1.Molecular characteristics of Shiga toxin-producing Escherichia coli in a dairy farm and market-sold raw livestock meats in Suzhou City

Objective:
To investigate the prevalence and molecular characteristics of Shiga toxin-producing Escherichia coli (STEC) in a large dairy farm and market-sold raw livestock meats in Suzhou City, so as to provide the evidence for evaluating human health risks of STEC.
Methods:
Bovine stool samples and breeding environmental samples were collected from a large dairy farm in Suzhou City, and beef, pork and mutton samples were collected from markets in Suzhou City. STEC strains were isolated and virulence genes were characterized in STEC strains using quantitative fluorescence PCR assay. The sensitivity to common antibiotics was tested using the broth microdilution plate method, and the genotypes of STEC were characterized using pulsed-field gel electrophoresis (PFGE).
Results:
A total of 624 samples were collected, including 110 adult cow stool samples, 170 calf stool samples, 60 farm environmental samples, 126 beef samples, 100 minced beef samples, 15 pork samples, 15 minced pork samples, 18 mutton samples and 10 ground mutton samples. A total of 12 non-O157 STEC strains were isolated, with a detection rate of 1.92%, and the detection rates of non-O157 STEC strains were 4.12%, 1.59% and 3.00% in calf stool samples, beef samples and minced beef samples, respectively, while non-O157 STEC strains were not detected in adult cow stool samples, environmental samples, pork samples, minced pork samples, mutton samples, or minced mutton samples. Among the 12 STEC strains, there were 4 strains carrying stx1 gene, 4 strains carrying stx2 gene and 4 strains carrying stx1 and stx2 genes. The 12 STEC strains showed the highest prevalence of resistance to ampicillin, ampicillin/sulbactam, cefotaxime and cefazoline (all were 41.67%), and were sensitive to imithiomycin, polymyxin, azithromycin, cefoxitin and ciprofloxacin, and there were 5 strains with multidrug resistance (41.67%). The 12 STEC strains were characterized with 11 genotypes and had no unique gene fingerprint patterns, with the Dice similarity coefficient ranging from 61.3% to 92.7%.
Conclusions
The detection of non-O157 STEC strains is high in calf stools, and non-O157 STEC strains show a level of resistance to common antibiotics and present molecular polymorphisms. The monitoring and management of STEC strains should be strengthened.
2.Effects of isoflurane delayed preconditioning on nuclear factor-κB during myocardial ischemia reperfusion injury in rabbits

Objective To explore the mechanism of the catdioprotection of isoflurane delayed preconditioning on myocardial ischemia reperfusion injury in rabbits.Method Thirty male New Zealand white rabbits,weighing 2.0 to 2.5 kg,were randomly divided into three groups(ten for each group):Control group(group C),I/R group(I/R group) ,2.0% isoflurane group(group S) .Group S was exposed to 2.0% isoflurane-100% oxygen for2 h.Group C and I/R group were exposed 2 h to 100% oxygen served as untreated controls.Twenty-four hours later I/R group and group S underwent 40 rain of coronary occlusion followed by 2 h of reperfusion.Blood samples were taken from arterial line at 20 min before occlusion(T1) ,20 rain after occlusion(T2) ,40 rain after occlusion(T3) ,1 h after reperfusion(T4) and 2 h after reperfusion(TS) for determination of plasma IL-10 levels and TNF-alevels by ELISA.At the end of the reperfusion,infarct size and area at risk were defined by Evans and TTC staining.The heart was harvested and levels of the nuclear factor kappa β(NF-κB)activity were determined by Western Blot,and ultrastructures were observed by electron microscopy.The data was expressed as,and were analyzed by using oneway ANOVA test with SPSS 13.0.P value less than 0.05 indicated statistical significance.Results The NF-κB activity of group S was significantly lower than that of group I/R(P<0.05).Group S significantly(P<0.05)reduced infarct size(19.7%±2.8% in group S) of the left ventricular area at risk as compared with control (37.8 %±1.7 % in I/R group).The injury of I/R group was worse than that of group S from the changes of the cellular structure under light microscope.Group S had a lower levels of TNF-α and also had a higher level of IL-10.Conclusions Isoflurane can inhibit NF-κB activity during myocardial ischemia reperfusion and modulate the cytokine expression,which may be one of molecular mechanisms of Isoflurane delayed preconditioning on cardioprotection.
3.Textual Research on Traditional Effect Verification of Belamcanda chinensis and Its Pharmacological Experimental Verification

