2.Autoimmune progesterome dermatitis.

Autoimmune progesterorie dermatitis describes an eruption which is influenced by the hormonal changes of the menstrual cycle. Recurrent premenstrual eruptions occur which correspond to the postovulation rise in serum prrigesterone. A 32-year-old women feveloped cyclic hives premenstrually and followed by spontaneous regression. Intradermal skin testing with crystal progesterone produced a generalized urticarial response after eight hours. The patient was treated with systemic corticostirod and antihistamine.

3.Effect of Coculture System with Autologous Cumulus Cells on Embryo Quality and Pregnancy Rates.

Despite the rapid development of assisted reproductive technologies (ART) in recent years, implantation rates after replacement of embryos into the uterine cavity remains low. Several techniques such as culture conditions based on formulations of human tubal fluid and various ART techniques as GIFT, ZIFT, TET have been adopted in recent years to improve embryo viability in vitro and implantation rates. Also, coculture of human IVF-derived embryos have been used in an effort to increase the number of viable embryos following IVF and to improve synchrony between the developing embryo and the uterine environment. The aim of this study was to evaluate whether the use of coculture with autologous cumulus cells has a significant beneficial effect on the development of embryos in vitro and its relation to the pregnancy rates in 120 patients with previous failed IVF-ET from September, 1995 to January 1998. We obtained the results from which significant improvement in the quality of viable embryos were observed using a coculture system with autologous cumulus cells, but pregnancy rates in this group of patients did not differ from the rate in the standard IVF group during the same period. Our study shows that a simplified short-term coculture system with autologous cumulus cells may help rescue moderate quality embryos to cleave regularly.

4.Treatment of angiofibroma with carbon dioxide laser and copper vapor laser.

Angiofibroma is a disfiguring facial deformity that constitutes pait of the multiple system involvement in tuberous sclerosis. We describe eight affected patients who have been treated with:arbon dioxide laserabration and subsequent copper vapor later treatment for residual angiomatous component, which resulted in conspicious cosmetic improvement without scarring or recurrence. This report documents that the treatment with carbon dioxide later and subsequent copper vapor laser is an efficient and safe remedy for treating multiple facial angioibromas.

5.Two cases of tinea faciei with atypical clinical manifestation.

Tinea faeiei is an unusual fungal infection which often is not recognized by clinieians. Because of light sensitivity, the frequent absence of typical clinical manifestation, and secondary changes due to steroid treatment, this condition is frequently misdiagnosed with other dermatoses. We described two patients who showed atypical facial lesions which were histologically diagnosed as tinea faciei. The case I was a 54 year old male who had multiple discoid plaques and nodules on the face. The other case was a 83 year old female who showed scaly plaques, nodules, pustules, and photosensitivity on the face. Histologic exarnination revealed fungus spores and mycelia within the inflamed hair follicles.

6.Treatment of Pelvic Osteolysis with a stable Cementless Acetabular Cup with Exchange of Polyethlene Liner and Morselized Bone Graft.

Purpose: We evaluated, retrospectively, twenty hips that had undergone revision surgeries because of massive pelvic osteolysis in order to determine the retention rate of acetabular metal shells and polyethylene liner exchanges.. Materials and Methods: We performed liner exchanges for massive osteolysis around stable HG acetabular cups with severe polyethylene wear in twenty-three hips, between June 1996 and May 2003. Clinical and radiological follow-up was available for 20 hips for more than 2 years. In 18 hips, we performed curettage of the granulomatous tissue and tightly packed morselized cancellous allografts into the screw holes or the peripheral rims for the acetabular osteolytic lesions. The mean follow-up period was 3.8 years (range, 2.4 to 9.3 years) and the mean of time from the primary total hip arthroplasties to the component exchanges, was 8.2 years (range, 5.6 to 12.4 years). Results: During the follow-up period, all of the hips were functioning well, and none required any subsequent repeat surgeries. Dislocations occurred three times after the repeat operations in one hip, which was treated successfully with an abduction brace. None of the hips demonstrated a progression of the pre-existing osteolytic lesions or the development of any new osteolytic lesions. At the final follow-up, none of the acetabular components demonstrated any evidence of loosening. Conclusion: Our results demonstrated that isolated liner exchanges and debridement of the granulomatous tissue, with or without bone grafting, can be an effective alternative solution to revision of the cup for massive osteolysis around well-fixed, cementless, acetabular cups in selected patients. Retention of the pre-existing cementless acetabular cup provides less intra-operative and post-operative morbidity. However, in order to determine the longevity of the retained cementless acetabular cups, further long-term studies are necessary.

