Regional arterial intra-chemotherapy (RAIC) was an effective method of combined therapy for pancreatic carcinoma, which could elevate the regional con-centration of anticancer agents for sufficient dosage in pan-creas, reduce the systemic adverse events, increase the toler-ation of chemotherapy.It could also increase the concentra-tion of anticancer agents in regional lymph node of pancreas, thus decrease the lymph node metastasis.The mechanism of RAIC for pancreatic cancer and the impact for lymph node metastasis needed to be further investigated.

Objective:To investigate the cognitive status of socialist core value among medical students, and to provide reference for the core value education in medical colleges and universities. Methods: 1024 medical students were selected with random cluster sampling method, and the socialist core value was investigated using self-designed questionnaire. Results:It was shown that 69 percent of medical students recognized the importance of socialist core values, but the contents had not been mastered. Social atmosphere, medical student's own short-coming and school education were all the factors leading to the weakness of socialist core values. Conclusion:It is quite necessary to cultivate medical students′socialist core values. Class, school, and society should play the main role to guide the medical students to establish the correct socialist core value.

Objective Study the effect of hysteroscopic surgery in treatment of small mass of caesarean scar pregnancy(CSP).Methods From January 2000 to January 2013,61 cases lesions ≤2.5 cm diameter of endogenous CSP undergoing treatment in Luohe Central Hospital were studied retrospectively.According to different treatment,they were divided into 21 cases in hysteroscopic surgery group,23 cases in methotrexate + operation group and 17 cases in bilateral uterine artery embolism + curettage group.Intraoperative blood loss,operative time,hospitalization time after treatment and β-hCG to normal time were compared among those three groups.Results The surgical blood loss,hospitalization time after treatment,β-hCG to normal time were (49.8 ± 6.2) ml,(3.5 ± 0.8) days,(21.2 ± 2.4) days in hysteroscopic group,(87.0 ±30.5) ml,(12.5 ± 1.0) days,(29.6 ±2.2) days in methotrexate + operation group,the difference was statistically significant (P ＜ 0.05).The operation time were (33 ± 4) minutes in hysteroscopic surgery group and (35 ± 6) minutes in methotrexate + operation group,which did not reached significant difference (P ＞ 0.05).Length of hospital stay after treatment of hysteroscopic surgery group is less than the bilateral uterine artery embolism + curettage group significantly (P ＜ 0.05).Operation time,surgical bleeding and β-hCG to normal time had no obvious difference between hysteroscopic surgery and in bilateral uterine artery embolism + curettage group (P ＞ 0.05).Conclusion Hysteroscopy surgery in treatment of small mass endogenous CSP is effective,shorter hospitalization time,quick recovery.

Objective To investigate whether clinical,endocrine,and sonographic characteristics of infertile women with polycystic ovary syndrome (PCOS) could predict the ovarian response to clomiphene citrate (CC) medication.Methods A prospective study was conducted,85 women with PCOS were given CC 50 mg per day for 5 days.In the case of an absent response,doses were increased to 100 daily in subsequent cycles.Clinical,endocrine,and sonographic characteristics were compared between responders and nonresponders.Results The cycle history (oligomenorrhea or amenorrhea) and antral follicle count (AFC) were significantly different in responders from those in nonresponders (P ＜ 0.05).There were no differences in age,serum testosterone,follicle stimulating hormone (FSH),luteotropic hormone (LH),body mass index (BMI),insulin resistant index (HOMA-IR),and glucose and insulin levels between the two groups (P ＞ 0.05).In addition,multivariate logistic regression analysis indicated that cycle history and AFC were the predictors of ovarian response to CC treatment.Conclusions The cycle history and AFC can be used as the simple and effective parameters to predict ovarian response to CC treatment in PCOS patients.

Objective:To investigate the medical students' cognition of ethical issues in assisted reproductive technology (ART) and discuss the importance of ethical education in medical colleges and universities.Methods:A questionnaire survey about the ethical cognition of assisted reproductive technology was conducted among 632 medical students.The questionnaire included the donation and management of gametes and embryos,and offspring problems.Results:Medical students agreed with most of the current ART-related laws and regulations.however,44.8％ of medical students considered that criteria of donating eggs should be relaxed,such as allowing women who do not receive ART treatment be paid for eggs.In addition,a total of 39.0％ of medical students supported the legalization of surrogacy technology in china.Conclusion:Medical students exists a certain misunderstanding in egg donation and surrogacy technology.We should give more training to medical students in ethical knowledge.In addition,our country can supplement and perfect the current laws and regulations to a certain extent.

This paper introduces reformation of course system for five-year clinical medicine specialty in three aspects: orientation of course arrangement ,basic structure and major features of new teaching model. By adjusting the teaching plan and setting up new course system optimally, we have made an active attempt on new teaching model for bringing up high-quality medical talent.

