1.An Experimental Study on Changes of Articular Cartilage following Injury of Ligaments and or Meniscus of the Knee Joint in Adult Rabbits

Osteoarthritic changes in experimental animals has been induced by several different methods. In order to study the changes of the articular cartilage induced by injuries of ligaments and/or meniscus of the knee joints, the author performed the experimental study that section of the medial collateral and anterior cruciate ligaments (group I) and section of the medial collateral and anterior cruciate ligaments combined with excision of the medial meniscus (group II) in twenty adult rabbit's knees caused instability. Histological changes of the articular cartilage were observed up to four months. The results were as follows: 1. The changes of the articular cartilage were gradually advanced depend on the time lapse after experimental procedures, in group I and ll. 2. The changes of the articular cartilage could be noticed from eighth week after experimental procedures in group I and from fourth week in group II, and more severe at sixteenth in group I and at twelfth week in group II. 3. The changes of the articular cartilage in group ll were appeared earlier and more severe than in group l. 4. The changes of the articular cartilage in the medial side of the knee joint were noticed more severe than in the lateral side.

2.Anatomical variants of paranasal sinus affecting the ostiomeatal unit.

It is well known that anatomic variations affecting the ostiomeatal unit (OMU) become one of the causes of mucosal abnormalities of paranasal sinuses (PNS). Findings of coronal plane CT scans of PNS were analyzed in 95 patients with sinusitis or sinusitis-like complaints. Anatomical variations were seen in 88 cases of 60 patients. Mucosal abnormalities were seen in 27 of 47 cases with concha bullosa, 23 of 37 cases with enlarged ethmoid bulla, 8 of 33 cases with Agger nasi, 20 of 26 cases with nasoseptal deviation, 7 of 8 cases with Haller cell, 3 of 6 cases with duoble middle turbinate, 4 of 6 cases with medially bent uncinate process, 1 cases with laterally bent uncinate process, and 33 of 40 cases with mixed variations. Mucosal abnormalities in cases with antomical variations were detected in 61 cases, including maxillary sinus(51 cases), anterior ethmoid sinus (26 cases), frontal sinus (4 cases), and mixed (32 cases). Mucosal abnormalities without anatomical variations were detected in 13 cases involving maxillary sinuses (7 cases), anterior ethmoid sinuses (5 cases), and frontal sinus(1 cases). In conclusion, as coronal plane CT scan has the capability of dolineating anatomical variations and mucosal abnormalities of PNS, it is considered useful in evaluating the treatment and prognosis of chronic or recurrent sinusitis.

4.MRI Findings of Orbital Hydroxyapatite Implants in Postoperative 5 months.

Fibrovascular ingrowth of 55 cases of intraorbital hydroxyapatite sphere was evaluated by magnetic resonance imaging about 5 months postoperatively at the Department of Ophthalmology, Yeungnam University College of Medicine from April 1993 through May 1998. Of them, evisceration was performed in 35 cases; enucleation in 14 cases, and secondary implantation in 6 cases. Fibrovascularization was classified into 4 groups by width and strength of enhancement, Grade 3-4 and 5 cases showed delayed fibrovascularization, Grade 1. Driling for peg could be performed within 1 month in the cases with Grade 3 fibrovasculariztion. In cases of delayed fibrovascularizated Grade 1 or Grade 2, driling should be performed after confirmation of complete fibrovascular ingrowth by repeated magnetic resonance imaging.

5.A Clinical Study of the Tibial Plateau Fractures

The tibial plateau fracture is a fracture of the proximal end of the tibia, involving the weight bearing articular surface. This fracture often presents some problems in treatment and prognosis, because it is frequently accompanied by soft tissue injuries such as collateral ligament, cruciate ligament and menisci. During a period of 3 1/2 years, from Febuary 1980 to July 1983, We observed 78 cases of tibial plateau fractures at Gang Nam Sacred Heart Hospital. Of the above cases, 40 could be followed for a period of anywhere from 3 months to 3 years. They have been analysed according to the cause, classification, method of treatment and final result of treatment. Among these 21(52.5%) were treated by cast immobilization, 3(7.5%) by skeletal traction, 16 (40%) by open reduction and internal fixation. Thirty two cases (80% ) out of 40 indicated the result of the Acceptable group according to Hohl and Lucks criteria.

6.A Clinical and Pathological Study of Recurrent Hematuria in Childhood.

