2.Multiple Chorioangiomas of Placenta: A case report associated with repeated abortions and intrauterine fetal death.

Chorioangioma is the most common primary tumor of the placenta. It is widely accepted to be formed from hamartomatous proliferation of angioblastic cells in early placenta. According to groups who systematically examined the placentas with the greatest scrutiny, the incidence of chorioangioma is 1~1.2% of all placentas examined. The clinical implication of chorioangioma has been emphasized due to its high association with hydramnios, antepartum and postpartum hemorrhage, premature onset of labor, or intrauterine fetal distress or death. The authors experienced a case of a 38-year old multigravida who had undergone 4 consecutive spontaneous abortions. The fifth pregnancy was carried to 35 weeks whereupon a dead fetus was delivered. The placenta was enlarged(950 gm) with multiple protruding nodules into the fetal surface. Microscopically, the nodule was composed of many vilous structures lined by chorionic epithelium. Within these structures was proliferation of endothelial lined capillaries some of which demonstrated ectatic change.

3.Unusual Cutaneous Manifestions of Connective Tissue Disease: II. Multiple Cutaneous Ulcerations in Dermatomyositis.

The second case of this serial reports under the title of unusual cutaneous manifestations of connective tissue diseases is a woman with primary idiopathic dermatomyositis who developed cutaneous ulcerations on her arms and legs, which is thought to b a rare manifestation of adult dermatomyositis. These multiple ulcerations are about bean sized, polymorphous, and grouped on the poikilodermatous lesional skin. In this patient these ulcerative lesions became regressive and healed with disfiguring scars after the activity of the disease has controlled by prednisolone and methotrexate.

6.Foreign Body Induced Bladder Stone After Bladder Neck Suspension: A Case Report.

Bladder stone formation may be associated with the intravesical foreign body. Frequency of bladder foreign body in female is less than male. We present a case of bladder stone formation for nylon suture materials in a 47-year-old female patient who has been treated by bladder neck suspension for stress incontinence. Patient was successfully treated by surgical removal of suture materials without recurrence of stress incontinence.

7.A Change of Serum Prostate Specific Antigen Correlated with Clinical Status of Prostate Cancer Patient.

PURPOSE: It has been reported that serum prostate specific antigen(PSA) generally increase in direct proportion to clinical status of prostate cancer, but some reports suggest that PSA level is not correlated with clinical stage in 10% of the patients of prostate cancer. We evaluate the value of PSA about clinical status of prostate cancer through comparing disease status and PSA of patient at initial diagnosis of prostate cancer. MATERIALS AND METHODS: A clinical study was retrospectively made on sixty patients with prostate adenocarcinoma. The patients were admitted to the Department of Urology between January of 1992 and December of 1996. Results were obtained through statistical analysis of the correlation between PSA level and clinical status by performing serum PSA, chest X-ray, bone scan, pelvic CT or pelvic MRI. At the time of the analysis patients were diagnosed with prostate adenocarcinoma pathologically. RESULTS: Mean PSA level of patients with bone metastasis and no metastasis was 125.4 and 97.89ng/m1, respectively and correlation between the two groups was not observed, so bone scan could not be replaced by measuring serum PSA. The number of hot uptake In bone scan wasn't correlated with serum PSA level. Gleason score wasn't correlated with serum PSA level. Multiple comparisons in groups which were classified by bone metastasis and lymphatic metastasis had no statistical significance. CONCLUSIONS: As the result of the above observations, we predict that there is a problem in evaluating clinical status with PSA as compared with the value at initial diagnosis or as a predictor of recurrence in patients with radical prostatectomy.

8.Correlation of Voiding Dysfunction with the Lesion Site of Cerebrovascular Accident.

PURPOSE: Cerebrovascular accident is a serious neurological event, it can have temporary or permanent effects on survivors, including memory, cognition and volitional control of voiding. However, the correlation between the brain lesion site and type of voiding dysfunction is less well understood. This study was performed to evaluate the type of voiding dysfunction on the anatomic lesion of brain in patients with urinary symptoms after a cerebrovascular accident. MATERIALS AND METHODS: Urodynamic evaluation was undertaken in 74 patients who had suffered from voiding dysfunction. Computerized tomography(CT) and magnetic resonance imaging(MRI) were performed to localize the lesion in central nervous system. RESULTS: Damage to basal ganglia and/or internal capsule(6 of the 11 patients) and cerebral cortex(12 of the 21 patients) showed frequent detrusor hyperreflexia, while multiple lesion(21 of the 39) exhibited frequent detrusor areflexia. Uninhibited relaxation of sphincter in detrusor hyperreflexia was only observed in cerebral cortex lesion. CONCLUSIONS: Correlation of bladder dysfunction with the anatomical lesion of cerebrovascular accident was not conclusive, but was suggestive of directions to be taken in the future studies for the improvement of the life quality and preservation of renal function

9.PAX2 Expression in Renal Dysplasia.

BACKGROUND: Renal dysplasia is the abnormal development of the kidney. The condition is usually detected in childhood along with other urinary tract anomalies, but can remain unnoticed until adulthood. It was recently reported that a PAX2 gene mutation plays a major role in the development of renal dysplasia. The aim of this study was to examine the expression of PAX2 in dysplastic kidneys of children and adults. METHODS: A total of 30 cases diagnosed with renal dysplasia after a nephrectomy were examined. PAX2 expression was evaluated using immunohistochemistry. Apoptosis was detected using an Apop Tag detection kit. RESULTS: In the dysplastic kidneys, there was strong PAX2 expression in the epithelia of the primitive ducts in both children and adults, but the degree was significantly lower in adults (p=0.007). However, the mesenchyme surrounding the primitive ducts of children showed stronger staining for the smooth muscle actin antibody and trichrome than the adults. The apoptosis index was significantly higher in the primitive duct epithelia than in the surrounding normal collecting duct epithelia (p=0.000). CONCLUSIONS: PAX2 is overexpressed in the primitive ducts of renal dysplasia, which is sustained until adulthood and is associated with increased apoptosis. However, a decrease in PAX2 expression in the dysplastic epithelia and mesenchymal cuff of adults suggest a gradual regression of the dysplastic elements with time.

10.A Case of Congenital Dislocation of the Knee.

Congenital dislocation of the knee is very rare skeletal deformity and was firstly described by Chatelaine in 1822. Since then there have been a few number of case reports and some series discussing the etiology and treatment. The tibia is displaced anteriorly in relation to the femur. It is generally subdassified as simple hyperextention, subluxation, and dislocation depending on the degree of the joint displacement and the severity of disease. Early gentle manipulation and serial splintage or plaster cast are recommended for treatment and were successful in the majority except the case of quadriceps contrarture or late correction. The authors experienced one case of congenital dislocation of the right knee and brief review of the related literatures was made.
