2.A study on the histological responses of port-wine stains treated by flashlamp-pumped pulsed dye laser(SPTL-1).

Port-wine stains (PWSs) are benign congenital vascular malformations that most commonly occur as isolated lesions on the face, neck and trunk. The major indication for therapy in most patient, regardless of age, is psychological burden imposed by the PWSs. A Candela SPTL-1 flashlamp-pumped pulsed dy laser (Candela Corp. Wayland, Mass.) using the principle of selective photothermolysis was used. Selective cutaneous vascular damages are influenced by the wavelength. pulse duration and energy density of the laser. To evaluate histopathological changes according to energy densities are essential to treat cutaneous benign vascular lesion by the iaser. 5 patients with PWSs were treated with the Candela SPTL-1 laser. Lesions were biopsied and analized immediately after laser exposure. 2 days and 7 days after laser exposure. The results were as follows : 1. Depth of penetration The penetration depth was increased from .2mm to 1.9mm with energies ranging from 6J/cm(2) to 10J/cm(2) respectively. 2. Nonvascular damages of epidermis & dermis Focal epidermal vacuolization has been observed in biopsy specimens immediately after laser exposure with energy density 6J/cm(2). As energy densities increased from 6J/cm(2) to 10J/cm(2), histopathological changes got worsened with appearance of vesicles. subepi dermal seperations and epidermal necrosis. 2. Vascular damages of dermis Multiple foci of agglutinated RBC, fibrin and platelet thrombi within the dermal blood vessels have been observed in biopsy specimens immediately after laser exposure. There was a pattern of "acute vasculitis" in the upper dermis and a prominent perivascular neutrophilic responses in the mid-dermis 2 days after laser exposure. Dermal vessels with smaller lumen and granulation tissue have been observed in biopy specimens 7 days after laser exposure. In summary, penetration depth of laser beam and nonvascular epidermal and dermal damages were directly proportional ot the energy density of the laser. But 7 days after laser exposure, epidermis and dermis showed normal appearance without detectable scarring. This study suggested that Candela SPTL-1 laser is safe and has no scar formation on the treatment of PWSs.

5.Study on Peripheral T - Cell Pepulation in Parients with Leprosy.

It is generally believed that the defense against mycobacterium leprae is largely mediated through cell-mediated immunity(CMI) and several investigators have reported a defective CMI in patients with leprosy. especially the lepromatous from. This study was undertaken to ennumerate the peripheral T-cell population in leprosy patients for evaluation of one aspect of its immune status. Fiftu-two patients with leprosy(26 tuberculoid, 17 active lepromatous, 9 inactive lepromatous) entered in this study. All the patients are under regular antileprosy chemotherapy for varing periods(10 months to 14 years). Peripheral blood T0lymphocytes were enumerated by the E-rosette technique and compared with normal healthy control. The results were as follows: The mean T-cell percentage in peripheral blood was 50.6% in 17 active lepromatous leprosy patients, 62.2% in 9 inactive lepromatous leprosy patients, 67.7% in 26 tuberculoid patients and 69.5% in 17 normal healthy controls, There was marked decrease in the peripheral T-cell ratio in active lepromatous group (p<0,005) and less marked decrease in inactive lepromatous group (0.01.

6.Total Dose Effect on Normal Skin of Hybrid Mice by Conventional Fractionated Irradiation.

Development of supervoltage treatment machine may minimize skin reaction by skin-sparing effect, but skin damage is still one of “the dose limiting factor” in radiation therapy. In spite of these importance, systemic histopathologic studies of skin in similar conditions which used in clinical treatment has not been performed so far. 60 mice were irradiated with conventional fraction (200x5/wk) and whole abdominal field (2x3 cm, from symphysis pubis to xyphoid process). Used machine was 250 KV, 24 mA. orthovoltage x-ray machine. Histopathological changes of acute skin reaction at the level of total irradiation dose were analyzed and possible mechanism of later chronic changes were investigated. Obtained results are as follows 1. In 1,000 rad irradiated group, only mild epidermal edema is noted. 2. In 2,000 rad irradiated group, slightly decreased number and size of hair follicles and appendages, dermal edema and scanty infiltration of inflammatory cells are visible. 3. In 3,000 rad irradiated group, marked increased capillary congestion and prominent infiltration of inflammatory cells are observed. 4. In 4,000 rad irradiated group, vascular wall thickening with proliferation of endothelial cells are prominent. Dermal thinning and hyalinization are newly developed. 5. In 5,000 rad irradiated group, complete desquamation of epidermis is not seen, despite of acceleration of all above mentioned changes.

