Objective To investigate the relationship between peripheral blood composite inflammatory in-dexes[systemic immune inflammatory index(SII),lymphocyte to monocyte ratio(LMR),eosinophil to neu-trophil ratio(ENR),neutrophil to lymphocyte ratio(NLR)]and the severity of respiratory adenovirus(AdV)infection and concurrent wheezing in children.Methods A total of 154 children with respiratory AdV infec-tion(study group)who were admitted to the hospital from June 2022 to May 2024 and 154 healthy children who underwent physical examinations(control group)were selected.Children with respiratory AdV infection were divided into mild group(86 cases)and severe group(68 cases)according to the pediatric sequential or-gan failure assessment(p-SOFA),and children were divided into wheezing group(79 cases)and non-wheezing group(75 cases)according to whether wheezing occurred in children with respiratory AdV infection.The pe-ripheral blood SII,LMR,ENR and NLR in each group were detected and compared,the correlation between peripheral blood SII,LMR,ENR,NLR and the severity of respiratory AdV infection in children was analyzed by Pearson method.The factors affecting wheezing in children with respiratory AdV infection were analyzed by univariate and multivariate Logistic regression analysis.The diagnostic value of peripheral blood composite inflammatory indexes for the severity of respiratory AdV infection and concurrent wheezing in children was e-valuated by drawing the receiver operating characteristic curve.Results The peripheral blood SII,ENR and NLR in severe group were higher than those in mild group and control group(P＜0.05),while the peripheral blood SII,ENR and NLR in mild group were higher than those in control group(P＜0.05).The peripheral blood LMR in severe group was lower than those in mild group and control group(P＜0.05),while the pe-ripheral blood LMR in mild group was lower than that in control group(P＜0.05).The p-SOFA in severe group was higher than that in mild group(P＜0.05).Peripheral blood SII,ENR and NLR in children with re-spiratory AdV infection were positively correlated with p-SOFA(r=0.512,0.439,0.507,P＜0.05),and LMR was negatively correlated with p-SOFA(r=-0.436,P＜0.05).The area under the curve(AUC)of the single detection of SII,LMR,ENR and NLR for diagnosing severe respiratory AdV infection were 0.785,0.809,0.784 and 0.834 respectively,and the AUC of combined detection was 0.916,which was larger than those of single detection(P＜0.05).SII,ENR and NLR in wheezing group were higher than those in non-wheezing group(P＜0.05),and LMR was lower than that in non-wheezing group(P＜0.05).Small airway disease,high SII,high ENR and high NLR were risk factors for wheezing in children with respiratory AdV in-fection(P＜0.05),and high LMR was a protective factor(P＜0.05).The AUC of the single detection of SII,LMR,ENR and NLR for diagnosing wheezing in children with respiratory AdV infection were 0.778,0.771,0.757 and 0.805 respectively,and the AUC of combined diagnosis was 0.884,which was larger than those of single detection(P＜0.05).Conclusion The peripheral blood SII,ENR and NLR in children with respiratory AdV infection increase,and the LMR decreases,which are closely related to the aggravation of the severity of the disease and the concurrent wheezing.The combined detection of four indexes has high value in the diagno-sis of the severity of the disease and the risk of concurrent wheezing.

Objective:To evaluate the efficacy and safety of upadacitinib in the real-world treatment of difficult-to-treat Crohn's disease (DTT-CD) .Methods:This multicenter, retrospective cohort study included patients diagnosed with DTT-CD according to the International Organization for the Study of Inflammatory Bowel Diseases (IOIBD) criteria, and treated at eight Chinese inflammatory bowel disease centers between January 2023 and March 2025. Clinical outcomes were assessed after 12 weeks of induction therapy with upadacitinib (45 mg qd), including clinical remission rate, clinical response rate, and incidence of adverse events.Results:Among 151 enrolled DTT-CD patients, the clinical remission rate was 47.0%, and the clinical response rate was 90.7% after 12 weeks of treatment. Adverse events occurred in 42 cases (27.8%) .Conclusion:Upadacitinib demonstrated favorable efficacy in inducing clinical remission in DTT-CD patients, with a good safety profile at the induction dose (45 mg qd) .

