2.Nurse's Adoption on a Planned Organizational Change.

With the dynamically changing environment of society, managing change is the vital element of organizations's survival and growth. Health care organizations have expended enormous resources to restructure patient care delivery. Despite the growing literature describing these organizational innovations, there is a paucity of credible data that reflects systematic measurement and evaluation of such changes. This study examined the nurses' psychological response toward the work process redesign, newly introduced by the nursing department in a acute care hospital. The aim of the study was to figure out how nurses's general perception of change and perceived attributes of change affected their acceptance of change during the organizational transition. This was descriptive-correlational. The sample for the study included 50 head nurses and 135 staff nurses. Data was analyzed using SPSS PC+, version 10.0. The major findings of the study were as follows: First, the mean score of the perception of change was 71.2 (SD=13.8) with the range of 0-100, which means nurses generally perceived change positively. There were significant differences in perception of change by gender and education level. Head nurses perceived change more positively than staff nurses. The higher education level showed the more positive view of change. Second, among the perceived attributes of change, trialability had the highest mean score, which means nurses perceived the change more positively if it is testable on a limited basis. Relative advantage was perceived the most negatively. Finally, factors influencing the acceptability of the work-process redesign were perceived comparability, complexity, relative advantage, and observability, which accounted for 43.7% of the variance in the acceptability of change.This study evaluated the preliminary effects of the nursing process for reengineering, focusing on nurses' acceptability towards change. The usefulness of this research study was to determine the factors influencing acceptance of organizational members during transitional periods of change and to suggest effective strategies for increasing adoption as well as for decreasing resistance to change.

3.Angioleiomyoma: A Clinicopathological Study of 59 Cases.

BACKGROUND: Angioleiomyoma of the skin is a rare smooth muscle tumor and may develop anywhere in the body. Comprehensive clinicopathological analysis of angioleiomyomas developed in Korean has not been reported in domestic dermatological journals. OBJECTIVE: The purpose of this study was to evaluate the clinical and pathological characteristics of angioleiomyoma and the relationship between the tumor and the nerve with immunohistochemical stains. METHODS: Fifty nine cases of angioleiomyoma were reappraised from the files of the pathology of Seoul National University, Green Cross Reference Laboratory Center and Korea Veterans Hospital. Brief clinical histories of all cases were examined and microscopic findings in 35 cases were classified according to subtypes. Paraffin blocks were recut and stained in 16 cases for desmin, smooth muscle actin and S-100 protein. RESULTS AND CONCLUSION: 1. Male to female ratio was 1:1.27. 2. The age of the patients was mostly over 20 years and the mean age was 51 years in men and 40 years in women. 3. Tumors occurred most frequently in the lower extremity(38 cases), and followed by upper extremity(18 cases), head(2 cases), and trunk(1 case). Women had predominance of the legs in location site and men showed equal distribution in the lower and upper extremities.

METHOD: This program is developed through literature review, survey and seminar. The program is consisted of six sessions twice a week for 6 weeks. This program is composed of health education on stroke, exercise, oriental nursing interventions, and therapeutic recreation. The outcomes have been evaluated on the basis of perceived health status, self-efficacy, U/E function, ADLs and subjective response. RESULTS: 1) After the 6 week program, the perceived health status had improved and the score of rehabilitation self-efficacy increased significantly. 2) After the 6 week program, the score of BADL increased significantly, but, the score of IADL was not statistically significant. 3) After the 6 week program, the amount of use and quality of movement of the affected U/E were increased significantly. 4) All of the participants showed satisfaction with this program. CONCLUSION: Considering these research results, the program is effective in improving functional abilities and self-management ability. Therefore this program could be continuously developed and implemented as a community based self-helf group program for post-stroke clients.

6.Effect of East-West Self-help Group Program for Rehabilitation of Post Stroke Clients.

PURPOSE: The purpose of this study was to evaluate the effect of an East-West Self-help program for Rehabilitation of post-stroke patients. METHOD: A quasi-experimental design was used. The subjects were 75 post stroke clients(Exp. group : 38, Cont. group:37). The subjects of the experimental group participated in the Self-help group program of six sessions, twice a week, during 6 weeks. The program consisted of health education of stroke, exercise, oriental nursing interventions, and therapeutic recreation. The obtained data were analyzed by using the repeated measure ANOVA of SPSS. RESULT: 1) The score of rehabilitation self-efficacy increased significantly in the experimental group as compared to the control group. 2) The score of BADL, IADL, amount of use & quality of movement of the affected U/E, and grip power increased significantly in the experimental group as compared to the control group. 3)The level of blood cholesterol decreased significantly in the experimental group as compared to the control group. CONCLUSION: Considering these research results, the program is effective in improving functional abilities and self-management ability. Therefore this program could be implemented as a community based self-help group program for post stroke clients.

