3.Comparative study of cisplatin plus gemcitabine versus cisplatin plus vinorelbine in patients with advanced stage non-small cell lung cancer

AIM: To explore the differences of efficacy and side-effects in advanced stage non-small cell lung cancer (NSCLC) patients treated with gemcitabine plus cisplatin or vinorelbine plus cisplatin. METHODS: Eligible patients were randomly assigned to GP (gemcitabine+cisplatin) group or NP (vinorelbine plus cisplatin) group. In GP group, 36 evaluable patients were treated with gemcitabine 1000 mg?m~ -2 IV on day 1 and 8 and cisplatin 75 mg?m~ -2 IV which was divided into 1-3 days dosing, in a 21 days per cycle manner. In NP group, 30 evaluable patients were treated with vinorelbine 25 mg?m~ -2 IV on day 1 and 8 and cisplatin 80 mg?m~ -2 IV which was divided into 1-3 days dosing, 21 days per cycle. All the patients at least received two cycles therapy. The response rate, median survival time (MST), l year survival, and side-effects were observed. RESULTS: The response rates were 41.6 % vs 36.7 %; MST were 10.3 months vs 9.6 months; 1 year survival rate were 44.4 % vs 40.0 %(P= 0.33 ) in GP group and NP group, respectively. 4.The efficacy of Fufangbanmao capsules combined with chemotherapy in the treatment of patients with metastatic colorectal cancer

Objective To investigate the efficacy and side effects of Fufangbanmao capsules combined with FOLFOX-4 (Oxaliplatin + 5-fluorouracil /Calcium folinate) regimen in the treatment of metastatic colorectal cancer (MCC). Methods A total of 107 patients with previously untreated MCC, who were admitted to the hospital between April 2006 and October 2008, were randomly divided into combination group (n = 54) and chemotherapy group (n = 53). In combination group, patients received Fufangbanmao capsules (750 mg twice daily) and FOLFOX4 regimen (oxaliplatin 85 mg/m2 day 1, infusion of CF 200 mg/m2 for 2 hours followed by bolus 5-FU 400 mg/m2 and a 22-hour infusion of 5-FU 600 mg/m2 ). Whereas the patients in chemotherapy group were treated with FOLFOX4 regimen. Results The effective rate was 44. 4% in combination group and 37. 7% in chemotherapy group with no significant difference (P = 0. 481). The median time to progression (TTP) was 11. 6 and 7. 9 months in combination group and chemotherapy group, respectively, with significant difference (P=0. 020). The difference was found in improvement of quality of life (QOL) between combination group and chemotherapy group (57. 4% vs 32. 1 % , P=0. 008). The side effects in two groups included gastrointestinal toxicities, neuropathy, alopecia and bone marrow suppression. However, Ⅲ/Ⅳ neutropenia was significantly less in combination group(37. 0%) when compared with chemotherapy group (58. 5%, P = 0. 043). Conclusions In first-line treatment of MCC, Fufangbanmao capsules combined with FOLFOX4 regimen showed synergic and enhanced effect for improving TTP and QOL, and also reduced the incidence of neutropenia.

Objective To explore the function of placental trophoblast cell apoptosis on the pathogenetic mechanism of pregnancy induced hypertension (PIH). Methods Apoptosis of trophoblast cells in 20 cases of PIH(PIH group) and in 10 cases of normal pregnancy (control group) were directly observed using the terminal-deoxynucleotidyl transferase mediated d-UTP nick end labeling (TUNEL) method. Background Whether negative suction during excimer laser in situ keratomileusis(LASIK) affect the structure and function of retina or not is in controversy,but it seems that temporary hypertention induced by negative suction is a key factor of impairment of retina in LASIK.ObjectiveThis study attempts to study the influence of transient high intraocular pressure (IOP) during LASIK on the contents of retinal amino acid.MethodsThe both eyes of 45 New Zealand white rabbits were suctioned for different periods (20s,45s,3min) with negative pressure generator during the LASIK to make the instantaneous high IOP models,and LASIK without negative suction was performed in the both eyes of 15 rabbits in the control group.The changes of the contents of retinal amino acids were evaluated with High Performance Liquid Chromatography(HPLC) at 0,7,10,14 and 28 days postoperatively and compared with those of control group.ResultsThere were no statistically significant differences in the contents of retinal amino acids among different time points after operation in negative suction for 20s group and 45s group,respectively(P>0.05).At negative suction for 3minuts,the content of glutamic acid in retina was significantly increased in comparison with control group in 7,10,14 and 28 days (P<0.05),and no statistically significant difference was seen in the contents of glutamic acid at postoperative instant group compared with control group(P>0.05).A statistically significant difference in the contents of glutamine,tryptophan,phenylalanine was revealed among postoperative 10 days,14 days and 28 days groups comparison with control group (P<0.05).ConclusionThe acute IOP elevation caused by negative suction during LASIK results in the reversible increase of retinal amino acids.The duration of negative suction time influent the reconstruction of retinal structure.
10.Clinical study on the early prediction of death for serum markers in infants with hemophagocytic syndrome

Objective To investigate the association with death for serum parameters at the time of diagnosis and its value in predicting the death in infants with hemophagocytic syndrome (HPS).Methods A retrospective case-control study was conducted on 108 children with HPS who were admitted to our center between July 2005 and July 2012.For each patient,demographic,laboratory data and outcome information were collected.The patients were divided into death and surviving groups based on the follow-up results.The relation between serum markers and death was examined using the COX proportional hazards model and decision tree.Results Of 108 infants with HPS,33 died corresponding to a fatality rate of 30.6% and 90.3% of deaths occurred within 8 weeks after diagnosis.Following features were significantly associated with death:white blood cells (WBC) <5 x 109/L (HR =9.08,95% CI 3.07 ~ 26.87),hemoglobin <80 g/L (HR =6.15,95% CI 1.68 ~ 22.49),albumin < 28 g/L (HR =4.63,95% CI 1.12 ~ 7.39),serum ferritin > 1 100 μg/L (HR =3.05,95% CI 1.28 ~ 16.75),trigeminal ganglion ≥4 mmol/L (HR =2.88,95% CI 1.51 ~ 8.60),and prothromin time ≥ 16 s (HR =3.60,95 % CI 1.28 ~ 7.24),and fever for more than 2 weeks (HR =5.39,95% CI 1.97 ~ 14.66).Decision tree demonstrated that the probability of death was as high as 100% for infants with WBC <5 x 109/L and hemoglobin < 80 g/L.The odds of dying was still 66.7% for infants who had WBC≥5 × 109/L but reported trigeminal ganglion ≥4 mmol/L after having fever for more than 2 weeks.Conclusion The first 8 weeks after the onset of HPS is the critical period of treatment.There are several easily available serum predictors of early mortality in HPS infants,particularly the WBC and hemoglobin level,which may help guide treatment decisions.
