1.Study and application of bone marrow stem cells in the treatment of liver disease

OBJECTIVE: The investigation of curing liver disease with stem cell makes a notable performance. This article intends to review the status of clinical and empirical study of stem cell transplants. DATA SOURCES: A computer-based search was conducted in Pubmed for English articles about bone marrow stem cells cure liver ailment published between January 1995 and June 2006 with the Keywords of "bone marrow stem cell, hemopoietic stem cells, mesenchymal stem cells, hepatic fibrosis". Meanwhile, relevant Chinese articles were searched in Chinese Journal Full-text Database with the key words of "bone marrow stem cell, hemopoietic stem cells, mesenchymal stem cells, hepatic fibrosis". STUDY SELECTION: Data were checked in the first trial and articles about the bone marrow stem cell and liver disease therapy are selected, while obvious unrelated literatures or repetitive study were excluded. DATA EXTRACTION: A total of 58 related English articles and 40 Chinese articles were collected, including 36 studies about bone marrow stem cells, 24 researches on mesenchymal stem cells and 14 about hemopoietic stem cells, 30 literatures of them in accordance with the inclusion criteria were reviewed. DATA SYNTHESIS: There are all kinds of stem cells in bone marrow that can differentiate other kinds of cells, including mesenchymal stem cells and hemopoietic stem cells. Both animal trials and clinical researches show that hemopoietic stem cells can differentiate into liver cells and hepatic oval cells and is mature to participate in the liver regeneration. Mesenchymal stem cells are homogeneous cells with multi-differentiation potentials and can be differentiated into liver cells and bile duct cells. Bone marrow stem cell therapy has many merits than traditional methods, for example simple materials, culture in vitro, passage and easy amplification, indicating a great achievement in clinic. CONCLUSION: We can use bone marrow stem cell therapy for all kinds of refractory hepatopathy, such as hepatitis, liver cirrhosis and hepatoma etc. It will not only improve haematogenesis and immune function, but also replace necrosis and lost hepatic cell, leading a perfect boundary of hepatopathy therapy.
2.Effect of acid sensing ion channels on respiratory regulation by central chemoreceptors

[ABSTRACT]AIM:Toinvestigatetheeffectofintracerebroventricular(icv)injectionofacidulatedartificialce-rebrospinal fluid ( aCSF ) on the respiratory reactions and the functions of acid sensing ion channels ( ASICs ) in this process.METHODS:Healthy adult SD rats (n=30) were divided into aCSF with pH 7.4 control group, aCSF with pH 6.5 group, amiloride control group, amiloride plus aCSF with pH 6.5 group, psalmotoxin 1 (PcTx1) control group and PcTx1 plus aCSF with pH 6.5 group.The electromyogram (EMG) of the diaphragm was monitored to observe the respirato-ry responses induced by icv injection of acidulated aCSF .The ASICs blockers were also injected into the lateral cerebral ventricle firstly and acidulated aCSF was injected following the ASICs blockers to observe the effect of ASICs on the respira -tory regulation by the central chemoreceptor .RESULTS:After icv injection of acidulated aCSF , the respiratory responses of the rats were excited (P<0.05).The respiratory excitation responses disappeared after icv injection of ASICs blocker amiloride.The degree of respiratory excitation was weakened after icv injection of ASIC 1a blockers-PcTx1.CONCLU-SION:ASICs play a key role in the respiratory regulation by the central chemoreceptor and ASIC 1a partly operates in this process.
3.Effect of Danhong injection on respiratory inhibition induced by hypoxia

