42.9%) male consorts whose wives were negative for HPV 16 and 18. Conclusively, these results suggest that HPV might be transmitted by sexual contacts in heterosexual couples.

2.Clinical Significance of a Cervical Cytologic Diagnosis of Atypical Squamous Cells of Undetermined Significance: Qualitification and HPV DNA Detection.

OBJECTIVE: To evaluate qualification and determine the usefulness of human papillomavirus(HPV) DNA testing in women with cytologic smears read as atypical squamous cell of undetermined significance (ASCUS). METHOD: The study was based on 88 women with cytologic smears reported of ASCUS favoring either a reactive process or squamous intraepithelial lesion (SIL) from July 1999 to June 2000. Cytologic examination by Papanicolaou smear was based on the Bethesda System and cervical biopsy was done via coloscopy. the presence of high risk HPV DNA was determined with Hybrid capture assay. RESULTS: The overall incidence of cervical intraepithelial lesion (CIN) was higher in the group with ASCUS favoring SIL than in the group with ASCUS favoring reactive process(42.2 % vs 18.6 %; p<0.05). Women with ASCUS favoring SIL were 9.1 times more likely to have high grade SIL(CIN II/III) develop than were women group with ASCUS favoring reactive process(95% confidence interval, 1.08-76.0). High-risk HPV DNA was demonstrated in 39.8%(35 cases) and was associated a 8.8 times increased likelihood of histologic conformation of CIN(95% confidence interval, 3.09-24.7). The sensitivity of HPV assay for any CIN was 74% and specificity was 75%, whereas the corresponding values for the repeat Pap smear was 67% and 33%, respectively. CONCLUSION: Women with ASCUS favoring SIL was associated with a relatively high risk that the women was harboring SIL. The addiction of a high-risk HPV DNA assay to cytologic examination appears to provide for early detection of high grade CIN in women with ASCUS.

3.Clinical Effect of Nicardipine(Perdipine(R)) on Cerebrovascular Diseases.

Twenty-five patients with cerebrovascular diseases were administered Nicardipine, a calcium antagonist, in daily dose of 60 mg per orally for 8-12 weeks during the year of 1983, to evaluate the efficacy of the drugs clinically. The following results were obtained. 1) General improvement rating was 80%. 2) Marked improvement(88%) were noted in subjective symptoms, especially of tinnitus, paresthesia and headache. significant improvement were noted in psychiatric(74%) and neurologic(71%) symptoms e.g disturbance of sleep, emotion, memory, speech, swallowing and muscle power. 3) In 13 hypertensive patients, average blood pressure lowered from 163/96 to 138/83 mmHg in 4 weeks and maintained thereafter. 4) major adverse effect was facial flushing, noted in 4 patients(16%), which were transient and mild and allowed to contiume medication.

4.A MEASUREMENT OF DISPLACEMENTS OF CAST FRAMEWORK BY TORCH SOLDERING AND ELECTRIC SOLDERING TECHNIQUES.

The purpose of this study was to investigate the displacements of cast framework by torch soldering and electric soldering techniques. Specimen had two cylinders and connecting bar that had sectioned with 0.3mm gap at mid point. 10 of total specimens were divided into two groups. In torch soldered group, soldering investment block was made and conventional torch soldering procedure was carried out. In electric soldered group, electric soldering was carried out on the master cast without soldering investment block by using electric soldering machine(Dentapunkt DP 7, Kulzer, Germany). After soldering procedure, three dimensional coordinates of two centroids of each cylinder were measured by three dimensional coordinate measuring machine. The intercentroidal displacement and global displacement were calculated and then, these values were compared and evaluated. The results were obtained as follows: 1. Intercentroidal distances of specimens decreased after both soldering procedures, and the decrease in intercentroidal distance was greater for torch soldered group than for electric soldered group 2. 5.The Classification of Mental Disorders in North Korean Psychiatry

Objectives:
Psychiatry in North Korea is still poorly understood. Therefore, it is necessary to study the psychopathology and diagnostic classification of North Korean psychiatry. This study examined North Korea’s psychopathology and diagnostic classification system and how it differs from international standards.
Methods:
This study reviewed North Korean medical books and medical journals available at the Information Center on North Korea. This literature review and qualitative content analysis examined 15 medical books and 227 medical articles.
Results:
North Korea’s psychiatric diagnostic classification is based on the International Classification of Diseases, 10th edition (ICD-10), but with some modifications to reflect the circumstances of North Korea. While the research journals mainly used the ICD-10 diagnostic criteria, North Korea has its own diagnostic criteria, which it refers to as the “existing diagnostic criteria.” ‘Neurotic disorders’ were the most common, both in the classification in the medical books and the list of disorders covered in the medical journals. There was an interest in ‘organic mental disorders,’ such as postconccusional syndrome and ‘mental disorders due to encephalic parasitism,’ but relatively little research on mood disorders. There were no studies on suicide and a view that alcohol is abused in capitalist countries.
Conclusion
This study examined the diagnostic classification system of North Korean psychiatry.Further analyses of the characteristics of the psychopathology used in North Korea can be used to understand North Korean society.

