1.An observation of ultrasonographic findings in thyroid disease

The authors analyzed ultrasonographically the total 45 cases of thyroid disease verified by historicaldiagnosis, functional diagnosis, and morphological diagnosis at the Departement of Radiology, Busan NationalUniversity hospital from June to Sept. 1982. The resuls obtained were as follows; 1. In the sex distributionfemale was 38 cases (84.4%), and male 7 cases (15.6%). 2. Among the total 4 cases thyroid adenoma was 24 cases(53.3%), nodular hyperplasia 8 cases (17.7%), thyroid carcinoma 7 cases (15.6%), diffuse hyperplasia 3 cases(6.7%), Hanshimoto's thyroiditis 1 cases (2.2%), subacute thyroiditis 1 cases(2.2%), tuberculous thyroiditis 1cases(2.2%) in orders. 3. On ultrasonogram, total 24 cases of thyroid adenoma showed a single nodule in 24 cases(100.0%), smooth outer margin in 23 cases (95.8%), capsular echo in 23 cases (95.8%), pure cystic nodule in 4cases (16.7%), and increased echogenicity in 17 cases (85.0%) among the 20 cases excluded the pure cystic nodules.4. Total 7 cases of thyroid carcinoma showed no capsular echo in 4 cases (57.1%), irregular outer margine in 4cases(57.1%), no pure cytic nodule in all cases, and enlargement of metastatic lymphnode in 4 cases (57.1%). 5.Total 8 cases of nodular hyperplasia showed enlargement of thyroid gland and multiple nodule in 8 cases (100.0%),and increased echogenicity in 7 cases (87.5%). 6. Total 3 cases of diffuse hyperplasia showed enlargement ofthyroid gland and increased echogenicity in 3 cases (100.0%). 7. Total 3 cases of thyroiditis showed decreasedechogenicity in 3 cases (100.0%) and enlargement of thyroid gland in 2 cases (66.7%). 8. The cold area visualizedon radionuclide scan could be differentiated from a solid mass and cystic one by utrasonogram.

Silicone gel breast implants may induce local(fibrous capsular contracture) or systemic(rheumatoid arthritis, systemic sclerosis, etc) complications. The exact mechanism of fibrous capsular contracture has not been fully understood. In the present study, we tried to find out the effect of silicone gel on the fibroblast proliferation which has been known as a major contributing factor in fibrous capsular contracture formation. In vitro, activated macrophages are known to secrete monokines which affect fibroblast proliferation and collagen synthesis. And tumour necrosis factor-alpha(TNF-alpha) and interleukin-6(IL-6), which were released by macrophages, were reported as potent stimulator of fibroblast proliferation. The goal of this study is to investigate the role of macrophages and tumour necrosis factor-alphaor interleukin-6 in the interaction of fibroblasts and silicone gel. We designed four groups, two experimental and two control, using Institute for Cancer Research(ICR) mouse peritioneal macrophage and silicone gel. For the preparation of the conditioned medium of macrophages, peritoneal macrophages were prepared and cultured for 24 hours on the silicone gel-coated and naked (not coated) surface [silicone gel-macrophage conditioned medium(SCM; experimental group) and normal polystyrene-macrophage conditioned medium(NCM; control group) respectively]. To correct the effect of 10% fetal bovine serum which was included in Rapid Prototyping and Manufacturing Institute (RPMI) 1640 medium and draw the effect only by macrophages, the RPMI 1640 medium with 10% fetal bovine serum was cultured by the same method on the silicone gel-coated and naked surface (silicone gel-macrophage free conditioned medium; SFM and normal polystyrene-macrophage free conditioned medium; NFM respectively). Each conditioned medium was added onto NIH 3T3 fibroblasts culture at a final 25% concentration of total culture medium and followed by the cultivation for 24 hours. For antibody neutralizing experiments, each conditioned medium was preincubated with polyclonal rabbit anti-mouse TNF-alpha antibody or polyclonal rat anti-mouse IL-6 antibody for 1 hour and then, conditioned medium with antibody was added to the culture medium of NIH 3T3 fibroblasts by the same method. After 24 hours cultivation, total number of viable fibroblast(cell growth), DNA synthesis and collagen synthesis of fibroblasts with each medium were measured by sulforhodamine B(SRB) assay, 3H-thymidine and 3H-proline incorporation respectively. The results were as follows: 1. In the experiment about the effect of the conditioned medium on the fibroblast activity, the experimental group(SCM), compared with the control group(NCM), showed a significant increase of the cell growth (p<0.01), a significant decrease of DNA synthesis(p<0.001), but no significant difference in the collagen synthesis. 2. In the experiment about the effect of polyclonal rabbit anti-mouse TNF-alpha antibody on the fibroblast activity, after the addition of antibody the experimental group, compared with the control group, showed a significant decrease of the cell growth(p<0.001), a significant increase of DNA synthesis(p<0.01), but no significant difference in the collagen syn thesis. 3. In the experiment about the effect of polyclonal rat anti-mouse IL-6 antibody on the fibroblast activity, after the addition of antibody the experimental group, compared with the control group, showed a significant decrease of the cell growth(p<0.001), a significant increase of DNA synthesis(p<0.0001), but no significant difference in the collagen synthesis. In conclusion, culture supernatants (conditioned medium) of peritoneal macrophages, activated by silicone gel, stimulate the NIH 3T3 fibroblast proliferation. TNF-alpha and IL-6, products of macrophage, are involved in the stimulation of NIH 3T3 fibroblast proliferation in an in vitro condition.