AIM: To investigate the effect of Danhong injection on respiratory inhibition induced by hypoxia and its related mechanism .METHODS:The electromyogram of the diaphragm was monitored to observe the respiratory re-sponses of the rats to hypoxia .The expression of acid-sensingion channel 1a (ASIC1a) in the brainstem after hypoxia was detected by the technique of immunohistochemical staining .RESULTS: The respiratory responses of the rats to hypoxia were initiatory excitation and consequent inhibition .In contrast to pre-hypoxia, the respiration of the rats in hypoxia group was inhibited 30 min after hypoxia (P<0.05), but the respiration of the rats in hypoxia plus Danhong group was still ex-cited (P<0.05), indicating that the respiration of the rats in hypoxia plus Danhong group was not yet depressed at the same time.The ASIC1a positive neurons were detected in the nuclei of trapezoid body and solitary tract .Compared with control group, the expression of ASIC1a was obviously enhanced after hypoxia .In contrast to hypoxia group , the expression of ASIC1a in the rats in hypoxia plus Danhong group was remarkably reduced (P<0.05).CONCLUSION:Danhong in-jection resists the occurrence of respiratory inhibition after hypoxia , and ASIC1a may participate in this process .
5.Vancomycin flush solution for prevention of catheter-related infections in oncology patients with PICC: a systematic review of the effectiveness

Objective To assess the efficacy of vancomycin flush solution for prevention of catheter-related infections in oncology patients. Methods Randomized controlled trials(RCTs) were searched and identified from Cochrane library, PubMed, EMBASE, CBM, CNKI and related journals were also scanned. In accordance with inclusion and exclusion criteria, we evaluated the quality of the included studies by Jadad scale and analyzed the data of catheter- related sepsis, catheter colonization, exit- site infection by RevMan 5.0. Results We included 5 randomized controlled trials (400 patients). Meta analysis showed that there were statistical differences between vancomycin- heparin mixture and heparin solution on the incidence of catheter-related sepsis, catheter colonization, but no difference was found on the incidence of exit-site infection. Conclusions The current evidences show that contrast with heparin flush solution, van comycin - heparin flush solution can significantly reduce the incidence, of catheter - related sepsis and catheter colonization. But vancomycin-heparin flush solution was more effective, and the results should be further confirmed in prevention of Gram-positive bacterial and fungal catheter-related infections or in non- neutropenic patients. Before draw the conclusions, further high-quality, large scale, double-blind randomized controlled trials are still needed.
6.The changes of the glycolipid and coronary artery lesions in coronary artery disease patients with isolated post-challenge hyperglycaemia

Objective To observe the changes of the glycolipid and coronary artery lesions in coronary artery disease patients with isolated post-challenge hyperglycaemia (IPH). Methods Hospitalized coronary artery disease patients diagnosed by coronary angiography were received oral glucose tolerance test. The labarotary test datas, including blood hematochrome, fasting and post-challenge plasma glucose levels, plasma lipid and insulin levels, were recorded. The number of diseased coronary artery and coronary lesion scores were calculated according to coronary angiogram for each patient. Continuous 77 patients with normal glucose metabolism(normal glucose metabolism group) and 104 patients with IPH (IPH group) entered the study. Results The occurrence of hypertension, acute coronary syndrome, multivessel coronary disease, revascularization were 84.6%(88/104), 82.7%(86/104), 69.2%(72/104), 70.2%(73/104) in IPH group and 63.6%(49/77), 70.1%(54/77), 50.6%(39/77), 54.5%(42/77) in normal glucose metabolism group, and there were significant differences(χ2=3.985-10.583, all P<0.05). The body mass index, triglyceride, total cholesterol, fasting glucose, post-challenge glucose, hemoglobin A1c, fasting insulin, insulin resistance index, coronary lesion scores were (27.42±3.55) kg/m2,(2.18±1.53) mmol/L,(4.73 ± 1.00) mmol/L,(5.98 ± 0.92) mmol/L, (14.11 ± 1.94) mmol/L, (5.90 ± 0.78)%, (10.07 ± 6.13) mU/L, 2.617 ± 1.535, (41.35 ± 27.21) points in IPH group and (25.11 ± 3.40) kg/m2, (1.52 ± 0.85) mmol/L, (4.22 ± 1.02) mmol/L, (5.24±0.62) mmol/L, (6.49±1.07) mmol/L, (4.97±0.39)%, (7.72±4.28) mU/L,1.785±0.974, (33.36±23.36) points in normal glucose metabolism group, and there were significant differences(t=3.035-33.698, P<0.05 or 0.01). Conclusions Coronary artery disease patients with IPH have much more seriously abnormal glycolipid metabolism and complex coronary artery lesions, and need specialized nursing management.
7.A quantitative study on work-related stressors of night shift nurses’in coronary care unit

