1.Utilization of Antihyperlipidemics in 28 Hospitals of Chongqing from 2004 to 2007

OBJECTIVE:To evaluate the status and tendency of the application of antihyperlipidemics drugs in Chongqing area. METHODS: The use of antihyperlipidemics drugs in 28 hospitals of Chongqing from 2004 to 2007 was analyzed statistically in respect of the consumption sum, DDDs and DDC. RESULTS: The mean annual incremental ratio of the consumption sum of antihyperlipidemics drugs reached over 33.80%. Simvastatin has been dominated the first place over the 4 years in terms of consumption sum and DDDs; the consumption sum of atorvastatin has been on the rise but that of Gemfibrozil decreased year on year with mean annual decrement of 60%. CONCLUSION: Statins assumed a dominant position among all the antihyperlipidemics drugs used in Chongqing area and its use has great potential.
2.The role of Survivin and VEGF in angiogenesis of colorectal tumor

Objective To identify the role of survivin and VEGF in angiogenesis of colorectal tumor by investigating tissue expression of survivin, VEGF and CD34 in human colorectal tumors. Methods Immunohistochemical staining for the paraffin sections by using the polyclonal antibodies of survivin, monoclonal antibodies of VEGF and CD34, was performed by the standard avidin-biotin-peroxidase technique. The percentage of positive cell and quantitative analysis were made by image analysis system, which also helped to calculate the amount of MVD. Results The immunoreactivity of survivin significantly increased in the transition from adenoma with low dysplasia to adenoma with high dysplasia (P <0.01). Similar changes in protein expression were observed for VEGF and MVD. The expression of survivin was closely related with that of VEGF. Both were positively correlated with MVD. Conclusion Survivin promotes angiogenesis of colorectal tumor together with VEGF. VEGF may be the potential causes of reexpression of survivin during colorectal tumorigenesis.
3.Effects of peroxisome proliferator-activated receptor gamma agonist pioglitazone on neurocyte apoptosis and expression of ICAM-1 after traumatic brain injury

Objective To investigate the effects of peroxisome proliferator-activated receptor gamma (PPARγ) agonist pioglitazone on delayed neuronal death,apoptosis of neurocytes,and expression of intercellular adhesion molecule-1 ( ICAM-1 ) following traumatic brain injury (TBI) in rats.MethodsThirty-six Sprague-Dawley rats were randomized into sham injury group,control group and pioglitazone treatment group,with 12 rats in each group.TBI model was established by modified Feeney method.Treatment grouP received intragastric administration of pioglitazone at a dosage of 10 mg/kg,and the sham injury group and the control group were lavaged with isometric 0.2% dimethyl sulphoxide.Paraffin sections of brain tissues collected at 48 hours after TBI were employed to observe delayed neuronal death,apoptosis of neurocytes and expression of ICAM-1 by Nissl staining,TUNEL staining and immunochemistry respectively.Results( 1 ) Cell loss rate of Nissl body in the treatment group [ ( 38.59 ± 1.97 ) % ]was significantly lower than that of the control group [ (51.25 ± 4.01 ) % ] ( P < 0.05 ),but was higher than that of the sham injury group [ (8.65 ± 1.23 ) % ] ( P < 0.01 ).(2) The number of apoptotic neurocytes of the treatment group (31.67 ± 4.76) was significantly lower than that of the control group (45.33 ± 4.68 ) ( P < 0.05),but was higher than that of the sham injury group ( 16.83 ± 2.04 ) ( P < 0.01 ).(3) The mean optical degree of ICAM-1 positive expression of the treatment group (0.26 ± 0.04) was significantly lower than that of the control group (0.31 ± 0.04) ( P < 0.05 ),but was higher than that of the sham injury group (0.10 ± 0.02 ) ( P < 0.01 ).ConclusionsThe PPARγagonist pioglitazone can reduce the apoptosis of neurocytes and protect neurons after TBI.Meanwhile,its suppression of ICAM-1 expression is probably a mechanism of the suppression of inflammatory reaction and neural protection.
4.Location and fine structure of melanocytes in human fetal scalp hair follicles

