Numerous medications have the potential to induce acute pancreatitis. However, isoniazid-induced acute pancreatitis is extremely rare. Drug-induced acute pancreatitis can be diagnosed by improvement after stopping the drug and recurrence of pancreatitis when rechallenged. We present a case of severe acute pancreatitis accompanied by multiple large pseudocysts after isoniazid treatment for pulmonary tuberculosis. We confirmed that isoniazid induced pancreatitis by rechallenging after treatment cessation. Most previous reports of isoniazid-induced pancreatitis have been clinically mild forms, and the patient fully recovered with supportive management. However, this case presents severe and permanent pancreatic damage that developed with 5 weeks of isoniazid treatment. When a patient presents with manifestations of pancreatitis during treatment of tuberculosis that includes isoniazid, the physician should consider isoniazid-induced pancreatitis.

PURPOSE: This study was conducted to study the attitude of Korean lawyers toward withdrawal of life sustaining treatment, and compare and analyze different types of their attitudes. METHODS: Research design of this project was Q methodology approach. The study population was 24 lawyers, aged from 32 to 69 years. Q sample to investigate the attitude of the lawyers toward withdrawal of life sustaining treatment included 34 statements obtained from literatures, TV debate, and depth interviews of 5 lawyers among the lawyers included. After listening to the purpose and method of the study, the 24 lawyers agreed to fill out a survey asking sociodemographic information, and the information was distributed in 9 scale Q-sample. RESULTS: The collected data were processed through QUANL PC program and sorted into 5 types as follows: The first type was 'Choosing to withdraw life sustaining treatment', the second 'Withholding life sustaining treatment' regardless of the cost, the third is neutral type that claims that humans have the right to decide the death and life, and demands the proper legalization to protect such rights, the fourth type agrees to withdrawal of life sustaining treatment, nevertheless, admits that one has a rigt to withhold one's own life treatment, categorized as self contradiction type. The fifth type believed that 'Life and death are providential' with the faith, therefore, such authority to decide life and death belongs to God, but not human beings. CONCLUSION: In conclusion, the lawyer's attitudes toward withdrawal of life sustaining treatment were grouped into five different types as follows: 'Choosing to withdraw life sustaining treatment', 'Withholding life sustaining treatment', 'Demanding legalization', 'Self contradiction type', and 'Life and death are providential'.

It is not the best way to treat a hopeless patient with life-sustaining medical devices until the heart beats stop. Advanced medical technology may prolong the life for a significant period without recovery from the disease. However, it would give an unbearable economic burden to the family and the society. In 2006, we decided not to operate 9 patients with traumatic intracranial hematomas. We examined those patients with special references to possible legal and ethical problems. It is reasonable to withhold a treatment after documentation that the family never wants any life sustaining treatment when the treatment does not guarantee the meaningful life.

PURPOSE: To identify the efficacy of spectacle wearing for 4 months before amblyopia treatment in children with anisometropic amblyopia. METHODS: The patients with anisometropic amblyopia without strabismus were selected for the present study. The patients were divided into 2 groups; patients who began amblyopia treatment while simultaneously wearing glasses (Group A, 16 patients) and patients who began amblyopia treatment after wearing spectacles for 4 months (Group B, 17 patients). Intermittent atropine penalization or part-time occlusion was provided for amblyopia treatment. Age and best-corrected visual acuity at the start and cessation of treatment, type of amblyopia, treatment method and duration of treatment were analyzed. RESULTS: There were no statistical differences among age, best-corrected visual acuity of the amblyopic and better eyes, type of amblyopia, or treatment method between the 2 groups. In Group B, visual acuity of the amblyopic and better eyes improved after 4 months of spectacle wearing. At the last visit, there were no statistical differences of visual acuity in the amblyopic eye between the 2 groups. In children with resolution of amblyopia, the treatment duration of Group A (15 patients) was 17.3 months, longer than the 4.7 months in Group B (13 patients) (p = 0.003). CONCLUSIONS: Wearing spectacles for 4 months prior to amblyopia treatment may be an effective method of shortening the treatment duration.

PURPOSE: An optimal treatment for Helicobacter pylori (H. pylori) infection in children has not yet been established. In this study, the efficacy and the tolerability of triple therapy with colloidal bismuth subcitrate (CBS), amoxicillin and metronidazole, were evaluated in comparison with dual therapy with CBS and amoxicllin. METHODS: Eighty-six children with H. pylori infection, aged 6 years to 14 years, were enrolled in this study. H. pylori infection was confirmed by endoscopic antral biopsy with an rapid urease test, culture and a modified Giemsa stain. The children were considered positive for H. pylori infection if culture was positive or if both modified Giemsa staining and CLO test were positive. The children were treated with one of the following three regimens. Dual therapy with CBS for 4 weeks and amoxicillin for 2 weeks (Group I. n=57: 1993.8-1995.1), Triple therapy with CBS for 4 weeks, amoxcillin for 2 weeks and metronidazole for 2 weeks (Group IIa. n=11: 1995.2-1995.7), and Triple therapy with CBS for 2 weeks, amoxicillin for 2 weeks and metronidazole for 2 weeks (Group IIb. n=18 : 1995. 8-1996. 6). A 50mg/kg/d dose of amoxcillin was aclministered, and that of CBS was 7-8mg/kg/d, and that of metronidazole was 20mg/kg/d. About one month after the cessation of treatment, eradication of H. pylori was evaluated with repeated endoscopic biopsy. RESULTS: H. pylori eradication rate was 61.4% (35/57) in Group I, 90.9% (10/11) in Group IIa, and in the absence of H. pylori 88.9% (16/18) of Group IIb was done by means of an urease test, culture and a modified Giemsa stain (P=0.012). The overall proportion of eradication of H. pylori infection was 89.7% (26/29) by triple therapy and 61.4% (35/57) by dual therapy (P=0.007). However, there was no statistically significant difference in eradication rate between the 2-week triple therapy (IIb) and the Denol 4-week triple therapy (IIa)(P=0.86). Side effects : All patients in three treatment groups were tolerated well with little and mild side effects (P=0.258). CONCLUSION: The 2-week triple therapy with CBS, amoxcillin, and metronidazole would be a highly effective and safe treatment regimen for H. pylori infection in children.