Through reviewing ancient and modern literatures,the effect evolution and disease treatment changes of Belamcanda chinensis were understood.The pharmacological experiments were used to verify its main effects.The combination of featured advantages of traditional Chinese medicine (TCM) and modern science and technology contributed to the promotion of TCM modernization.It had important significance for the development of effective components,selection of disease types in the treatment for research and development of new TCM drugs.The indication of Belamcanda chinensis was verified from textual research.The treatment of disease types by Belamcanda chinensis was verified from medical books.The treatment of disease types by Belamcanda chinensis compound was analyzed based on the Pujifang database management system.Main indications of Belamcanda chinensis were summarized.Modern pharmacological studies on anti-inflammatory mechanism of main components of Belamcanda chinensis were combined to screen animal models and investigation indexes for the preliminary verification of the efficacy of Belamcanda chinensis.The comprehensive application of classical herbal medicine books and prescription database analysis results showed that removing phlegm and relieving sore throat were the efficacy of Belamcanda chinensis,which was an important medicine in the treatment of pharyngitis and sore throat.In the modern research,serum of experimental group,IL-4 in throat tissues,as well as IgE and LTC4 level in serum and lung tissues were significantly reduced compared to the model group (P<0.05).It was concluded that the treatment effect of Belamcanda chinensis extract to chronic pharyngitis may be through the decreasing of IgE level in serum and lung tissues,inhibiting IL-4 expression in serum and throat tissues,and the LTC4 expression in serum.
4.Expression of MG7-Ag and follow-up mucosal pathological changes in patients with Suspected malignant gastric ulcer

Objective To evaluate the diagnostic value of expression of MG7-Ag diflferentiation and guidance of follow-up mucosal biopsy in the patients with suspected malignanct gastric ulcer.Methods The expression of MG7-Ag in 58 patients who were suspected as having malignant gastric ulcer under routine endoscopy was studied by immuno-histochemical staining.The follow-up endoscopy was pefformed 1 to 72 weeks(mean 7 weeks)later and biopsy was taken.Results There were 19 cases of gastric carcinoma in 30 patients with positive MG7-Ag expression(63.3%)and only 5 in 28 patients with negative MG7-Ag expression(P<0.01).Conclusion The positive expression of MG7-Ag is correlated with gastrie carcinoma in patients with suspected malignant gastric ulcer.
5.Persistent Malfunction of Glymphatic and Meningeal Lymphatic Drainage in a Mouse Model of Subarachnoid Hemorrhage

Subarachnoid hemorrhage (SAH) is a devastating cerebrovascular event that often is followed by permanent brain impairments. It is necessary to explore the pathogenesis of secondary pathological damages in order to find effective interventions for improving the prognosis of SAH. Blockage of brain lymphatic drainage has been shown to worsen cerebral ischemia and edema after acute SAH. However, whether or not there is persistent dysfunction of cerebral lymphatic drainage following SAH remains unclear. In this study, autologous blood was injected into the cisterna magna of mice to establish SAH model. One week after surgery, SAH mice showed decreases in fluorescent tracer drainage to the deep cervical lymph nodes (dcLNs) and influx into the brain parenchyma after injection into the cisterna magna. Moreover, SAH impaired polarization of astrocyte aquaporin-4 (AQP4) that is a functional marker of glymphatic clearance and resulted in accumulations of Tau proteins as well as CD3⁺, CD4⁺, and CD8⁺ cells in the brain. In addition, pathological changes, including microvascular spasm, activation of glial cells, neuroinflammation, and neuronal apoptosis were observed in the hippocampus of SAH mice. Present results demonstrate persistent malfunction of glymphatic and meningeal lymphatic drainage and related neuropathological damages after SAH. Targeting improvement of brain lymphatic clearance potentially serves as a new strategy for the treatment of SAH.

6.Research on the feasibility of a magnetic-coupling-driven axial flow blood pump.

A new-designed axial flow blood pump, dived by magnetic coupling and using internal hollow brushless DC motor and inlet and outlet in line with impeller, was tested in mimic circuit. The results showed good performance of the new pump and indicated that its hydrodynamic characteristic can meet the demands of clinical extracorporeal circulation and auxiliary circulation.

7.The effect of ultrasound monitoring of inferior vena cava collapse index guiding fluid replacement on circulation in elderly patients during induction of general anesthesia