Objective:To investigate the clinical application value of cystatin C(CysC), neutrophil gelatinase-associated lipocalin(NGAL), urea nitrogen(UREA), blood creatinine(Scr)and urinary neutrophil gelatinase-associated lipocalin(NGAL)in early diabetic nephropathy and their relationship with aging.Methods:A total of 140 diabetic patients aged 60 and above who were admitted to Beijing Hospital from July 2022 to November 2022 and 59 healthy people aged 60 and above who underwent physical examination in Beijing hospital during the same period were selected as the study subjects.In January 2023, 157 healthy people over 20 years old who underwent physical examination in Beijing Hospital were used as another group of research objects to explore the relationship between various indicators and aging.According to the urinary microalbumin/urinary creatinine(UACR)ratio, the patients were divided into three groups: 40 patients with simple diabetes mellitus(DM), 57 patients with early diabetic nephropathy(EDN), and 43 patients with diabetic nephropathy(DN).59 healthy subjects were used as control group(NC group).Serum NGAL, Scr, Cys C, UREA and urinary NGAL were detected, and NGAL was determined by latex immunoturbidimetry.Scr was determined by sarcosine oxidase method.Cys C was determined by immunoturbidimetry.Urease-glutamate dehydrogenase method was used to determine UREA.Receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic value of urea alone and combined diagnosis in EDN.The relationship between serum NGAL, Scr, Cys C, UREA and urinary NGAL and gender and aging were analyzed in 157 healthy individuals.Results:Compared to the NC group, serum NGAL, Scr, CysC, and UREA levels were significantly higher in the EDN and DN groups(all P<0.01), and the differences between EDN group, DN group and NC group were statistically significant( P<0.01).The values of these markers increased across the NC, DM, EDN, and DN groups.ROC curve shows: The area under curve(AUC)of serum NGAL, Scr, Cys C, UREA, urine NGAL, combined diagnosis 1 and combined diagnosis 2 were 0.655, 0.760, 0.789, 0.753, 0.628, 0.831, 0.827, respectively.The sensitivity of combined diagnosis 1 and combined diagnosis 2 was higher(63.2% and 66.7%), and the specificity of serum NGAL was higher(96.6%).The AUC values of Cys C, co-diagnosis 1, and co-diagnosis 2 were large and correlated(AUC=0.789, 0.831, 0.827, rs=0.501, 0.573, 0.567), and the diagnostic value of Cys C, co-diagnosis 1, and co-diagnosis 2 for EDN was high(AUC=0.789, 0.831, 0.827, Yoden index=0.442, 0.530, 0.531; sensitivity 56.1%, 63.2%, 66.7%; specificity 88.1%, 89.8%, 86.4%).Healthy individuals over 20 years of age: serum NGAL[(127.5±35.5)μg/L vs.(111.5±32.4)μg/L, P=0.004], Scr[(83.4±11.2)μmol/L versus(63.4±11.0)μmol/L, P<0.001], Cys C[(0.8±0.2)mg/L vs.(0.7±0.2)mg/L, P<0.001], and UREA levels were higher in males than females[(5.1±1.1)mmol/L vs.(4.6±1.1)mmol/ L, P=0.002]; urinary NGAL was higher in females than males[22.0(13.0, 37.8)μg/L vs.15.0(10.0, 30.5)μg/L, P=0.025].Cys C( F=16.582, P<0.001), urinary NGAL levels( F=-3.533, P<0.001)basically increased with age. Conclusions:CysC showed a higher diagnostic value for EDN than UREA, Scr, blood NGAL, and urine NGAL in patients aged 60 and above Combined diagnosis 1 and combined diagnosis 2 have higher value in monitoring the progress of DN.The levels of Cys C and urinary NGAL showed a gradually increasing trend with the increase of age, and the levels of serum NGAL, Scr, Cys C and UREA were higher in males than in females.Urinary NGAL was higher in females than males.

