1.Chancroid: Report of Three Case.

Chancroid is an acute localized, autoinoculable venereaI disease caused by Haemophilus ducreyi, characterized clinicaIly by painful ulceration at the site of incubation, and frequently accompanied by regional lymphadenopathy, and short incubation period. Three cases of chancroid were seen in Won Ju city, Kangwondo, Korea. All of the patients were male, and painful ulcers developed on genital region in 2 to 3 days after sexual contacts. Direct smear showed the characteristic "school-of-fish" arrangement of Haemophilus ducreyi by Gram stain. There were no regional lymphadenopathies and VDRL test were negative. Treatments were done with sulfisoxazole (Gantrisin) in case I, with sulfamethoxydiazine(Bayrena) and streptomycin in case 2, and with sulfamethoxydiazine and tetracycline in case 3 with exccllent results.

2.Effect of External Factors on Diastase Activity in Water.

Many factors exert an influence on enzyme activity and thus on the rate of reactions that they catalyse. The most important of these factors are pH, temperature, substrate concentration, and the concentration of some inhibitors present. A solution of the enzyme diastase, which breaks down molecules of the polysaccharide starch to the disaccharide maltose by hydrolysis, was provided. Activity of these enzyme was measured by the rate at which starch was removed from the reaction mixture. These experiments were designed to study this reaction rate under varying conditions and the following results were obtained. 1. The range of optimum pH for this enzyme at room temperature was 4.0-7.0 and the optimum pH was 5.0. 2. The range of optimum temperatures for this enzyme at pH 7.0 was 30-50 degrees C and the optimum temperature was 40 degrees C. 3. The relationship between the enzyme activity and substrate concentration could be expressed by the Michaelis-Menten equation. The limiting velocity of these enzyme at room temperature and pH 7.0 was 415 microgram starch removed/ml of reaction mixture/min and Km, Michaelis constant, was 343 microgram/ml. 4. Inhibitors NaCl and HgCl2 blocked this enzyme activity completely at 1% and 0.01% respectively.

3.Drug Eruption & Liver Damage due to Diaminodiphenyl - Sulfone (DDS): Report of a case.

We report a case of drug eruption and liver damage due to diaminodiphenyl suIfone(DDS) ingestion in a 17-year-old female. This patient had taken DDS for 20 days, 100mg-200mg daily to treat an unknown skin disease. Thereafter, she had generalized erythematous eruption on whole body, icteric sclera and fever. Liver function test showed abnormality (SGOT 514 unit, SGPT 710 unit, alkarine phosphotase 4. 3 B.U., total biIirubin 7. Oml/dl, direct bilirubin 4. 8mg/dl). The adverse reactiions to DDS are gastrointestinal intolerance, hemolytic, anemia, methemoglobinemia, agranulocytoais, hepatitis, neuritis, psychosis and a skin rash described as a fixed drug eruption, erythema multiforme, exfoliative dermatitis and toxic epidermal necrolysis. In Korea, DDS has been used for the treatment of skin diseases of various types for a long time without prescription, especially in rural areas. This trend is a significant sociomediical problem in Korea.

6.Deep Vein Thrombosis after Cementless Total Hip Replacement Arthroplasty Using Doppler Ultrasound.

Venous thromboembolic disease is a frequent complication after total hip arthroplasty. However, in Korea, the low incidence of deep vein thrombosis after total hip arthroplasty was reported. In this study, we present the results of 82 consecutive patients(90 hips) who had a cementless total hip replacement with a Anatomic Medullary Locking Component(AML: Depuy, Warsaw, Indiana, USA) between August 1995 and September 1996 at Inha University Hospital, Korea. Postoperatively, none of the patients were treated with any known prophylatic method for deep vein thrombosis except elastic stockings. Both preoperatively and six weeks after surgery, all patients were studied by an radiologist using a portable Doppler ultrasonic velocity detector with a transmission frequency of 5 megahertz. Contrast phlebography was also done in all patients at the same time as the Doppler ultrasound. In addition, coagulation assays, a complete blood count, blood typing, and serum chemical-profile tests were done for all patients. In comparing the results of these laboratory tests from the DVT group and from the non-DVT group, we found that only eight patients(9.8%) out of eighty-two patients had DVT after total hip arthroplasty. Thrombi were found in the superficial femoral vein in five cases and in the common femoral vein in three cases. Though two(2.4%) patients showed suspicious symptoms of pulmonary embolim, their perfusion lung scans were negative. The DVT group show a significantly shorter activated partial thromboplastin time than did the non-DVT group(P<0.05). In addition, the DVT group showed significantly higher serum total protein than the total protein level of the non-DVT group(P<0.01). In this two groups, there was no difference between the results of Doppler ultrasound and the results of contrast phlebography. Consequently, the incidence of deep vein thrombosis after total hip replacement arthroplasty in Korean patients is significantly lower than incidence in patients from countries other than Korea. Doppler ultrasound was a non-invasive and the most reliable diagnostic technique for deep vein thrombosis.