Objective To investigate the relationship between the ratio of neutrophils to lymphocyte and prehypertension and its clinical significance. Methods In May 2015,three thousand one hundred and twenty-six subjects who underwent physical examination in Tangshan People's Hospital Health Examination Center were enrolled in the study. 120 cases of pre hyperton were selected as the observation group,and 120 with normal blood pressure were used as the control group. The neutrophils and lymphocyte counts of the subjects were detected. The relationship among the two groups of test indicators,the ratio of neutrophils to lymphocyte counts and blood pressure was analyzed. Results The ratio of neutrophil and lymphocytes in the observation group was (2. 28± 0. 39), significantly higher than that in the control group ( 1. 41 ± 0. 32) ( t＝ 1. 196 P ＝0. 028). According to the different ratio of neutrophils / lymphocytes,the patients were divided into 3 groups,group A (1. 12 ± 0. 23), group B ( 1. 73 ± 0. 08), group C ( 2. 57 ± 0. 39), aling with the ratio increasing, the blood pressure increased,the systolic blood pressure of group A,B,C was (115±9) mmHg,(117±11) mmHg,(128 ±6) mmHg,respectively. The difference was statistically significant (F＝4. 404,P＝0. 013),the diastolic blood pressure of group A,B,C was (73±5) mmHg,(78±7) mmHg,(86±4) mmHg,respectively. The difference was statistically significant (F＝4. 553,P＝0. 027). Multivariate Logistic regression analysis showed that the ratio of neutrophils to lymphocytes was affected by systolic blood pressure,blood glucose,high density lipoprotein,BMI and triglyceride. Conclusion There is correlation between the ratio of neutrophil to lymphocyte and prehypertension, suggesting that inflammation may be involved in the occurrence and development of hypertension.

Objective: To investigate the clinical and histological features, diagnosis and differential diagnosis of myofibroma/myofibromatosis. Methods: The clinical data and pathology features of nine cases of myofibroma/myofibromatosis were collected from August 2011 to November 2016 in Affiliated Drum Tower Hospital, Nanjing University Medical School and Children′s Hospital of Nanjing Medical University. Immunohistochemistry(IHC), PDGFRB molecular analysis and ETV6-NTRK3 gene fusion were performed and relevant literature reviewed. Results: There were 7 males and 2 females, with age ranging from 3 days to 18 years (mean 5 years). The tumors were located in head and neck (eight cases) and trunk (one case). Clinically, the tumors presented as freely movable nodules. Microscopically, they appeared biphasic with alternating light- and dark-staining areas. The light-staining area consisted mainly of plump myoid spindle cells with eosinophilic cytoplasm arranged in nodules, short fascicles, or whorls.The dark-staining area was composed of round or polygonal cells with slightly hyperchromatic nuclei or small spindle cells arranged around a distinct hemangiopericytoma-like vascular pattern. IHC showed the tumor cells in the light-staining area were strongly positive for vimentin and SMA, while cells in dark-staining area were strongly positive for vimentin, and weakly for SMA. Tumor cells were negative for desmin, S-100 protein, h-Caldesmon, CD34 and STAT6. Analysis of PDGFRB mutations was performed in seven cases. Two cases showed 12 exon point mutation c. 1681 c>T(p.R561C), one case showed 14 exon point mutation c. 1998C>G (p.N666K). ETV6-NTRK3 gene fusion was not detected by fluorescence in situ hybridization in four patients under three years old. All cases were followed for 6 to 68 months, with two recurrences. Conclusions Myofibroma/myofibromatosis is an uncommon benign myofibroblastic tumor of infancy and childhood. The tumor can appear biphasic, and may show PDGFRB point mutation which is of potential diagnostic value.

Objective To explore the color preference in hospitalized patients with mental disorders and healthy adults. Methods Abstract color preference test was conducted in 1951 clinic patients with mental disor-ders and 1005 healthy adults by use of dual comparison method. Results The order of color preference for pa-tients with mental disorders was blue(4.72),green(4.67),red(4.52),yellow(4.46),orange(4.38),purple (3.92),gray (3.56),white (3.43) and black (2.96). There were significant differences between patients with mental disorders and healthy adults in preference to purple(Z = 2.934,P = 0.003),black(Z = 3.730,P =0.000),orange(Z =-2.113,P = 0.035),yellow(Z =-2.093,P = 0.036),white(Z =-2.499,P = 0.012) and gray(Z=-3.204,P=0.001). The preference to black(χ2=22.409,P<0.001)and gray(χ2=134.086, P < 0.001) differed significantly among schizophrenia , mood disorders and neurosis. Schizophrenia patients mostly preferred blue,mood disorders patients mostly preferred red,and neurotic patients mostly preferred green. Conclusions The color preference in hospitalized patients with mental disorders is different from that in normal adults. There are differences in color preference among patients with different types of mental disorders.

Objective:To construct a biospecimen bank of patient derived organoids (PDOs) from pancreatic cancer tissues and to explore the feasibility of PDOs drug sensitivity assay technology to guide chemotherapy drug selection for pancreatic cancer.Methods:Pancreatic cancer tissue specimens obtained after surgical resection and puncture biopsy from Mar 2020 to Dec 2022 at Drum Tower Hospital, Nanjing University School of Medicine were collected. Pancreatic cancer PDOs were cultured in vitro and histologically identified; PDOs were treated with gemcitabine, Nab-paclitaxel, fluorouracil, Oxaliplatin, and Irinotecan and cell viability was measured to analyze the correlation between PDOs drug sensitivity and the actual clinical treatment response.Results:The PDOs can reproduce the pathological features of corresponding tumor tissues; the sensitivity of different PDOs to the same chemotherapeutic drug is significantly different; The sensitivity of PDOs was highly consistent with the actual treatment effect of the corresponding patients 75.76% (25/33); organoid organ-based susceptibility testing had predictive value for the treatment response of patients (AUC=0.733, 95% CI: 0.546-0.919, P<0.05). Conclusion:A biobank of pancreatic cancer PDOs was successfully constructed, and the drug susceptibility test results were significantly correlated with the actual medication response of patients, suggesting that the drug susceptibility test technology based on PDOs has the potential to guide individualized chemotherapy for pancreatic cancer.
