We studied 105 patients with trasmural myocardial infarction who were admitted to Soonchunhyang university hospital from 1984-1988, and evaluated the clinical and coronary angiographic findings accordin to the age. The following results were obtained: 1) Among 105 patients with myocardial infarction, the prevalent age were sixth and seventh decade(27.6%) and the ration of male to female was 2:1, But the raion of male to female was 5.3:1 under the 50 years of age. 2) In frequency of risk factor of coronarhy heart disease, cigarrette smoking was present in 66.7%, hypertensio in 44.8% and hypercholesterolemia in 28.6%. The mean number of risk factor was 1.3 under the 50 years of age and was 1.7 over the 50 years of age. 3) Anterior myocardial infarction was most common under 50 years of age whereas anterior and inferior myocardial infarction occur similarly over the 50 years of age. 4) By coronary angiography, one vessel disease of stenosis of left anterior descending coronary artery was common findings under 50 years of age, but multivessel disease was common findings over the 50 years of age.

Aortic root diameter by two dimensional and M-mode echocardiography in predicting prosthetic aortic valve size preoperatively was measured in 10 adult patients undergoing aortic valve replacement. Correlation of aortic root diameter measured by two dimensional echocardiography and actual prosthetic valve size implanted by aortic valve replacement was excellent. Correlation coefficient was statistically significant (r=0.91, P<0.001). Correlation of aortic root diameter measured by M-mode echocardiography and actual prosthetic valve size was also excellent. Correlation coefficient was statistically significant (r=0.86, P<0.001). This study demonstrates that aortic root diameter by two dimensional and M-mode echocardiography can accurately predict prosthetic aortic valve size in patient undergoing aortic valve replacement.

Myoglobin has been shown to be elevated in the serum after myocardial infarction. Myoglobin is one of the first to appear in the serum and its quantification may thus be used in the diagnosis of the early phase of acute myocardial infarction. The purpose of this study was to clarify the time course of myoglobin elevation in the early phase of acute myocardial infarction. We compared this with the time course of serum CPK and obtained following results: 1) Serum myoglobin becomes significantly elevated at 4 hours following onset of chest pain and apears earlier than CPK. 2) Serum myoglobin time curve peaks at 8 hours, whereas the CPK time surve peaks at 16 hours following onset of chest pain. 3) Mearsurement of serum myoglobin in the early phase of acute myocardial infarction can serve as valuable aid in diagnosis of myocardial infarction with other findings.

Metabolic reprogramming of cancer cell is one of essential hallmarks of cancer. Otto Warburg first demonstrated that cancer cells utilized more glucose and enhanced glycolytic pathway in the presence of oxygen in 1926. Scientific observations of basic and clinical research in several decades supported that cancer-specific metabolism can be an emerging target for treatment of cancer. Metabolic reprogramming is regulated by both oncogenic signaling and tumor suppressor genes associated with critical signaling pathways in metabolism. These changes provided energy, substrates for cell growth and proliferation, favoring microenvironment, and important for redox balancing for cancer cells. Recent advance of several tools for evaluation comprehensive metabolic profiles of cancer cells provided us to identification of metabolic Achilles' heel of cancers including thyroid cancer. This approach can be a useful strategy for advance in treatment of cancer patients.

Purpose: To evaluate the proportion of bevacizumab and the reason for its usage in wet age-related macular degeneration (AMD). Methods: Retrospective analysis of medical records was performed for 1,541 patients who received ranibizumab, aflibercept, or bevacizumab injection to treat wet AMD. The proportion of bevacizumab among the entire set of injections was identified. The reason for selecting bevacizumab was additionally identified. Results: During the study period, a total of 2,929 anti-vascular endothelial growth factor (anti-VEGF) injections were performed; 2,236 (76.3%) were ranibizumab or aflibercept injections and 693 (23.7%) were bevacizumab injections. The most common reason for bevacizumab usage was ‘having a 0.1 or worse best-corrected visual acuity or being unable to assure reimbursement due to the development of extensive scarring or geographic atrophy’ (297 bevacizumab injections, 42.9%). The second most common reason was ‘the inability to assure reimbursement such as extrafoveal choroidal neovascularization (CNV) or early CNV without definite fluid in the foveal region’ (201 bevacizumab injections, 29.0%). Conclusions Bevacizumab was used in 23.7% of the anti-VEGF injections to treat wet AMD. When analyzing patients’ treatment burden and financial impact, the results of the present study may provide useful information. Further multi-center studies are required to evaluate more precisely the usage of anti-VEGF drugs.

Purpose: To evaluate the proportion and reasons for ineligibility to re-register for extended health insurance at 5 years in patients diagnosed with neovascular age-related macular degeneration (AMD) and registered for extended health insurance. Methods: This retrospective study was performed in patients diagnosed with neovascular AMD and registered for extended health insurance with follow-up for at least 5 years. The criteria for re-registration for extended health insurance were determined along with the ineligibility for re-registration. Results: In total, 263 patients were included in the analysis. Of these, 148 (56.3%) did not satisfy the criteria for re-registration. No active treatment was performed in 98 patients during the last 6 months of the study period (no recurrence, 51 patients; additional treatment was not considered beneficial due to retinal damage even without disciform scar formation, 44 patients). Macular disciform scar formation was noted in 50 patients (33.8%). Older age (p = 0.013), poor visual acuity (p = 0.004), and retinal angiomatous proliferation (p < 0.001) were associated with ineligibility for re-registration due to severe retinal damage. Conclusions Among the patients who were initially registered for extended health insurance, 56.3% failed to satisfy the re-registration criteria. The primary reason was advanced retinal damage. The results of this study provide useful information for patient education and to establish long-term treatment strategies.
