Analyses were done on one hudred and twenty caaes of bleeding gastric ulcers diagnosed by emergency endoscopy for the past five years. The reaults are as the following: Much more cases were found in male than female and the most prevalent age group was 6th decade. The body of stomach along the lesser curvature was the most common bleeding site. No age related difference was noted in bleeding sites. Most common type of ulcer was in the round and shallow form. Five of six cases with exposed vessels showed atrophic changes in surrounding mucosa. Among the probable precipitating factors, analgesica, alcohols and certicosteroids were found in such order,

4.Two Cases of Superficial Migratory Thrombophlebitis.

Superficial migratory thrombophlebitis (SMT) represents multiple, tender, often cordlike nodules, usuallyion the legs but occationally on the arms. In some instances the nodules slowly migrate along the course of a vein. It can occur in association with carcinoma of internal organ or other diseases but also idiopathically. In 1865 Trousseau first wrote about that patients who have carcinoma involving an internal organ are prone to develop thrombophlebitis in one or more superficial veins so he drew attention to the fact that thrombophlebitis may be a first presenting sign of carcinomatosis. In fact, it is not uncommon for the ordinary thrombotic lesions to be observed several months or even a year before the carcinoma would otherwise diagnosed. So the clinical manifestation of SMT may be very important dermatologic finding to find hidden cancer of visceral organ. We present two cases of SMT, first case is a 25-year-old male patient who had multiple, tender nodules on both lower extremities and second case is 32-year-old male patient who had multiple, tender, linear, often cordlike nodules along the inner aspect of right lower extremity. Because we could not find any internal carcinoma or other diseas esexcept histories of hepatitis, we regard them as idiopathic type, but continuous follow up observation must be needed under the importance of possible presence of internal carcinoma.

5.A Case of Herpes Gestationis.

Herpes Gestationis is a rare polymorphous, vesiculobullous disease of pregnancy and puerperium. It is characterized by rapidly progressive skin eruption heraled initially by severe generalized pruritus. Erythematous papules and urticaria like plaques rapidly appear followed by grouped, tense, vesiculobullous eruptions. A 23 year-old pregnant woman was admitted to our departrnent of dermatology hecause of erythematous patches with pruritic vesiculobullous and pustular eruption on the whole body since third week of conception, Biopsy specimen of bullous skin lesion showed spongiosis of epidermal cells, mild necrosis of basal cells, subepidermal bulla, edema, of papillary derrnis and perivascular infiltration of inflammatory cells. Ultrastructural findings showed mild necrosis of basal cells, bulla above or below basement membrane zone and partial separation of basal larnina. Direct immunofluorescences of bullous lesion and normal appearing skin demonstrated deposition of IgG in linear pattern along basement rnembra,ne zone. After treatment with systemic corticosteroid and antihistamin, the lesions were cleared, leaving residual hyperpigmentation, and she delivered normal female baby.

6.Serum IgE and Peripheral Blood T-Lymphocytes in Parientis with Atopic Dermatitis.

During recent years, many investigators studied to confirm the pathogenesis of atopic dermatitis but it remains obscure. However, two major proposals, the B-padrenergic blockade hypothesis of Szentivanyi and the immunological hypothesis, have been advanced. The significant role of IgE in the pathogenesis of atopic dermatitis was considered since the discovery of elevated IgE levels in atopic dermatitis in 1969. However, recent various reports have indicated the existence of immune defects not only in humoral but also in cellular immune status of patients with atopic dermatitis. This study was undertaken to assess the humoral and cellular immune status of patients with atopic dermatitis using in vitro immune parametera The relationship between the humoral and cellular immune status of atopic subjects and the severity of disease was examined reapectively. The effect of the corticosteroid administration on the serum IgE levels of the patients with atopic dermatitis was followed. Eighty-three patients with atopic dermatitis, diagnosed and treated at the Department of Dermatology, Soon Chun Hyang University Hospital from October, 1988 to September, 1981 and twenty-one normal healthy subjects were included in this: study. Serum lgE levels were measured by Phadebas IgE (paper radio-immunosorbent technique) and peripheral blood T-lymphocytes (early and total) wer enumerated by the E-rosette technique.

8.Pyogenic Arthritis in Renal Transplant Recipients.

Acute pyogenic arthritis in renal transplant patient is one of the serious problem resulting in significant morbidity and mortality. The purpose of this paper is to analyze seven patients in whom acute pyogenic arthritis developed following renal transplantation. Pyogenic arthritis developed in seven (0.7%) out of 958 renal transplant patients whose transplanted kidney functioned well for more than two years. The age of the patients ranged from 37 to 65 years with a mean of 48 years. All patients were male. Pyogenic arthritis developed between the period of one and 12 years (mean: 4.5+/-4.2 years) following transplantation. Causative organisms were Staphylococcus aureus (three), E. coli (two), Salmonella (one) and Acinetobacter bananii (one). Involved joints were four knees, one hip, one elbow and one finger. Two patients improved on a conservative regimen of bed rest and antibiotics and five patients required open drainage. However, pyogenic arthritis recurred in three years, four years and seven years after renal transplant in one patient. Looking at the probable risk factors of age, sex, immunosuppresants, diabetics and acute graft rejection, diabetics and older age (over 40 years old) are highly associated with post renal transplant pyogenic arthritis.

9.Ganglion in extensor tendon of the hand: case report

Ganglion is a common clinical entity, usually superficial in location, therefore easily diagnosed and most common tumors of the hand which represent 50 to 70% of all soft tissue tumors of the hand. The soft mucin filled cyst is attached to the adjacent underlying joint capsule, tendon, or tendon sheath. The most common ganglion is the ganglion on the dorsal side of wrist, with its origin at the scapulolunate ligament and is usually located between the extensor digitorum communis of the index finger and the extensor pollicis longus tendons. Intratendinous ganglion is very rare lesion that originates within the tendon, which has been reported 10 cases in earliest articles and 1 case in Korean article. We report one case of ganglion that arose within the extensor digitorum communis of the middle finger.

10.The Bankart Procedure in the Traumatic Recurrent Dilocation of the Shoulder

Fifteen shoulders in 15 patients with traumatic recurrent anterior dislocation were performed from Sept. 1993 to April 1994. The position in which the dislocation occurred were abduction and external rotation in 10 cases, but most likely direct injuries in the 13 cases. The average interval between the initial and second dislocation was 5.4 months, and the average frequency of dislocation was 15 times per year. Physically, crank test in 13, fulcrum test in 12, sulcus test in 4, and jerk test in 3 cases were positive. The classic Bankart lesions were 4 cases, the bony Bankart lesions in 11 cases, so all of them had an abnormal tension on the inferior glenohumeral ligament. The Hill-Sachs lesions were associated in 14 cases, the Superior Labrum Anterior Posterior lesion in 1 case. Traditional Bankart procedure in 12 cases, traditional method and shift surgery simultaneously for inferior laxity in 3 cases were performed. In 4 cases of bony Bankart lesion with large bone fragment, anterior capsule and subscapularis muslce was splitted during exposure. Velpeau sling was applied in a daytime for postoperative immobilization, exercise with 90° of flexion but no external rotation was permitted during 3 weeks after operation. After than 140° of flexion and 40° of flexion and averag 57° of external rotation. There was no recurrence in our series. The result at follow up were exellent in 9 cases, good in 6 cases.