Objective To investigate the location,fine structure of melanocytes in human fetal scalp hair follicles.Methods The scalp with hair follicles was obtained from a dead fetus of 6 months of age,and divided into two parts.One part was embedded in paraffin,tissue sections were prepared with a width of 7 μm and stained with NKI/beteb,monoclonal antibodies to HMB-45,tyrosinase and tyrosinase-related protein 1(TRP1),respectively.The other part with hair follicles was treated with collagenase type Ⅱ 0.1 g/L and trypsin,then,cell suspension was collected and cultured.After 14-day culture,follicle melanocyte cells (FMC)were separated from keratinocytes by differential trypsinization,and fibroblasts were removed with geneticin.Following three times of pure passage,FMC were seeded and fixed on mica for scanning electron microscopy(SEM)and atomic force microscope(AFM)scanning.Results Histopathological examination showed that NKI/beteb positive cells located at the outer root sheath of human hair follicles,and these cells stained negatively for HMB-45,tyrosinase and TRP1 antibodies.However,in the hair bulb,lots of cells expressed HMB-45,tyrosinase and TRP1 antigens.After fibroblasts and keratinocytes were removed,two kinds of melanocytes remained in the culture:one was small in number and showed abundant melanin,which was lost after subsequent passage;the othgr was large in number and had no melanin initially,but proliferated very rapidly.After three passages,almost all the melanocytes were positive for NKI/beteb.As SEM and AFM showed,most cultured melanocytes appeared fusiform with two(rarely three)dendrites,and the cell body was round or oval with a few melanosomes scattered in but no clear secondary branches on the dendrites.Conclusions The melanocytes in outer root sheath of hair follicles from the fetal scalp are presumed as melanocyte stem cells or their progenies.In vitro,these cells proliferate very rapidly during early phases,but the morphology and function of them still remain immature,which is unfavorable for melanosome transport.

6.Application of Liposomal Doxorubicin in the Treatment of Refractory Non-Hodgkin’s Lymphoma

Objective To observe the efficacy of liposomal doxorubicin combined with cyclophosphamide, vincristine and prednisone in the treatment of refractory Non-Hodgkin’s lymphoma.Methods Liposomal doxorubicin (40mg/m2) was given by intravenous drip in the 1st day. Cyclophosphamide (750mg/m2) was given by intravenous injection in the 1st day. Vincristine (2mg) was given by intravenous injection in the 1st day. Prednisone (100mg/m2) was given orally from the 1st to the 5th day. A cycle was repeated every 3 to 4 weeks. Every patient took at least 2 cycles of the regimen.Results A total of 13 patients were assessed in the group. Among them, 7 were completely release (53.8), 4 were partially release (30.67) and 2 remained the same (15.35). The B symptom of 7 patients in the 9 with that disappeared, and that of the other 2 patients was improved obviously. The most common adverse effects were slight gastrointestinal reactions and the grade Ⅲ bone marrow suppression in a few patients. Conclusion The regimen of liposomal doxorubicin combined with cyclophosphamide, vincristine and prednisone is effective in the treatment of refractory Non-Hodgkin’s lymphoma with tolerable toxicity. It may be a salvage chemotherapeutic regimen deserving further study.
7.Efficacy of laparoscopic surgery for rectal cancer

Objective To investigate the safety and clinical outcome of laparoscopic resection of rectal cancer. Methods The clinical data of 347 patients with rectal cancer who were admitted to the Shanxi Tumor Hospital from May 2004 to July 2008 were prospectively analyzed. Of all the patients, 343 met the inclusion criteria,and they were randomly allocated to laparoscope group (n = 169) and open group (n= 174). The diameter of the tumors, number of lymph node dissected, length of rectum resected, morbidity, the mean operation time, number of patients receiving blood transfusion, time to out-of-bed activity, first flatus, bowel movement and liquid diet were observed. All data were analyzed using the t test and chi-square test. The survival rate was calculated using the Kaplan-Meier method. Results The diameter of the tumors, number of lymph node dissected, length of rectum resected and number of patients receiving blood transfusion in the laparoscope group were (4.3 ± 1.3 ) cm, 7 ± 5,(19.1±2.2)cm and 4, and they were (4.2±1.3)cm, 7 ±5, (19.0±2.3)cm and 8 in the open group,respectively, with no significant difference between the two groups ( t = 0. 629, - 0. 726, 0. 562, x2 = 1. 264,P >0.05). The mean operation time in the laparoscope group was 19 minutes longer than that in the open group (t = 7. 904, P < 0.05 ). The time to out-of-bed activity, first flatus, bowel movement and liquid diet in the laparoscope group were 0.6, 0.3, 0.3 and 0.6 days earlier than those in open group( t = - 6. 392, - 3.581, - 3. 802,- 3. 493, P < 0.05 ). There were no significant differences in postoperative infection, anastomotic leakage, intestinal obstruction and deep vein thrombosis between the two groups ( x2 = 0. 236, 0. 354, 0. 000, 0. 000, P >0.05). A total of 167 patients in the laparoscope group and 172 patients in the open group had been followed upuntil 1 may, 2010. The 1-, 2-year survival rates were 94.0% and 82.6% in the laparoscope group and 95.3% and 91.2% in the open group. There was no significant difference in the 2-year survival between the two groups (x2 =0.541, P >0.05). The survival time of the patients in the laparoscope group and open group were 55.9 and 57.9 months, respectively. Conclusions Laparoscopic surgery is safe and feasible for patients with rectal cancer, with quick recovery after the operation.
8.Investigation and Analysis of Medication Safety among Rural Residents in Yongzhou City of Hunan Province

