1.Clinical observation of voriconazole prevention and cure on invasive fungal infection in patients with hematological malignancy

ObjectiveTo study the therapeutic effect and safety of voriconazole prevention and cure on invasive fungal infection (IFI) in patients with hematological malignancy.MethodsSeventy-six patients with hematological malignancy were divided into observation group (39 cases) and control group (37 cases) by random digits table.The control group was given voriconazole of intravenous drip.The observation group was given voriconazole of intravenous drip sequence oral administration.The rates of occur IFI and adverse effect were compared between the two groups.ResultsIn observation group and control group,the confirmed diagnose rates were 17.9%(7/39) and 18.9%(7/37),clinical diagnose rates were 23.1%(9/39)and 27.0% (10/37),para-diagnose rates were 59.0%(23/39) and 54.1%(20/37),there was no significant difference between the two groups(P> 0.05).In observation group and control group,the paropsia rates were 12.8%(5/39) and 21.6%(8/37),transaminase elevated rates were 12.8%(5/39) and 43.2%( 16/37),there were significant differences between the two groups(P < 0.05 ).ConclusionThe voriconazole of intravenous drip sequence oral administration and intravenous drip has a nice effect to IFI,and intravenous drip sequence oral administration could reduce the adverse effect.
2.Ultrastructural Study of Human Epidermal Keratinocytes Cultured in Low Calcium Medium

A low calcium medium developed for epidermal keratinocytes were prepared according to the MCDB 153 modified formula and used in human epidermal keratinocyte culture compared with DMEM culture system. The observation by contrast microscopy and electron microscopy showed that in the low calcium medium keratinocytes grew as a monolayer of high proliferation and had many characteristics of basal cells, with a more rounded shape and large intercellular spaces. Increasing the calcium ion concentration in the medium or changing the other culture conditions the cells in these cultures could be induced stratification and terminal differentiation. The results suggest that the growth, proliferation and differentiation of cultured human epidermal keratinocytes can be controlled and regulated someway.
3.Evaluation of hCDC4 expression and its correlation with the clinicopathological significance in oral squamous cell carcinoma

Objective To investigate human cell division control protein 4(hCDC4) expression and its correlation with the clini‐copathological features in oral squamous cell carcinoma(OSCC) .Methods We freshly collected 52 samples of surgically resected OSCC tissues and 12 samples of normal tissues .hCDC4 expression in the samples was detected by immunohistochemical staining . The correlation between hCDC4 protein expression and clinicopathological feature was analysed .OSCC cells and Tca8113 were transfected with hCDC4‐siRNA ,cell proliferation and c‐Myc and Cyclin E protein expression were determined by using M TT and Western blot .Results The hCDC4 protein expression in normal tissues was significantly up‐regulated compared to those in OSCC tissues (83 .3% vs .25 .0% ,P < 0 .05) .Clinicopathological analysis revealed that reduced hCDC4 expression was associated with large tumor size ( ≥ 4 cm) and high clinic stage ( Ⅲ + Ⅳ ) (P< 0 .05) .hCDC4 knockdown by siRNA led to increased cell prolifera‐tion and c‐Myc and Cyclin E protein accumulation in Tca8113 cells .Conclusion Loss of hCDC4 may promote tumor progression by resulting in c‐Myc and Cyclin E protein accumulation in OSCC .
4.Analysis of clinical and pathological characteristics of breast carcinoma in elderly female patients