Objective:To investigate the effect of ultrasound monitoring of inferior vena cava collapse index (IVC-CI) guiding fluid replacement on circulation in elderly patients during induction of general anesthesia.Methods:A total of 71 elderly patients who underwent elective surgery under general anesthesia and tracheal intubation at Hunan Provincial People′s Hospital from April 2021 to September 2022 were randomly divided into control group (35 cases) and observation group (36 cases) using a random number table method. Before anesthesia, both groups of patients underwent IVC ultrasound examination and calculated the IVC-CI value. For patients with IVC-CI≥40%, the observation group was given 8 ml/kg of crystal solution before anesthesia induction, while the control group was not treated. The incidence of hypotension, the use of vasoactive drugs, and the total infusion volume from anesthesia induction to skin incision were recorded in two groups. Mean arterial blood pressure (MBP), heart rate (HR), oxygen saturation (SpO 2), cardiac index (CI), and cardiac volume variability (SVV) before anesthesia (T 0), 5 min after induction (T 1), 1 min after tracheal intubation (T 2), 5 min after tracheal intubation (T 3), 10 min after tracheal intubation (T 4), and 1 min before skin incision (T 5) were recorded and compared between the two groups. Results:The incidence of hypotension (27.8% vs 60.0%) and utilization rate of vasoactive drugs (25.0% vs 48.6%) in the observation group were lower than those in the control group, and the total infusion volume during anesthesia induction was higher than that in the control group, with statistical significance (all P<0.05). SVV, CI and MBP at T 1, T 3, T 4 and T 5 were significantly different from those at T 0 in the control group ( F=3.85, 14.66, 3.96, all P<0.05). SVV, CI and MBP at T 1, T 3, T 4 and T 5 in the observation group were significantly different from those at T 0 ( F=3.51, 13.20, 4.35, all P<0.05). There was no significant difference in SVV, CI, MBP, HR and SpO 2 between 2 groups (all P>0.05). Conclusions:For the elderly patients with preoperative IVC-CI≥40%, pre-filling with 8ml/kg crystal solution before anesthesia induction can significantly reduce the incidence of hypotension and the utilization rate of vasoactive drugs in the elderly patients during anesthesia induction.
8.Etiological characteristics of a foodborne diseases outbreak of Salmonella enteritidis in Suzhou

ObjectiveTo determine the pathogenic cause in a foodborne diseases outbreak of Salmonella enteritidis in a company in Suzhou City, and provide evidence for epidemiological investigation and guidance for clinical treatment. MethodsRelevant specimens were examined for Salmonella, Shigella, Staphylococcus aureus and Vibrio parahaemolyticus. Furthermore, for the isolated Salmonella enteritidis, a micro broth dilution method was used for antimicrobial susceptibility testing, and pulsed field gel electrophoresis (PFGE) was used for molecular typing. ResultsA total of 44 strains of Salmonella enteritidis were detected from 43 anal swabs of the patients in the outbreak, 7 anal swabs of canteen employees, 31 retained food specimens and 6 environmental specimens. A total of 15 antimicrobial susceptibility testings showed that the 44 strains had the same antimicrobial resistance spectrum, which was 100% resistant to cefazolin, ampicillin, ampicillin/sulbactam, polymyxin E and nalidixic acid, suggesting a multi-drug resistance to more than three antibiotics. PFGE cluster analysis showed that the 44 strains had a 100% of genetic similarity. ConclusionThe outbreak is caused by the consumption of food contaminated with Salmonella enteritidis. The isolated strains have multi-drug resistance, which could guide appropriate antimicrobial treatment based on the antimicrobial susceptibility testing.
9.Clinical efficacy study of transvaginal ischia spinous fascia fixation in the treatment of pelvic organ prolapse

Objective :
To explore the safety and effectiveness of transvaginal ischia spinous fascia fixation for pelvic
organ prolapse.
Methods :
The retrospective analysis of 124 patients who underwent surgical treatment for stage III
- IV pelvic organ prolapse was conducted. Among them , 53 cases of transvaginal ischia spinous fascia fixation (IS⁃ FF) were performed as a study group (ISFF group) while 71 cases of transvaginal sacrospinous ligament fixation (SSLF) were performed as a control group (SSLF group) . The operation time , postoperative hospitalization days , preoperative and postoperative hemoglobin values , indwelling urinary catheter time , postoperative pain scores , and the occurrence of complications were compared between the two groups , and the efficacy of the operation was objectively evaluated by using the staging method of pelvic organ prolapse (POP⁃Q) . Also the scores of the pelvic floor impact questionnaire⁃7 (PFIQ⁃7) , the pelvic floor dysfunction questionnaire⁃20 (PFDI⁃20) , and the questionnaire of quality of life 12 (PISQ⁃12) were used to evaluate the patients ′ postoperative quality of life.
Results :
The operation time and postoperative hospitalization days of patients in the ISFF group were less than those in the SSLF group , and the differences were statistically significant (P < 0. 05) . The preoperative and postoperative hemoglobin values , retention time of urinary catheter, postoperative pain scores , and hospitalization costs of patients in the two groups were compared , and the differences were not statistically significant. At the 3 ⁃month postoperative outpatient follow⁃up , the objective success rate was 100% in two groups. The median follow⁃up time of patients in both groups was 24 months ( 12 - 41 months) , and there were 2 cases of recurrence in the ISFF group , with a recurrence rate of 3. 77% and a subjective success rate of 96. 23% . While there were 3 cases of recurrence in the SSLF group and 2 cases of loss of visit , with a recurrence rate of 4. 34% and a subjective success rate of 95. 65% . 1 patient in the SSLF group presented with a pelvic hematoma with a diameter of about 5 cm after surgery. The hematoma disappeared after hemostasis and other symptomatic treatment. There was no organ injury or blood transfusion in both groups.
Conclusion
Transvaginal ischia spinous fascia fixation is a safe and effective treatment for pelvic organ prolapse , and it has the advantages of short operation time , fast postoperative recovery , fewer complications , and improvement of patients ′ quality of life.