OBJECTIVE:To provide reference for ensuring the medication safety of rural residents well.METHODS:A questionnaire survey was conducted among 162 rural residents of 27 townships in 11 counties (districts) from Yongzhou city of Hunan province.The survey data was analyzed statistically.RESULTS:A total of 5 670 questionnaires were issued and 4 376 valid questionnaires were recovered,with an effective recovery rate of 77.2%.73.6% residents did not considered the incompatibility among drugs when receiving two or more drugs;the proportion of increasing or decreasing drug dosage and changing drugs at will was in high level.Most of the residents who had common drugs in the family did not pay enough attention to the custody of the reserved drugs and lacked of basic knowledge;the proportion of residents who bought drugs mainly from pharmacies was the highest (50.7%);in addition,nearly 20% bought drugs mainly through the Intemet and other ways.More than half of the residents surveyed did not understand the drug instructions;after ADR occurred,more than half of the residents surveyed would stop or change their drugs voluntarily;more than 70% of the residents surveyed did not know or did not pay attention to characteristics and incompatibility of drugs.CONCLUSIONS:Because of the low level of education,the lack of common sense of safe drug use in local rural residents and unbalanced allocation of medical resources,the phenomenon of irrational drug use is widespread and the safety of medication is not ideal;there is a lot of room for improvement.
9.Potential categories of problematic mobile phone use and sleep quality in adolescents: the chain mediating role of interpersonal distress and depression

Objective:To explore relationships between problematic mobile phone use and interpersonal distress, depression and sleep quality using a combined variable- and person-centered approach.Methods:From December 2021 to January 2022, a total of 851 middle school students were measured by the mobile phone addiction index scale, the college students interpersonal comprehensive diagnostic scale, the center for epidemiologic studies depression scale, and the Chinese version of Pittsburg sleep quality index. Common method bias test, descriptive statistics, and correlation analysis were performed using SPSS 26.0 statistical software, and Mplus 8.8 software was used to construct structural equation models for latent profile analysis.Results:The score of problematic mobile phone use was(47.23±14.50), the score of interpersonal distress was(8.72±5.68), the score of depression was(14.80±10.28), the score of sleep quality was(5.51±2.78). Variable-centered analysis revealed that problematic mobile phone use had a significant direct effect on sleep quality (effect value=0.22, 95% CI=0.11-0.34), accounting for 40.74% of the total effect. The mediating effect of depression was found between problematic mobile phone use and sleep quality (effect value=0.08, 95% CI=0.03-0.14), accounting for 14.81% of the total effect. Interpersonal distress and depression had a chain mediating effect between problematic mobile phone use and sleep quality(effect value=0.22, 95% CI=0.16-0.30), accounting for 40.74%(0.22/0.54) of the total effect. Person-centered analysis revealed that problematic mobile phone use was divided into three potential profiles: mild use group(26.1%), moderate use group(54.7%) and severe use group(19.2%). Depression(moderate use group: effect size=0.05, 95% CI=0.02-0.10; severe use group: effect size=0.07, 95% CI=0.02-0.12) and interpersonal distress-depression(moderate use group: effect size=0.13, 95% CI=0.09-0.18; severe use group: effect size=0.21, 95% CI=0.15-0.28) had a significant relative mediating effect between problematic mobile phone use profiles and sleep quality. Conclusion:Adolescent problematic mobile phone use is heterogeneous, with both variable-centered and person-centered results indicating that depression and interpersonal distress-depression mediated the effects of adolescent problematic mobile phone use on sleep quality.
10.ROSIER scale is useful in an emergency medical service transfer protocol for acute stroke patients in primary care center: A southern China study

Objective: The aim of the study is to validate whether the Recognition Of Stroke In the Emergency
Room (ROSIER) scale can be used by general practitioners (GPs) in an emergency medical service
(EMS) protocol to transfer stroke patients from primary care center to advanced hospital with acute
stroke center. Methods: GPs prospectively performed the ROSIER scale and the Cincinnati Prehospital
Stroke Scale (CPSS) on suspected stroke patients as a transfer protocol. All patients were immediately
transferred to the Level-II hospital for further treatment. Results: 468 of the 512 suspected stroke
patients met the inclusion criteria in this study. The ROSIER scale showed a diagnostic sensitivity
of 83.13% (95% confidence intervals [CI] 79.74-86.52%) and specificity of 80.88% (95% CI 77.32-
84.44%). The CPSS showed a diagnostic sensitivity of 78.01% (95% CI 74.26-81.76%) and specificity
of 70.59% (95% CI 66.46-74.72%). The Kappa statistic value of the ROSIER scale and the CPSS
were 0.601 and 0.454, respectively. The area under the curve (AUC) of ROSIER scale was large than
the CPSS (AUC 0.855 vs. 0.791). However, the difference was not significantly different.
Conclusions: This study suggest that ROSIER and CPSS could be used in an EMS protocol to transfer
stroke patients from a primary care center to an advanced hospital offering thrombolysis service
