2.Establishment of nursing specialist team on nutrition support and cultivation of nutrition support nurse

Objective To regulate nutrition support nursing, facilitate the development of nursing specialty, cultivate nutrition support nurse. Methods Establish nursing specialist team on nutrition support. 30 nurses received theory training, skill training, clinical practice, and completion reporting. Conduct nutrition knowledge management survey. Establish standardized wards of nutrition support nursing and conduct special inspection. Results Nursing specialist team on nutrition support was established, and 29 nutrition support nurses were cultivated. Before training, nurses' knowledge of nutrition and nutrition nursing management score were (3.46 ± 0.47), (0.70 ± 0.24) points, and the score were (4.63±0.14), (0.90±0.27) points after training, results was statistically significant (t=12.848, 2.981, P<0.01). Before and after special inspection of enteral nutrition, enteral nutrition pump and special infusion tube cases were 106,60 and 106, 60, results were statistically significant (χ2=4.747, 4.471, P<0.05). Before and after special inspection of parenteral nutrition, correct infusion and speed cases were 90, 92 and 61, 54, results were statistically significant (χ2=4.642, 7.644, P<0.05 or 0.01). Conclusions The development of nursing specialty of nutrition support was promoted, nurses′ comprehensive ability and self-identity was improved, management quality of nutrition support was.
3.Clinical application of MR diffusion kurtosis imaging in spastic hemiplegic cerebral palsy

Objective To explore the clinical applicability of diffusion kurtosis imaging (DKI)in children with spastic hemiplegic cerebral palsy.Methods 22 children with spastic hemiplegic cerebral palsy and age-matched 22 normal children were selected in this study,and were treated with routine brain MRI and DKI to test the white matter area of tractus corticospinalis on brainstem plane, anterior limb of internal capsule,posterior limb of inter capsule,genu of corpus callosum and the splenium of corpus callosum.The fractional alanisotropy(FA)value and mean kurtosis (MK)value of deep gray nuclei area in thalamus,lenticular nucleus and head of caudate nucleus were examined.Furthermore,gross motor function classification system (GMFCS)was performed for the children in research group.Two independent sample t test was applied in the comparison of groups.The correlation between DKI parameters and cerebral palsy gross motor function was analyzed by Spearman analysis.Results As for MK value,there were statistically significant differences in all different brain areas between the research group and the control group(P<0.05).And for FA value,there was statistically significant difference only in white matter area between the research group and the control group (P<0.05).However,there was no statistically significant difference in cerebral deep gray nuclei area (P>0.05).The GMFCS score was negatively correlated with the FA value,and was positively correlated with the MK value.Conclusion DKI can reflect the damaged situation of gray matter in children with cerebral palsy,and its parameter values can be used to monitor the children with cerebral palsy as evaluation indicators.
4.Distribution of vascular endothelial growth factor gene polymorphism in Uyghur Ethnic patients with urolithiasis in south Xinjiang

Objective To determine vascular endothelial growth factor(VEGF)-460 gene polymorphism in Uyghurs and its relationship to urolithiasis in south Xinjiang. Methods Polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP),gene sequencing and genetic analysis methods were used in 200 urolithiasis patients of Uyghurs, and 200 healthy Uyghurs. Results The distribution of genotype and allele had no significant difference between urolithiasis patients and normal controls (P>0. 05). The frequencies for the CC,TT and CT genotypes in patients with urolithiasis and normal controls were 1.5 %, 29.0 %, 69.5 % and 0. 5 %, 27.5 %, 72.0 %, respectively. The frequencies for C and T allele were 36.2%,63.7% and 36.9% ,63.1%, respectively. Conclusions The results of VEGF-460 gene polymorphisms indicate no significant relationship between patients with turolithiasis and normal controls in Uyghurs in south Xinjiang,which may not be urolithiasis susceptibility genetic locus.
5.Classification of main pancreatic duct and treatment strategy after linear stapler closure of pancreatic neck in laparoscopic pancreaticoduodenectomy