7.A study of the characteristics of craniofacial skeleton on orthognathic surgical cases with skeletal Class III malocclusion.

The purpose of this study was to evaluate the characteristics of craniofacial skeleton on orthognathic surgical cases with skeletal Class III malocclusion. For this study, 74 students at the dental college of Chosun University volunteered as a normal occlusion group. They had well-balanced faces and good occlusions with acceptable Class I molar relationship. They had not received orthodontic treatment and had no signs or symptoms of temporomandibular joint dysfunction. 45 malocclusion patients enrolled for orthognathic surgical treatment with skeletal Class III malocclusion at the Department of Orthodontics, College of Dentistry, Chosun University. On the basis of this study, the results of this study were as follows: 1. Skeletal Class III malocclusion was largely due to the overgrowth of mandible in man and the undergrowth of maxilla in woman. 2. The mandible was antero-inferiorly overgrown by large MP-HP angle and large gonial angle in orthognathic surgical cases with skeletal Class Iii malocclusion. And also, upper incisors were severely labioversioned, but on the other hand lower incisors were linguoversioned. 3. In female, lower-third facial height was characteristically shortened in comparison with middle-third facial height and also, lower facial throat angle was sgrall in male.

8.A comparison study of Pharmacia CAP system, fluoroallergosorbent test and radioimmunoassay for serum lgE and eosinophil cationic protein measurement.

Results from two in vitro assays, Pharmacia CAP system(CAP) and fluoroallergosorbent test(FAST), for detecting serum specific IgE to three allergens, mugwort, Derraatophagoides pteronyssinus, and eat epithelium, were compared with skin prick test(SPT) results in 53 atopie dermatitis patients with positive SPT and 25 non-atopic controls. Using SPT results as the reference standards, CAP achieved higher sensitivity than FAST in determination of three allergens. CAP for measuring total IgE and eosinophil cationic protein(ECP) was also compared with FAST and ECP radioimmunoassay(RIA), respectively. In comparison of two assays for total serum IgE, the values obtained with CAP were significantly higher than those with FAST. In comparison of CAP and RIA for measuring ECP, the values obrained with CAP were significantly higher than those with RIA and the differences between ECP values of two assays were significant in high ECP group(>8.3 micro gram/1). These results suggest that CAP is the preferred test and seems to be more appropriate method for detection of specific IgE and ECP in patients with allergic digeases.

9.Histologic study of chemical peel in guinea pig after pretreatment with tretinoin and tretinoin-combined pretreatment kit.

The purpose of this study was to evaluate the histologic difference that occured after trichloroacetic acid(TCA) chemical peel in an animal model that was pretreated with Tretinoin alone or Tretinoin-based combined pretreatment kit. Eight Hartley white guinea pigs were used in our study. The dorsal skin of the guinea pigs was divided into six equal squares(2x2 cm). Upper two areas of these six were not pretreated, middle two areas were pretreated for 4 weeks with Tretinoin alone and lower two areas were pretreated for 4 weeks with Tretinoin-based combined kit. Each guinea pig underwent chemical peel with 50% TCA. The wounded areas were biopsied at post-peeling 3 weeks and 6 weeks. The histology revealed that those animals pretreated with combined kit healed quicker than the animals pretreated with Tretinoin alone. This study implies that if patients are treated with Tretinoin-based-combined pretreatment kit before undergoing chemical peel, the pretreatment time will be shortened.
