1.The treatment effect of two transplantation of pteryginm combine mitomycin C

Objective To compare the treatment effect of amniotie membrane transplantation(AMT) combine mitomycin C(MMC) and corneal limbal stem cell autograft transplantation combine mitomycin C surgery.Methods 234 pterygium patients(239eye) were randomly divided into two groups: routine resection and AMT combine mitomycin C,limbal epithelial autograft transplantation combine mitomycin C. Results The recrudescnce (3.25 % ) of AMT combine mitomycin C is lower than routine resection and limbal epithelial autograft transplantation combine mitomycin C(4.31% ). But there was no significant difference between these two groups. Conclusion The treatment of AMT combine mitomycin C is as good as Iimbal epithelial autograft transplantation combine mitomyein C and the recrudescnce is lower.
2.Serum levels of β2-microglobulin and lactic dehydrogenase in patients with multiple myeloma and their clinical significance

Objective To evaluate the clinical significance of the serum β2-microglobulin (β 2-MG) and lactic dehydrogenase (LDH) in multiple myeloma (MM) patients.Methods The serum levels of β2-MG and LDH in 63 MM patients (MM group) and 20 healthy person (control group) were measured by biochemical assay.Results The serum levels of β 2-MG and LDH in MM group were higher than those in control group[(3.81 ± 0.62) mg/L vs.(2.43 ± 0.91) mg/L and (296.4 ± 34.7) U/L vs.(145.5 ±17.8) U/L,P < 0.05].Furthermore,the serum levels of β 2-MG and LDH increased gradually with MM clinical staging (P <0.05).After VAD chemotherapy,the serum levels of β 2-MG and LDH in clinical response MM patients(54 cases) were significantly decreased [(2.51 ± 1.36) mg/L vs.(3.57 ± 0.82) mg/L and (159.1 ± 35.2) U/L vs.(285.3 ± 87.6) U/L,P < 0.05],while those in no response patients (9 cases)were unchanged (P> 0.05).Conclusion The serum levels of β 2-MG and LDH can be taken as a clinical index to classify the clinical phase,prognosis and effectiveness of chemotherapy in MM patients.
3.Repeat renal transplantation after allograft loss from BK virus nephropathy—Report or 4 cases

Objective To summarize experience and prognosis of repeat renal transplantation after graft loss due to BK virus nephropathy (BKVN).Methods The clinical data of 4 adult patients undergoing repeat transplantation after previous allograft loss due to BKVN were collected and analyzed retrospectively.Results Three of four patients had documented allograft loss caused by BKVN and underwent retransplantation 5 months,9 months and 9 months respectively after hemodialysis with confirmed clearance of viremia.Allograft nephrectomy was performed on 1 of 3 patients 4 months before retransplantation.Maintenance immunosuppression was CsA + MMF + Pred,Tac + MMF + Pred and CsA + Pred in these 3 patients respectively.During the follow-up period of 9 months,5 months and 26 months,viremia kept negative and allografts function stabled normally without recurrence of BKVN.The cause of allograft loss was not illustrated in the other patient before retransplantation,which was performed without dialysis or allograft nephrectomy.BK virus was not monitored routinely after the operation.Four months later,his serum creatinine rose up to 400μmol/L and BKVN recurrence was proved by pathological analysis of the biopsy samples of the first and the second transplantation.Tac was switched to CsA and his serum creatinine declined to 260 μnol/L at 20th month.Conclusion Retransplantation can be performed on the patients with previous allograft loss due to BKVN.Allograft nephrectomy,clearance of viremia,monitoring BK virus and timely adjustment of immunosuppression were the keys to guarantee successful retransplantation.
4.CT-Guided Percutaneous Puncture Biopsy of Thoracic and Abdominal Masses

Objective To sum up the application and the technique of CT-guided percutaneous puncture biopsy of thoracoabdominal masses.Methods 107 cases of thoracoabdominal masses were made percutaneous puncture biopsy,in which chest disease 81 cases,abdominal disease 26 cases.Biopsy was done using 18~20 G Franseen needle and 18 G coaxial automatic cutting needle.Routinely chose vertical plane angle,horizontal plane angle and vertical plane angle on one side of the body outline when needle was been entered.Results The successful rate by first puncture was 100%,and the total verification rate was 92.52%.The rate of thoracic complication was 11.11%.No obvious abdominal complication was found.Conclusion The technique of CT-guided percutaneous puncture biopsy of thoracoabdominal masses is simple,practical,high accuracy and only few complications.It should be emphasized that the needle angle should be having definite reference and the needles should be chosen accurately.
5.High Risk Factors for Pulmonary Fungus Infection in Intensive Care Units of Neurosurgery

