1.Clinical evaluation of the laparoscopic intrafascial supracervical hysterectomy

ObjectiveTo observe the clinical effect of laparoscopic intrafascial supracervical hysterectomy.Methods The complications during and after the operation of laparoscopic intrafascial supracervical hysterectomy of 46 patients were reviewed and summarized retrospectively. Results Laparoscopic intrafascial supracervical hysterectomy of 46 cases had been done well without significant complications,such as injury of intestine,bladder and ureter.Four cases had complications in short term,one was pelvic hematoma,one was pelvic infection,and two were hematocele in cervical canal.There were five cases had complications in long term.Residual cavity periodic hemorrhage in two cases,cervical gland cyst,pelvic parcel effusion and cervical polypoid hyperplasia each in one case.ConclusionsThere are less complications during the laparoscopic intrafascial supracervical hysterectomy,while much more after the surgery.Therefore,the surgical method needs to be improved.
2.Clinical analysis of minimally invasive percutaneous plate osteosynthesis for distal femoral fractures with LISS

Objective To summarize the therapeutic effect of minimally invasive percutaneous plate esteosynthesis (M1PPO) with less invasive stabilization system (LISS) in distal femoral fractures. Method Twenty-eight patients with LISS in distal femoral fractures between June 2004 and May 2005 were analyzed retrospectively. Results The average following up period was ( 17.6 ± 0.1 )months (6-30 months) all 28 cases,only one case of delayed union,the others were no complications. According to Johner-Wruh score standard,28 cases were excellent 16,good 9,fair 2 and poor 1,the rate of excellent and good was 89.3%. Conclusion The therapeutic effect of MIPPO with LISS in distal femoral fractures is satisfactory.
3.Relationship between matrix metalloproteinase-9 activity and microemboli formation in patients with cerebral infarction and its clinical significance

Objective To investigate the relation between plasma matrix metalloproteinase 9 (MMP 9) levels and transcranial Doppler (TCD) detected microemboli in patients with cerebral infarction and evaluate the clinical significance. 4.Comparison of hip arthroplasty and internal fixation for the treatment of intertrochanteric fractures

Objective To compare the effect of hip artificial replacement and DHS internal fixation for the treatment of intertrochanteric fracture, and investigate the difference of the two method. Methods Eighty-six pa- tients with intertrochanterie fracture patients were treated with hip arthroplasty, and dynamic hip screw. The pa- tients were followed up for 6 months, and compared the operation rime, blood loss, postoperative complications, sur- vival rate, postoperative hip function score and lose situation. Results There was no significant difference in opera- tion time,blood loss,postoperative complications,in local symptoms hip varus deformation occured in the DHS inter- hal fixation because lag screw cut-our and the collapse of the medial cortex, hip and thigh pain oecured in artificial re- placement,hip function score dropped 9.5 % in DHS and 9.2% in artificial replacement. Conclusion For elderly patients, eomminuted fracture and osteoporosis, and other types of unstable intertrochanteric fracture patients, the primary purpose of surgery is to provide to patients more activities of the early conditions,so hip arthroplasty can be used to the treatment of intertrochanteric fracture,as a reasonable replacement surgery.
5.Infection status of Toxoplasma gondii and its related knowledge and behav-ior among special population in Changzhou City