PURPOSE: The purpose of this study was to describe feelings and actions of nurses following withdrawal of life-sustaining treatment from children being cared for by the nurses. METHODS: Data were collected by in-depth interviews with 7 nurses from different hospitals where children receiving nursing care had life-sustaining treatment withdrawn. The interviews were conducted from August 2016 to February 2017 when all data were saturated. Interviews lasted 30~90 minutes and were conducted 2~3 times per participant. Data were analyzed using Giorgi's phenomenological research methodology. RESULTS: The following factors constituted experiences of nurses working in pediatric wards when life-sustaining treatment was withdrawn from children: “agony and conflict in the aspects of care”, “heavy mind and regret for exhausting care”, “intentionally avoiding parents' sadness”, “comforting sadness in the heart” and “orientation in the role of caring for children undergoing withdrawal of life-sustaining treatment.” CONCLUSION: Findings indicate that support systems and intervention programs need to be developed so that nurses can understand and wisely deal with experiences of withdrawal of life-sustaining treatment from children who receive care from nurses.

BACKGROUND: Home-use devices designed for cosmetic purposes have recently gained popularity. Small, low-energy, low-cost intense pulsed light (IPL) systems provide safe, convenient, and private treatment for several medical conditions. OBJECTIVE: In this study, we aimed to assess the efficacy and safety of a home-use IPL device for hair removal, acne treatment, achieving a whitening effect for hyperpigmentation, and wrinkle reduction. METHODS: Five IPL hair removal treatment sessions were performed at 1-week intervals. A total of 10 sessions were performed in a split-face manner for other medical conditions. Four independent randomized controlled trials were performed using a blind evaluation method. All subjects were followed up twice to evaluate the maintenance of efficacy. RESULTS: A significant reduction in hair was observed via trichoscopy following two sessions of treatment (p<0.001). A mean hair reduction of 80% was observed at week 4; however this returned to the previous state upon cessation of treatment. Acne grading decreased from 2.6 to 1.1 at week 8, and IPL treatment induced a significant reduction in melanin index after 2 weeks. The average values of the difference of R2 (Cutometer(R)) between IPL-treated areas and baseline were higher than those of the difference of R2 between control area and baseline. CONCLUSION: This home-use IPL device was effective for hair removal and treatment of acne, hyperpigmentation, and wrinkles.

BACKGROUND: The aim of this study was to evaluate the longitudinal changes of trabecular bone score (TBS) during and after bisphosphonate (BP) treatment in postmenopausal Korean women with osteoporosis. METHODS: We analyzed 191 patients who took BP and underwent bone mineral density (BMD) test for the period from January 2010 to December 2015. The mean follow up period during treatment and after treatment was 22.8 months and 18 months, respectively. The TBS and BMD values were evaluated by the percent changes relative to the baseline. RESULTS: In 191 patients, who treated with BPs, L-spine BMD increased 3.65±0.5% and TBS increased 0.26±0.4% from baseline during first 1 year. At 2 to 4 years, the changes of BMD and TBS from baseline gradually increased up to 9.3±3.25% and 2.69±0.98% and both results showed statistically significant correlation. In 86 patients who stopped BPs, L-spine BMD decreased -0.54±1.07% and TBS increased 0.33±1.96% from baseline during 3 years follow up period. CONCLUSIONS: Lumbar spine TBS increase over time with BPs treatment although the changes were less than that of BMD. Also, it preserve for years after stopping treatment, as the changes of lumbar spine BMD. The results of BMD and TBS showed significant correlation during treatment but not during drug withdrawal.

Verruca plana is a subtype of warts, which are one of the most common dermatological diseases. A 37-year-old man presented with multiple asymptomatic skin-colored, 1∼2-mm, flat-topped papules on both arms, neck, and face. The patient had ulcerative colitis, which had been treated with immunosuppressants for 15 years. After skin biopsy, verruca plana was confirmed. The patient was treated with 5% imiquimod cream for 6 months. However, only mild improvement was observed. Acitretin was then added to the treatment regimen. After 44 weeks of treatment, acitretin was stopped. Dramatic and rapid clinical improvement was achieved after 3 weeks of treatment, and no sign of recurrence after treatment cessation has been reported for 60 weeks. Consequently, the combination of oral acitretin and topical 5% imiquimod cream should be recommended for the effective and safe treatment of recalcitrant verruca plana in immunosuppressed patients.
