1.Expression and significance of kai1 and nm23 mRNA in esophageal carcinoma

Objective To investigate the expression of kai1 and nm23 mRNA in esophageal carcinoma, and to explore the clinicopathological significance of kail and nm23 mRNA in the progression of esophageal carcinoma. Methods To detect mRNA expression of kai1 and nm23 in 46 cases of esophageal carcinoma and 34 cases normal esophageal tissues closely adjacent to carcinoma by in situ hybridization.Results The incidence of kai1 (91.3 %>64.7 %, χ2 =8.65, P =0.003) and nm23 (43.5 %>5.9 %, χ2=13.86,P <0.001) in esophageal carcinoma was significantly higher than in normal esophageal tissues . The expression of nm23 was correlated with the pathological stages (r =-0.379, P =0.009) and lymph node metastasis (χ2 =6.47, P =0.011). The expression of mRNA had positive correlation between kai1and nm23 in esophageal carcinoma (r =0.298, P =0.007). Conclusion The expression of nm23 may play an important role in the occurrence, development and clinicopathological features of esophageal carcinoma. Combined detection of kai1 and nm23 may be helpful in further directing the clinical therapy of esophageal carcinoma.
2.Clinical Effect of Febuxosta on the Patients with Primary Gout Complicated with Mild to Moderate Renal Insufficiency

Objective:To explore the curative effect and safety of febuxosta for the patients with primary gout complicated with mild to moderate renal insufficiency.Methods:60 male patients with primary gout complicated with mild to moderate renal insufficiency were enrolled in the affiliated Hospital of integrated traditional Chinese and Westem Medicine,Nanjing University of Chinese Medicine from January 2015 to August 2016 and randomly divided into two groups,the control group (n=30) was treated by allopurinol,and the study group (n=30) was treated by febuxosta treatment.The blood uric acid (BUA) and hepatorenal function were compared in the treatment period,and the incidence of adverse reactions and the recurrence of gout in post-treatment were compared.Results:At 1,2 and 3 month after treatment,the serum level of BUN in the study group was significantly lower than that of the control group (P<0.05).The renal function of all patients were significantly improved compared with pre-treatment (P<0.05),but the no significant difference was found between two groups at the same time(P>0.05).The control group showed the adverse reactions with limb discomfort,drowsiness,nausea,abdominal distension,diarrhea and itch of skin,and the study group showed limb discomfort,drowsiness and abdominal distension,and one patients presenting allergic skin rash and discontinuation of allopurinol.The total incidence of adverse reactions in the study group was significantly lower than that of the control group (P<0.05).Conclusions:Febuxosta could effectively and safely decrease the blood uric acid levels and improve the renal function of patients with primary gout complicated with mild to moderate renal insufficiency.
3.Upper Limb Movement Deficiency in Children with Autism and Related Factors

Objective To explore the deficiency of upper limb movement in children with autism and its related factors. Methods 30 au-tism children aged 7-13 years old, and 30 healthy children aged 7-8 years from February to May, 2014 were enrolled. Their range of motion, comprehensive strength and cooperation, endurance, and muscular tension were tested. Results All the indexes of autism children lagged be-hind the healthy children (P<0.01), expect the muscular tension of the right upper limb (P>0.05). The endurance of boys was better than girls (P=0.020), and there was no difference in other aspects between them (P>0.05). The type of autism for more upper limb movement cor-related with obstruction of Jianjing acupoint (GB21) (r=0.515, P=0.013), and was significantly different with the type of less limb move-ment (χ2=8.533, P=0.003). Conclusion Compared with healthy children, the joint motion is relatively flexible, but lack of strength, and with poor persistence and unbalance development between left and right upper limbs. Motor rehabilitation should be conducted according to their movement features and the condition of meridian.
4.Clinical manifestations analysis of amiodarone-induced adverse reactions in our hospital from 2008 to 2014

Objective To analyze the occurrence characteristics and related factors of amiodarone-induced adverse re-actions (ADR)in our hospital so that we can provide reference for clinical safety,rational drug use. Methods Amio-darone-induced ADR reports of inpatient and outpatient in our hospital from January 2008 to June 2014 were collected,and amiodarone-induced ADR clinical manifestations, treatment and outcome and other information were statistically analyzed. Results In amiodarone-induced ADR reports of the reported 120 cases in our hospital, ADR proportions of men and people with advanced age, original cardiovascular diseases or injections, were slightly high ,and there was a low incidence of severe ADR (16.67%). The main clinical manifestations were liver damage (36.67%), phlebitis(30.83%),allergic-like reactions and anaphylactic shock(11.67%),rash(7.50%),hypotension and arrhythmias(5.83%),thyroid dysfunction (4.17%) and lung damage (3.33% ). After drug reduction, drug withdrawal, active clinical treat ment and other treatments, 35 cases (29.17%) cured and 76 cases (63.33%) improved, with 8 cases (6.67%) of sequelae and 1 case of death (0.83%). Conclusion Amiodarone is a kind of relatively safe antiarrhythmic drugs, but its ADR manifestations are various. We should restrict indications and closely monitor ADR in order to reduce the oc-currence of serious adverse events in clinical work.
5.Pregnancy outcome with Rh blood group alloantibody: analysis of 71 pregnant women

【Objective】 To retrospectively investigate the antibody distribution and pregnancy outcome of pregnant women with Rh alloantibody in Guangzhou, and summarize the prevalence, diagnosis and treatment experience of hemolytic disease of newborn (HDN) caused by Rh alloantibody, so as to provide data for the prevention, diagnosis and treatment of Rh-HDN. 【Methods】 A total of 17 345 pregnant women in Guangzhou from January 2014 to December 2020 were selected for irregular antibody screening test.Those with Rh antibody were followed up for delivery, and the clinical and laboratory examination results of pregnant women and newborns were analyzed. 【Results】 A total of 71 cases (0.41%, 17/17 345) with Rh alloantibodies were detected.Among them, anti-D, anti-E, anti-Ec, anti-C, anti-Ce, anti-c, anti-e accounted for 26.76% (19/71), 46.48% (33/71), 9.86% (7/71), 7.04% (5/71), 5.63% (4/71), 2.82% (2/71) and 1.41% (1/71), respectively.Among the 71 pregnant women, 34 gave birth to children with HDN, with the total prevalence rate of 47.89%, among whom 100%, 78.94% and 42.42% were anti-c, anti-D and anti-E, respectively.And 71.43% (5/7) of the children who underwent transfusion were with anti-D.Although the yield rate of anti-E was the highest, it involved low morbidity and mild symptoms, which preferred to occur in the first fetus.No significant difference in gestational age, birth weight and the occurrence time of jaundice was notice between the anti-D group and anti-E group, but the total bilirubin of the anti-E group was lower while the Hb level were higher than those of the anti-D group (P<0.05). Two children died, and others were cured by phototherapy, albumin, IVIG and blood transfusion. 【Conclusion】 The publicity of Rh-HDN for early prevention and treatment should be strengthened to improve the cure rate and the prognosis.
