1.The efficacy of tibial shaft fractures treated with interlocking nails

Objective To investigate the difference of efficacy of interlocking nails and dynamic compression plate in tibial shaft fractures.Methods The clinical data of 137 cases with tibial shaft fractures were reviewed retrospectively.They were divided into two groups according to the various treatments which included interlocking nails group(72 cases with 79 fractures) and dynamic compression plate group(65 cases with 70 fractures).The efficacy,healing fineness rate,function score after operation and complications were compared.Results After operation,the patients were followed up 21.4 months on average.The postoperative callus time,weight-bearing limb time,healing time and postoperative fixation time taken in interlocking nails group was shorter than that in dynamic compression plate group [(2.36 ± 0.46) months vs.(3.12 ± 0.57) months,(2.42 ± 0.51) months vs.(3.14 ± 0.63) months,(7.65 ± 1.29) months vs.(9.33 ±1.54) months,(12.64 ± 3.76) months vs.(16.38 ± 3.15) months],and there was significant difference (P< 0.05).The healing fineness rate was 97.5%(77/79) in interlocking nails group,and 92.9%(65/70) in dynamic compression plate group,and there was nosignificant difference (x2 =1.76,P >0.05).The incidence of comphcations in interlocking nails group was higher than that in dynamic compression plate group[25%(2H9) vs.12.9%(9H0)],and there was significant difference(P < 0.05).Condmions Interlocking nails have better biomechanical features with high union rate,low infective rate and good recovery of joint function.The clinical result of interlocking nails for the tibial shaft fractures is also better than that of dynamic compression plate.
2.A comparative study of explicit and implicit group-bias of off-farm workers

Objective To investigate the way off-farm workers show bias on their own group ,rural residents and urban citizens .Method The participants were 78 off-farm workers from 18 to 25 years of age ;The scales and the modified affective priming paradigm were adopted to measure their explicit and implicit attitudes respectively on these groups through recording the participants' reaction on computer .Results (1)The order of three groups sorted by implicit scores in condition of group-relevant words as primes from high to low was urban citizens ,off-farm workers ,rural residents(0.169±0.217,-0.016±0.116,-1.209±1.686 respectively),but the order of explicit scores were reverse(0.7920±1.461,0.4469±1.514,07920±1.461 respectively),and both the difference between the implicit scores and the difference between explicit scores were very significant(between the implicit scores of group bias on off-farm workers and urban citizens:t=7.165,P<0.01,and on rural residents:t=6.038,P<0.01;between the explicit scores of group bias on off-farm workers and rural residents:t=9.666,P<0.01,and on off-farm workers and urban citizens:t=7.828,P<0.01).(2)The scale scores of group identify ,collective self-esteem and self-esteem were significantly related to the scale scores of group .bias on the off-farm workers and rural residents but not other scores(P<0.01,P<0.01,P<0.05 respectively).Conclusion Although off-farm workers evaluated their own group and rural residents more positively than urban citizens in the obviously way ,they accepted the superiority of urban citizens .implicitly ,and the explicit group-bias was relative to self-esteem ,collective self-esteem and group identify.
3.Dengue Type Ⅰ Virus Isolated in Guangzhou Detected by Using TaqMan MGB Real-time PCR

OBJECTIVE To detect serotype of dengue virus in sera from patients with fever.METHODS A pair of degenerated primers and one MGB probe were designed targeting the conserved region at the E gene of dengue type 1 virus.TaqMan MGB real-time PCR assay was developed with plasmid including E gene of dengue type 1 virus as standard sample.The sera of 10 patients with fever were used to extract RNA,and convert into cDNA.Then cDNA were detected by TaqMan MGB real-time PCR assay and the amplified products were analyzed at the same time.RESULTS The sera of 9 patients from 10 samples were observed to generate a fluorescent signal,and about 100 bp fragment was obtained simultaneously.CONCLUSIONS Dengue fever on 2006 in Guangzhou is caused by the dengue type 1 virus.TaqMan MGB real-time PCR assay is rapid and sensitive to detect dengue virus infections.
4.Identification of difference of gene expression in the visceral adipose tissue of the lean type 2 diabetic rats

Objective To investigate the difference of gene expression in the visceral adipose tissue of the lean type 2 diabetic rats. Methods The difference bands were generated by the representational difference analysis ( cDNA RDA) ; the final difference products was ligated into the pUC-18 vector, subcloned and sequenced, and the obtained expressed sequence tags ( ESTs) were given bioinformatics analysis, and then the expression level of known gene was verified by semi-quantitative RT-PCR preliminarily. Results Six novel ESTs and 1 known gene [ rat lipoprotein lipase ( LPL) gene) were obtained in visceral adipose tissue of rat. One of the ESTs was partially similar to mouse musculus ERA-like GTPase (Era) gene. LPL gene was up-regulated in the visceral adipose tissue of the lean type 2 diabetic rats and those fed with fat-enriched food. Conclusion Six novel ESTs and 1 known gene in high expression are obtained from visceral adipose tissue of rat. LPL gene among them is up-regulated and may be related to the high fat and high calorie diet, hyperlipidemia, insulin resistance and molecular pathogenesis of the lean type 2 diabetic rat.