7.An Evaluation of the Accuracy of Mini-Wright Peak Flowmeters in Patients with Asthma and Chronic Obstructive Pulmonary Disease.

BACKGROUND: The peak flowmeter is very useful in monitoring of out-patients as well as those in emergency departments because of its convenience and simplicity with low cost. There have been many studies aimed at determining the accuracy and reproducibility of the peak flow meter in normal population. However, there is a paucity of reports regarding its accuracy in patients with chronic obstructive pulmonary disease(COPD) or asthma. The accuracy of the peak expiratory flow(PEF) measured with a mini-Wright peak flowmeter was assessed by a comparison with the results of a mass flow sensor. METHODS: The PEF measurements were performed in 108 patients aged 19-82 years presenting with either a chronic obstructive lung disease or asthma before and after inhaling salbutamol. The PEF measurements from the mini-Wright flowmeter were compared with those obtained by the calibrated mass flow sensor. RESULTS: The average of the readings taken by the mini-Wright meter were 37-39 l/min higher than those taken by the mass flow sensor. The average percentage error of the mini-Wright meter were higher, ranging less than 300 l/min. The mean of the differences between the values obtained using both instruments (the bias)±limits of agreement(±2 SD) were 37.1±90 l/min for the PEF(p<0.001). CONCLUSIONS: The mini-Wright peak flowmeter overestimated the flows in patients with COPD or asthma. It was also found that the accuracy of the mini-Wright peak flowmeter decreased in its mid to low range. The limits of agreement are wide and the difference between the two instruments is signigicant. Therefore, the measurements made between the two types of machines in patients with asthma or COPD cannot be used in terchangeably.

9.Computed tomography attenuation values of normal upper abdominal organs in Korean before and after contrast enhancement

CT is a highly accurate method of detecting and clarifying the narure of space-occupying lesions within theliver nd pancreatic disease. It was found to be a reliable, non invasive method for detecting the lesion. A normalrange of attenuation values were obtained from 71 CT examinations of the upper abdomen, that is liver, spleen,pancreas, aorta, before and after contrast enhancement in whom no radiologic or laboratory abnormality of theseorgans were detected from March to Sept. 1983. The results were as follows; 1. The age distribution was from 28years to 71 years. The sex ratio was 45 male to 26 female. 2. Mean Hounsfield Units (HU) in liver before contrastenhancement and after contrast enhancement liminute, 3 minutes, 5 minutes, 10 minutes were 56.1±3.85, 75.0±4.77,82.0±3.71, 84.3±2.76 and 88.2±3.95 HU respectively. 3. Mean Hounsfield Units in pancreas before contrastenhancement and after contrast enhancement 1 minute, 3 minutes, 5 minutes, 10 minutes were 43.0±4.30, 66.5±4.27,71.2±2.39, 74.4±3.00 and 79.2±2.(0 HU respectively. 4. Mean Hounsfield Units in spleen before contrastenhancement and after contrast enhancement 1 minute, 3 minutes, 5 minutes 10 minutes were 53.0±4.31, 71.4±2.84,75.3±2.70, 80.5±3.04 and 83.2±2.14 HU respectively. 5. Mean Housfield Units in aorta before contrastenhancement and after contrast enhancement 1 minute, 3 minutes, 5 minutes 10 minutes were 45.1±5.39, 86.7±6.86,92.5±5.33, 106.8±4.12 and 114.1±5.02 HU respectively. 6. The most significant finding was that the livernormaly had the highest attenuation values of any of the organs measured. When another viscera in upper abdomenhad a attenuation value greater than that of the liver, this reflected abnormal lesions.

10.Distal Radius Osteosarcoma.

PURPOSE: We analyzed the clinical outcome of osteosarcoma developed in distal radius and the effect of delayed treatment on prognosis. MATERIALS AND METHODS: Twelve patients with distal radius osteosarcoma were analysed. We categorized patients into two groups of standard treatment or non-standard treatment. The patients of standard treatment group are all stage IIB and non-standard treatment group includes five stage IIB and one stage III. RESULTS: Five-year overall survival and disease-free survival rates of standard treatment group were 100% and 83%. Five-year overall survival rate of non-standard treatment group was 44%. Between two group, there are differences in age, tumor size, surgery type, symptom duration. CONCLUSION: Distal radius osteosarcoma have good prognosis than other extremity osteosarcoma. Survival rate of non-standard treatment group were lower than standard treatment group. Although the prognosis of non standard treatment group is poorer, the duration till death was longer than that of other sites with similar condition. Further multi-institutional study should be needed.
