2.Evaluation of laryngeal carcinoma by computed tomography

The limitation so direct or indirect laryngoscopy and laryngogram in detemining the exact site and anatomiclocation of laryngeal carcinoma were well documented by many authors. As compared with laryngoscopy and laryngogram, CT study for laryngeal cancer is more exact and accurate method demonstrating anatomic sites of involvement, invasion into deep soft tissue spaces of endolarynx, destruction of laryngeal cartilages and cervical metastasis. Fourteen laryngeal cancer patients proven by laryngoscopic biopsy were further examined by computed tomography for staging. The authors compared laryngoscopic findings with those of computed tomography, and their clinical, surgical and computed tomographic findings were analysed. The results were as follows; 1. All patients were proved as squamous cell carcinoma. They were 12 males and 2 females aged over 50 yrs. 2. Common clinical symptoms were hoarseness, dysphagia and swallowing difficulty. The pirmary anatomic sites determined by CT were 8 transglottic, 2 glottic, 2 supraglottic and 1 pyriform sinus respectively. They were 2 T1. 7 T2, 1 T3, 3 T4 by TNM systems, respectivly. (One case was difficult to evaluate exactly). 3. Invasion into deep soft tissue spaces of endolarynx, cartilage destruction, and neck metastasis were relatively predominant in transglottic caracinomas. 4.CT was superior in evaluating tumor invasion, especially into deep soft tissue spaces of endolarynx, laryngeal cartilages and metastasis ot soft tissue and lymph nodes of neck. However CT had some limitation in determining primary site of laryngeal cancer.

3.Influence of Health-Promoting Behaviors on Quality of Sleep in Rotating-Shift Nurses.

PURPOSE: The purpose of this study was to identify quality of sleep and health-promoting behaviors in rotating-shift nurses and to explore the influence of health promoting behaviors on quality of sleep. METHOD: Participants were 161 staff nurses working in an irregular three shift system in one of three general hospitals located in Kyungpook province. Data were collected using the Pittsburgh Sleep Quality Index and the Health Promoting Lifestyle Profile-II and were analyzed using hierarchical multiple regression analysis. RESULTS: The majority of the participants had very poor quality of sleep and performed a low level of health promoting behaviors. In terms of health promoting behaviors, nutrition and stress management significantly influenced quality of sleep of these nurses. CONCLUSION: Results indicate the importance of encouraging rotating-shift nurses to enhance their health promoting behaviors in order to improve quality of sleep.

4.Mauriac Syndrome in a Patient with Type I Diabetes Mellitus.

Mauriac syndrome consists of a triad of poorly controlled diabetes, profound growth retardation and hepatomegaly. We experienced a case of Mauriac syndrome in an 18-year-old girl who had poorly controlled diabetes mellitus, short stature, hepatomegaly and central obesity. Also at the time of examination, she had complications of diabetic cataract and nephropathy. Fourteen years prior to admission, she was diagnosed as diabetes mellitus at a hospital. Thereafter, she had been managed with irregular insulin injection. On physical examination at admission, her height was 135cm(<3 percentile) and her weight was 39kg(<3 percetile). She was short and obese. The liver was 5 cm palpable below the right subcostal margin. Her sexual maturation was Tanner stage I. On ophthalmologic examination, the cataracts were observed on both eyes and diabetic retinopathy was absent. Diabetic nephropathy could not be confirrned by kidney biopsy due to her mother's refusal. We studied the hormonal, radiographic and histological abnormalities. The hormonal study was normal and the bone age was by delayed as much as 10 years. The liver biopsy revealed glycogen accumulation in hepatocyte. She was consistent with Mauriac syndrome. She was managed by strict diabetic control with insulin therapy, diabetic diet and intensive education. She was discharged with well controlled blood glucose. Five months later, growth acceleration and sexual maturation have not been observed, but hepatomegaly subsided. (J Korean Pediatr Soc 2000;43-837-841)

5.Predictors of Hypertension among Middle-aged and Elderly Self-employed Workers: Results from a Baseline Survey of the Korean Longitudinal Study of Aging.

PURPOSE: The purpose of this study was to investigate the individual and working environment-related factors affecting hypertension among middle-aged and elderly self-employed workers in South Korea. METHODS: This was a secondary data analysis of data from the Korean Longitudinal Study of Aging (KLoSA). The study sample included 1,319 middle-aged and elderly self-employed workers who were diagnosed without hypertension and older than 45 years at Wave 1 of the KLoSA. Data were analyzed using descriptive statistics and Cox proportional hazards regression analysis using SPSS 20.0. RESULTS: About 20% of the middle-aged and elderly self-employed workers were diagnosed with hypertension within about 8 years from the baseline. the significant predictors of hypertension among the participants were age, diabetes mellitus, body mass index, frequency of health examinations, working hours per week, the presence of regular day off, and the type of job. CONCLUSION: Middle-aged and elderly self-employed workers are vulnerable to work-related health problems. In order to prevent hypertension among self-employed workers, it is necessary to improve the work environment-related factors as well as individual life styles.

6.Expression of the Human Erythrocyte Glucose Transporter (Glut-1) in Renal Cell Carcinoma.

