1.Clinical survey of splanchnicectomy for pain palliation with advanced pancreatic carcinoma

Objective To investigate the clinical effect of splanchnicectomy for pain palliation with advanced pancreatic carcinoma.Methods Altogether 27 cases were treated from January 2005 to February 2008,retrospective study was used to analyze the clinical data of them.All the patients had diverse extent pain,among these 16 cases who had underwent splanchnicectomy (therapy group),others were in control group.Results Pain extent was scored according to visual analogue scale (VAS),preoperative score of therapy group (6.06±1.93) scores,postoperative score 3 months(2.67±1.68) scores,6 months(2.69±1.75) scores,the discrepancy had statistical significance (P<0.01),preoperative score of control group (5.91±1.87) scores,postoperative score 3 months(5.11±2.03) scores,6 months(5.33±2.25) scores,the discrepancy had no statistical significance.Intergroups contrast,showing significant difference (P<0.05).Conclusion For the patients who request to deal with primary disease,splanchnicectomy is manipulated easy,short of complication,significantly relief of pain,and deserve to be spreaded.
2.Diagnosis and treatment of secondary sclerosing cholangitis after billary tract operation

Objective To investigate the diagnosis and treatment of secondary sclerosing cholangitis after billary tract operation.Methods The clinical clata of 7 cases with secondary sclerosing cholangitis after billary tract operation treated in our hospital from January 2000 to June 2008 were retrospectively analyzed.Results Recurrent episodes of bacterial cholangitis were present in all patients,and they all had history of biliary tract operation.After definite diagnosis,2 cases underwent operation and 5 cases underwent conservative treatment.Among the latter,1 case underwent endoscopic sphincterotomy and 1 case had liver transplantation.Of all the patients,2 cases recovered(2/7),2 cases died(2/7) and 3 cases are still undergoing treatment.Conclusions The prognosis of secondary sclerosing cholangitis after billary tract operation is relatively poor.The key measure is prevention,and early diagnosis together with rational treatment can have certain therapeutic effect.
3.Transplantation of en-bloc kidneys and single kidney from pediatric donors to pediatric recipients: 11 cases report

Objective To investigate the therapeutic principle of en-bloc kidney and single kidney transplantation from pediatric donors to pediatric recipients.Method A retrospective analysis of 11 pediatric kidney transplants into pediatric recipients was performed.The age of donors and recipients was 33 days to 48 months,and (9.1 ± 3.4) years (4.6 14.3 years) respectively.Result During the follow-up period of 1 to 22 months,the patient survival rate was 100%.Complications included delayed graft function in 1 case (managed by peritoneal dialysis),urine leak in 2 cases (treated by reoperation),hydronephrosis in 2 cases (treated by extracting ureteral catheter) and vascular thrombosis in 1 case.Due to thrombosis,one graft was lost.Of the remaining 10 recipients,all had excellent long-term function.At the last follow-up,their serum creatinine levels were 65.5 ±13.6 μmol/L (49-83μmol/L),and transplanted renal ultrasound examination showed no abnormality.Conclusion Kidney grafts from pediatric donors can be successfully transplanted to pediatric recipients,but the therapeutic principle is different from that in adult kidney transplantation.
4.Multiple b value diffusion-weighted MRI based on intravoxel incoherent motion model applied to lymphoma-pilot study