Objective:To investigate the classification of main pancreatic duct and treatment strategy after linear stapler closure of pancreatic neck in laparoscopic pancreaticoduodenectomy (LPD).Methods:The records of 51 consecutive patients with LPD who were treated by linear staple closure technique of pancreatic neck from February to December 2022 from Binzhou Second People′s Hospital, Shijingshan Campus, Beijing Chaoyang Hospital, Capital Medical University, Rizhao Hepatobiliary-Pancreatic-Splenic Surgery Research Institute, Chaoyang Central Hospital, Shandong Juxian People′s Hospital, Weihai Municipal Hospital, Binzhou Central Hospital, and Affiliated Hospital of Chifeng University were retrospectively reviewed. According to the visibility, position and diameter of the main pancreatic duct at the stump of the pancreas, the type of main pancreatic duct was divided into type I, type Ⅱ, type Ⅲa and type Ⅲb. The number of cases in each main pancreatic duct classification and the corresponding treatment strategies were examined.Results:A total of 51 cases of LPD were successfully completed. Of these patients, the males comprised 56.9%(29/51), and females comprised 43.1%(22/51), with age ranging from 31 to 88 years old. The type of the main pancreatic duct at the stump of the pancreas included 7 cases (13.7%) of type Ⅰ, 39 cases (76.5%) of type Ⅱ, 2 cases (3.9%) of type Ⅲa, and 3 cases (5.9%) of type Ⅲb. Corresponding treatment strategies were adopted according to different main pancreatic duct types, the main pancreatic duct was successfully found, and a support drainage tube was inserted.Conclusion:After linear stapler closure of pancreatic neck, corresponding treatment strategies should be adopted according to the classification of the main pancreatic duct, which would help to improve the success rate of finding the main pancreatic duct and placing a support drainage tube.
6.Comparison of Protective Effects between Aboveground and Underground Parts of Astragalus membranaceus on Immunosuppression Model Mice

OBJECTIVE:To compare the protective effects model mice between the aboveground and underground parts of Astragalus membranaceus on immunosuppression ,and to provide reference for further utilization and development of A. membranaceus. METHODS :A total of 240 ICR mice were divided into 4 batches,60 mice in each batch ,with half male and half female. Each batch of mice were randomly divided into blank group ,model group ,A. membranaceus aboveground part and undergroud part low-dose and high-dose groups (3,6 g/kg,by crude drug )according to body weight and sex ,with 10 mice in each group. Blank group and model group were given normal saline intragastrically. A. Membranaceus groups were given corresponding concentration of drug intragastrically ,10 mL/kg,once a day ,for consecutive 30 days. Except for blank group , other groups were intraperitoneally injected with cyclophosphamide 40 mg/(kg·d)for consecutive 3 days,since 24th day of treatment,to establish immunosuppression model. The levels of serum immunoglobulin (IgG,IgM,IgA),inflammation factors [nitric oxide ,interleukin-2(IL-2),IL-6,tumor necrosis factor-α] and half hemolysis value were detected in each group. Body weight ,thymus index ,spleen index ,phagocytic index ,activity of natural killer (NK)cell,splenic lymphocyte proliferation ability,dinitrofluorobenzene-induced delayed metamorphosis reaction in mice (by weight difference between left and right ears ) and the number of hemolytic plaque were determined. RESULTS : Compared with blank group , the serum levels of immunoglobulin,body weight ,thymus index ,spleen index ,phagocytic index ,NK cell activity ,the proliferation ability of splenic lymphocyte,the number of hemdytic plaque and half hemolysis value were decreased significantly in model group (P<0.05), while inflammation factor level as well as weight difference between left and right ears were increased significantly (P<0.05). Compared with model group ,above indexes of mice in A. membranaceus groups were improved significantly ,in dose-dependent manner(P<0.05). Compared with A. membranaceus undergroud part group ,above indexes of A. membranaceus aboveground part group were improved significantly (P<0.05). CONCLUSIONS :Aboveground and underground part of A. membranaceus both have pretective effect on immunosuppression model mice ,and the effect of aboveground part of A. membranaceus is stronger than underground part of A. membranaceus .
7.Changes of expression of miR-155 in colitis-associated colonic carcinogenesis.

