1.Hemocoagulase vs. Epinephrine in Endoscopic Mucosal Resection:Prospective Randomized Controlled Study

Objective To compare the anti-hemorrhage effect and safety of hemocoagulase and epinephrine (1∶10 000) during endoscopic mucosal resection (EMR) or polyps electrocautery. Methods Between April and November 2007,totally 60 patients received EMR or endoscopic piecemeal mucosal resection (EPMR) in our hospital because of local lesions in the esophagus,stomach or colon. The patients were randomly divided into hemocoagulase and epinephrine groups (30 cases in each) before receiving the operation. After the resection,the hemorrhage at the surgical area was measured. During the operation,the blood pressure,heart rate,and adverse effects including palpitation,dizziness,pain,and nausea were recorded. Results Immediate hemorrhage was detected in 1 case of hemocoagulase group (1/30,3%),and 6 cases of epinephrine group (6/30,20%,?2=2.588,P=0.108). No hemorrhage was observed at 24 hours in both the groups. No significantly difference was noticed in the injection volume between the two groups [(11.0?6.0) ml (5-30 ml) in hemocoagulase group vs. (11.9?7.0) ml (5-28 ml); t=-0.535,P=0.595]. In epinephrine group,the mean artery blood pressure and heart rate increased significantly after the injection [(96.9?7.9) mm Hg vs. (99.9?8.1) mm Hg,t=-3.005,P=0.005; 79.8?7.9/min vs. 84.3?8.1/min,t=-3.585,P=0.001]. Whereas,no such differences were noticed in hemocoagulase group. Five cases in epinephrine group showed transient dizziness and palpitation,and 2 patients who had esophageal lesions complained of pain at the injection site. None of the hemocoagulase group had postoperative complications. Conclusions Hemocoagulase shows same anti-hemorrhage effect as epinephrine during EMR without leading to adverse effect.
2.Influence of new Xuefuzhuyu decoction on IgG and C3, C4 of peripheral blood in atherosclerosis rat

Objective To explore the influence of new Xuefuzhuyu decotion on IgG,C3 and C4 of peripheral blood in atherosclerosis rats.Methods Healthy adult Wister rats were selected and randomly divided into blank group,AS model group,new Xuefuzhuyu decoctioin low dosage group,the new Xuefuzhuyu decoctioin high dosage group,and positive control group,with 13 cases in each group.Vitamin D3 joint high fat feeding method was adopted to establish atherosclerosis rat model.Medicine lavage began on the first day of experiment.Low dosage new Xuefuzhuyu decoctioin,high dosage new Xuefuzhuyu decoctioin,and atovastatin were administrated to the new Xuefuzhuyu decoctioin low dosage group,the new Xuefuzhuyu decoctioin high dosage group,and positive control group respectively,and distilled water 15 ml/kg · d-1 was administrated to both the blank group and the AS model group.The weight of body was detected each week in each group for regulating the administrated dosage.After medicine lavage was performed for successive 8 weeks,blood was obtained from abdominal aorta for detecting IgG,C3,C4 by immune scattering method.Results The value of IgG,C3 and C4 were (8.110±0.720)g/L,(1.591±0.143)g/L,(0.390±0.33)g/L in the AS model group,(8.000±0.840)g/L,(1.564± 0.132) g/L,(0.396±0.36)g/L new Xuefuzhuyu decoctioin low dosage group,(7.250±0.740)g/L,(1.390±0.150)g/L,(0.326±0.153)g/L new Xuefuzhuyu decoctioin high dosage group and (7.240±0.400) g/L,(1.414 ± 0.104) g/L,(0.330±0.164) g/L in the positive control group,all of which werehigher than the blank group (5.810±0.820)g/L,(0.943±0.124)g/L,(0.330±0.164)g/L with statistically significant difference (P<0.05).Each value of IgG,C3 and C4 in the new Xuefuzhuyu decoctioin high dosage group and the positive control group were lower than the AS model group and the new Xuefuzhuyu decoctioin low dosage group,with a statistically significant difference(P<0.05).There was no significant difference between new Xuefuzhuyu decoctioin high dosage group and the positive control group (P>0.05).Conclusion New Xuefuzhuyu decoction can ameliorate IgG,C3 and C4 of peripheral blood in atherosclerosis rats.Its function was similar to atorvastatin.
3.Risk factors of early rebleeding after endoscopic variceal ligation and/or endoscopic injection of fibrin tissue adhesive