Objective To investigate the prevalence of Toxoplasma gondii infection and related knowledge and behavior among special population in Changzhou City,so as to provide the evidence for formulating effective measures of toxoplasmosis prevention. Methods The pregnant women and patients with neoplasia in Changzhou hospitals,and livestock and poultry breeding or processing workers were selected as the subjects of the study. Venous blood samples were collected from each partici-pant for detecting IgG and IgM antibodies against T. gondii by ELISA. A questionnaire investigation on knowledge and behavior about T. gondii infection was conducted. Results Among the total 300 respondents investigated from March to May,2015,the prevalence of T. gondii infection was 16.3%(49/300). Totally 52 respondents knew the knowledge about the prevention and con-trol of T. gondii infection with an awareness rate of 17.3%(52/300). The proportion of participants who frequently contacted with cats/dogs(25.0%,13/52)in the group who knew the knowledge about the prevention and control of T. gondii infection was lower than that of participants(50.8%,126/248)in the group who did not know the knowledge about the prevention and control of T. gondii infection,and the difference was statistically significant(X2=11.51,P<0.05). The proportion of participants sepa-rating chopping boards for raw and cooked food(61.5%,32/52)in the group who knew the knowledge about the prevention and control of T. gondii infection was statistically higher than that of the participants(9.3%,23/248)in the group who did not know the knowledge about the prevention and control of T. gondii infection,and the difference was statistically significant (X2=78.43,P<0.001). There was a negative correlation between the awareness rate of the knowledge about the prevention and con-trol of T. gondii infection and the infection rate of T. gondii. The infection rate of T. gondii(5.8%,3/52)in the group who knew the knowledge about the prevention and control of T. gondii infection was lower than that(18.5%,46/248)of the group who did not know the knowledge about the prevention and control of T. gondii infection,and the difference was statistically significant (X2=5.14,P<0.05). Conclusions The awareness rate of the knowledge about the prevention and control of T. gondii infec-tion among special population in Changzhou City is low. The health education on the knowledge of the prevention and control of toxoplasmosis should be strengthened,in order to improve the awareness of personal hygiene and change the unhealthy lifestyles and dietary habits.
6.Effect of self-designed closed negative pressure drainage combined with sponge dressing on refractory wounds

Objective To investigate the effect of self-designed closed negative pressure drainage combined with sponge dressings on refractory wounds.Methods Sixty patients with phase III-IV pressure ulcers were randomly divided into experiment group and control group in equal number.The self-designed closed negative pressure drainage combined with sponge dressing was applied in the experiment group and in the control group the conventional dressings were used.The two groups were compared in terms of hyperplasia of fresh granulation tissue,time for filling the defect and the healing time and the medical expense.Results Compared to the control group,the time for hyperplasia of fresh granulation tissue,the time for filling the defect and the healing time in the experiment group were all significantly shorter,and the medical expense of the experiment group was significantly less(all P<0.01). Conclusion The self-designed closed negative pressure drainage combined with sponge dressings in the treatment of phase III-IV refractory pressure ulcers may effectively shorten the healing time,improve the curative effects and reduce the economic burden of patients.
7.Analyais of Toxoplasma gondii infection and related factors among special population in Changzhou City,Jiangsu Province

Objective To understand the prevalence of Toxoplasma gondii infection and related factors among special popu?lation in Changzhou City,so as to provide the evidence for formulating toxoplasmosis prevention measures. Methods The ve?nous blood was collected from participants in Changzhou City for detecting IgG and IgM antibodies against T. gondii by ELISA;and an epidemiological questionnaire investigation was also conducted. Results Among the total 400 respondents detected,the prevalence of T. gondii infection was 15.0%and the positive rates of IgG and IgM were 15.0%and 0.8%,respectively. The T. gondii infection rates of pregnant women,HIV/AIDS patients,patients with neoplasia,and livestock and poultry breeding and processing workers were 11.0%,11.0%,24.0%and 14.0%,respectively. As the age increased,the infection rate of T. gondii showed an upward trend,with the highest infection rate(21.6%)among the group of respondents aged above 50 years. The in?fection rates of groups with different education levels were statistically different(χ2=11.443,P<0.05),and there was a trend that the infection rate decreased with the increase of education level. The prevalence of T. gondii infection in pregnant women was not significantly associated with the number of pregnancies and gestational age. The infection rate of T. gondii in the live?stock and poultry breeding and processing workers increased with the length of their working years. Conclusions The preva?lence of T. gondii infection among the special population in Changzhou City is high. Therefore,it is necessary to enhance the health education on the knowledge and information of prevention and control of toxoplasmosis,and to improve the awareness of personal protection,the hygiene practices and diet habits. In addition,more attention should be paid to T. gondii monitoring in the special population.
8.Clinical analysis on treatment of lateral clavicular fracture and acromioclavicular joint dislocation with clavicular hook plate