Objective:to investigate age-related changes in high-sensitivity cardiac troponin T(hs-cTnT)levels among middle-aged and elderly individuals, and to establish reference values for the 99th percentile upper reference limit(99th URL)of hs-cTnT within this demographic.Methods:This research is designed as a cross-sectional study.Hs-cTnT test results were collected from outpatients at Beijing Hospital between January 2018 and December 2023.The final sample included 5, 677 outpatients aged 45 to 85 years(65±10), all of whom were free from cardiovascular diseases and other chronic heart-related conditions.Multiple linear regression was utilized to assess the impact of gender and age group on hs-cTnT concentrations within the middle-aged and elderly populations.Furthermore, a generalized additive model for location, scale, and shape(GAMLSS)was employed to generate percentile curves of hs-cTnT by gender in this cohort, with the aim of determining the 99th URLs for both middle-aged and elderly individuals.Results:Multiple linear regression analyses indicated that both gender and age significantly influenced hs-cTnT levels in middle-aged and elderly populations( β60-69 years=1.399, β≥70 years=5.306, βgender=-2.650, all Pvalues<0.001).The hs-cTnT data for both males and females conformed to Box-Cox-Cole-Green(BCCG)distributions, with percentile curves fitted at the 25th, 50th, 75th, and 99th percentiles according to gender.The percentile values of hs-cTnT in both males and females demonstrated a progressive increase with advancing age.Notably, males exhibited higher 99th percentile values than females across all age groups. Conclusions:The GAMLSS approach is employed to establish continuous reference intervals for hs-cTnT in the middle-aged and elderly population.This effort aims to provide a theoretical foundation for reference values specific to this demographic in China and to offer guidance for clinical diagnosis and treatment.

Objective:To evaluate the clinical outcomes of endoscopic-assisted polyether ether ketone (PEEK) patient-specific implant (PSI) revision for over-resected mandibles caused by the mandibular angle osteotomy.Methods:A retrospective analysis was conducted on 24 patients [8 males, 16 females, aged 19-57 (32.5±9.5) years] with 39 over-resected mandibles that underwent PEEK-PSI mandibular angle revision surgery at the Affiliated Friendship Plastic Surgery Hospital of Nanjing Medical University from January 2019 to December 2023. Preoperative cone-beam computed tomography (CBCT) data were used to design and fabricate customized PEEK PSIs based on individual anatomical requirements. An intraoral incision approach with endoscopic assistance was employed to meticulously dissect soft tissue attachment around the angle region, followed by the implantation of a customized PEEK PSI. Postoperative CBCT scans were performed for 3D reconstruction, with root mean square error (RMSE) and maximum deviation (MaxD) as accuracy metrics. Patients′ satisfaction was assessed preoperatively and ≥6 months postoperatively using the face questionnaire (FACE-Q) scores, which included overall facial appearance, lower face and jawline, appearance distress, psychological health and social function.Results:All 24 patients achieved satisfactory recovery with primary healing of intraoral incisions. No complications such as infection, nerve injury, or implant rejection occurred during follow-up period. Patients′ facial appearance and jaw line contouring were significantly improved. Fine anatomical fitting between PEEK-PSI and defect areas was observed: RMSE ranged from 0.117 to 0.315 mm, and MaxD was (5.485±1.300) mm. FACE-Q scores demonstrated significant improvements after surgery in overall facial appearance [(49.8±5.4) vs (65.0±5.3) scores], lower face and jawline [(42.5±5.3) vs (56.1±4.6) scores], appearance distress [(60.0±6.9) vs (70.6±6.5) scores], psychological health [(62.0±5.0) vs (70.8±5.3) scores], and social function [(60.3±4.3) vs (69.3±5.8) scores] (all P<0.001). Conclusion:Endoscopic-assisted PEEK-PSI revision for over-resected mandibles following mandibular angle osteotomy exhibits high surgical precision and safety, effectively restoring mandibular contour and significantly enhancing patients′ satisfaction.

