1.Screening of drug resistent gene by cyclical packaging rescue of hepatocellular carcinoma retroviral cDNA libraries.

Multidrug resistant genes are highly expressed in hepatocellular carcinoma that seriousty affects the effect of chemotherapy. Screening of resistant genes from HCC cells and studying its mechanism of drug resistance will be helpful to improve the effecacy of chemotherapy for hepatocellular carcinoma. Here we described an alternative method called cyclical packaging rescue (CPR). First we constructed a retrovirus cDNA library of hepatoma cells and used it to infect fibroblasts. Then we added drugs to screen survival cells. The survival cells, stably integrated helper-free retroviral libraries, were recovered rapidly after transfection with plasmids expressing retroviral gag-pol and env genes. Through this method, retroviral RNAs were directly repackaged into new infectious virions. Recovered retroviral supernatant was then used to reinfect fresh target cells. When performed in concert with selection using functional assays, cDNAs regulating functional responses could be identified by enrichment through multiple rounds of retroviral library recovery and retransmission. Using CPR, we obtained several cDNAs. After a preliminary detection, we found Ribosomal protein S11 (RPS11), Ribosomal protein L6 (RPL6), Ribosomal protein L11 (RPL11), Ribosomal protein L24 (RPL24) possibly had drug resistant function.

2.Inhibition of HBV Release by BST-2.

Bone marrow stromal antigen 2 (BST-2) is a kind of host restriction factor. Since it was discovered to be responsible for the defect in virion release of HIV-1 mutants lacking the accessory gene vpu in 2008, it was thought to mainly restrict the viruses by directly tethering viral particles at the plasma membrane. Recent reports suggest that BST-2 also can inhibit the the release of HBV particles, which are budding in the intracellular vesicles, expanding the antiviral spectrum of BST-2. Futhermore, the machanism that BST-2 used to restrict HBV release in multivesicular bodies (MVBs) is similar to that used to restrict HIV at the plasma membrane. However, HBV have evolved strategies to antagonize the antiviral action of BST-2. There are two different opinions about the antagonist. One is HBV inactivated BST-2 by HBx requiring a hepatocyte-specific environment. Another thought envelope protein HBs counteract the antiviral action of BST-2. In this review, we focus on the current advances in the anti-HBV activity of BST-2.

3.Plasma Concentration Monitoring of Vancomycin in ICU Patients and Individualized Medication Adjustment

Objective: To monitor vancomycin plasma concentration results by clinical pharmacists in order to participate in individualized medication adjustment.Methods: Clinical pharmacist monitored the blood concentration of vancomycin in special patients, and then designed the individualized medication according to the monitoring results and clinical manifestations of patients.Results: Through the individualized medication adjustment, clinical pharmacist enhanced the efficacy and reduced the incidence of adverse reactions, and improved the level of clinical rational drug use as well.Conclusion: Clinical pharmacist can provide better service for clinics through the practice of individualized precision pharmacy, and reasonable and safe pharmacy service can be provided for patients.

5.Pulmonary manifestation in children with systemic lupus erythematosus

Objective To observe the incidence,clinical types,and prognosis of pulmonary involvement in children with systemic lupus erythematosus(SLE),and to probe the relationship between the relative factors.Methods The clinical data of 61 children with SLE,who treated in Children's Hospital of Shanghai from Jan.2006 to Dec.2011 were analyzed retrospectively.The lung manifestations of the patients were classified according to their clinical symptoms and signs,and lung imaging examination (chest X-ray and CT).Other data of the SLE patients with lung injury or without lung injury were collected,including the course of the disease,activity index [SLE disease activity index (SLEDAI) scoring 0-4 was divided into no activity,5-9 was divided into low activity,10-14 was divided into moderate activity,and more than 15 as severe activity],blood sedimentation,the values of serum complement and the positive rate of dsDNA in autoantibody,and a comparison was made and statistical analysis was performed.Results Pulmonary injury occurred in 17 cases with an incidence rate of 27.9% (17/61 cases),and the average course of the disease was (8.76 ± 6.96) months.The clinical manifestations of 17 cases included fever,cough,dyspnea,hypoxemia,chest pain,lower breath sound,and lung rale,which were not non-specificity.According to imageological examination,in 17 cases there were 2 cases with pulmonary hypertension (the incidence rate was 11.8%),4 cases with acute lupus pneumonitis (the incidence rate was 23.5%),6 cases with interstitial pneumonia (the incidence rate was 35.3%),7 cases with lesion of pleura (the incidence rate was 41.2%),10 cases with pulmonary infection (the incidence rate was 58.8%).There were no statistical differences between patients with and without lung injury(infectious and non-infectious)in the course,the values of serumn complement,the positive rate of dsDNA and activity index.Lung lesions of the 17 patients recovered quickly after early diagnosis and active treatment.Conclusions Pulmonary manifestation in children with SLE is not uncommon,and it can happen at any stage of the disease,not just on the stage of lupus activity,but it usually indicates disease was not controlled.Pulmonary manifestation of SLE in children involve many parts of lung,including pulmonary parenchyma,interstitial,blood vessels,pleura,etc.Early diagnosis and active treatment can improve the short-term prognosis of lung injury with lupus,and long-term outcome remains to be observed.
6.The nursing care for patients with hyperperfusion syndrome occurring after carotid artery stenting;initial experience in 9 cases

