1.Study on the anti-inflammatory and analgesic effects of xiangqingzhitong spray

Objective Xiangqingzhitong spray ( XQ spray) is a kind of class V new Chinese drugs .This paper studied the anti-inflammatory and analgesic effects of XQ spray to provide experiment evidence for clinical application . Methods Mice were divided into blank control A group , yunnan baiyao aerosol A group , voltaren A group and low , middle and high dose A group of XQ spray.Analgesic effects were studied by observing effect of XQ spray for pain threshold in electrical stimulation test .Mice were divided into blank control B group , yunnan baiyao aerosol B group , voltaren B group and low , middle and high dose B group of XQ spray .Analgesic effects were studied by observing the effects of XQ spray for pain threshold in tail pain tenderness test .Mice were divided into blank control C group , yunnan baiyao aerosol C group , voltaren C group and low,middle and high dose C group of XQ spray .Anti-inflammatory effects were observed by the models of ear swelling induced by xylene in mice.Rats were divided into blank control D group , yunnan baiyao aerosol D group , voltaren D group and low, middle and high dose D group of XQ spray .Anti-inflammatory effects were observed by the models of paw edema induced by carrageenin in rats . Results Electrical stimulation test showed that middle and high dose A group of XQ spray could enhance the pain threshold and the analgesia percentage reached 90%after the administration of 15 min and 30 min.Compared with control group , significant difference was found in medium and high dose A group (P<0.05).High dose B group of XQ spray could enhance pain threshold in tail pain tenderness test markedly after the administration of 15 min and 30 min and the difference was significant compared with control group ( P<0.01).Middle dose C group could inhibit the ear swelling induced by xylene in mice and the difference was significant compared with control group ([4.41 ±4.55]mg vs [9.15 ±3.94]mg,P<0.05).High dose C group could inhibit the ear swelling induced by xylene in mice and significant difference was found compared with control group ([3.62 ±3.56]mg vs [9.15 ±3.94] mg,P<0.01). High dose D group could inhibit the paw edema induced by carrageenin in rats after the administration of 30 min and there was signifi-cant difference compared with control group ([1.19 ±0.94]mg vs [2.54 ±1.28]mg,P<0.01). Conclusion XQ spray has sig-nificant anti-inflammatory and analgesic effects .
2.Literature analysis on drug-induced diseases of vital organs caused by traditional Chinese medicine injection

ObjectiveTo study the occurrence and influence factors of drug-induced injuries of vital organs caused by traditional Chinese medicine injection.MethodsChina Hospital Knowledge Database (CHKD) from 1993-2013 and case reports concerning adverse effects and drug-induced diseases caused by traditional Chinese medicine injection from Wanfang Data Knowledge Service Platformwere retrieved. Indicator systemwasestablished and relevant contentsweresummarized and analyzed.Results34 types of traditional Chinese medicine injectionswereincluded and 699 drug-induced diseasesweresummarized, taking up 53.98% of the total adverse effects. Among them, the top three included in the drug-induced diseaseswere acanthopanax injection, safflower injection and Mailuoning injection. The non-conformance between the traditional Chinese medicine injections in the literature and instructions mainly reflects unreasonable solvent selection, large compatibility concentration and usage and dosage beyond those specified in the instructions. ConclusionThe occurrence of drug-induced injuries of vital organs caused by traditional Chinese medicine injectionis relatively high, drug instructions shall be strictly followed in accordance with requirements during clinical application.
3.Laboratory detection indicators and pregnant information for predicting severe preeclampsia in pregnant women

Objective To use the laboratory detection indicators and pregnant information to predict severe preeclampsia in preg-nant women.Methods 80 pregnant women with severe preeclampsia were collected as the disease group,and 444 cases were select-ed as the control group.Their laboratory detection indicators and pregnant information were compared.between the two groups.The Logistic regression and the ROC curve were used to analyze the predictive value of each indicator for predicting severe preeclamp-sia.Results BMI,MAP,HCT,PLT in the first trimester of pregnancy and BMI,MAP in the second trimester of pregnancy were positively correlated with severe preeclampsia.HCG and PAPP-A in the first trimester of pregnancy were negatively correlated with severe preeclampsia.The single indicators with large predicting value for severe preeclampsia included PAPP-A in the first trimester and MAP in the first and second trimesters of pregnancy.The combining MAP in the first and second trimester of pregnancy had the largest predictive value.Conclusion PAPP-A in the first trimester and MAP in the first and second trimester of pregnancy can predict subsequent severe preeclampsia.
4.Analysis of Influential Factors for Clinical Rational Use of Chinese Patent Medicine

