1.Clinical Analysis of Postoperative Complications of 96 Aged Patients with Cerebral Impairments

Objective To sum up the precaution and treatment of the postoperative complications of the aged patients with cerebral impairments. Methods The clinical data of 96 aged patients with cerebral impairments were reviewed, 76 patients of which had postoperative complications. Results Among the 76 patients with postoperative complications, 24 cases died, and the death rate is 31 7%. 11 patients died of lung failure due to lung infection, 4 patients died of massive bleeding of upper digestive tract, 3 patients died of heart failure, 4 patients died of renal failure and the other 2 patients died of recurrent hematoma. Conclusions The postoperative death rate of the aged patients with acute cerebral impairment is fairly high, and the main death cause is postoperative complications, so we should treat this kind of patients in time and prevent the occurrence of complications.

3.Medical knowledge discovery system research based on computer--epidemiological data mining of complications in diabetes mellitus.

In this paper, a systematic architecture of medical data mining based on computer was provided for epidemiological analysis. Complications in diabetes mellitus were used as the cases under discussions on redundancy elimination, normalized storage, knowledge induction and visual expression of medical data. 3022 pieces of census records from Tianjin General Hospital were researched to find the solution of quantitative mining from qualitative data and knowledge discovery. From the qualitative data mining of 43 kinds of complications in diabetes mellitus, we found 18 knowledge rules with significant statistical meaning on concurrency relation, e. g. hyperlipoidemia, coronary disease, hypertension and cerebrovascular disease. And knowledge tree was noted to be an effective visual expression method for showing the rules generated from the above system. Medical analysis system based on data mining and knowledge discovery could generate effective knowledge rules from medical record database, which was found to be especially useful for epidemiological analysis and national health survey. So how to cooperate with community medical care and hospital information system in the near future is practically significant.

4.Mediating effects of social participation quality on the relationship between perceived social constraints and mental health status in first stroke patients

Objective:To explore the relationships among perceived social constraints, social participation, as well as anxiety and depression in first stroke patients, and to further analyze the mediating effect of social participation on these variables.Methods:A total of 216 first stroke patients in the Department of Neurology of Affiliated Hospital of Qingdao University from April to December 2020 were recruited by convenience sampling and investigated by general information questionnaire, Social Constraints Scale (SCS), Chinese version of Impact on Participation and Autonomy Questionaire (IPA-C) and Hospital Anxiety and Depression Scale (HADS).Results:The total score of SCS, IPA-C, depression and anxiety were 33.49 ± 6.81, 43.42 ± 9.62, 8.05 ± 4.15 and 8.61 ± 2.59. Social constraints were positively correlated with social participation as well as anxiety ( r=0.644, 0.383, both P<0.05). Social constraints were positively correlated with social participation as well as depression ( r=0.482, 0.371, both P<0.05). The quality of social participation partially mediated the relationship between social constraints and anxiety (intermediary effect was 0.119), and also partially mediated the relationship between social constraints and depression (intermediary effect was 0.270). Conclusions:First stroke patients experience high level of social constraints, low quality of social participation and severe anxiety and depression. Social constraints can affect anxiety and depression through social participation. Medical staff should build a good environment to meet the needs of stroke patients of social participation, help patients to establish a correct psychological coping style, reduce patients' avoidance of social participation due to perceived constraints and exclusion, and thus promote the mental health of patients.
5.Electroacupuncture can promote hippocampal neurogenesis relieving chronic cerebral hypoperfusion

Objective:To observe the expression of noggin mRNA in the hippocampus of rats with chronic cerebral hypoperfusion, and explore the effect of electroacupuncture (EA) at the " baihui" and " dazhui" acupoints on their learning and memory and on hippocampal neurogenesis.Methods:In total, 120 Sprague-Dawley rats had cerebral hypoperfusion induced by permanent bilateral ligation of the common carotid artery. The 104 successfully induced were divided at random into a model group and an EA group, each of 52. The EA group was given EA on the baihui and dazhui acupoints for 20 minutes daily for seven days followed by a two-day break. The output current was 1mA at 15Hz. No special treatment was given to the model group. After one, two, four and six weeks of treatment, 6 rats were given BrdU injections to observe the proliferation and differentiation of neural stem cells. Learning and memory were assessed using the Morris Water Maze. The expression of noggin mRNA and neurogenesis in the dentate gyrus of the hippocampus were measured using reverse-transcription polymerase chain reactions and immunohistochemistry.Results:After one, two and five weeks of intervention, the average learning and memory ability of the EA group were significantly better than those of the control group. The average expression of noggin mRNA was significantly higher in the EA group than in the model group at the same time points. Compared with the model group, there were more BrdU-positive cells in the hippocampal dentate gyrus in the EA group, and the number decreased with the prolongation of ischemia. Pearson correlation analysis showed that the levels of noggin mRNA in the hippocampus of both groups were positively correlated with their number of BrdU-positive cells. The correlation was stronger in the EA group than in the model group.Conclusions:Electroacupuncture can promote hippocampal neurogenesis in rats with chronic cerebral hypoperfusion by regulating the expression of hippocampal noggin mRNA, thereby improving their spatial learning and memory ability.
6.Shenling Baizhusan Improves Spermatogenesis in Hyperuricemia Oligoasthenospermia Mice by Regulating Nrf2/ARE Pathway

