Objective To retrospectively analysis and compareabout Bryan artificial cervical disc arthroplasty with ante-rior cervical decompression and fusion (ACDF) on the clinical efficacy for“Skip”cervical spondylosis. Methods From February 2002 to May 2012, 49 cases were treated with Bryan artificial cervical disc arthroplasty (artificial cervical disc replacement surgery group, 18 cases) or anterior cervical decompression and fusion (ACDF group, 31 cases), 29 males and 20 females. Each case was evaluated at the moment of preoperatively, 3 months, 6 and 12 months and last follow-up after surgery by the Japanese Orthopedic Association (JOA), Neck Disability Index (NDI), Visual Analog Scale (VAS), Cervical sagittal curvature, the total cervical spine range of motion(ROM),middle segments of motion. MRI was also used to assess to adjacent segment disc degeneration, spinal cord compression and signal change situation. Results All patients were followed up for more than 24 months. The score of the JOA, NDI, VAS in the two groups of patients improved significantly after surgery than before surgery. In addition, the VAS score in last follow-up were significantly different between the two groups, but other index each time in the two groups showed no significant difference. In last follow-up, the result of artificial cervical disc arthroplasty group were better than ACDF group on the incidence of axial symptoms, the total cervical spine range of motion (ROM) and middle segments of motion. The incidence of axial symptoms in artificial cervical disc arthroplasty group were 11.1%,ACDF group were 45.2%. ROM in arti-ficial cervical disc arthroplasty group were 35.5°±5.9°,ACDF group were 24.5°±6.2°. Middle segments of motion in artificial cer-vical disc arthroplasty group were 7.3°±1.4°,ACDF group were 10.1°±1.6°. The above comparison of the datas were statistically different. There are two cases of adjacent segment degeneration in ACDF group without need to surgery. Conclusion Bryan artifi-cial cervical disc replacement surgery effectively retained the overall motion of the cervical spine, reduced the motion of middle segments, thus avoiding adjacent segment degeneration and the incidence of postoperative axial symptoms.

Objective To compare the effect of 8-week high-intensity interval training(HIIT)and moderate-intensity continuous training(MICT)on body composition and cardiovascular risk biomarkers in middle-aged and elderly obese women,so as to provide theoretical and practical basis for future in-tervention.Methods Thirty-four middle-aged and elderly obese women were randomly divided into an HIIT group(n=13),a MICT group(n=12)and a CON group(n=9).All subjects underwent a laboratory VO2max test before intervention to determine their corresponding exercise load. Then,HIIT group rode a power bicycle for 2 minutes at 80%~85%VO2max intensity and 1 minute at 30%VO2max intensity,re-peated 10 times, while MICT group rode at 65%VO2max intensity for 30 minutes,3 times a week for 8 weeks.CON group did exercise. Before and after intervention, all groups were measured their body weight, body fat, muscle mass, body fat rate,as well as the serum levels of high-sensitivity c-reac-tive protein(hs-CRP),endothelin-1(ET-1),lipoprotein(a)[Lp(a)]and homocysteine(Hcy). Results Af-ter intervention, there was a significant decrease in the body fat and body fat rate(P<0.05)and a sig-nificant increase in muscle mass(P<0.05)in the HIIT group, while there were no significant changes in the above three values in the MICT group(P>0.05).Moreover,there was a significant decrease in hs-CRP and ET-1 of the HIIT group(P<0.01),and a significant increase in Hcy and Lp(a)of the CON group(P<0.01).Meanwhile,the decrease in hs-CRP(P<0.05),Lp(a)(P<0.05)and Hcy(P<0.01)of the HIIT group,as well as that in Hcy(P<0.05)of the MICT group were significantly higher than the CON group. What's more,no significant differences were observed between HIIT and MICT groups in their effects(P>0.05).Conclusions HIIT is superior to MICT in improving body composition and reducing serum levels of hs-CRP and ET-1 in middle-aged and elderly obese women. Moreover,it has a positive effect on improving chronic inflammatory state and vascular endothelial function,to a certain extent, and lowering their risk of suffering from cardiovascular diseases.

Objective To summarize the nursing experience of delayed closure of chest with acute renal injury after switch operation and underwent peritoneal dialysis in neonates and to improve the therapeutic effect. Methods To summarize the curative effects and perioperative nursing experience of one case of the complete transposition of great arteries with intact interventricular septum neonate who underwent delayed closure of chest with acute renal injury and peritoneal dialysis after Switch operation under general anesthesia and extracorporeal circulation in November 2017 in our department. Results The child was postponed to close the chest after surgery. Low cardiac output syndrome and acute renal function injury occurred 1 hour after operation. Through monitoring hemodynamic indexes during ICU, the child recovered after timely treatment of low cardiac output syndrome, maintaining stabilization of circulation, diuresis, peritoneal dialysis, keeping water, electrolyte and acid-base balance, nursing care for delayed closure of chest and other related treatment. Postoperative assisted mechanical ventilation time was 168 hours, postoperative ICU hospitalization time was 12 days, and postoperative total hospitalization time was 19 days. Conclusion The infants who have complete transposition of the great arteries and the intact interventricular septum after Switch operation have many complications and rapid changes in the state of illness. Rigorous and meticulous nursing plays a key role in reducing the postoperative complications and improving the achievement ratio of the operation.

Objective To understand the current postoperative application situation of severity degree evaluation for children patients with congenital heart diseases (CHD) in China and the confronting problems,in order to offer reference to improve clinical severity prediction and nursing ability. Methods Cochrane Library, PubMed,EMbase,CINAHL,CBMdisc,CNKI,Wanfang database and VIP database were searched. Languages were not limited. The literature which was published from 2000 to 2016 was included. The literature which was related to postoperative severity degree evaluation for the children patients with CHD was included. The quality of included literature was critically evaluated and the data were extracted with QUADAS evaluation system by three reviewers. They also completed the literature analysis. Results Twelve articles were included. There were only 2 articles published in nursing journals. Pervasive evaluation methods were mainly applied in the people with diseases of respiratory system. There was only 1 article which studied CHD. The application effects of evaluation methods were different. The application effect of pervasive evaluation methods in CHD was limited. Two studies showed that the application effect of the specific score was good, but there was no multicenter or large sample verification. Conclusions The application of postoperative severity degree evaluation methods for children patients with CHD in China is limited. The recognition degree of nurses is not high. Relative verification research is not enough. Further studies of the specific score are necessary. We suggest that the reasonable application of evaluation methods should be promoted based on verifying the application effects of evaluation methods and in combination with the training of nurses, which guarantees the safety of children patients.