Objective To evaluate treatment effect of nodal lymphoma by using multiple b value diffusion-weighted MRI based on intravoxel incoherent motion (IVIM) model. Methods From November 2012 to November 2013, 22 patients with pathology confirmed lymphoma in Guangdong General Hospital were chosen. Patients were examined on a 1.5 T MR scanner with plain MRI scan and multiple b value diffusion-weighted MRI scan before and after cycle two of chemotherapy. According to chemotherapy response which evaluated after cycle 2 of chemotherapy, lymphoma nodes were divided into four groups:complete response (CR), partial response (PR), stable disease (SD), progressive disease (PD). Kruskal-Wallis H test was used to compare prechemotherapy D value, f value among CR, PR, SD groups. Nemenyi test was used to compare prechemotherapy D value between the two of CR, PR, SD groups. One-way ANOVA was used to compare D*value among CR, PR, SD groups. Paired-sample t test was used to compare D, f, D*value between before and after chemotherapy in PR group. CR and PR group were ascribed to curative group, and SD group ascribed to poor response group. ROC curve was used to evaluate the predictive efficiency of parameters derived from IVIM. Results Twenty-two lymphoma patients were scanned before chemotherapy and 21 patients were scanned after cycle 2 of chemotherapy. There were 49 lesions in CR group, 17 lesions in PR group, 8 lesions in SD group and no lesions in PD group. Prechemotherapy D value of CR, PR, SD group were (0.63±0.26)×10-3, (0.57±0.10)×10-3,(0.42±0.04)×10-3 mm2/s, respectively. There was significant difference among the three groups (H=12.944,P=0.002). There was no statistically difference of prechemotherapy D value between CR and PR group (χ2=0.072,P=0.965). Prechemotherapy D value was lower in SD group than that in CR group (χ2=12.090,P=0.002) and PR group (χ2=10.684,P=0.005). There was no statistically difference of prechemotherapy f value among CR, PR, SD groups (χ2=2.312,P=0.315) or D*value (F=0.535,P=0.588). D value significantly increased after chemotherapy in PR group [(1.03±0.37)× 10-3 vs.(0.63 ± 0.26)× 10-3 mm2/s, t=-4.781, P=0.001]. f value significantly increased after chemotherapy in PR group [(9.39 ± 4.52)% vs.(6.44 ± 3.25)%, t=2.294, P=0.036]. D* value slightly increased after chemotherapy but with no statistical difference in PR group [(99.72 ± 42.12)× 10-3 vs.(90.37 ± 45.33)× 10-3 mm2/s, t=-0.579, P=0.570]. When a D value of 0.48 × 10-3 mm2/s was used as the threshold value for predicting chemotherapy response, the best results were obtained with sensitivity of 100.00%and specificity of 75.76%.Conclusions Prechemotherapy D value can predict chemotherapy response and D value can monitor chemotherapy response in lymphoma.
5.Pretreatment of SS31 prevents hypoxia/reoxygenation-induced apoptosis in renal tubular epithelial cells

Objective To investigate whether the pretreatment of SS31 could alleviate hypoxia/reoxygenation (H/R)-induced injury by inhibiting p66Shc.Method The cultured rat renal proximal tubular cell line NRK52E cells were exposed to 24-h hypoxia (5% CO2,1% O2,and 94% N2) followed by 6-h reoxygenation (5% CO2,21% O2,and 74% N2).SS31 was added to the culture medium 4 h prior to the treatment.Then the cell viability,apoptosis,ROS and MTP were determined.In addition,Western blotting was used to detect the expression of p66Shc and p-p66Shc.Result H/R induced apoptotic cell death,accompanied with activation of total and p-p66Shc in NRK52E cells.Total p66Shc and p-p66Shc were detected at low levels in control NRK52E cells,and their levels were dramatically increased in cells after H/R treatment.Pretreatment with 100 μmol/L SS31 significantly prevented cell death and attenuated total p66Shc and p-p66Shc levels after H/R.Conclusion This study revealed that SS31 pretreatment serves a protective role against H/R-induced apoptosis of human renal tubular epithelial cells by suppressing p66Shc.
6.Effects of Crude Extract of Cape Jasmine on Learning and Memory Function in Experimental Dementia Animal Model

ObjectiveTo assess the effects of crude extract of Cape Jasmine in the experimental Alzheimer's model induced by Aβ25-35 and the memory acquisition impaied model induced by ibotenic acid(IBO).MethodsAlzheimer's dementia of mice was induced by Aβ25-35 i.cv. treatment and the impaired memory acquisition model was induced by IBO injected into the forebrain nucleus basalis. The effects of crude extract of Cape Jasmine on the learning and memory function of model animals were evaluated with Morris water maze and step-through test in 3 dose groups(12-5, 25, 50 mg/kg). Donepezil(0-75 mg/kg)was used in the positive control group.ResultsMorris water maze test showed that the spatial learning and memory ability of the model mice significantly improved in three crude extract of Cape Jasmine groups(P<0-05). Meanwhile, the impaired function of the rats induced by IBO significantly improved in the medium dose group(P<0-01). The electric shock latent period in step-through box test prolonged and the frequency of electric shock decreased within 3 minutes in three groups.ConclusionCrude extract of Cape Jasmine can improve the learning and memory function of mice or rat in the model group.
7.Anti-depressant Mechanisms of Wuling Jun Powder by Gene Microarray Analysis