OBJECTIVETo investigate the changes of miR-155 and its target genes in colitis-associated carcinogenesis.
METHODSColitis-associated colon cancer was induced by azoxymethane (AOM) and dextran sulfate sodium (DSS) in C57BL/6 mice. Mice of three different stages during the development of colon cancer were obtained, named AD1, AD2 and AD3, respectively. A control group of mice without any treatment and a DSS only group representing chronic inflammation without cancer were set up as well. Colon tissue was collected and expression of miR-155 in the colon tissues was measured by real-time fluorescent quantitative PCR. TargetScan and PicTar were used to predict potential target genes of miR-155, which were then preliminarily screened with our gene expression microarray database of AOM-DSS mouse model. Regular PCR was used to confirm the changes of the expression of these potential target genes in AOM-DSS mouse model.
RESULTSColitis-associated colon cancer was effectively induced by azoxymethane and dextran sulfate sodium in C57BL/6 mice. Histological examination revealed that the evolution process was sequentially from normal, mild dysplasia, moderate dysplasia, and severe dysplasia to adenocarcinoma in the AOM-DSS mouse model. The level of miR-155 was gradually elevated with the formation of colitis-associated colon cancer. There was no significant difference between the levels of miR-155 expression in the DSS group (0.005 6 ± 0.003 7) and control group (0.012 0 ± 0.005 1) (P > 0.05), but the level of miR-155 in the AD3 group (0.054 4 ± 0.027 0) was significantly higher than that of the DSS group (0.005 6 ± 0.003 7)(P < 0.01). No significant change of miR-155 expression was found in the DSS only group. The relative expression levels of miR-155 in the control group, DSS only group and AD3 group were 0.012 0 ± 0.005 1, 0.005 6 ± 0.003 7, 0.054 4 ± 0.027 0, respectively. Data analysis with the gene expression microarray showed that Tle4, Kcna1, Itk, Bcorl1, Cacna1c, Rspo2 and Foxo3 were potential target genes of miR-155 in the AOM-DSS mouse model. Changes of Kcna1 and Cacna1c in the AOM-DSS mouse model were validated to be consistent with the changes obtained with the gene expression microarray.
CONCLUSIONThe up-regulation of miR-155 is related to colitis-associated carcinogenesis, but is irrelevant to chronic inflammation in the mouse model.

8. Expression of tyrosine and threonine protein kinase in carcinogenic process of colorectal cancer and its relationship with prognosis

Objective:
To investigate the expression of TTK (tyrosine and threonine protein kinase) in the process of colorectal cancer (CRC) development and its relationship to prognosis in CRC patients.
Methods:
Colitis-associated colon cancer model was induced by azoxymethane (AOM) and dextran sulfate sodium (DSS) in C57BL/6 mice. Mice at four different stages of colon cancer development were obtained, named AD1 (inflammation of the recovery), AD2 (mild dysplasia), AD3 (adenoma) and AD4 (adenocarcinoma), as well as negative control (no treatment). The expression of TTK was measured by real time fluorescent quantitative PCR (qPCR) and immunohistochemical staining in mouse colon tissues and 24 pairs of CRC specimens. The relationship between TTK and prognosis was analyzed in a set of CRC genome-wide gene expression microarray data that was obtained from Gene Expression Omnibus (GEO) of National Center for Biotechnology Information (NCBI).
Results:
The genome-wide microarray data from mouse AOM-DSS model indicated that the expression of TTK mRNA was gradually elevated during the development of colon cancer. The subsequent qPCR results showed that TTK mRNA levels in negative control, AD1, AD2, AD3 and AD4 groups were 1.05±0.42, 1.10±0.03, 1.38±0.15, 1.33±0.17 and 2.12±0.22, respectively. And TTK expression in AD2, AD3 and AD4 groups were significantly higher than that in negative control (