Objective To analyze the clinical and pathological characteristics of breast carcinoma in elderly female patients and to offer the standard clinical diagnosis and treatment for breast carcinoma.Methods A total of 178 patients admitted to Breast Center of Beijing Hospital from January to December 2011 were enrolled in this retrospective study.Patients were divided into non-elderly group and elderly group.Clinical features,imaging diagnosis,operation patterns and pathological diagnosis were recorded.The data were analyzed by using SAS 9.1 software.Results The patients diagnosed as breast carcinomas in BI-RAD 4-5 categories by mammography were much more in the elderly group than in the non-elderly group (82.1% vs.63.0%,P<0.05).The diagnostic accuracy rates of ultrasonography and mammography in the elderly group were comparable(P> 0.05),while in the non-elderly group,the diagnostic accuracy rate of ultrasonography was higher than that of mammography (P<0.05).The incidence of concurrent diseases was higher in the elderly group than in the non-elderly group (P<0.05).The patients undergoing breast resection operation were much more in the elderly group than in the non-elderly group (14 cases vs.3 cases,P<0.05).The pathological diagnosis analysis showed that there were no significant differences in the number of ER-positive cells and PR-positive cells,tumor size,histological grade,pathological stage between the two groups (all P>0.05).The incidence of lymph node metastasis was lower in the elderly group than in the non-elderly group (P<0.05).The percentage of Ki-67 positive cells was lower in the elderly group than in the non-elderly group (P<0.05).The percentage of CerbB-2 strongly positive (+++) cells was lower in the elderly group than in the non-elderly group (5.1 % vs.24%,P<0.05).The invasive ductal carcinoma was the major histological type of breast cancer in both two groups,but there were more mixed type and other type carcinomas in the elderly group than in the non-elderly group (P<0.05).Conclusions Ultrasonograpy and mammography have better sensitivity for breast cancer in elderly female patients.The surgical protocols are often influenced because more concurrent diseases have occurred in the elderly.There are significant differences in some prognostic factors between the two groups,which may imply a better prognosis in elderly patients.
5.Absorption mechanism of aesculin across Caco-2 monolayer mode

AIM:To research the absorption mechanism of aesculin across Caco-2 monolayer model.METHODS:The Caco-2 cell monolayers drug transport model was assigned to study the double transport mechanism of aesculin to explore the absorption of aesculin according as time and drug concentration determined through HPLC and the P_ app was calcalated.RESULTS:In the Caco-2 monolayer model,the transport of aesculin form Apical to Basolateral was similar to the transport form basolateral to apical.CONCLUSION:The main mechanism of the aesculin intestinal absorption in the Caco-2 monolayer model is passive transference.

7.Evaluation of ligamentum teres hepatis with multi-slice spiral CT:an initial experience

Objective To recognize the features of the ligamentum teres hepatic(LTH) in multi-slice spiral CT,and to preliminarily explore its clinical value.Methods A total of 130 patients without liver disease underwent abdominal scanning with a 16-slice spiral CT scanner.LTHs,as focused points,were observed by using postprocessing techniques such as multi-planar reformation(MPR),slab maximum intensity projection(slab-MIP) and slab volume rendering(slab-VR).Results The LTH was detected by multi-slice spiral CT in 90.8%(118/130).On these oblique-sagittal MPR,slab-MIP and slab-VR images,LTH presentedas slightly high density smooth cord-like structure in accordance with its anatomical morphology and position.The mean diameter of fissure portion of normal LTH was(5.44?0.96) mm(range 3.2-7.2 mm) measured at its middle region,and that of free portion of normal LTH was(3.43?0.91) mm(range 1.6-6.0 mm).Conclusions Multi-slice spiral CT demonstrates the LTH clearly,and has a potentialto evaluate the LTH before abdominal operation in which the LTH is to be used as an autogenous repairmaterial.
8.Therapeutic analysis of unilateral adrenalectomy for Cushing syndrome in patients with adrenocorticotropic hormone independent bilateral macronodular adrenal hyperplasia