OBJECTIVE:To provide reference for rational use of Chinese patent medicine. METHODS:The influencing fac-tors of rational use of Chinese patent medicine was investigated in respects of the selection of Chinese patent medicine,drug combi-nation,usage and dosage,clinical evaluation,pharmacoeconomic evaluation. RESULTS&CONCLUSIONS:Chinese patent medi-cine is a characteristic product of traditional Chinese medicine. It is a complex and important issue to use Chinese patent medicine rationally. It should be considered in many ways,not only pay attention to the principle of clinical use of Chinese patent medicine, but also need to carry out clinical evaluation of Chinese patent medicine;at the same time,the economics of Chinese patent medi-cine should be focused on.
5.Expression of IL-8 and CXCR2 on Keratinocytes from Psoriatic Lesions

Objective To investigate the expression of IL-8 and CXCR2 on keratinocytes from psoriatic lesions and their roles on clinical and pathologic manifestations. Methods The chemotaxis of psoriatic lesional keratinocytes was detected by micropore loculus test. The concentration of IL-8 was determined in the cultured supernatants of psoriatic keratinocytes by ELISA. The expression of CXCR2 on keratinocytes from affected skin was tested by flow cytometry. Results The chemotaxis for neutrophils by the cultured supernatants of psoriatic lesional keratinocytes was significantly stronger than that by controls. The concentration of IL-8 in the cultured supernatants of psoriatic lesional keratinocytes was also increased. The expression of CXCR2 on psoriatic keratinocytes was significantly increased. Conclusions The psoriatic epidermal hyperproliferation may be correlated with up regulation of IL-8 production and CXCR2 expression on psoriatic keratinocytes. At the same time, the psoriatic inflammation may be partly related to the increase of secretion of IL-8, which has chemotactic capacity, by keratinocytes. IL-8 and CXCR2 may be involved in the pathogenesis of psoriasis.
6.Association of radial arterial calcification damage with bone mineral density and bone metabolism biomarkers in uremia patients

Objective To investigate the association of radial arterial calcification damage with bone mineral density (BMD) and bone metabolism biomarkers in uremia patients.Methods Sixty-seven incident hemodialysis patients were recruited into uremic group.Serum creatinine,calcium,phosphorus,lumbar spine and femoral neck BMD were measured.Parathyroid hormone (iPTH),25OHD,1,25(OH)2D,fibroblast growth factor (FGF) 23,bone specific alkaline phosphates (BAP) and osteocalcin (BGP),type Ⅰ collagen pyridine crosslinked C-telopcptidc (ICTP) were detected.Radial artery calcification was analyzed by von Kossa staining and transmission electron microscopy.Arterial type Ⅰ collagen (Col Ⅰ) expression was examined.Twenty-three healthy cases received serum and BMD examination only as control.Results Uremic patients presented higher serum phosphate,iPTH,FGF23,lower serum calcium,25OHD,1,25 (OH)2D (all P < 0.05),and lower lumbar spine and femoral neck BMD (all P < 0.01) compared to controls.Significant calcium deposit was observed in radial arteries in 24 uremic cases (35.8%),including 10 cases of diabetes.Immunohistochemistric assay confirmed that Col Ⅰ expression increased around calcification site and electron microscope revealed that more calcium and phosphorus plaque attached among collagen fibers.No correlation was showed between iPTH and radial artery calcification (r =-0.08,P =0.306),but after stratified by iPTH levels,correlation of iPTH and calcification was found in low iPTH (< 150 ng/L) group and high iPTH group (> 300 ng/L) (r =-0.41,0.31,P=0.044,0.023).Diabetes,lumbar spine and femoral neck BMD,ICTP,FGF23 were correlated with arterial calcification (r =0.62,-0.25,-0.43,0.34,0.86,P =0.000,0.001,0.012,0.018,0.000).Multiple regression analysis showed femoral neck BMD,ICTP,FGF23 levels were independently associated with radial arterial calcification (β =-0.221,0.181,0.260,P =0.021,0.024,0.036).Conclusion In uremic patients,reduced BMD,abnormal bone turnover rate,especially accelerated bone reabsorption,and increased serum FGF23 level are independently associated with radial artery calcification.
7.Medial tentorial dural arteriovenous fistula with venous sinus thrombosis leading to thalamic dementia: a challenging diagnosis of 4 cases