The clinico-pathological observation was done on 28 children with recurrent hematuria, who had been admitted to the Pediatric ward of NMC from January 1981 to July 1991. The results were as follows; 1) Most of the children with recurrent hematuria were over 6 years of age(24 cases, 85.7%)and the sex ratio was about 2.5:1. 2) IgA nephropathy (17 cases, 60.7%) was the leading pathologic lesion and mild focal nonspecific glomerulonephritis (5 cases), membranous nephropathy (2 cases), minimal chage (2 cases), Alport syndrome (1case), abnormal glomerular basement membrane (1 case) followed. 3) Types of recurrent hematuria ware gross in 22 cases and microscopic in 6 cases. Flank pain was noted only 1 case. The prodromal events of hematuria were upper respiratory infection in 15 cases and severe exercise in 2 cases. 4) There were no significant differences in clinical features and laboratory findings between IgA nephropathy and other pathologic lesions, except for higher frequency of proteinuria in former group. 5) During the follow-up periods, three cases were resulted in chronic renal failure and they were two cases of IgA nephropathy Class IV and one case of Alport syndrome. And one case of IgA nephropathy was resulted in nephrotic syndrom.

8.2 cases of vasculo-Beh et's disease involving intracranial artery.

Both vein and arterial involvements with various manifestations in Beh et's disease is defined as vasculo-Beh et's disease. Venous lesions such as thrombosis or varices are more common than arterial lesions. Aneurysms or arterial steno-occlusion involving large intracranial arteries are very rare. We report two cases of vasculo-Beh et's disease involving intracranial arteries. The one case presented with carotid-cavernous sinus fistula due to internal carotid aneurysm, and the other case was accompanied by cerebral infarct due to intracranial arterial stenosis.

9.Worker's demands and Group Health Service Agency's Intention for Worksite Medical Care.

OBJECTIVES: This study was performed to survey worker's demands for worksite medical care and agency's intention for offering worksite medical care in Group Health Service Agency. METHODS: One study subjects were employers, workers and office personnels in 167 factories. They were asked for medical care utilization behavior and worksite medical care demand. 121 employers(72.5%), 145 office personnels(86.8%) and 816 workers(60.5%) answered the questionnaire. The other subjects were doctors, nurses and administrators in 66 Group Health Service Agencies nationwide. They were asked for intention of offering worksite medical care in Group Health Service Agency through the mail survey. 31 doctors(47.0%), 38 nurses(57.6%) and 34 administrators answered the questionnaire. RESULTS: 87 percentage of doctors and 94 percentage of nurses had experiences of being requested to prescribe and laboratory examination. All nurses, 94 percentage of administrators and 76 percentage of doctors thought that it was necessary to give medical treatment at worksite. Also in worksite 85 percentage of employers, 86 percentage of office personnels and 81 percentage of workers answered that they accepted the worksite medical care when it would be given. CONCLUSIONS: Worksite medical care should be permitted because almost of all workers and employers requested worksite medical care, and doctors and nurses in Group Health Service Agency were willing to give worksite medical care and there was necessity to prevent occupational cerebrovascular disease.

10.Deep Learning Approach Using Diffusion-Weighted Imaging to Estimate the Severity of Aphasia in Stroke Patients

Background:
and Purpose This study aimed to investigate the applicability of deep learning (DL) model using diffusion-weighted imaging (DWI) data to predict the severity of aphasia at an early stage in acute stroke patients.
Methods:
We retrospectively analyzed consecutive patients with aphasia caused by acute ischemic stroke in the left middle cerebral artery territory, who visited Asan Medical Center between 2011 and 2013. To implement the DL model to predict the severity of post-stroke aphasia, we designed a deep feed-forward network and utilized the lesion occupying ratio from DWI data and established clinical variables to estimate the aphasia quotient (AQ) score (range, 0 to 100) of the Korean version of the Western Aphasia Battery. To evaluate the performance of the DL model, we analyzed Cohen’s weighted kappa with linear weights for the categorized AQ score (0–25, very severe; 26–50, severe; 51–75, moderate; ≥76, mild) and Pearson’s correlation coefficient for continuous values.
Results:
We identified 225 post-stroke aphasia patients, of whom 176 were included and analyzed. For the categorized AQ score, Cohen’s weighted kappa coefficient was 0.59 (95% confidence interval [CI], 0.42 to 0.76; P<0.001). For continuous AQ score, the correlation coefficient between true AQ scores and model-estimated values was 0.72 (95% CI, 0.55 to 0.83; P<0.001).
Conclusions
DL approaches using DWI data may be feasible and useful for estimating the severity of aphasia in the early stage of stroke.
