1.The New Methold of Total Hip Replacement through Anterolateral “S” Approach

The twelve patients, whose ages ranged from 18 years to 87 years, 5 of males and 7 of females, were admitted to Department of Orthopaedic Surgery of Korea University Hospital from January, 1978 to November, 1979 for total hip replacement surgery: 5 cases of the post traumatic femur neck fracture, 2 case of idiopathic aseptic necrosis, 2 cases of post infectious bony ankylosis of the hip, 1 case of sequele of Legg-Perthes disease, 1 case of secondary osteoarthritis secondary due to congenital acetabular dysplasia and acetabular dysplasia secondary to old infection. For these cases, the authors carrid out the surgery of total hip replacement through the new method of anterolateral “S” approach,combined with innominate osteotomy and autogenous bone graft of resected femoral head. Regular type of eleven Muller prosthesis and one Charnley prosthesis were adequately inserted in the enlarged acetabular socket. The results obtained are as follows: 1. This approach had the following advantages; such as the sufficient exposure of the operative field without the greater trochanteric osteotomy, the shortened operative time, the decreased amount of blood loss and the easy widening of the acetabular socket during the surgery. 2. There was no difficulty to perform simultaneously innominate osteotomy with autogenous bone graft of resected femoral head to the ilium during the same operative period. 3. The release of the rectus femoris from the anteroinferior iliac spine does not cause any weakness of the knee extension. 4. In all 12 patients, the end results of the function after this new method revealed saticefactory.

2.Studiedies on the Pollution Bacteria in the River Water of Baek Ma.

The purpose of this study was to survey bacteria which was known as a fecal bacteria living in the human and animal gastro and intestinal tract. The colony density of the fecal coliform and fecal streptococci was 280 and 130 in 100ml sample amounts respectively. Staphylococcus aureus and Pseudomonas aeruginosa which is usually exist in swimming pool and was known as indicator of the pool water contamination was detected.

3.The Effect of Augmentation with The Synthetic Polyester Ligament in The Tear of the Posterior Cruciate Ligament: A Preliminary Report.

The purpose of this study is the present the short rerm result of arthroscopic reconstruction for PCL injury. Since January l990, authors have implanted the synthetic polyester ligament (ABC ligament, Surgicraft, U.K) into the substance of ruptured PCL proper under arthroscopic control. The follow up period ranged from 24 months to 61 months (average,35months). The main cause of injuries were traffic accidents in 38. Thirtyfive of patient were men and three were women, range in age from 18 to 70 years (average, 37 years). 18 were acute injury and operated at average 12 days after injury and 20 knee were chronic injury and operated at average 9 months after injury. There were 25 case that had combined ligament injuries (l2 ACL injury, ]0 MCL injury, 8 LCL injury elc). In 15 knees, the tear was in the mid substance area and in 13 it was near femoral attach site and in lO it was near tibial attach site. Postoperative result were evaluated by roentgenographic evaluation of posterior sagging, KT-20 (0 arthrometer and Lysholm knee score. Difference were improved from 12.5 mm (mean: pre.op) to 3.8m (mean: post.op) on posterior drawer test. Both knee showedminimal difference on KT-20N) study (2.l mm on 201b) at the time of follow up. The average Lysholm knee score were 84.7. By Clancy criteria, the result were exceflent 9 case (24%), good in 20 case (53%), fair in 7 case (18'Po) and failui e in 2 case (5%). On based of this study, arthroscopic augmentation with ABC ligament in PCL injury is very effective treatment but more longer follow-up and more case must he needed to accept this method.

4.Multiple Trichoepithelioma Associated with Milia.

We present a case of multiple trichoepithelioma associated with milia. A 12-year-old girl developed symmetrically distributed, numerous, skin-colored papules on her face, some of which were studded with milia-like whitish materials. Histopathological findings of these lesions revealed the typical findings of trichoepitheliomas with overlying milia in the upper dermis.

5.Mineral Water Investigation on 10 Area in Seoul.

We are calling the erupting ground water as drug water or mineral water in Korea and thinking those water and food making water. Authors tried to investigate the evidence of contamination by user on the 10 erupting ground water and gained following results: 1. All of the mineral waters on 10 area in Seoul were unfittable to drinking water standard. 2. In the view of the bacteriology 80% were contaminated by coliform group. 3. The highest value of the free carbon dioxide contents were 652.96 ppm at Sam Sun Mineral Water.

6.Lower Extremity Rotational Profile in Students of Elementary School.

We studied 600 normal lower extremities of Korean students of elementary school in order to establish normal values for the rotational profile and to define any differences between ethnic groups. Rotational profile examined at this study included medial and lateral rotation of the hip and thighfoot angle. The mean value of medial rotation of the hip was 48 degrees with a range of 35 to 64 degrees in male subjects, and 50 degrees with a range of 35 to 60 degrees in female subjects. The mean value of lateral rotation of the hip was 57 degrees with a range of 43 to 75 degrees, and 53 degrees with a range of 40 to 72 degrees in female subjects. Unlike medial rotation, lateral rotation of the hip was greater in male than in female subjects by a mean difference of 4 degrees(T-test, p<0.05). The mean value of thigh-foot angle was 10 degrees with a range of -2 to 23 degrees in male subjects, and 12 degrees with a range of 0 to 21 degrees in female subjects. The total range of hip rotation was slightly greater in Korean than in Caucasians or in Chinese, but, to conclude this, more large study by the same methods will be needed in other areas of Korea.

8.Two cases of cutis verticis gyrata due to cerebriform intradermal nevus.

Cutis verticis gyrata(CVG) is an unusual conformation of the scap characterized by the presence of folds and furrows due to thickening of the corium and the subutlneous tissue. In sccordance with etiological factor and associated findings, CVG occurs as primarary disease or secondary to a variety of local or systemic diseases. Cerebriform intradermal nevus(CIN) is a cause of CVG that can usually be recognized clinically and for a certainty histologically. We report two cases of CVG due to CIN. Not only did they have CVG due to CIN on the scalp, but also they had associated congenital nevi on the face and whole boispy, respectively.

CT findings of seventy seven cases in sixty six patients were compared with the conventional radiographicfindings to evaluate the utility and the merits of the high resolusin CT in trauma, The resuls were as follows: 1.CT was helpful in the management in 50 cases(64%) of total 77 cases. 2. CT was useful in detection and evaluationof traumatic lesion in shoulder, wrist and ankle as well as spine, pelvis, hip and knee. 3. In difficultdiagnostic dilemas in trauma of the bones and the joints with the complex anatomy, CT should be consideredroutinely when appropriate.

10.Spontaneous Testicular Hemorrhagic Necrosis Masquerading as a Testis Tumor .

Spontaneous testicular hemorrhagic necrosis is a rare disease usually associated with testicular torsion. Partially involved and suspicious testis tumor cases are also defined from orchiectomy specimens. Herein, a spontaneous hemorrhagic necrosis, without any testicular torsion, but with involvement of the whole testicle and epididymis, is reported. A 21 year old patient, who presented with a painless left testicular enlargement of several days duration was believed, based on physical examination, ultrasonography and elevation of serum LDH, to have a testicular tumor. Diagnosis was made only after radical orchiectomy and histopathological examination.