ObjectiveTo explore the possible mechanisms of Wuling Jun Power by the DNA microarray technique.MethodsAn experimental depression model was established by exposing the mouse to a chronic mild stress procedure. The total RNA was extracted reverse-transcripted and hybrided to the mouse 1-2 cDNA microarray (Clontech). The difference of expression profiles between control model, Wuling Jun powder and fluoxetine-treated groups were analyzed by the Image 2-1 Software.Results130 genes were significantly altered in stress group compared with the control groups. Among them, 116 genes were up-regulated and 14 genes were down-regulated. Meanwhile, 85 genes significantly changed in the Wuling Jun powder treated group with 34 genes up-regulated and 51 genes down-regulated compared with the model groups. For the Fluoxetine-treated group, 133 genes significantly changed with 35 genes up-regulated and 98 genes down-regulated compared with the model groups. These genes were associated with many aspects of life including receptor activity, protein kinases, inflammatory factors, transferrin, neurogenesis and so on.ConclusionMultiple genes were affected by the stress exposure. Altered changes of some genes were normalized by Wuling Jun powder and Fluoxetine treatment. In general, the mechanisms of Wuling Jun powder and Fluoxetine are similar, but also with minor difference.
8.The impact of CD4+ CD25+ regulatory T cells on the mouse spontaneous liver transplant tolerance

Objective To examine the contribution of CD4+ CD25+ regulatory T cells to liver transplant tolerance. Methods After injection of anti-CD25 monoclonal antibody (mAb, PC61), mouse orthotopic liver transplantation was performed and survivals were determined. The paraffin-embedded sections of hepatic allografts were cut and stained with hematoxylin and eosin (HE). Furthermore, the effect of CD4+ CD25+ regulatory T cells on proliferative response of CD4+ T cells and cytotoxicity of CD8+ T cells was examined by depleting these regulatory T cells. Results Depletion of these cells in the recipients but not in the donors before liver transplantation caused rejection. Histological analyses of hepatic allografts with PC61 treatment showed extensive leukocyte infiltration and tissue destruction, whereas those in the control group showed minimal changes. Moreover, elimination of CD4+CD25+ T cells resulted in the enhancement of both proliferative response of CD4+ T cells and cytotoxicity of CD8+ T cells against donor-type alloantigen. Conclusions These results suggest that CD4+CD25+ regulatory T cells were important for tolerance induction to hepatic allografts.
9.Change of serum myeloperoxidase and lipoxin A4 level in coronary heart disease patients with anxiety and/or depression.

OBJECTIVE:
To investigate the change of serum myeloperoxidase (MPO) and lipoxin A4 (LXA4) in coronary heart disease (CHD) patients with anxiety and depression and its clinical significance.
METHODS:
From December 2010 to February 2011, 143 CHD patients and 44 non-CHD patients (the control group) hospitalized in the Department of Cardiology at the Second Xiangya Hospital were enrolled. The hospital anxiety and depression scale (HADS) was used to evaluate the psychological state of all patients and the CHD patients were assigned to an anxiety and depression group (n=57) or a non-depression and anxiety group (n=86). The serum levels of high sensitive C-reactive protein (Hs-CRP), MPO, and LXA4 were examined, and the ratio of MPO and LXA4 (M/L) was calculated.
RESULTS:
The levels of Hs-CRP, MPO, and LXA4 as well as M/L ratios in both CHD groups were significantly higher than those in the control group (all P<0.01). Compared with the non-anxiety and depression group, the levels of MPO and LXA4, and M/L ratios in the anxiety and depression group increased (all P<0.05). Correlation analysis showed that MPO was positively correlated with the score of HADS-total (HADS-t), HADS-anxiety (HADS-a), or HADS-depression (HADS-d), while LXA4 was negatively correlated with HADS-t or HADS-d. Multiple ordinal logistic regression analysis revealed that higher HADS-t score, stable angina, unstable angina, and acute myocardial infarction were the independent impact factors for the elevation of M/L ratio.
CONCLUSION
Anxiety and depression may aggravate the inflammatory response in CHD patients. The imbalance between inflammation and anti-inflammation may be part of the mechanism.

10.Study of molecular mechanism and antigen expression of CisAB01 blood group.

OBJECTIVETo explore the molecular mechanism of CisAB01 subtype in the ABO blood group system, and to investigate the expression of A and B antigens in red blood cells (RBCs).
METHODSFor 5 unrelated individuals with the CisAB phenotype, the molecular basis for the blood type was studied with serological assay, DNA sequencing and haplotype analysis. Bioinformatics analysis was carried out to investigate the changes in structure and function of relevant enzymes. Expression of A and B antigens in RBCs of CisAB01 was detected by flow cytometry.
RESULTSAll of the 5 samples were found to have a CisAB01 subtype. The underlying mutations, 467C>T and 803G>C in exon 7, have resulted in replacement of amino acid P156L and G268A. The mean fluorescence intensity (MFI) of A antigen in CisAB01 cases was 135, while the control group was 172. The B antigens in CisAB01 cases (MFI=38) showed significant decrease in MFI compared with the control group (MFI=164).
CONCLUSION803G>C mutation of the ABO gene probably underlies the CisAB01 subtype. Fluorescence intensity of A antigens in CisAB01 subtype cases is slightly lower than the normal type, while the B antigen was significantly lower.
