1.Discussion on schizophrenia from the liver in TCM

Schizophrenia is roughly equivalent to insanity in TCM.Premorbid personality traits of schizophrenia are similar to the characteristic of stagnation of liver in TCM.The stagnation of liver-qi is a usually cause and main differentiation type of schizophrenia.Early location of this disease is toward to the liver.The stagnant qi of the liver turns into fire,up to brain to interfere the activity of mind,then leads the disease into the acute phase;Stagnation of the liver-qi and the heart-fire affect other organs and lead the disease to its chronic phase:The change of solar term or dramatic disturbances of climate change often leads liver yang to interfere the mind,resulting in recurrence or relapses of schizophrenia.We should pay attention to rule of liver in the treatment,rehabilitation,prevention of recurrence in schizophrenia.When treating schizophrenia,we need pay attention to soothing liver,without inhibiting the sthenic liver-energy;suppressing the hyperactive liver by calming the liver and removing heat from the liver.Soothing liver and strengthening spleen,dispersing liver-qi and regulating stomach are the common treatment.For preventing degeneration,while nourishing liver and kidney,clearing heat and calming the mind should be used at same time;while nourishing liver yin,wind-dispersing,eliminating phlegm,clearing-stasis should be used at same time.Cleaning liver heat,nourishing yin,activating blood and detoxification should be paid attention to when preventing and treating the side effects caused by antipsychotic drugs.
2.Reconstruction of failed urethroplasty with different tissues and materials for hypospadias

Objective To study the efficacy and complications of reconstruction of failed urethro-plasty for hypospadias with pedicle flap , bladder mucosa , buccal mucosa , and biological patch . Methods 23 patients were enrolled from Jul .2005 to Dec.2011.8 patients, with good local skin condition , were performed with pedicle flap urethroplasty .The other 15 patients, with bad local skin condition or with long segment urethral stricture , were performed with free grafts , including 6 bladder mucosa , 7 buccal muco-sal and 2 biological patch. Results Of the 23 cases, 7 cases were cured by one phase operation .There were 16 (16/25) cases had complications.3 (3/16) cases were failed because of serious infection (2 pedi-cle flap, 1 bladder mucosa ) .The failed cases were implemented with urethroplasty 6 months later by the buccal mucosa installments operation .4 (4/16) cases had solitary urethral fistula (1 pedicle flap, 2 bladder mucosa, and 1 buccal mucosal), who were successfully treated with simple fistula repair 3 to 6 months later. 9 ( 9/16) cases had urethral stricture ( 2 pedicle flap , 3 bladder mucosa , 3 buccal mucosal , and 1 biologi-cal patch graft ) , who were treated with urethral sound and got good result .We had reconstructed the urethra using mucosa graft onlay urethroplasty .All of the 23 patients were followed up with an average of 14.5 ( 6-24) months.23 cases were satisfied with the stretched penis , urination and no need to expand the urethra more than 6 months.3 cases were not satisfied with penile appearance .After communication, these patients did not require a further penis orthopedic surgery . Conclusions Pedicle flap, bladder mucosa , buccal mucosa and biological patch can be used in urethral repair and construction of failed urethroplasty for hypos -padias.Urethral sound dilation plays an important role in hypospadias repair .
3.Value of perfusion CT quantitative analysis for predicting tumor regression grade after chemoradiotherapy in patients with rectal cancer

Objective To explore the value of perfusion CT quantitative analysis for predicting tumor regression grade (TRG) after chemoradiotherapy in patients with rectal cancer.Methods From June 2016 to June 2018,94 rectal cancer patients diagnosed and treated in Cangzhou Central Hospital of Hebei Province were selected and were divided into reaction group (TRG 3-4) and non-reaction group (TRG 0-2) according to the results of surgical specimens.Perfusion CT was performed in both groups before treatment,and chemoradiotherapy and surgery were used.Baseline data and perfusion CT results including blood flow,blood volume,mean transit time (MTT),permeability surface (PS) were compared between the two groups,and receiver operating characteristic (ROC) curve was used to evaluate the predictive efficacy of perfusion CT indexes for chemoradiotherapy responsiveness.Results In this study,a total of 23 cases (24.47%) were responsive to chemoradiotherapy,and 71 cases (75.53%) were not responsive to chemoradiotherapy.Blood flow in reaction group [(38.60 ±7.13) ml · 100 g-1 · min-1] was significantly lower than that in non-reaction group [(67.39 ± 11.33) ml · 100 g-1 · min-1,t =3.273,P =0.001].MTT in reaction group was significantly longer than that in non-reaction group [(11.12 ±2.19) s vs.(6.88 ± 1.32) s,t =4.500,P <0.001].There was no significant difference in blood volume [(4.62 ±0.73) ml/100 g vs.(5.01 ± 1.04) ml/100 g] and PS [(13.72±3.82) ml · 100 g-1 · min-1 vs.(11.40 ±2.59) ml · 100 g-1 · min-1] between the two groups (t =0.818,P =0.415;t =0.409,P =0.683).The best cut-off points of blood flow and MTT for predicting chemoradiotherapy responsiveness were 50.89 ml · 100 g-1 · min-1 and 8.99 s,the area under the curve (AUC) was 0.825 and 0.922,and the AUC of combined prediction of chemoradiotherapy responsiveness was 0.982,which was significantly better than that of single prediction (Z =2.868,P =0.004;Z =2.051,P =0.004).The accuracy (91.49%) and specificity (90.14%) of combined prediction of chemoradiotherapy responsiveness were significantly better than those of single prediction (blood flow:accuracy 75.53%,specificity 73.24%;MTT:accuracy 79.79%,specificity 78.87%),and the differences were statistically significant (x2 =8.800,P =0.012;x2 =6.766,P =0.034).Conclusion Blood flow and MTT in perfusion CT have great predictive value for chemoradiotherapy responsiveness in patients with rectal cancer.
4.Analysis of the status and influencing factors of Helicobacter pylori infection of electrical power system workers in Shanghai

Objective To investigate Helicobacter pylori (HP) infection status and influencing factors of electrical power system employees in Shanghai. Methods A total of 29 986 employees in Shanghai electrical power system who underwent physical examination were selected as the research subjects, and received 13C-urea breath test (13C-UBT). The HP infection rate was recorded. Data including gender, age, body mass index, smoking history, drinking history, medication history, educational level, consumption of high-salt and high-fat food, presence of vomiting, and awareness of HP infection related treatment were also collected. The relationship between these factors and HP infection was analyzed. Results 13C-UBT showed that of 29,986 subjects examined, 17 580 were HP positive, and the positive rate was 58.63%. There was significant difference in HP infection rate between men and women (P < 0.05). There was a significant difference in HP infection rate among different age groups (25-40, 41 ~ 55 and 56-70 years old) (P <0.05). People who consumed high-salt and high-fat food and those who displayed no significant difference in HP infection rate (P <0.05). There were significant differences in the HP infection rate between people with and without history of HP infection (P <0.05), between smokers and non-smokers (P <0.05), and between those who knew that HP infection needed treatment and those who did not (P < 0.05). No significant difference was found in age, body mass index, educational level, drinking history and medication history (P>0.05). Logistic regression analysis showed that male, ignorance of HP infection related treatment, smoking and history of family HP infection were independent influencing factors for HP infection. Conclusion The HP infection rate was relatively high among employees in Shanghai electrical power system, and the main influencing factors of HP infection were male, ignorance of HP infection related treatment, smoking and history of HP family infection.