7.Total Antioxidant Capacities in Patients with End Stage Renal Disease and Effect of Hemodialysis.

BACKGROUND: Cardiovascular disease is the major cause of mortality in patients receiving hemodialysis(HD) for end stage renal disease(ESRD). After renal failure, antioxidant levels increase, possibly in response to increased generation of free radicals. As a result, increased lipid peroxidation may contribute to increased risks of atherosclerosis. The aims of this study was to investigate the distribution of total antioxidant capacity of plasma in Korean adults and ESRD patients, and effects of HD. METHODS: Ninety five patients(41 men and 54 women, mean ages 54.7+/-28.3 years) receiving regular HD for ESRD were recruited. Venous blood samples were taken immediately before HD in 65 patients, and before and after HD in 30 patients. Control subjects were healthy individuals(61 men and 51 women, mean ages 42.7+/-10.8 years). Total antioxidant capacity of plasma and serum uric acid concentration were assessed. RESULTS: Reference range of plasma total antioxidant capacity in Korean population is 1.04 ~ 1.52 mmol/L. Total antioxidant capacities in male(1.32+/-0.11 mmol/L; mean+/-SD) were higher than those of female(1.24+/-0.12 mmol/L, P<0.001). Total antioxidant capacities in ESRD patients(1.72+/-0.26 mmol/L) were higher than controls(1.28+/-0.12 mmol/L, P<0.001). Total antioxidant capacities in pre-HD samples(1.55+/-0.16 mmol/L) were higher than post-HD(1.33+/-0.10 mmol/L). Plasma total antioxidant capacities and serum uric acid concentrations showed positive correlation(r = 0.69, P < 0.0001). DISCUSSION: The increase in total antioxidant capacity in ESRD patients might be due to high serum uric acid. Plasma total antioxidant capacities decreased after HD in ESRD patients due to decrease of uric acid concentration.

8.One Case of Menetrier's Disease.

Menetrier's disease is a rare disease characterized by the presence of large rugal folds involving part or all of the stomach. Patients with hypertrophic gastropathy often have distressing abdominal symptoms, weight loss and edema due to gastric protein loss. The 48-year-old male patient was admitted to the Hangang Sacred Heart Hospital with cheif complaints of indigestion and epigastric pain. The diagnosis of Menetriers disease is established by radiologic, endoscopic, and pathologic examination. He was treated with soft diet, antacid, H2- receptor antagonist, and IV albumin. We report a case of Menetriers disease with brief review of literatures.

10.Anatomical variation of the Main and Right Portal Vein on Indirect Portogram: Correlated with CT and Hepatic Angiogram.

PURPOSE: To describe variations of the main and right portal veins as visualized by indirect portograms and to examine the surgical implications of these findings. MATERIALS AND METHODS: A retrospective review was conducted of 632 indirect portograms of 632 patients in whom third-order branches were visualized. All patientsalso underwent dynamic CT and AP and oblique hepatic angiography. RESULTS: Variations of the main portal veinwere found in 165 patients(26.1%) and involved an immediate trifurcation of the main portal vein in 102 patients(16.1%), early division of the right posterior portal vein in 53(8.4%), simultaneous division into fourbranches in three (0.5%), five branches in two (0.3%), a replaced P4 originating from the right portal vein in two(0.3%), and an accessory P6 originating from the main portal vein, a replaced P6 originating from the main portalvein, or a replaced left lateral portal vein originating from the right portal vein in one (0.2%), respectively.Of 468 patients who had a common right portal vein, absence of the right posterior portal vein was seen in 26 patients(5.6%) and an accessory subsegmental branch originating from the right portal vein in 17 patients(3.6%). CONCLUSIONS: Recognition of such variations of the portal vein assists in the localization and appreciation ofthe hepatic segmental anatomy, in preoperative evaluations for hepatic resections, and in radiologic interventional procedures through the portal vein.