7.Glomus Tumor of the Sinonasal Tract: Two Case Reports and a Review of Literature.

Herein we describe two cases of nasal glomus tumor. Histological findings were typical, save for one which was quite large (3.1 cm in its greatest dimension) with an invasive growth pattern and increased ki-67 labeling index (up to 10%). These features raised a red flag of similarity to a recently described "invasive glomus tumor of nasal cavity", suggesting a more aggressive form of glomus tumor. However, objective criteria for this possibility is lacking at present and more similar case studies are needed to establish a truly aggressive form of glomus tumor.

8.Comparative Study of Time - dependent Changes of Arterial Oxygen Saturation between upper Extremity and Lower Extremity in Spinal Anesthetic Patients.

To study the effect of one part of hemodynamic changes following spinal aneethesia on the time-dependent change of aterial oxygen saturation (SaO2) from the upper extremities and lower extremities, we analyzed the ASA class I, 30 patients (Group I, Tetracaine only (n =15), 40+/-6 years, 58+/-7kg, 162+/-6cm, Group II, Tetracaine+/-Epinephrine (n=15), 41+/-6 years, 59+/-6kg, 161+/-7cm) scheduled for urologic surgery under lower spinal anesthesia. The results were as follows: 1) There was no significant difference of preanesthetic SaO2 between upper and lower extremities in two groups (p>0.05). 2) There was no significant diference of time-dependent changes of postanesthetic SaO, in the upper extremities as compared with preanesthetic value (p=0.05). 3) There was significant ifference of time-dependent changes of postanesthetic SaO2, at 5 min, 10 min and 15 min in the lower extremities of Group I, on the other hand, at 10 min and 15 min in Group II as compared with control value (p<0.05). 4) There was significant difference of postanesthetic SaO at 10 min and 15 min in the lower extremities between two groups (p<0.05). 5) It took more longer to reach the maximum level of SaO2, after inal anesthesia in Group II as compared with Group I (p<0.05).

9.Comparison of Diclofenac or Fentanyl for Pain Following Tonsillectomy.

BACKGROUND: This study was designed to compare intramuscular diclofenac with intravenous fentanyl in the prevention of pain after tonsillectomy and adenoidectomy for day-surgery. METHODS: We studied 60 patients, aged 3-13 years, undergoing tonsillectomy with or without adenoidectomy. Patients were randomly assigned to receive placebo injecton (Group 1), intramuscular diclofenac 1 mg/kg (Group 2) or intravenous fentanyl 1 microgram/kg (Group 3) after induction of anesthesia. Pain was evaluated by using an observer pain score for the first 30 min, 60 min and 4 hrs postoperatively. RESULTS: It is statistically significant that group 2 and group 3 had lesser pain than group 1 at 30 min and 60 min. But there is no difference among any groups at 4 hr postoperatively. CONCLUSIONS: We conclude that diclofenac may have advantages compared to fentanyl in safety and convenience for the treatment of pain after tonsillectomy in children.

10.Evaluation of Government Assisted Visiting Nursing Services of Health Center in 2000.

PURPOSE: This study is a fact-finding research to understand the status of visiting nursing services operated by health centers in Korea and it aims to provide basic information for policy development on operation and management of visiting nursing services in health centers. METHOD: This study investigates the results of visiting nursing services in 242 health centers from Jan. 10 through Dec. 30, 2000, where 3,106 visiting nurses were employed by the public work program. RESULT: In 2000, 129,401 new household as service recipients was identified and that was 0.9% of Koreas total households (15,137,000), and 5.8% of low income households (2,242,000). The highest high risk group was dementia patients(aver. 55.2/1,000 person). Average number of households visited by visiting nurse were 4.5 households per day and the first-visited houses per visiting nurse were 1.1 households per day. The re-visiting rate was 71.3%. Total 4,059,130 service items were provided and assessment ranked the highest with 33.7%. The satisfaction level of clients on the nurses was an average of 3.17 points in the scale of 4 and the nursing service was a 2.60 points in a scale of 3. CONCLUSION: Visiting nursing service should continue to provide comprehensive healthcare services in cost-effective ways while cooperating with others.