7.Two Cases of Nickel Dermatitis.

Nickel sensitivity is the most common cause of allergic metal dermatitides,, particularly in women. Women are uaually sensitized. by the objects wom close to the skin ('e.g. earring, necklace fasther, watch, suspenders, brassiere clips, etc.), and men are by occupational exposures (e.g. plating or printing industries, tools, etc.). Nickel dermatitis is frequently developed on the sites where seems to have been directly contact witb nickel substance, but secondary eruptions may develop on the areas where seems to have not been directly exposed to nickel, so that nickel dermatitie is sometimea misdiagnosed as atopic dermatitis, nummular eczema, stasis dermatitis, or other dermatitides. The authora prcsent two cases of nickel dermatitis confirmed by patch test with 2. 5% nickel sulfafte; first case-a 22 years old female who has erythemat,ous maculopapular patches on her ears, around neck, left wriat, elbows, and abdomen; second case tkat was previously misdiagnosed as mummular eczema-a 18 years old female who has large oozing and crusted patches on both lateral aspects of her right thigh and back, The authors reviewed the incidence of nickel sensitivity during past years in korea and the clinical aspect of distribution of nickel dermatitis.

8.Therapeutic Trial of Dinitrochlorobenzene ( DNCB ) Sensitization in Patients with Warts.

Warts are benign turnors caused by oncogenic human papovavirus which has been clearly identified on electronmicroscopy, although repeated atternpts at culture have not been successful. Various therapeutic modalities have been tried for the treatment of warts, however thete is no saisfactory method and the clinical course as well as the prognosis is always variable. Recently several authors have reported successful therapeutic results in the treatment of warts by DNCB sensitization method based on the findings that warts are often regressing spontaneously and that the spontaneous regression seems to depend on host's ability to mount an immune response against the wert virus. The present study was undertaken to evaluate the DNCB sensitization method for possible routine use in the treatment of warts. Fifteen cases including five verruca vulgaris and ten verruca plana juvenilis patients were sensitized with 1000ug of DNCB in acetone solution on either side of their inner forearms. Seven or ten days after sensitization, several warts, which were unintentionally selected, were challenged with 0.05 to 0.1ml of weaker DNCB solution(50ug-25ug/0.1ml)at weekly interval. Four patients with verruca vulgaris and three patients with verruca plana juvenilis did not return after 1 to 4 times of challenge, however six out of these 7 patients showed a slight decrease in size and number of the warts. One patient with verruca plana juvenilis developed marked irritative and delayed cutaneous hypersensitivity reaction to the 3 times of challenge, so that this patient was excluded from further study. The remaining 7 patients consisted of I verruca vulgaris and 6 verruca plana juvenilis patients showed complete disappearance of all warts including those which were never challenged, after 1 to 13 times of DNCB challenge application. The authors reviewed the possible therapeutic mechanism of DNCB sensitization in the wart patients, the possible advantage and disadvantage of this method and concluded that this method can be cautiously used in seleted patients with warts whose response to the conventional treatment methods are poor.

9.Dietary Intakes, Serum Lipids and Hematological Indices in Female Adolescent Smokers.

This study was due to investigate the dietary intakes, serum lipids and other hematological indices in female adolescent smokers. The subjects were 85 smokers, whose average pack-year(smoking years on the basis of one pack of cigarettes per day) was 1.26 and 87 nonsmokers who were female high school students in Seoul. An anthropometric measreument was performed and % body fat was also analyzed by the Bioelectrical Impedance Fatness Analyzer(GIF-891). Dietary intakes and food habits were examined through questionnaires and nutrient intakes were analyzed by the Computer Aided. Nutritional analysis program for professional(CAN-pro). Serum TG, HDL-cholesterol and total-cholesterol levels were measured with test kits. Serum glucose, albumin, GOT and GPT were measured by automated dry chemistry system, SPOTCHEM 4410. Hemoglobin, hematocrit, RBC, WBC and MCV were determined by Semi Automated Microcell Counter(F-520). All data were statistically analyzed by SAS PC package program. There was no significant difference in the anthropometric measurements between smokers and nonsmokers. The caloric intake in adolescent smokers tended to be higher than that of nonsmokers but the difference was not statistically significant. In addition, there was no significant difference between smokers and nonsmokers in biochemical indices. Analysis of serum lipids showed that the serum levels of total-cholesterol and LDL-cholesterol of nonsmokers were unexpectedly significantly higher(p<0.05) than those of smokers. Overall results indicate that smoking itself with short pack-year in healthy female adolescent did not seem to influence apparent health and nutritional status.