Using the technique of fluorescent-labled mRNA differential display, new apoptosis related gene 2ass-bnip3 of type 2 diabetic cardiomyopathy was found, the sequence of 1594 bp with coding 187 amino acids was obtained by the full-length clone, and its structural and functional predictions were performed. 2ass-bnip3 may play an important role in the development of diabetic cardiomyopathy via a regulatory pathway of calcium regulation and apoptosis.

Betacellulin (BTC) is one of "islets regeneration factors" which received more and more attention these years. BTCe is the C-terminal 50-residue region of mature BTC protein and can bind with erbB-1?erbB-4 receptor. It has the same mitogenic activity on cells as the whole section. BTC and BTCe can improve the level of glucose-stimulated insulin secretion (GSIS) during islets culture in vitro though they had no effect on the acute insulin secretion. BTCe also effectively ameliorated the hyperglycemia of STZ-induced diabetic rats by a single plasmid injection into muscle of rats. It is supposed that BTCe promote the proliferation of PDX-1 positive cells or repair some signal transduction pathway. Perhaps the latter is more important.

Objective To analyze the clinical indications,efficacy and safety of Chinese minimally invasive percutaneous nephrolithotomy (MPCNL) in treating upper urinary calculi based on our experience.Methods From June 1992 to September 2010,a total of 10,452 patients (6060 males and 4392 females)with a mean age of (47.6 ± 13.7) years (7 months-93 years) received MPCNL in our center.The mean stone burden was (777.4 ± 740.3) mm2 (20 - 4 080 mm2 ).The data of stone burden,operative techniques,operating time,stone-free rate,major complication,hospital stay and stone composition were investigated. Results Of the 10 452 cases,11 801 procedures were performed on 10 876 (5493 left and 5383right) renal units,including 10 102 first stage procedures,1604 secondary procedures,86 third procedures and 9 fourth procedures.There were 11 830 tracts established,including 373 (3.15% ) tracts of 14 F,7867 (66.50%) tracts of 16 F and 3590 (30.35%) tracts of 18 F.There were 1207 (10.20%),9174(77.55%) and 1449 (12.25%) punctures located in upper,middle and lower pole,respectively.956(8.79%) renal units were managed with multiple tracts,which including 2 tracts in 846 (7.78%) units,3tracts in 85 (0.78%) units,4 tracts in 18 (0.17%) units and 5 tracts in 7 (0.06%) units.Pneumatic lithotripsy was used in 8563 (72.56%) procedures,Holmium:YAG laser lithotripsy was used in 2981(25.26%) procedures and Pneumatic lithotripsy + Holmium: YAG laser lithotripsy was used in 257(2.18%) procedures.762 (7.29%) cases needed ESWL to clean the stone after MPCNL.The average operating time was ( 101.3 ± 44.2) min ( 10 -240 min).The stone-free rate of MPCNL was 89.9%,which increased to 93% by adjunctive ESWL.And the mean hospital stay was ( 13.2 ± 6.4) days (2 - 72 days).The major complications happened on 321 (3.07%) cases,including 294 (2.81% ) cases of blood transfusion,12 (0.11% ) cases of sepsis,2 (0.02%) cases of renal abscess,9 (0.09%) cases of pleura injury,2 (0.02%) cases of colon injury and 2 (0.02%) cases of death.53 (0.51%) cases needed selective renal arterial embolization to achieve hemostasis.The main stone compositions were analyzed in 4345 cases.Calcium oxalate,calcium phosphate,magnesium ammonium phosphate,uric acid,ammonium urate,carbapatite and cystin were 91.74%,90.33%,14.91%,17.77%,4.83%,8.47% and 0.51%,respectively. Conclusions MPCNL is an effective and safe treatment option for all kinds of upper urinary calculi in patients at all ages with a high stone free rate and low major complication rate.
8.Identification of variations of gene expression of visceral adipose and renal tissue in type 2 diabetic rats using cDNA representational difference analysis.