Objective To study the independent risk factors of early rebleeding after endoscopic variceal ligation (EVL) and/or endoscopic injection of fibrin tissue adhesive.Methods Data of 370 patients who had cirrhosis and accepted 396 procedures of EVL and/or endoscopic injection of fibrin tissue adhesive were retrospectively studied.Independent risk factors for early rebleeding were determined by Logistic regression analysis.Results Results of all the factors that were significantly different between the re-bleeding and non-rebleeding patients,the portal vein diameter,ascites volume,Child-Pugh score and serum albumin were independent ones of early rebleeding after EVL and/or endoscopic injection of fibrin tissue adhesive ( P < 0.05).Larger volume of ascites,wider portal vein diameter,reduced albumin,Child-Pugh score greater than 10 were indicative factors of rebleeding.Conclusion The early rebleeding rate after EVL and/or endoscopic injection of fibrin tissue adhesive is determined by portal vein diameter,ascites volume,ChildPugh score,and serum albumin.
4.LSD1 negatively regulates the expression of tumor suppressor gene SIRT3 in pancreatic cancer cell line PANC-1

Background and purpose: Lysine specific demethylase 1(LSD1) is an important chromatin modifier. It epigenetically regulates gene expression pattern through chromatin modification and participates in maintenance of tumor malignant properties, such as oncogenesis, development, invasion, migration and metabolic transformation. SIRT3 (sirtuin 3) is a mitochondria localized tumor suppressor and regulates tumor metabolic transformation and oxidative stress. The correlation between LSD1 and SIRT3 has never been reported before. This study aimed to elucidate the correlation between LSD1 and SIRT3 with gene transcriptional regulation methods. Methods: RNA interference technique, co-immunoprecipitation assay(CoIP), chromatin immune-precipitation assay(ChIP) and ifrelfy luciferase activity assay were employed to elucidate the correlation between LSD1 and SIRT3 in pancreatic cancer. Results:mRNA and protein levels of SIRT3 were signiifcantly elevated in LSD1 knock-down PANC-1 cells. LSD1 interacts with PGC-1α, an important regulator of SIRT3 gene expression. LSD1 and PGC-1αoccupied the same region in SIRT3 promoter region through ChIP analysis. Luciferase activity assay validated LSD1 as a negative regulator of PGC-1αin SIRT3 gene transcriptional regulation. Conclusion:LSD1, as an important tumor promoter, negatively regulates the expression of tumor suppressor gene SIRT3, these results provide important clues for the role that LSD1 plays in aberrant metabolism and oxidative stress.
5.Clinical and Esophageal Dynamic Characteristics of Patients with Achalasia of Cardia Evaluated by Chicago Classification Criteria