Objective To explore the work-related stressors of night shift nurses’in coronary care unit(CCU).Method Semi-structured interviews were conducted among 6 night shift nurses in CCU with phenomenological method to analyze work experience of night shift.Result CCU night shift nurses’work-related stressors were found to be horror,frustration,overwork,interpersonal communication stress and shortage of work experience.Conclusion Strengthening psychological communication and emergency training to nurses,adopting incentive mechanism and organization support can relieve nurses’physical and psychological stress and improve work initiative.
8.Hemocoagulase vs. Epinephrine in Endoscopic Mucosal Resection:Prospective Randomized Controlled Study

Objective To compare the anti-hemorrhage effect and safety of hemocoagulase and epinephrine (1∶10 000) during endoscopic mucosal resection (EMR) or polyps electrocautery. Methods Between April and November 2007,totally 60 patients received EMR or endoscopic piecemeal mucosal resection (EPMR) in our hospital because of local lesions in the esophagus,stomach or colon. The patients were randomly divided into hemocoagulase and epinephrine groups (30 cases in each) before receiving the operation. After the resection,the hemorrhage at the surgical area was measured. During the operation,the blood pressure,heart rate,and adverse effects including palpitation,dizziness,pain,and nausea were recorded. Results Immediate hemorrhage was detected in 1 case of hemocoagulase group (1/30,3%),and 6 cases of epinephrine group (6/30,20%,?2=2.588,P=0.108). No hemorrhage was observed at 24 hours in both the groups. No significantly difference was noticed in the injection volume between the two groups [(11.0?6.0) ml (5-30 ml) in hemocoagulase group vs. (11.9?7.0) ml (5-28 ml); t=-0.535,P=0.595]. In epinephrine group,the mean artery blood pressure and heart rate increased significantly after the injection [(96.9?7.9) mm Hg vs. (99.9?8.1) mm Hg,t=-3.005,P=0.005; 79.8?7.9/min vs. 84.3?8.1/min,t=-3.585,P=0.001]. Whereas,no such differences were noticed in hemocoagulase group. Five cases in epinephrine group showed transient dizziness and palpitation,and 2 patients who had esophageal lesions complained of pain at the injection site. None of the hemocoagulase group had postoperative complications. Conclusions Hemocoagulase shows same anti-hemorrhage effect as epinephrine during EMR without leading to adverse effect.
9.Simultaneous silence of bcl-2 and XIAP increases radiosensitivity of UMSCC12 head and neck cancer cells

Objective To investigate the radiosensitization effect of the simultaneous silence of bcl-2 and XIAP on human head and neck cancer cell line in vitro.Methods Four groups of UMSCC12 cells were transfected with siRNA-bcl-2,siRNA-XIAP,siRNA-bcl-2 plus siRNA XIAP,and siRNA control,respectively.The silence efficiency was tested by Western blot assay. Apoptosis was evaluated with the activities of caspase-3 and caspase-9,and radiosensitization effect was evaluated with clonogenic assay.Results Bcl-2 and XIAP protein expressions were effectively eliminated in the cells simultaneously transfected with siRNA-bcl-2 and siRNA-XIAP.Transfection of cells with bcl-2 siRNA increased radiationinduced apoptosis ( t =5.32,6.27 ; P < 0.05 ),but transfection with XIAP siRNA did not impact cell apoptosis.Since the simultaneous transfection of the above two siRNAs,the activities of caspase-3 and caspase-9 were increased by 1.36 and 1.34 times ( t =11.47,6.22 ; P < 0.05 ) in nonirradiated cells and increased by 1.72 and 1.98 times ( t =12.02,20.14; P < 0.05 ) in irradiated cells,respectively.The colonic assay showed that the SERs were 1.06,1.15 and 1.41 for the cells transfected with XIAP siRNA,bcl-2 siRNA and both siRNAs,respectively.Conclusions Compared to single silence of XIAP or bcl-2,simultaneous silence of XIAP and bcl-2 offers a potential approach to improving the radiosensitivity of the head and neck cancer cells,and apoptosis might contribute to the enhancement of radiosensitization.
