5.Doppler Echocardiographic Assessment of Diastolic Pressure Gradient and Mitral Valve Area in Mitral Valvular Disease.

The pressure gradient across the mitral valve and atrioventricular pressure half time were measured by Doppler echocardiography during cardiac catheterization in 15 patients with mitral stenosis. Among these 15 patients with mitral stenosis, 6 patients were combined with aortic insufficiency and 4 patients with mitral insufficiency. Mitral valve area(MVAe) was measured by Doppler echocardiographic pressure half time and mitral valve area(MVAc) was measured by cardiac catheterization data(modified Gorlin's formula). Mean diastolic pressure gradient(MDPG) and peak diastolic pressure gradient(MDPG) measured by Doppler echocaridgraphy were compared with pulmonary wedge pressure. Obtained results were as follows; 1) Thewe was significant correlation between MVAe and MVAc regardless of mitral regurgitation or aortic regurgitation(r=0.09). 2) There was significant correlation between mean diastolic pressure geadient(MDPG) and mean pulmonary wedge pressure(r=0.69). But MDPG were lower than mean pulmonary wedge pressure in patients with mitral regurgitation or aortic regurgitation, which may be due to left ventricular volume overload. 3) The correction coefficiency between peak diastolic pressure gradient(PDPG) measured by Doppler echocardiography and mean pulmonary wedge pressure(r=0.59) was slightly lower than the correlation coefficiency between mean diastolic pressure gradient(MDPG) and mean pulmonary wedge pressure(r=0.69).

7.A Case of Woringer-Kolopp Disease.

Woringer-Kolopp disease, so called pagetoid reticulosis, is most likely represent a chronic localized from of reticulosis. A 24-year old male patient visited with asymptomatic slowly enlarging, scaly single patch on his left thigh for 10 years duration. Histopathologic studies were taken with H-E stain, D-PAS, transmission electronmicroscopy and monoclonal antibody(OK6) tissue stain. Infiltration of atypical cells showed in the lower portion of epidermis like pagetoid patterned, none in dermis. It seems to be that the cell in this case probably originated from the stimuIated T-lymphocyte.

8.Prevalence of Hepatitis C Antibody in Patients with Cryoglobulinemia.

BACKGROUND: Mixed cryoglobulinemia occurs predominantly in association with chronic liver diseases, infections, autoimmune diseases, and malignancies. Mixed cryoglobulinemia has been classified as essential if no primary disease is identified. Recently, mixed cryoglobulinemia has been reported in cases of hepatitis C virus(HCV) infection in foreign countries, but there have been few reports on the subject In our country. The aim of this study was to assess the prevalence and clinical features of HCV infection in patients with cryoglobuline mia in Korea. METHODS: Eighty-five patients with detectable serum cryoglobulin were studied between June 1996 and January 1997 at Hanyang university Hospital. Anti-HCV antibodies (Ab) were tested In all patients by particle agglutination method (ASAN HCV. PA Kit) and confirming a positive results, were tested by the microparticle enzyme immunoassay method (IMx, Abbott). Medical records were analyzed retrospectively to compare the clinical features between anti-HCV Ab positive and anti-HCV Ab negative patients. RESULTS: Anti-HCV antibodies were found in 12 out of 85 (about 14%) patients with cryoglobulinemia. Six of these patients were diagnosed as rheumatoid arthritis and 4 as osteoarthritis, and 2 as fibromyalgia syndrome. Nine of the 12 patients had no remarkable abnormalities in their liver function test results and there were no statistical differences between anti-HCV _Lb positive and negative groups. There were no meaningful differences between the two groups in immunologic data such as rheumatoid factor, C reactive protein, antinuclear antibody, and antiperinuclear factor. CONCLUSIONS: We found the association of HCV infection and cryglobulin in Korean patients with cryoglobulinemia and recommend routine anti-HCV Ab testing in patients with cryoglobulinemia even if they have normal liver function test results.