OBJECTIVE By analyzing the high risk factors for pulmonary fungus infection in intensive care units of neurosurgery,this paper may be regarded as a good reference for early diagnosis and treatment. METHODS According to the domestic diagnostic standard on pulmonary fungus infection,we analyzed the clinical data of 58 patients with pulmonary fungus infection in department of neursurgery. RESULTS Totally 117 strains of fungi were isolated from the 58 cases.Candidiasis was the most frequent type,accounting for 92.3%.Some factors,such as the severity of underlying diseases,long-time coma,long-term use of broad-spectrum antibiotic,abuse of glucocorticoid,the open airway and some traumatic intubations might be regarded as the high risk factors for pulmonary fungus infection.Fluconazole showed good clinical effects on the treatment of fungus infection. CONCLUSIONS It is important to eliminate the high risk factors for pulmonary fungus infection,to provide early diagnosis and to use prophylactic antifungal agent,which can reduce the incidence rate of pulmonary fungus infection.
6.The role of endothelin-1 and angiotensin Ⅱ in acute kidney injury due to over-exertion in rats

Objective To study the role of endothelin-1 (ET-1) and angiotensin Ⅱ (AngⅡ) in over-exertion induced acute kidney injury (OTIAKI) by observing the changes in ET-1 and AngⅡ contents in plasma and renal tissue and their relationship with OTIAKI in exhausted rats. Methods 40 male Wistar rats were randomly divided into two groups: control group (CN, n=8) and exhaustion group (ES, n=32). The exhaustion group, depending on the recovery time after exhaustion, was further divided into 4 subgroups (8 each): immediate subgroup (ESI), 6h after exhaustion subgroup (ES 6h), 12h after exhaustion subgroup (ES 12h) and 24h after exhaustion subgroup (ES 24h). The animal model of OTIAKI was reproduced by exhausting swimming, while the rats in control group were not forced to swim. The contents of serum urea (Ur) and creatinine (Cr) in each group were serially measured. The renal specimens were observed with a light microscope to study their morphologic changes. Renal cell apoptosis was detected using terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate nick-end labeling (TUNEL) assay. The contents of ET-1 and AngⅡ in plasma and renal tissue were measured by radio-immunoassay (RIA). The correlation between the content of ET-1 and the levels of serum Ur and Cr were was analyzed by Pearson method, and the correlation between the content of AngⅡ in renal tissue and cell apoptosis was analyzed by Spearman method. Objective To observe the effect of recombinant human tumor necrosis factor receptor Fc fusion Protein (rhTNFR:Fc) treatment on IgM-RF,IgG-RF,IgA-RF of patients with rheumatoid arthritis.Methods A randomized,active-comparator controlled,parallel group study were conducted.110 patients were enrolled and were randomly divided to the treatment group,in which patients were treated with twice weekly subcutaneous injection of rhTNFR:Fc (25 mg) (rhTNFR:Fc treatment group,n=55),and the MTX froup,in which MTX (the mean dosage was 15 mg/week) (MTX group,n=55) for 24 weeks.Blood routine,IgM-RF,IgG-RF,IgA-RF,and disease activity score 28 (DAS28) were monitored.Student's t-test was used for statistical analysis.Results The level of IgM-RF decreased significantly in the rhTNFR:Fc treatment group 24 week(29±16) U/ml later.However,the level of IgG-RF(145±20) U/ml,IgA-RF(153±34) U/ml increased significantly in the rhTNFR:Fc group,and the level of IgG-RF (62±14) U/ml,IgA-RF (66-±19) U/ml decreased significantly in the MTX group.Conclusion Although rhTNFR:Fc,is effective in treating the clinical symptoms of RA,it seems to affect RF producing-B cells either directly or indirectly.
8.Change of serum myeloperoxidase and lipoxin A4 level in coronary heart disease patients with anxiety and/or depression.