PURPOSE: The objective of this work is to determine whether the degree of expression of Human Erythrocyte Glucose Transporter (Glut-1) protein might correlate with survival rate, tumor stage and pathologic findings in renal cell carcinoma (RCC). MATERIALS AND METHODS: 94 patients who underwent radical nephrectomy at our department for RCC were included in this retrospective study. Biologic significance of Glut-1 immunohistochemical staining according to the survival rate, tumor stage and pathologic findings in patients with RCC was evaluated. Immunohistochemical staining was performed using a primary antibody which was polyclonal rabbit anti-human Glut-1. RESULTS: In 67 of 94 patients, Glut-1 staining was demonstrated in the plasma membrane of cancer cells. Positive staining of Glut-1 in clear cell type was significantly higher than chromophobe type and chromophill type (p=0.01, 0.04). Positive staining for Glut-1 did not show any significant difference between localized and metastatic RCC. Positive staining for Glut-1 did not show any significant difference with tumor stage (p=0.09) or nuclear grade (p=0.20) or survival rate in localized RCC. CONCLUSIONS: In summary, Glut-1 protein express not in the normal portion of kidney but in the RCC. Expression of Glut-1 protein does not correlate with tumor stage, nuclear grade, or survival rate. These findings suggest that RCC may have another type of glucose transporter.

9.The Effect of Antioxidant Vitamins Supplementation on Blood Pressure, Plasma Lipids, Folate, and Homocysteine Levels in Smokers and Non-Smokers of College Students in the Gyeonggi Area.

This study was performed to investigate the effect of vitamin C and E supplementation on blood pressure, plasma lipids, folate, and homocysteine levels in smokers and non-smokersof college male students in Gyeonggi Area. The nutrient intakes were determined by a 24hr-recall method. The subjects were divided into six groups: vitamin C supplementation group (n: smokers = 10, nonsmokers = 10), vitamin E supplementation group (n: smokers = 10, nonsmokers = 10), vitamin C and E supplementation group (n: smokers = 10, nonsmokers = 10), respectively. There were no significant differences between the smokers and nonsmokers in terms of anthropometric measurements. Systolic and diastolic blood pressure were significantly higher (p < 0.05) in smokers than that of non-smokers. There was no significant difference in energy and other nutrients intakes between smokers and non-smokers. In plasma lipids levels, smokers had higher plasma triglyceride, LDL-cholesterol, VLDL-cholesterol, total cholesterol concentration than that of non-smokers (p < 0.05). HDL-cholesterol level of smokers had a tendency to be lower than that of nonsmokers. In smokers, AI, TPH, LPH were significantly higher than that of non-smokers (p < 0.01). Plasma folate, homocysteine levels were not significantly different between smokers and non-smokers. The effect of antioxidant vitamins supplementation in smokers: In vitamin C supplementation group, HDL-cholesterol level was significantly increased (p < 0.01) and AI, TPH, LPH were significantly decreased (p < 0.01). In vitamin E supplementation group, HDL-cholesterol level was significantly increased (p < 0.05). In vitamin C and E supplementation group, LPH was significantly decreased (p < 0.05). The effect of antioxidant vitamins supplementation in non-smokers: HDL-cholesterol level was significantly increased (p < 0.05) and AI, TPH, LPH were significantly decreased (p < 0.05) by vitamin C supplementation group. Plasma homocysteine level was decreased by vitamin E supplementation group in non-smokers p < 0.01). The results of this study showed that smoking had a tendency to increase plasma lipids levels that factor into the risk of coronary heart disease. It is considered that antioxidant vitamin supplementation in smokers had a tendency to decrease cardiovascular disease than in nonsmokers.

10.Effect of Lidocaine and Metoclopramide on Vascular Pain Associated with Intravenous Propofol Injection.

BACKGROUND: Propofol has a high incidence of pain with intravenous injection, and different methods have been used to minimize the incidence and severity of this pain. In this study, we have compared the effect of lidocaine pretreatment with that of metoclopramide pretreatment on propofol injection pain. METHODS: Ninety healthy female patients scheduled for general anesthesia were randomly divided into three groups; saline group (n = 30), lidocaine group (n = 30) and metoclopramide groups (n = 30). Each patient received 2 ml of pretreatment solution (normal saline, 1% lidocaine, 0.5% metoclopramide) via 18 G angiocatheter inserted in the antecabital fossa after applying an arm tourniquet inflated to 50 mmHg. The tourniquet was released 1 minute later, followed by intravenous injection of 2.5 mg/kg of propofol. The assesement of pain was made at the induction of anesthesia and in the recovery room and the severity of pain was classified as none, mild, moderate, severe by one observer. RESULTS: The severity and incidence of pain diminished significantly in the lidocaine group and the metoclopramide group compared with the saline group at the induction of anesthesia (P < 0.05) and there was no significant difference between the lidocaine group and metoclopramide group. We had similar results in the recovery room and one patient from the saline group and the metoclopramide group had no recall regarding injection pain. CONCLUSIONS: Intravenous metoclopramide pretreatment is as effective as intravenous lidocaine pretreatment in alleviating the propofol injection pain.
