1.Hairy Blue Nevus.

A 2-month-old female infant presented with a bluish-black, flat, hairy patch, 1.2cm in diameter, on the posterior aspect of her neck since birth. Histologic sections from the lesion showed numerous diffusely scattered dermal melanocytes throughout the dermis. The histopathologic and clinical findings were not fully oonsistent with any other known dermal melanocytosis. The diagnosis of “hairy blue nevus” was proposed for this unique lesion.

We report three cases of atypical skin eruptions in leukemias presenting as scrotal ulcers, herpes simplex and urticaria pigmentosa that are unique in their morphology and site of presentation. Therefore any skin lesion that develops in patients with leukemia should be examined by biopsy.

6.A Case of Sebaceous Hyperplasia with Linear Arrangement.

We report a case of sebaceous hyperplasia showing linear arrangement in a 13-year-old girl. The lesion was found at birth and became prominent gradually. Multiple small discrete yellowish papules were arranged in a linear pattern on the temporal srea of the face. Unusual, incidental histopathologic finding was a sebaceous lobule lying in a dilated infundibular portion, which appeared to represent transfollicular extrusion of sebaceous lobule as a natural phenomenon.

7.A Case of Arteriovenous Fistula Associated with Macrotia.

We report a case of arteriovenous fistula involving the external ear in a 27- year-old male patient, who presented with pruritic erythematous scaly patch with multiple pulsating nodules on the thickened and hypertrophied left auricle.The blood flow, measured by laser Doppler velocimetry, was increased markedly on the affected site comparing with the other (710.33mV vs 8.4mV, 663.33mV vs 107. 33mV). We suggest that macrotia must be considered as the one of the clinical manifestations of cutaneous arteriovenous fistula.

8.A Case of Bullous Pemphigoid Treated with Plasmapheresis and Pulse Cyclophosphamide.

A 54-year-old woman with severe bullous pemphigoid (BP) associated with insulin dependent diabetes mellitus (DM), who showed unresponsiveness to conventional therapy with corticosteroids in combination of either cyclosporme or dapsone, was successfully treated with plasmapheresis followed each time by 3 successive days of pulse therapy of cyclophosphamide (500mg, intravenously). After six times of plasmapheresis, anti-basement membrane zone (BMZ) antibody titer decreased from 1:1280 to 1:40 and no new lesions developed at all. In severe refractory BP patients with uncontrolled DM, plasmapheresis is one of the valuable treatment modalities for a short period and the need for corticosteroids thus avoiding corticosteroid induced side effects. Herein we report a case of BP with uncontrolled DM who showed an excellent response to a low dose of corticosteroid and 150 mg oral azathioprine following plasmapheresis and cyclophosphamide pulse therapy.

10.A case of onychogryphosis associated with onychomycosis.

We present a case of onychogryphosis of both great toe nails associated with onychomycosis of all toe nails occurring in a 90-year-old female patient. She also had senile dementia. Affected nail plates were thickened, increased in length, and curved like the horn of a ram. The nail plates were removed surgically form the nail bed. Direct microscopic examination revealed many hyphae with potassium hydroxide. Trichophyton rnbrum was isolated from the culture in Sabouraud glucose agar media. On histopathologic examination, we found many hyphae in the nail plate. We suggest an onychomycosis as one of the causes of onychogryphosis.