OBJECTIVE:
To investigate the change of serum myeloperoxidase (MPO) and lipoxin A4 (LXA4) in coronary heart disease (CHD) patients with anxiety and depression and its clinical significance.
METHODS:
From December 2010 to February 2011, 143 CHD patients and 44 non-CHD patients (the control group) hospitalized in the Department of Cardiology at the Second Xiangya Hospital were enrolled. The hospital anxiety and depression scale (HADS) was used to evaluate the psychological state of all patients and the CHD patients were assigned to an anxiety and depression group (n=57) or a non-depression and anxiety group (n=86). The serum levels of high sensitive C-reactive protein (Hs-CRP), MPO, and LXA4 were examined, and the ratio of MPO and LXA4 (M/L) was calculated.
RESULTS:
The levels of Hs-CRP, MPO, and LXA4 as well as M/L ratios in both CHD groups were significantly higher than those in the control group (all P<0.01). Compared with the non-anxiety and depression group, the levels of MPO and LXA4, and M/L ratios in the anxiety and depression group increased (all P<0.05). Correlation analysis showed that MPO was positively correlated with the score of HADS-total (HADS-t), HADS-anxiety (HADS-a), or HADS-depression (HADS-d), while LXA4 was negatively correlated with HADS-t or HADS-d. Multiple ordinal logistic regression analysis revealed that higher HADS-t score, stable angina, unstable angina, and acute myocardial infarction were the independent impact factors for the elevation of M/L ratio.
CONCLUSION
Anxiety and depression may aggravate the inflammatory response in CHD patients. The imbalance between inflammation and anti-inflammation may be part of the mechanism.

9.CDK5 and epithelial-mesenchymal transition related proteins are abnormally expressed in head and neck squamous cell carcinoma

Objective To explore the expressions of Cyclin-dependent kinase 5 (CDK5) and Epithelial-Mesenchymal Transition (EMT) related proteins including N-cadherin, Vimentin and E-cadherin in head and neck squamous cell carcino? ma (HNSCC), and to determine the relationship between the expression of CDK5 and prognosis. Methods The expression levels of CDK5 and EMT related proteins were evaluated by immunohistochemistry in 55 patients who were diagnosed as HN?SCC. They were also analyzed in different clinical pathological factors. The correlation of CDK5 and EMT related proteins as well as the relationship between the expression of CDK5 and prognosis were also analyzed. Results The expression level of CDK5 was significantly higher in patients with lymph node metastasis than that in patients with non-lymph node metastasis (91.67%vs 30.23%, P<0.05). It’s also higher in T3-T4 stages than that in T1-T2 stages (85%vs 20%, P<0.05). The ex?pression levels of N-cadherin and Vimentin were significantly higher in patients with lymph node metastasis than those in patients with non-lymph node metastasis (75.00%vs 6.98%;91.67%vs 27.91%, all P<0.05). However, the expression level of E-cadherin was significantly lower in patients with lymph node metastasis (8.33%vs 86.05%, P<0.05) compared to that in patients without. CDK5 was positively correlated with N-cadherin and Vimentin, but negatively correlated with E-cad?herin (rs=0.512, 0.443,-0.363, all P<0.01). The 3-year survival rates were significantly lower in patients with high expres?sion of CDK5 (37.5%) than that in patients with low expression of CDK5 (87%, Log-rankχ2=12.678, P<0.01). Conclusion CDK5 and EMT related proteins were activated abnormally in HNSCC with lymph node metastasis. CDK5 may be a new bio?logical marker for prognosis of HNSCC.
10.Study of molecular mechanism and antigen expression of CisAB01 blood group.

OBJECTIVETo explore the molecular mechanism of CisAB01 subtype in the ABO blood group system, and to investigate the expression of A and B antigens in red blood cells (RBCs).
METHODSFor 5 unrelated individuals with the CisAB phenotype, the molecular basis for the blood type was studied with serological assay, DNA sequencing and haplotype analysis. Bioinformatics analysis was carried out to investigate the changes in structure and function of relevant enzymes. Expression of A and B antigens in RBCs of CisAB01 was detected by flow cytometry.
RESULTSAll of the 5 samples were found to have a CisAB01 subtype. The underlying mutations, 467C>T and 803G>C in exon 7, have resulted in replacement of amino acid P156L and G268A. The mean fluorescence intensity (MFI) of A antigen in CisAB01 cases was 135, while the control group was 172. The B antigens in CisAB01 cases (MFI=38) showed significant decrease in MFI compared with the control group (MFI=164).
CONCLUSION803G>C mutation of the ABO gene probably underlies the CisAB01 subtype. Fluorescence intensity of A antigens in CisAB01 subtype cases is slightly lower than the normal type, while the B antigen was significantly lower.