OBJECTIVESTo identify differences in gene expression in renal and visceral adipose tissue in type 2 diabetic rats using cDNA representational difference analysis (RDA) and to explore the molecular pathogenesis of type 2 diabetes and its chronic vascular complications.
METHODSA rat model of type 2 diabetes was generated by administration of a high fat and calorie diet combined with a low dose of streptozocin (STZ) injected into the tail vein. The difference bands were generated by cDNA representational difference analysis (cDNA RDA). The final difference products were ligated into the pUC-18 vector and sequenced. A bioformatics analysis was performed on the obtained expressed sequence tags (ESTs), and then the expression levels of known and novel genes were verified by semi-quantitative reverse transcription-PCR (RT-PCR). At the same time, full-length cDNA of a novel gene was cloned in silico.
RESULTSThe type 2 diabetic rats in this experiment experienced hyperglycemia, lipidemia, lower insulin sensitivity and normal body weight. We obtained 9 novel ESTs and 2 novel genes from renal tissue of rats and 6 novel ESTs and 1 known gene, the rat lipoprotein lipase (LPL) gene from their visceral adipose tissue. The 2 novel genes (RS91 and RS2) from the renal tissue were both very similar to serine (or cysteine) proteinase inhibitor, clade F and eukaryotic translation initiation factor 3 and subunit 5 (EIF-3 epsilon). The expression of both novel genes and the LPL gene were upregulated in renal and visceral adipose tissue of type 2 diabetic and fat-enriched rats. Full-length cDNA of the novel gene RS91 was cloned in silico.
CONCLUSIONS(1) The rat model of type 2 diabetes generated in this study was ideal because the disease in the animals closely mimicked type 2 diabetic patients. (2) cDNA RDA is a flexible, inexpensive, more accurate, sensitive and highly effective technique for identifying differences in gene expression. (3) Six novel ESTs and 1 known gene were obtained from rat visceral adipose tissue. The LPL gene was upregulated in adipose tissue of type 2 diabetic and fat-enriched rats, a result possibly related to the diabetic animals' high fat and calorie diet, lipidemia, insulin resistance (RI) and molecular pathogenesis. (4) Nine novel ESTs and 2 novel genes were obtained from the renal tissue of rat. We believe the 2 novel genes to be the serine proteinase inhibitors clade F and EIF-3 epsilon in rats. The upregulation of the 2 novel genes in renal tissue of type 2 diabetic rats and may have been related to their renal impairment.

9.Expression and identification of type 1 diabetes associated autoantigen IA-2.

OBJECTIVESTo obtain prokaryotic expressed IA-2 recombinant protein and to identify its immunological activity.
METHODSThe complimentary DNA (cDNA) coding for the intracytoplasmic part of IA-2 (IA-2ic) was amplified from human fetal brain RNA, and was subcloned into the PinPoint Xa-1 T vector to construct recombinant expression plasmid, and was then expressed in E. coli JM109 cells as a fusion protein with a biotinylated peptide sequence at the aminoterminus. The biotinylated fusion protein was then purified by affinity chromatography and was subsequently dialyzed. Finally, its immunogenicity was evaluated by enzyme linked immunosorbent assay (ELISA).
RESULTSThe purified IA-2ic fusion protein resolved on sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) as a single Coomassie brilliant blue stained band with a molecular weight of 59 kDa and its immunogenicity was confirmed by ELISA.
CONCLUSIONSE. coli expressed IA-2ic fusion protein has immunological activity. It can be used for detection of IA-2 autoantibodies (IA-2A) and for further studies on type 1 diabetes in future.

10.Clinical efficacy and safety of modified proceedure for prolapse and hemorrhoids combined with partial internal anal sphincterotomy in the treatment of annular mixed hemorrhoids

Objective To explore the clinical efficacy and safety of modified PPH combined with partial internal anal sphincterotomy in the treatment of circular mixed hemorrhoids.Methods Patients with annular mixed hemorrhoids were divided into two groups by a completely randomized controlled method.54 patients in the experimental group were treated with modified PPH combined with partial internal anal sphincterotomy,while 51 patients in the control group were treated with conventional PPH.The postoperative indicators,perioperative and long-term complication rates of the two groups were compared,and the clinical efficacy and safety were observed.Results The operation time in the experimental group was(48.35±4.37)minutes,which was higher than that in the control group(36.42 ±6.21)minutes(P<0.05).The incidence of postoperative anastomotic stenosis in the experimental group was 1.9%,lower than 15.6% in the control group(P<0.05).Anal pain,urinary retention,first defecation time,long-term anal distention,the experimental group was significantly better than the control group,the difference was statistically significant(P<0.05);The hospitalization time in the experimental group(4.8±0.62)days was not significantly different from that in the control group(5.1±0.54)days(P>0.05).The amount of intraoperative bleeding and anastomotic bleeding in the experimental group[(17.28±2.22)ml,3.7%]were not significantly different from those in the control group[(16.75± 2.13)ml,3.9%](P>0.05).Conclusion Compared with conventional PPH,the modified PPH combined with partial internal anal sphincterotomy slightly increases the operation time,but does not increase the risk of anastomotic bleeding,the incidence of rectal fistula,the amount of surgical bleeding,and the length of hospital stay.It can significantly improve postoperative anal pain,urinary retention,long-term distention symptoms,shorten the time of first defecation,ease the difficulty of defecation,and significantly reduce postoperative anastomotic stenosis,The long-term efficacy and safety are good.