BacKground:AchaIasia of cardia is categorized into 3 subtypes by Chicago cIassification criteria defined by high-resoIution manometry( HRM). These different subtypes present different cIinicaI and esophageaI dynamic characteristics. Aims:To study the cIinicaI and esophageaI dynamic characteristics of patients with achaIasia of cardia categorized by Chicago cIassification criteria. Methods:Twenty-five untreated achaIasia of cardia patients from January 2012 to ApriI 2014 at Beijing Friendship HospitaI,CapitaI MedicaI University were enroIIed. CIinicaI data incIuding cIinicaI symptoms, manifestations of endoscopy and barium meaI radiography and data of HRM were anaIyzed retrospectiveIy. Results:Of the 25 patients,5 patients(20. 0%)were cIassified as type Ⅰ,15(60. 0%)as type Ⅱ and 5(20. 0%)as type Ⅲ. AII patients compIained as having dysphagia,and 26. 7%( 4/15 ) of type Ⅱ patients had chest pain. Incidences of regurgitation in typeⅠ,ⅡandⅢwere 60. 0%(3/5),53. 3%(8/15)and 20. 0%(1/5),respectiveIy,no significant difference was found among the three types(P>0. 05). Incidences of diIatation of esophagus in type Ⅰ,ⅡandⅢwere 60. 0%(3/5),73. 3%(11/15)and 20. 0%(1/5),respectiveIy,no significant difference was found among the three types (P=0. 11). No differences in Iength of Iower esophageaI sphincter(LES),abdominaI LES Iength,LES resting pressure, upper esophageaI sphincter( UES)resting pressure and integrated reIaxation pressure( IRP)among the three types were found(P>0. 05). Incidences of muItipIe swaIIowing and spontaneous UES reIaxation were 32. 0%(8/25)and 24. 0%(6/25),respectiveIy,no significant difference was found among the three types(P>0. 05). Conclusions:Of the patients with achaIasia of cardia ,typeⅡis more common. Large-sampIe muIticenter studies are needed for further research.
6.Changes of cardiac energy metabolism and structure during ischemia/reperfusion injury of liver in rats

AIM:To investigate the effect and mechanism of liver ischemia/reperfusion(I/R)injury on the changes of cardiac energy metabolism and structure.METHODS:48 healthy Wistar male rats were randomly divided into 6 groups as follows(n=8 in each group):control group(CTL),simply ischemia for 30 min without reperfusion(I group);reperfusion following ischemia for 30 min(I/R group);2 h reperfusion following ischemia for 30 min(I/R 2 h group);4 h reperfusion following ischemia for 30 min(I/R 4 h group)and 6 h reperfusion following ischemia for 30 min(I/R 6 h group).The level of serum endotoxin was measured.The levels of insulin and insulin antibody in heart were detected by radioimmunoassay.The contents of MDA,MPO and lactic acid in heart were also determined.RESULTS:During the process of liver I/R injury,the level of endotoxin increased in I group and I/R group and declined gradually for long time during reperfusion,but was still longer than that in CTL group(P0.05).CONCLUSION:During the process of liver I/R injury,endotoxin is absorbed from intestine and impairment of liver detoxication leads to endotoxemia,which might play a role in the changes of the energy metabolism and structure in heart.
7.The influence of E-cadherin/β-catenin on the glycolysis effect in PANC-1 cells

Background and purpose:Lower expression of E-cadherin is associated with metastasis of cancer cells, however, the correlation between E-cadherin and glucose metabolism has seldom been reported. This article studied the correlation between E-cadherin and glycolysis effect in PANC-1 cells.Methods:Through treatment of transforming growth factor β (TGF-β) in PANC-1 cells to decrease E-cadherin expression, knock-down the gene of E-cadherin interaction protein β-catenin, and overexpressing of E-cadherin, the effects of E-cadherin on the glucose uptake and lactate production ability and on the expression of key glycolytic genes were assessed.Results:E-cadherin negatively regulated the glycolytic effect of PANC-1 cells by inhibiting glucose uptake and lactate production (P<0.05). Moreover, E-cadherin interacting partner β-catenin signiifcantly promoted glucose metabolism transformation in PANC-1 cells (P<0.05). Moreover, key glycolysis regulator sirtuin 3 (SIRT3) could lower E-cadherin expression.Conclusion:Lower expression of E-cadherin induced the transformation of glucose metabolism transformation in PANC-1 cells and manipulation of E-cadherin expression level could change the glycolysis effect. Moreover, through maneuver glycolysis process could inhibit high metastatic potential of pancreatic cancer cells.
8.Grade of membership analysis of multidimensional health status in adult twins

