3.Calf Contouring through Gastrocnemius Partial Tenotomy.

A muscular calf is esthetically unappealing in Asian women. Treatment by liposuction alone has poroven to be difficult due to a small amount of fat. We established a new method of transecting half of the gastrocnemius tendon in order to improve the contour of the calf. Thirty-five patients were operated on during the past 3 years. The medial or lateral half of the gastrocnemius tendon was cut using an endoscopic-guided 3M AGEE blade through a 1cm-sized incision. The portion of the muscle above the site of the transection was elevated from the underlying soleus muscles. We checked the calf circumference and evaluated the clinical results at 6 months postoperatively. An average 10% reduction in calf circumfernce was measured, showing a significant esthetic improvement. There were no significant changes in gait analysis or in the Cybex test. There were no specific complications related to the procedure. Through our clinical experience in 35 cases, we were able to prove that tenotomy is a effective, simple and easy method without severe morbidity.

5.Cryopathic Hemolytic Anemia: A case report.

The cryopathic hemolytic anemia is an autoimmune hemolytic anemia induced by cold antibody. Authors report a case of cryopathic hemolytic anemia confirmed by immunohematologic examination. A 22-year-old man was admitted at Kyung Hee Medical Center in March, 1994, with a past history of Kleinerfelter's syndrome and complained jaundice as well as dizziness. Direct antiglobulin test using polyvalent and anti-C3d monovalent antisera was positive, but was negative against anti-IgG, anti-IgA, and anti-IgM monovalent antisera. Cold agglutinin titer was as high as 1:1024 at 4 degrees C and anti-I specificity was confirmed by using cord bloods. The patient was not improved despite transfusion of washed red cells and administration of prednisolone, and on 16th day of hospitalization he manifested sudden episode of headache and loss of conscioussness, soon followed by death.

6.A FEM analysis for initial stress on the upper canine by orthodontic force of intrusion arch wire activation.

The purpose of this study was to find the distribution and measurement of compressive and tensile stress when intrusi- on arch wire is forced engage with upper canine and to analysis stress at each section through FEM. And we compare compressive and tensile ratio at each section. The results were as follows. 1. At FA point and cemento-enamel junction of upper canine, compressive and tensile force ratio is about the same. 2. At apex, compressive force is the four times as tensile force. ; In intrusion, we show root resorption at apex. 3. At Cemento-enamel junction, the compressive and tensile force show the maximun value except FA point.

8.Clinical Results of Nd:Yag Laser Posterior Capsulotomy.

From Feb. 1987 to Feb. 1988, 93 cases of the Q-switched Nd : Yag laser posterior capsulotomy by using of choherent Model 7900 were carried out and 80 cases(76 patients)had been followed up for more than 2 months. The results were as follows: 1. 72 aphakic eyes and 8 pseudophakic eyes(12 traumatic cataract and 11 congenital cataract and 57 adult and senile cataract) were followed. 2. Postoperative visual acuity of 0.7 or better was achieved in 54 eyes(67.5%) and 1.0 or better in 32 eyes(40%). 3. Complications were transient increased IOP 17 eyes(21.2%) and Vitreous pro lapses with anterior hyaloid rupture 15 eyes (18.7%) and anterior uveitis 2 eyes(2.5%) and cystoid macular edema 1 eyes(1.2%) and corneal edema 1 eyes(1.2%) but most cases were without permanent damages.

10.A Case of Granular Cell Tumors.

Granular cell tumors are uncommon benign tumors occurring most commonly on the tongue, skin and subcutaneous tissue. They are most often seen as a solitary nodule in adults. Multiple lesions are very rare, particularly in children. We report a case of multiple granular cell tumors in a 13-year-old boy on his left arm which was presented as firm, flesh-colored, match-head to pea-sized, tender nodules for 6 months. Histologically, the tumors are nonencapsulated nodules, composed of large, polyhedral cells with an abundant eosinophilic granular cytoplasm. By immunohistochemistry, the granular cells are strongly positive for S-100 protein.