7.Clinical Study of Conservative Treatment in the Carpal Tunnel Syndrome

Twenty-four hands of eighteen patients of carpal tunnel syndrome were treated with two or times injection of 5 mg dexamethasone and three weeks splinting at the department of orthopedic surgery, Korea University Hospital From Jan. 1980 to Feb.1985. The results obtained were as follows; 1. Physical findings consisted of sensory disturbances in sixteen hands, positive Tinel sign in thirteen, positive Phalen test in eighteen and thenar atrophy in eight. 2. On electromyographic study, delay of motor conduction velocity of the median nerve was noted in twelve cases and sensory nerve velocity in fifteen. 3. Fifteen patients(eighteen cases) were followed up from 6 months to 30 months. Twelve cases(60%) were improved and three cases were recurred.

8.The Surgical Treatment of Trochanteric Fracture

The incidence of trochanteric fracture of the femur is increasing, which is likely to continue for many years because of an increase in traffic accidents and the population of elderly people. The primary goal in the treatment of an elderly patient with an intertrochanteric hip fracture is to return the patient to his prefracture activity without any complication, as soon as possible. Rapid mobilization with stable internal fixation helps to prevent skin ulceration, pneumonia, urinary stasis, thromboembolic disease and other complications of confinement to bed in the elderly. Stable internal fixation depends mainly upon the fracture type and operative techniques. A clinical study of operative techniques was done in twenty three patients with trochanteric fracture of the femur who had been admitted and treated surgically at the Orthopaedic Department of W.M.B.H. during of two years from Mar. 1982 to Feb. 1984. The following are brief descriptions of the operative techniques. l. An L-shape incision and refiection of the vastus lateralis muscle provided wide exposure and facilitated anatomical reduction under direct vision. 2. The nail insertion site was selected at a slightly eccentric position anterior to rather than exactly midway from the opposite lateral cortex of the lesser trochanter. Therefore the nail could be laid rigidly between the thick anteromedial cortex and the posteromedial calcar portion. 3. The best stable position among the fracture line, the neck shaft angle and the nail plate could be obtained by the yoking procedure. It also allowed for later proximal migration of the shaft fragment and fracture impaction without impingement on the base of the barrel.

9.Avulsion of the Tibial Attachment of the Posterior Cruciate Ligament of the Knee Joint (Seven Cases Report)

The posterior cruciate ligament plays as basic stabilizer of the knee. Therefore the knee becomes unstable when tibial insertion of the posterior cruciate ligament is avulsed. The diagnosis may be delsyed or missed because of surgeon's 1sck of awareness or rare condition. The authors treated seven cases of avulsion fracture of posterior cruciate ligsment at the tibial attachment ares ranged from ten to thirty-two years. There were five male and two female. The results were as follows : 1. The cause of all seven cases injury was traffic accident. 2. The physical findings were seven positive posterior drawer test, seven cases of abrasion or laceration over the anterior proximal tibial surface and antero-medial surfsce of the knee joint. 3. The associated injury was comminuted fracture of the tibia and femur (1 case), rupture of collateral ligsment and comminuted fracture of the tibia (1 case) and zygomatic fracture (1 case). 4. The treatment methods were four cancellous fixation, one K-wire fixation after epiphyseal plate fusion but each one pull out wire and dexon suture before epiphyseal plate fusion. 5. The motion of knee joint was within normal limit and no instsbility was inspected at the three months after reduction.

10.Investigation of the pharmacologic response of sweat glands in the lesions of psoriasis, vitiligo and localized scleroderma(morphea).

In a previous communication, we described a relatively simpli. method for assaying quantities of sweat using the skin surface hydrometer. In order to evaluate the pharmacologic response of weat glands in psoriasis, vitiligo and loalized scleroderma, we measured the basal skin conductance and the quantity of sweat induced by intradermal injections of pilocarpine nitrate in the lesional and per ilesional skin. The results were as follows : 1. In psoriatic plaques, based skin surface hydration and the sweat response to pilocarpine was markedly reduced (p<0.05), but there were no significant differences in normal perilesional skin. 2. In generalized vitiligo, basal skin surface hydration and the sweat gland response was normal. In the case of the segmental type, there was a slightly decreased sweat response in the lesion but, not significant statistically. 3. In the morphea plaque, basal skin surface hydration and the sw.at response to pilocarpine was markedly reduced but was normal in perilesional skin. This study presents evidence of the functional changes in the development of sweat secretion and surface hydration in the lesions of psoriasis, morphec and segmental type of vitiligo. The demonstration of ahnormalities in the pharmacolopic response to pilocarpine induction led us to suggest the possibility that some degree of sweat gland involvement is induced by the pathogenetic mechanisms of these diseases.
