1.Determination of Early Renal Damage with Ultrasound Doppler Renal Blood Flow Measurement in Essential Hypertension

Objective:To evaluate the value of ultrasound Doppler renal blood flow measurement in diagnosing early renal damage of hypertension. Methods:Eighty patients with hypertension and 20 normotensive subjects were included in this study. They were classified into four groups according to blood or urine β2-MG contents and urinary albumin excretion rate(UAER). All patients were measured by ultrasound Doppler renal blood flow. Correlation analysis was performed between β2-MG or UAER and the parameters of renal blood flow measurement. Results:(1)Peak velocity in systole(Vs)、end diastolic velocity(Vd) decreased and pulsed index(PI)、resistance index(RI) of segmental and interlobar artery increased in all hypertensives with early renal damage. PI of interlobar artery more than 1.00 and/or RI more than 0.60 is associated with early renal damage in hypertensives;(2)Hypertensives with diabetes and early renal damage have higher Vs and lower Vd and significant increased RI、PI. Compared with hypertensives with early renal damage,hypertensives with diabetes had more serious renal damage at the equivalent DBP level. Conclusion:Ultrasound Doppler renal blood flow measurement may be clinically useful in the noninvasive evaluation of early renal damage of hypertension.
2.Non-invasive immunodiagnosis of Schistosomiasis japonica: the detection of specific antibodies in saliva.

OBJECTIVETo assess the feasibility of using saliva for Schistosomiasis japonica diagnosis.
METHODSSchistosoma japonicum infected animal model was established. Pairs of saliva and serum samples from rabbits and chronic schistosomiasis patients were collected. Anti-schistosoma specific antibodies in saliva and serum were detected by indirect ELISA.
RESULTSThe specificities of antibody detection of rabbit saliva and serum were 93% (28/30) and 97% (29/30), respectively, and the sensitivities of antibody detection of rabbit serum and saliva were 100% (24/24) and 88% (21/24), respectively. A significant correlation (r = 0.5307, P = 0.0038 < 0.05) existed between anti-SEA IgG levels in serum and saliva. As with those in serum, anti-SEA IgG levels in saliva could reflect the state of infection and treatment. The sensitivity of antibody detection was 91% (29/32) for patient saliva samples and 100% (32/32) for their sera. 8 samples were positive in 140 normal saliva samples (i.e. 6% false positive rate) and 6 samples were positive in 156 normal serum samples (4% false positive rate). There was a significant correlation (r = 0.4227, P = 0.008 < 0.05) between specific antibodies in saliva and serum.
CONCLUSIONThe detection of specific antibodies in saliva can be used as a non-invasive immunodiagnosis method of Schistosomiasis japonica.

3.Reconstruction of laryngotracheal defect using the musculo-periosteum flap of the sternocleidomastoideus with clavicular periosteum.

OBJECTIVE:
To investigate the effect of the musculo-periosteum flap of the sternocleidomastoideus with clavicular periosteum on the reconstruction of extensiveness laryngotracheal defect.
METHOD:
Retrospectively studied 49 patients, who admitted in our department from 1996 to 2005 years for severe laryngotracheal defect. There were 28 males and 21 females aged from 15 to 69 years old (mean age was 47 years old). The causes of laryngotracheal defect were laryngotracheal stenosis (31 cases) and surgery for thyroid carcinoma invading trachea (18 cases). All of 49 cases were treated with the graft of musculo-periosteum flap of the sternocleidomastoideus with clavicular periosteum and placed a silicon T-tube stenting for 3 to 6 months.
RESULT:
All of 49 cases were successfully decannulated with recuperative normal airway patency and effective phonation. The follow up ranged from 2 to 10 years, and the effect of operation was steady.
CONCLUSION
The musculo-periosteum flap of the sternocleidomastoideus with clavicular periosteum is an ideal graft for the reconstruction of cervical extensiveness tracheal defect.

4.Efficacy of crizotinib for 28 cases of advanced ALK-positive non-small cell lung cancer.

OBJECTIVEThis study aims to evaluate the efficacy and safety of crizotinib for advanced ALK-positive non-small cell lung cancer (NSCLC) patients.
METHODSTwenty-eight patients with advanced ALK-positive NSCLC were given orally crizotinib 250 mg b. i.d., and were followed up to evaluate the therapeutic efficacy and safety.
RESULTSAmong the 28 patients, the objective response rate (ORR) was 71.4% (20/28) and disease control rate (DCR) was 92.9% (26/28). Three patients achieved complete response. Seventeen patients had partial response. The most common drug-related adverse events were mild flickering vision and gastrointestinal reaction. Eleven patients experienced flickering vision. Nine patients had nausea and vomiting. Eight patients had diarrhea. They were all reversible and of grade I or II. Only one patient had grade III myelosuppression. Among the 28 patients, 16 cases were disease-free and 12 cases had progressive disease, with a progression-free survival of 8.2 months.
CONCLUSIONSCrizotinib is effective and tolerable in the treatment of advanced ALK-positive NSLCC. However, its long-term treatment efficacy requires to be further studied.
