2.Clinical application of Halo Apparatus

Ten cases of unstable spine were treated with various surgical procedures under control of Halo apparatus and following results wen obtained. 1. Six cases of unstable cervical spine were treated with Halo apparatus and were able to obtain solid fusion for permanent stability. 2. Three cases of tuberculous kyphosis were corrected with halo-pelvic hoop on, and were able to obtain mean correction angle of 42 (65%) with little danger. 3. With halo on, staged operations such as anterior and posterior osteotomy, gradual distraction, anterior and posterior fusion of the spine for correcting deformity were carried out securely. 4. Average duration of halo application was eight weeks, no more twelve weeks, and there was no irreversible complication with halo apparatus per se.

3.A Clinical Study of Ankle Fractures

The authors have reviewed 71 cases (70 patients) of the ankle fractures that were treated through the admission in Orthopedic Department, Paik Foundation Hospital, from June 1972 to April 1978. The average duration of the follow-up waa 15 months. The summary of the results were as follows; 1. Of the 71 cases, male was 54 cases and female 17 cases. The average age of the patients was 32. 2. The main cause of the injuries was traffic accident, and the other causes were sliding down and falling from a height, industrial accident, and sports injury in order. 3. By the classification of Lauge-Hansen, the pronation-external rotation and,supination-adduction types were commoner than the other types. 4. Open reduction was performed in 48 cases, and closed reduction in 23 cases. 5. The average duration of the cast immobilization after the closed reduction was 8 weeks, and that of the open reduction was 6 weeks. 6. The result of the treatment was better in those cases of the open reduction than that of, the closed reduction. 7. The good clinical results could be achieved by obtaining the anatomical position of the talus in the mortise and the reduction was achieved more satisfactorily when the key role of the lateral malleolus in complete reduction was well understood.

7.Comparison of the Hybrid Capture Assay and Polymerase Chain Reaction for the Detection of Hepatitis B Virus DNA.

BACKGROUND: Various molecular methods such as polymerase chain reaction (PCR) and DNA hybridization have been introduced to diagnose the hepatitis B more accurately. Recently, Hybrid Capture Assay (HCA) was developed, which uses the signal amplification solution hybridization capture assay with chemiluminescent detector. So we evaluated the sensitivity and clinical utility of the HCA and PCRs for the detection of hepatitis B virus DNA (HBV DNA) and compared these results with serologic markers. METHODS: We analysed the 50 samples from the hepatitis B patients using enzyme immunoassay, HCA and nested PCRs with two different primer sets. The primers of PCR I and PCR II were targeted to pol and core region respectively. RESULTS: In 18 cases, HBV DNA were detected by HCA in which the positive rates by PCR I and PCR II were 55.6%, and 88.9%, respectively. And in 32 cases in which HBV DNA by HCA was negative, the positive rates by PCR I and PCR II were 6.2% and 31.3%, respectively. In 44 cases which were positive for HBsAg, the positive rates for HBV DNA were 38.6% by HCA, 27.3% by PCR I, and 56.8% by PCR II. In cases positive for HBeAg, the positive rates were 93.3% by HCA, 60.0% by PCR I and 80.0% by PCR II. In cases positive for anti-HBe and negative for HBeAg, the positive rates were 10.3% by HCA, 10.3% by PCR I, and 44.8% by PCR II. CONCLUSIONS: Both HCA and PCR compensated each other yet as to the accurate investigation of the viral replication in patients with hepatitis B and the sensitivity was better in HBV PCR with primers to core region than to pol region.

8.Retinal Pigment Epithelial Detachment in Posterior Scleritis.

Posterior scleritis must be one of the most underdiagnosed treatable conditions in ophthalmology, partly because its manifestations are so protean and partly because the diagnosis is rarely considered. Although ultrasonography and computer tomogram are ancilliary tests, a careful examination of the posterior segment of the eye including the area of the ora serrata, macula and disc is essential to discover the presence of a posterior scleritis. We experienced prolonged retinal pigment epithelial detachments which had appered at early phase of posterior scleritis.

9.Posttraumatic Middle Cerebral Artery Occlusion: Case Report.

Although obstruction of the carotid artery after head trauma or non-penetrating injuries of the neck had been well known, reports of occlusion of middle cerebral artery following closed head trauma are rare. Up to now Jacque, et al, have found only 22 cases of posttraumatic middle cerebral artery occlusion including their own one case in the world literatures since DeVeer and Browder reported the first case in 1942. We report a case of posttraumatic middle cerebral artery occlusion. The patient developed right hemiplegia, aphasia and mental confusion 2 days following closed head trauma. Left cerotid angiograms revealed complete occlusion of the horizontal portion of the left middle cerebral artery distal to the origin of the lenticulostriate branches. Previously reported cases are review and possible pathophysiological mechanisms discussed.

10.Clinical Evaluations After Cataract Operation.

It is hard to advice for fitting contact lens in the case of monocular lens extraction in rural area because of exposure to dust laden air and elderly people in most cases. Questionares for comfortability, Side effects and the method of visual correction were obtained in 200 cases of cataract operation in rural area. Most of the advanced age group replyed that the unsatisfactory and difficulty for fitting contact lens, therefore it is desirable method to obtain good adaptation and binocualr vision with P-A (Phakic and Aphakic) spectacles by intensive training.