Objective:To investigate the clinical outcomes of using autologous mandibular outer cortex "sandwich" grafting to augment mandible for correcting lower facial asymmetry with normal occlusal relationships.Methods:A retrospective analysis was conducted on the clinical data of patients with lower facial asymmetry treated at the Department of Plastic Surgery, the Affiliated Friendship Plastic Surgery Hospital of Nanjing Medical University, from January 2016 to December 2019. Preoperative cone-beam CT (CBCT) scans of the maxillofacial region were obtained to acquire three-dimensional data of the maxillofacial skeleton. Computer-aided design was used to determine the osteotomy range of the mandibular outer cortex and design osteotomy guide template. The outer cortex of the larger mandibular side (donor site) was harvested according to the osteotomy guide template and sectioned. The segmented mandibular outer cortex was then contoured to match the arc of the recipient side’s mandibular outer cortex and fixed to the inner side of the recipient mandibular outer cortex, thus increasing the width and thickness of the expanded mandible. Follow-up was conducted at 7th day and 6 months postoperatively, CBCT scan was performed to measure the changes in ramus height (Co-Go), mandibular body length (Go-Me), and mandibular outer cortex thickness, and volume, and the patient satisfaction with facial appearance (1 to 5 points, with higher scores indicating higher satisfaction), were assessed to evaluate surgical outcomes. Statistical analysis was performed using SPSS 12.0 software. Paired t-tests were used to compare patient satisfaction scores preoperatively and six months postoperatively. Repeated measurement ANOVA was used to compare Co-Go and Go-Me measurements preoperatively, 7th day, and 6 months postoperatively. If a statistically significant difference is found, further analysis using post-hoc testing(Tukey’s HSD test) will be conducted to examine the data. Results:Sixteen patients with lower facial asymmetry were included, comprising of 5 males and 11 females, aged 18 to 40 years, with an average age of 25.2 years. Surgical method included contralateral mandibular outer cortex "sandwich" grafting to the expanded mandible in 9 cases and simultaneous genioplasty in 7 cases. Follow-up ranged from 6 months to 5 years, with an average follow-up of 18.6 months. All patients experienced numbness of the lower lip postoperatively, which resolved within six months, and no severe complications occurred. The symmetry of lower facial contour improved significantly and remained stable. Patient satisfaction score for facial appearance increased from (1.63±0.62) points preoperatively to (4.19±0.75) points six months postoperatively( P<0.01). The differences in Co-Go on the donor side and Go-Me on the recipient side across the three time points(preoperatively, 7th day and 6 months postoperatively) were not statistically significant(all P>0.05). However, the differences in Go-Me on the donor side and Co-Go on the recipient side across the three time points were statistically significant(all P<0.05). On the donor side, the mandibular outer cortex thickness decreased by a maximum of 6 mm on 7th day postoperatively and increased by a maximum of 2 mm at 6 months postoperatively compared to 7th day. On the recipient side, mandibular outer cortex thickness increased by a maximum of 6 mm on 7th day postoperatively and decreased by a maximum of 2 mm at six months postoperatively compared to 7th day. The volume of the mandibular outer cortex on the recipient side increased by (4 415.94±1 017.21)mm 3 at 7th day postoperatively compared to preoperatively, and decreased by (202.63±300.85)mm 3 at 6 months postoperatively. Conclusion:For lower facial asymmetrical with normal occlusal relationships and no occlusal plane deviation, contralateral mandibular outer cortex "sandwich" bone grafting can effectively increase the width and volume of the mandible on the grafted side, achieving favorable clinical outcomes.