Objective:To investigate the imaging characteristics and diagnosis of thalamic dementia caused by medial tentorial dural arteriovenous fistula (TDAVF).Methods:The clinical and imaging data of 4 patients with medial TDAVF diagnosed at Nanjing Brain Hospital from August 2022 to September 2023 were retrospectively collected, summarized and analyzed.Results:Four patients were all males, with an average age of 67 years. The duration of cognitive impairment varies from 13 days to 2 months, with an average of about 1 month. The average score on the Mini-Mental State Examination (MMSE) was 13, while the average score on the Montreal Cognitive Assessment (MoCA) was 15. One patient was unable to complete these tests. MRI showed bilateral thalamic swelling, with scattered microbleeding lesions on diffusion-weighted imaging and susceptibility-weighted imaging ( n=3). Enhanced scans showed patchy or clumpy enhancement of the bilateral thalamus. Magnetic resonance angiography showed abnormal blood vessels along the straight sinus area ( n=4), while magnetic resonance venography showed no straight sinus development ( n=4). Magnetic resonance spectroscopy showed neuronal swelling and damage ( n=3), with no increase in choline peak. Arterial spin labeling perfusion imaging showed decreased perfusion in the lesions. Digital subtraction angiography confirmed TDAVF with straight sinus thrombosis. Two patients underwent embolization, with significant improvement in postoperative memory impairment. One patient underwent surgical clipping, while the other was transferred to other hospital for treatment. Conclusions:Patients with medial TDAVF often present with thalamic dementia. Imaging examinations show typical bilateral thalamic swelling with microbleeds, and early visualization of vascular shadows, without development of straight sinus. These features would be beneficial for the early diagnosis of TDAVF.
8.Application of multidisciplinary team model on prevention of perioperative pulmonary infection among elderly patients

Objective? To explore the effects of multidisciplinary team (MDT) model on prevention of perioperative pulmonary infection among elderly patients. Methods? From August 2017 to May 2018, we selected 162 elderly patients with surgery in Department of Gastrointestinal Surgery at Renji Hospital affiliated to School of Medicine, Shanghai Jiaotong University. All of the patients were divided into groups according to hospitalization order. A total of 88 patients from August to December 2017 were in control group, and 74 patients from January to May 2018 were in intervention group. Control group provided routine nursing. On the basis of routine nursing, intervention group carried out the MDT intervention model. We compared the incidence of perioperative pulmonary infection and hospital stay of patients between two groups. Results? The incidence of perioperative pulmonary infection of intervention group (4.1%) was lower than that (13.6%) of control group with a statistical difference (P<0.05). The hospital stay of patients of intervention group (8.502±2.542) days was less than that (9.342±2.343) days of control group also with a statistical difference (P<0.05). Conclusions? MDT intervention can reduce the perioperative pulmonary infection among elderly patients and shorten the average hospital stay of patients.
9.Correlation between shared leadership and male nurses' identity perception

Objective To understand the status quo of shared leadership and identity perception among male nurses,and to analyze the correlation between them.Methods By convenient sampling,a total of 210 male nurses from 6 ClassⅢ hospitals in Zhejiang Province were investigated with Shared Leadership Questionnaire and Perceived Insider Status Scale from August to September 2017.Pearson correlation analysis was used to analyze the correlation between shared leadership and identity perception.A total of 210 questionnaires were distributed,and 204 valid questionnaires were collected.Results The scores of shared leadership and identity perception of these male nurses were (59.86±11.45) and (17.69±4.59) respectively,and there was a positive correlation between the two (r=0.271,P< 0.05).Conclusions The level of shared leadership and identity perception of male nurses is lower than middle level,and shared leadership level affected their identity perception level.It is suggested that hospital managers take measures to ensure that male nurses share leadership level in order to improve their level of identity perception and promote the retention and development of male nurses.
10.An approach to screen fetal agenesis of the corpus callosum at 11-13(+6) weeks.

OBJECTIVETo detect structural changes in the brain in fetuses with agenesis of the corpus callosum (ACC) and holoprosencephaly (HPE) in the first trimester.
METHODSThe ultrasound data were analyzed retrospectively in 620 normal singleton fetuses between 11 and 13(+6) gestational weeks, 5 fetuses diagnosed to have ACC, and 13 fetuses with HPE. The midbrain diameter (MD) and falx diameter (FD) were measured and their ratio (MD/FD) was calculated for comparative analysis.
RESULTSNo significant difference was found in the MD, FD, and MD/FD ratio between fetuses with ACC and HPE (P>0.05). Compared to the normal fetuses, all the fetuses with ACC and HPE showed significantly increased mean MD and MD/FD ratio (P<0.05); 4 (80%) fetuses with ACC and 11 (84.6%) with HPE had a reduced FD. All the fetuses with ACC and HPE had MD/FD ratios greater than 1, which were below 1 in all the normal fetuses.
CONCLUSIONIn the first trimester, fetuses with ACC and HPE have measurable abnormalities in the midbrain and falx area of the brain, and these changes, represented by abnormal midsagittal MD, FD and their ratio, can be of value in detecting ACC or HPE in fetuses in the first trimester.