Objective To analyze the clinical features of hyperperfusion syndrome occurring after carotid artery stenting, and to discuss its nursing measures. Methods Among 220 patients who received carotid artery stenting, nine developed hyperperfusion syndrome after stent implantation. Their clinical materials were retrospectively analyzed. The nursing measures, including properly controlling blood pressure, relieving brain edema, monitoring cerebral blood flow, medication with sedation drug, stopping or reducing antiplatelet therapy, close observation of blood pressure and clinical symptoms, cooperation with physicians to control the blood pressure and to dynamically make reexamination, etc. Results Of the nine patients with hyperperfusion syndrome, complete recovery was achieved in eight at the time of discharge and death due to intracranial hemorrhage occurred in one. Conclusion The key point of nursing for patients with hyperperfusion syndrome is close cooperation with physicians to control the patient ’s blood pressure so as to ensure a proper cerebral blood flow.
7.HPLC Fingerprint and chemical pattern recognition of Semen Cassiae

Objective To establish a method for the quality control of Semen Cassiae.Methods A high performance liquid chromatographic method was developed to establish the fingerprint of Semen Cassiae,and 32 samples from various batches of Semen Cassiae were analyzed.Cluster analysis and principal component analysis were applied to study HPLC fingerprint and chemical pattern recognition.Results The fingerprint of Semen Cassiae was set up.Conclusion The method could be used for the quality control and comprehensive evaluation of Semen Cassiae.
8.HPLC Fingerprint and chemical pattern recognition method of Radix Paeoniae Alba

Objective To establish a method for quality control of Radix Paeoniae Alba.Methods A high performance liquid chromatographic method was developed to establish the fingerprint of Radix Paeoniae Alba and 47 samples from various batches were analyzed.Cluster analysis and principal component analysis(PCA) were applied to study on HPLC fingerprint and chemical pattern recognition method. Results The result of PCA and cluster analysis showed that the samples were divided into two types.The HPLC fingerprinting of Radix Paeoniae Alba,showing 10 characteristic peaks,was established from 47 batches of Radix Paeoniae Alba.Conclusion The method provides an academic reference for the quality control of Radix Paeoniae Alba.

10.Short-term therapeutic effect of modified total pelvic floor reconstruction in treatment of severe pelvic organ prolapse

Objective To evaluate short-term clinical effects of modified total pelvic floor reconstruction surgery in treatment of severe pelvic organ prolapse.Methods Thirty-nine cases with severe pelvic organ prolapse including vault prolapse diagnosed by pelvic organ prolapse quantification (POP-Q) system received modified total pelvic floor reconstruction surgery.Clinical parameters associated peri-operative period and 12 months after surgery and complications were analyzed Results Median operation time was 70 minutes (30-240 minutes),median blood loss was 100 ml (10-200 ml).Seventy-seven percent (30/39) patients were able to micturate spontaneously in the next morning after surgery with postvoid residual volume less than 100 ml (0-650 ml).No severe intra-operative complications were recorded and the rate of postoperative morbidity was 20% (8/39 ).Median post-operative hospital stay was 4 days (1-11 days).The patients were followed up at median 24 months(13-29 months).According to POP-Q system evaluation,the successful rate of operation reached 100% .Two cases (5%,2/39) were recorded as symptomatic recurrence which manifested as posterior wall prolapse within 24 months after operation.During follow-up,8% (3/39) patients were found to have erosion within 7 months after surgery,and urgent urinary incontinence was observed in 5% (2/39) cases,while constipation occurred in 8% (3/39) cases.The most remarkable complication was dyspareunia (36%,5/14); while 50% (7/14) experienced better sexual life after surgery.Conclusions Modified total pelvic reconstruction is a safe,efficient and micro-invasive surgery in treatment of severe pelvic organ prolapse.However,its influence on post-operative sexual life should be concerned.