Objective To analyze the effect of clavicular hook plate(CHP) in treating distal clavicular fracture and acromioclavicular joint dislocation. Methods Eighteen patients with distal clavicular fracture(NeerⅡ) and acromioclavicular joint disclocation(Tossy Ⅲ) were treated by CHP. Results Sixteen patients were observed after operation,according to Lazzcano curative effect standard, fifteen cases were excellent, take 93.75 percent,1 case was good, with 6.25 percent, the excellent and good rates were 100.00 percent. The phenomenon such as the steel plate fracture,loose,escape hook and acromioclavicular joint re-dislocation were not happened. Conclusion CHP is an effective,scientific and convenience way for distal clavicular fracture and acromioclavicular joint dislocation,it worth being promoted.
9.Application of laser speckle perfusion imaging in predicting wound healing time of burn patients

Objective To explore the application effect of laser speckle perfusion imaging (LSPI) in predicting wound healing time of burn patients.Methods LSP1 was performed in 84 adult burn patients hospitalized in department of burns of Tongren Hospital of Wuhan University & Wuhan Third Hospital within post injury hour (PIH) 24 to 72 to detect the blood perfusion values of the wounds.The wound healing time was recorded.The 128 wounds were divided into superficial group (wound healing time shorter than or equal to 14 d,n =57) and deep group (wound healing time longer than 14 d and shorter than or equal to 28 d,n =71) according to the healing time.The blood perfusion values of the two groups were compared.Data were processed with t test or chi-square test.The receiver operating characteristic (ROC) curve was drawn and Youden index was calculated to determine the optimal critical blood perfusion value of wound healing time of the two groups,and the validity of the critical value was assessed by Kappa consistency test.Results (1) The blood perfusion value of woundsin superficial group was (6.8 ± 1.8) perfusion unit (PU),which was significantly higher than (3.5 ± 1.3) PU in deep group (t =11.404,P <0.01).(2) The total area under ROC curve of blood perfusion value to predict wound healing time was 0.931 (with 95% confidence interval 0.887-0.975,P <0.01).Combined with Youden index,5.52 PU was chosen as the optimal critical value of wound healing time of the two groups,with sensitivity of 76.9% and specificity of 94.7%.(3) The healing time of 44 wounds predicted was shorter than or equal to 14 d,and the healing time of 84 wounds predicted was longer than 14 d and shorter than or equal to 28 d,while the actual number of wounds was 57 and 71,respectively.The Kappa coefficient of consistency test was 0.754 (P <0.01).Conclusion LSPI is a useful method to predict the healing time of burn wounds.
10.Microskin grafting by spraying in burn management.

OBJECTIVETo explore the practicability of microskin grafting by spraying in burn management.
METHODSRazor thin autologous skin from pigs or burn patients was harvested and cut to pieces of 0.2 - 0.5 mm in size and suspended in normal saline. The suspension was put into a bottle with outlet and pumping device. The microskin suspended in the saline was sprayed to the burn wound and/or onto the alloskin sheets. The microskin distribution was detected by digital image analysis technique. In animal experiments, the burn wound development and pathomorphological changes after operation were observed. In burn patients who would receive microskin grafting, spraying method was used with the traditional flotation method as control. The treatment results and the operational procedures were compared between these two kinds of operation styles.
RESULTSThe microskin dispersion degree with spraying was much smaller than that with flotating method. In animal experiment with spraying method, the wound healing time was 23.2, 24.5 and 38.3 days in 3 groups, respectively, with the area ratio of donor to wound of 110, 120 and 150. In clinical study, The average operating time was 133.3 min with spraying method and 165.6 min with flotation method respectively (P < 0.05). The area ratio of donor to wound was 118.8 with spraying and 17.6 with flotation methods, respectively (P < 0.01). The one time wound coverage rate was 92.6% with spraying and 79.7% with flotation methods (P < 0.01). The wound healing time was 29.7 days with spraying and 37.3 days with flotating methods, respectively (P < 0.05).
CONCLUSIONSpraying method of microskin grafting might be a good method in major burn treatment. The advantages with this method included well-distributed microskin, simpler handling, saving of donor skin, shortening of operating time and less time needed for the wound healing. It might be recommended for other wound covering materials.