Objective To investigate the therapeutic outcomes of unilateral adrenalectomy for Cushing syndrome in patients with adrenocorticotropic hormone (ACTH) independent bilateral macronodular adrenal hyperplasia.Methods 22 patients diagnosed with Cushing syndrome caused by ACTH independent bilateral macronodular adrenal hyperplasia from January 2005 to December 2015 were retrospectively concluded.There are 17 male patients and 5 female patients with the median age of 46.5 years.All patients were presented with Cushing syndrome such as moon face, buffalo hump and other typical symptoms of Cushing syndrome.The laboratory tests showed disappearance of cortisol rhythm, elevated 24-hour urinary free cortisol, suppressed or normal ACTH and unsuppressed low dose dexamethasone suppressing test.CT scan showed bilateral macronodular adrenal hyperplasia in all patients.Results All the patients received unilateral adrenalectomy.Pathological results showed adrenal cortex nodular hyperplasia.16 patients had reexamination in the endocrine department.The median follow-up time was 26 (17-118)months for these 16 patients.In the follow-up between 3 to 9 months after surgery, laboratory test showed serum and urinary cortisol level returned to normal range and Cushing syndrome also disappeared in these 16 patients.CT scan showed no recurrence.And no adrenal insufficiency occurred.One patient had recurrence one year after surgery and two patients turned better after surgery but had recurrence at 3 years and 10 years after surgery.Two patients received contralateral adrenalectomy and cortisol hormone replacement therapy after surgery.Another patient with recurrence refused second surgery.No recurrence was observed in other patients.Four patients were followed up by telephone, and all recovered well after surgery.18 patients had hypertension before surgery and their blood pressure significantly decreased after surgery.Among them 13 cases blood pressure returned to normal range.5 patients had elevated blood glucose before surgery, and 3 patients' blood glucose returned to normal range.Conclusions Unilateral adrenalectomy for Cushing syndrome in patients with ACTH independent bilateral macronodular adrenal hyperplasia is safe and effective.
9.Comparison of hPepT1 transfected MDCK cells to hPepT1 transfected HeLa cells

Aim To screen a more suitable transfection recep-tor,and improve the efficiency of constructing cell lines highly expressing human peptide transporters 1 (hPepT1 ).Methods The recombinant plasmid pcDNA3.1 (+)-hPepT1 was transfect-ed into MDCK cells and HeLa cells by LipofectamineTM 2000 transfection reagent,respectively.The monoclonal cells were se-lected and cultured.Expression of hPepT1 mRNA and protein were determined by qRT-PCR and Western blot,respectively. The uptake capacity of Glysar in transfected cells was examined. Results Compared with wild type cells,the expression of hPepT1 and the uptake of Glysar in transfected MDCK cells and HeLa cells significantly increased (P <0.05).Although the up-take of Glysar in HeLa cells was higher than that of MDCK cells,on the contrary,the expression of hPepT1 and the uptake of Glysar in MDCK-hPepT1 cells was higher than that of HeLa-hPepT1 cells.Conclusion MDCK cells may serve as a more suitable transfected receptor for the construction of a cellular model with high expression of hPepT1 ,which would make the construction of a cell model highly expressing hPepT1 more effi-cient.
10.Endoscopic radial incision in patients with benign stricture of esophageal anastomosis

Objective To evaluate clinical efficacy and safety of endoscopic radial incision( ERI) for benign stricture of esophageal anastomosis. Methods Clinical data of 17 patients with benign stricture of e?sophageal anastomosis undergoing ERI from October 2013 to September 2014 were retrospectively studied. Im?provement of clinical symptom and treatment?related complication or discomfort were intensively analysed. Re?sults All 17 patients successfully received ERI procedures, and the mean operating time was 10 minutes with a mean of 4 incisions. Obvious bleeding and mis?cut of normal mucosa occurred in 1 case, and this patient was cured by endoscopic hemostasis, gastrointestinal decompression and administration of antibiotics. Heartburn oc?curred in 5 patients and disappeared spontaneously without other complications or discomfort. Dysphagia score decreased from 3?11 to 0?90 in the second day after ERI(P<0?01).The mean follow?up time was 15?5 months ( range 9?20 months) . The dysphagia score showed no significant difference between the follow?up period and the second day after ERI ( P>0?05 ) . Conclusion ERI is simple, safe and effective for treating benign stricture of esophageal anastomosis.