Objective To construct profiles of health status based upon physical,mental and social support items in adult twins of Qingdao.Methods Grade of Membership(GoM) model was applied to a set of 31 indicators to construct ideal profiles.Results Four health profiles were identified: pure type Ⅰ(healthy),pure type Ⅱ(personality disorders),pure type Ⅲ(psychological symptoms) and pure type Ⅳ(physiological symptoms).The most frequently occurring combination in this population was profile Ⅰ,Ⅱ,Ⅳ(14.74%),followed by profile Ⅰ,Ⅱ,Ⅲ,Ⅳ(13.44%),and then type Ⅰ(11.08%).Only 13.56% of subjects fell completely into one single pure type.Conclusions One healthy type and three non-healthy types are determined.Most individuals exhibit some of the characteristics of two or more types,holding partial membership in multiple categories.
9.Quantitative evaluation of the degrees of qualitative syndromes commonly encountered in patients with coronary heart disease.

Objective: To establish a quantitative model for evaluating the degree of traditional Chinese medicine (TCM) syndromes often seen in patients with coronary heart disease (CHD). Methods: Medical literature concerning clinical investigation of TCM syndromes of CHD was collected and organized, and the "Hall for Workshop of Metasynthetic Engineering" expert symposium method was applied. First, the 100 millimeter scaling was used for combining with scoring on degree of symptoms to establish a quantitative criterion for classification of symptom degree in CHD patients, and the model was established by using comprehensive analytic hierarchy process as the mathematical tool to estimate the weight of the criterion for evaluating qualitative syndromes in various layers by specialists. Then the model was verified in clinical practice and the outcomes were compared with fuzzy evaluation from the specialists. Results: A total of 287 clinical observation forms on CHD cases were collected, and 167 forms were available after excluding any irregular forms. The results showed that basic coincidence rate between the outcomes derived from specialists and those from the model was 68.26% (114/167), and part coincidence rate was 88.62%(148/167). Conclusion: This model, with good rationality and feasibility, has a high coincidence rate with fuzzy evaluation from specialists, and can be promoted in clinical practice. It is a good quantitative model for evaluating the degree of TCM syndromes of CHD.
10.Cerebrotendinous xanthomatosis: a family report and literature review

Objective:To summarize and review a Chinese family with cerebrotendinous xanthomatosis (CTX) so as to improve understanding of the disease.Methods:The proband was admitted to the Department of Neurology, the Second Hospital of Hebei Medical University on May 30, 2019. The medical history, neuro-imaging, pathology, CYP27A1 gene of the proband and CYP27A1 gene of her family were analyzed. Clinical features of similar cases from published literatures were retrieved and systematically summarized.Results:The proband was a 39-year-old female who was admitted to the Second Hospital of Hebei Medical University due to weakness of both lower limbs lasted for more than five years and aggravated for one year with speech slurred. The proband manifested with mental retardation, bilateral pyramidal tract impairment and cerebellar lesions, and had cholesterol crystal in xanthomas and compound heterozygous mutations of c.435G>A and c.1263+1G>A in CYP27A1 gene. The proband′s sister had the same mutation as the proband′s. The proband′s mother was the carrier of c.435G>A mutation, and father was the carrier of c.1263+1G>A mutation. Seventeen related cases concerning CTX with detailed clinical data were searched with major domestic databases. Combined with this case, clinical features with the frequency more than 50% were pyramidal sign, mental decline, ataxia, dysarthria, achilles tendon neoplasm, cataract, high arch foot.Conclusions:The onset of CTX is hidden, which can be diagnosed by its pathology and CYP27A1 gene detection. The possibility of CTX should be considered when there are unexplained clinical manifestations in common diseases such as pyramidal sign, mental decline, ataxia, dysarthria, achilles tendon neoplasm, cataract, high arch foot and so on.
