3.Uretero-Ileo-Cutaneous Anastomosis for Urinary Diversion -6Year Experience in 52 Patients-.

The records of fifty two cases of the ileal conduit at the Seoul National University Hospital during a 6-year period from January 1, 1975 to December 31, 1980 were reviewed. 1. There were 42 males and 10 females (ratio of 4.2:1) ranged in age from 5 to 74 years with peak incidence in the 6th decade. 2. The most common indication of this procedure was bladder tumor and other indications were neurogenic bladder, bilateral ureteral stricture, genitourinary tuberculosis, actinomycosis, urethral stricture. urethral cancer and colon cancer. 3. Seventy seven percent of patients with abnormal renal function preoperatively improved postoperatively. 4. Operative mortality was 3.8%. Early complication included wound infection and disruption, sepsis, obstruction and urine leakage in ileoureteral junction, pneumonia, fecal leakage from ileoileal junction, postoperative acute renal failure, bleeding from ileal loop, and urethral bleeding. Late complication included intestinal obstruction, parastromal dermatitis, acute pyelonephritis, ureteral obstruction and incisional hernia. 5. Thirty three of 43 patients developed bacteriuria, 30.3% of the patients showed mixed infection and major causative organisms were Pseudomonas, Enterobacter, E. coli and Serratia.

4.Difference of Wear Damage of the Miller-Galante 1 and 2 Polyethylene Tibial Components.

PURPOSE: The purpose of this study was to examine the effects of change of polyethylene resin type and manufacturing method on the wear of polyethylene tibial component. MATERIALS AND METHODS: Thirteen MG I and 10 MG II components were retrieved at revision surgery. Each polyethylene tibial component was graded for surface wear damage. Density profiles of the polyethylene were measured to examine the extent of oxidative degradation of the polyethylene. RESULTS: The primary damage mode of the MG I retrievals was scratching and metallic debris, but the primary damage mode of the MG II was delamination (P<0.05). For the implants with implantation time of 5 years or more, the MG II polyethylene had a higher density value (0.959+/-0.002 g/cc, n=7) compared with the MG I (0.948+/-0.004 g/cc, n=11). Both delamination and polyethylene degradation increased with length of implantation time for the MG II components (P<0.01). Even after gamma sterilization, the directly molded 1900 MG I components experienced significantly less delamination and degradation than the machined GUR 415 MG II tibial components. CONCLUSIONS: This study suggests that delamination of polyethylene tibial components is influenced by resin type and/or manufacturing method

5.Drug Eruption & Liver Damage due to Diaminodiphenyl - Sulfone (DDS): Report of a case.

We report a case of drug eruption and liver damage due to diaminodiphenyl suIfone(DDS) ingestion in a 17-year-old female. This patient had taken DDS for 20 days, 100mg-200mg daily to treat an unknown skin disease. Thereafter, she had generalized erythematous eruption on whole body, icteric sclera and fever. Liver function test showed abnormality (SGOT 514 unit, SGPT 710 unit, alkarine phosphotase 4. 3 B.U., total biIirubin 7. Oml/dl, direct bilirubin 4. 8mg/dl). The adverse reactiions to DDS are gastrointestinal intolerance, hemolytic, anemia, methemoglobinemia, agranulocytoais, hepatitis, neuritis, psychosis and a skin rash described as a fixed drug eruption, erythema multiforme, exfoliative dermatitis and toxic epidermal necrolysis. In Korea, DDS has been used for the treatment of skin diseases of various types for a long time without prescription, especially in rural areas. This trend is a significant sociomediical problem in Korea.

6.Invasive Cervical Cancer in Women Aged 35 or Less.

The incidence in women less than 35 years of age is increasing despite an overall decrease in newly diagnosed invasive cervieal cancer among all age groups. However the influence of young age on prognosis remains controversial. So 163 patients of invasive cervical cancer in women aged 35 or less treated from Jan. 1, 1984 to Dec. 31. 1993 at the Department of Obstetrics and Gynecology, Kosin Medical College, Pusan, Korea, were studied retrospectively. We obtained the following results; The 5-year survival rates by clinical stage were 88.7Y% in stage I b, 71.5% in stage IIa, 31.2% in stage IIb and O% in stage III or above. The lymph node metastasis rates by stage in RAH group, were 21.3% in stage I b and 31.8% in stage IIa. The lymph nodc metastasis rates by lesion size in RAH group, were 19.2% in 2cm below, 20.7% in 2-4cm and 40.0% in 4cm above. The 5-year survival rates in RAH group were 74,9% in lymph node positive group and 89.0% in lymph node negative group. The recurrenee rates by clinieal stage were 0% in stage Ia, 20.6% in stage I b, 22.2% in stage IIa, 50.0% in stage Ilb and 90.9% in stage III or above.