ObjectiveTo investigate the effect and mechanism of Shenling Baizhusan on the treatment of oligoasthenospermia with hyperuricemia (HUA). MethodThirty-two male Kunming (KM) mice were randomly divided into blank group (n=6), model group (n=6), high-dose Shenling Baizhusan group (n=7), low-dose Shenling Baizhusan group (n=7), and febuxostat group (n=6). Except for the blank group, all other groups received intraperitoneal injection of potassium oxazinate suspension (600 mg·kg-1) for 7 days. After modeling, the high-dose Shenling Baizhusan group and the low-dose Shenling Baizhusan group were orally administered with 20.14 g·kg-1 and 10.07 g·kg-1 of Shenling Baizhusan, respectively. The Febuxostat group was orally administered with 0.25 g·kg-1 of Febuxostat, while the blank group and model group were orally administered with the same volume of physiological saline. Oral administration was performed once a day for 14 consecutive days, after which samples were collected. Biochemical methods were used to measure serum uric acid (UA), superoxide dismutase (SOD) and malondialdehyde (MDA) in testicular tissue. Hematoxylin-eosin (HE) staining was used to observe the histopathological changes in testicular tissue and evaluate the spermatogenesis function. Automated sperm analyzer was used to measure sperm density and motility. Single-cell gel electrophoresis (SCGE) was used to assess sperm DNA integrity. Terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL) was used to detect testicular cell apoptosis rate. Western blot analysis was performed to measure the protein expression levels of Kelch-like ECH-associated protein 1 (Keap1), nuclear factor E2-related factor 2 (Nrf2), heme oxygenase-1 (HO-1), B-cell lymphoma-2 (Bcl-2), Bcl-2-associated X protein (Bax), and Caspase-3 in testicular tissue. Real-time polymerase chain reaction (PCR) was conducted to evaluate the mRNA expression levels of Keap1, Nrf2, and HO-1 in testicular tissue. ResultCompared with the blank group, the model group showed elevated serum UA level (P<0.01), decreased testicular spermatogenesis function, sperm density, and motility (P<0.01), and increased sperm trailing rate and testicular cell apoptosis rate (P<0.01). Compared with the model group, the high-dose Shenling Baizhusan group showed significant improvements in the above-mentioned indicators (P<0.05, P<0.01). Additionally, the expression levels of Keap1, Bax, and Caspase-3 in testicular tissue were reduced, while the expression levels of Nrf2, HO-1, and Bcl-2 increased (P<0.05, P<0.01). The mRNA level of Keap1 decreased (P<0.05, P<0.01), while the mRNA levels of Nrf2 and HO-1 increased (P<0.05, P<0.01). ConclusionShenling Baizhusan can significantly improve HUA oligoasthenospermia, and its mechanism may be related to the Nrf2/antioxidant response element (ARE) signaling pathway.
7.Circulating biomarker- and magnetic resonance-based nomogram predicting long-term outcomes in dilated cardiomyopathy.

BACKGROUND:
Dilated cardiomyopathy (DCM) has a high mortality rate and is the most common indication for heart transplantation. Our study sought to develop a multiparametric nomogram to assess individualized all-cause mortality or heart transplantation (ACM/HTx) risk in DCM patients.
METHODS:
The present study is a retrospective cohort study. The demographic, clinical, blood test, and cardiac magnetic resonance imaging (CMRI) data of DCM patients in the tertiary center (Fuwai Hospital) were collected. The primary endpoint was ACM/HTx. The least absolute shrinkage and selection operator (LASSO) Cox regression model was applied for variable selection. Multivariable Cox regression was used to develop a nomogram. The concordance index (C-index), area under the receiver operating characteristic curve (AUC), calibration curve, and decision curve analysis (DCA) were used to evaluate the performance of the nomogram.
RESULTS:
A total of 218 patients were included in the present study. They were randomly divided into a training cohort and a validation cohort. The nomogram was established based on eight variables, including mid-wall late gadolinium enhancement, systolic blood pressure, diastolic blood pressure, left ventricular ejection fraction, left ventricular end-diastolic diameter, left ventricular end-diastolic volume index, free triiodothyronine, and N-terminal pro-B type natriuretic peptide. The AUCs regarding 1-year, 3-year, and 5-year ACM/HTx events were 0.859, 0.831, and 0.840 in the training cohort and 0.770, 0.789, and 0.819 in the validation cohort, respectively. The calibration curve and DCA showed good accuracy and clinical utility of the nomogram.
CONCLUSIONS
We established and validated a circulating biomarker- and CMRI-based nomogram that could provide a personalized prediction of ACM/HTx for DCM patients, which might help risk stratification and decision-making in clinical practice.