8.Evaluation of Enzyme Immunoassay for the Diagnosis of pulmonary Tuberculosis.

BACKGROUND: The diagnosis of tuberculosis has been based on the detection of tubercle bacilli by acid-fast stain of smear or cultures, and recently the serologic diagnosis of tuberculosis has been provided a means of sensitive and specific detection of Mycobacterium tuberculosis. We evaluated the utility of enzyme immunoassay using determiner Tuberculosis Glicolipids(TBGL) antibody kit(Kyowa Medex Co. Ltd, Japan) to detect anti-TBGL antibody for diagnosis of pulmonary tuberculosis. METHODS: Anti-TBGL antibody assay was performed to the form 44 patients with active pulmonary tuberculosis(17 patients with smear positive, 7 patients with only culture positive, 20 patients with clinically active tuberculosis) and 80 controls (30 healthy controls, 24 patients with non-tuberculous respiratory diseases, 26 patients with inactive tuberculosis). We compared the sensitivity and specificity of anti-TBGL antibody with culture and AFB stain. RESULTS: Anti-TBGL antibodies were detected in 16 of 17(94%) smear positive patients, 4 of 7 patients with only culture positive and 16 of 20(80%) smear negative patients who had been clinically diagnosed as active pulmonary tuberculosis. Nine(35%) out of 26 patients with inactive tuberculosis, one(4%) out of 24 patients with non-tuberculous respiratory diseases and no one of healthy control had a positive antibody response. Overall sensitivity, specificity of the anti-TBGL antibody assay were 82%, 88%, respectively and sensitivities and specificities of culture and AFB smear 64%, 97%, and 49%, 100%, respectively. Anti-TBGL antibody titers in patients with active tuberculosis were significantly higher than control grup(P<0.05). Conclusions : The anti-TBGL antibody assay was sensitive, rapid and convenient. This assay will be useful as a tool for the diagnosis of tuberculosis in combination with other conventional methods.

9.Management of 24 Lower Ureteral Obstruction -Especially Internal Stent Indwelling-.

The lower ureter is not so frequent site of the urinary tract obstruction. In lower ureteral obstruction, the obstruction should be relieved by any method for maintaining renal parenchyme. A clinical studies were made on the 24 ureters of the lower ureteral obstruction. This studies were mainly focused on the method of management by internal stent indwelling. The following results were obtained. 1. In etiology, acquired lower ureteral strictures were most common ; tuberculosis in 10 ureters, radiation in 6 ureters, pelvic lymphadenopathy in 4 ureters, and iatrogenic stricture in 3 ureters. Congenital UVJ stricture was only one ureter. 2. In the method of management, open surgeries were done in 10 ureters and endourologic methods in 14 ureters. 3. In 5 failures of the first management, open surgeries were done in 3 ureters and endourologic methods in 2ureters. The tuberculosis was the most of their etiology. 4. Complications in internal stent indwelled cases were flank pain, severe vesical irritation, migration into the bladder and the obstruction of the lumen of stent.

10.Effects of Social Support and Self-efficacy on Hope among Nursing Students.

PURPOSE: This study was conducted in order to explore the factors affecting the hope in nursing students. METHODS: A total of 166 nursing students participated in the study. Data was analyzed using SPSS WIN 18.0. RESULTS: The mean scores for social support, self-efficacy, and hope were above-average level. There were significant mean differences in hope according to religion, academic achievement, and satisfaction with nursing majors. Hope correlated positively with social support and self-efficacy. The significant predictors of hope included social support and satisfaction with the nursing major. The regression model explained 12.7% of hope. CONCLUSION: To improve hope in nursing students, nursing educators should focus on promoting their social support and satisfaction with the nursing major.
