1.Compatibility Stability of Chinese Material Medica Injection in Centralized Pharmacy Admixture

OBJECTIVE: To study the compatibility stability of Chinese material medica injection for reference of clinical rational administration. METHODS: The pertinent literature about the compatibility stabililty of Chinese material medica injection during 1999-2006 retrieved from CHKD were statistically analyzed. RESULTS: The analysis on temperature, pH, corpuscle and concentration et exactly and directly revealed that Chinese material medica injection was stable under any infusion condition, which serves as reference for clinical medication. CONCLUTION: Because of the complexity of the components and process of preparation or extraction, the Chinese material medica injection often induce adverce reaction during intervenous drop infusion, thus great importance should be attached to the compatibility stability in centralized pharmacy admixture to ensure the clinical medication safety and decrease the incidence of adverse drug reaction.
2.Determination of the Contents of Tanshinone Ⅱ_A in Compound Jinpu Tablets with HPLC

OBJECTIVE:To establish the HPLC method for determination of tanshinone Ⅱ A in compound Jinpu tablets METHODS:HPLC was performed on Shim-pack CLC-ODS(4 6mm?250mm,5?m) with methanol-water(15∶5) as mobile phase Flow rate was 1 5ml/min and detecting wavelength was 270nm Tanshinone Ⅱ A in compound Jinpu tablets was extracted by reflux method RESULTS:The calibration curve was linear in the concentration range of 0 01 333~0 10 667?g/ml(r=0 9 998,n=5) CONCLUSION:This method is simple,accurate,repeatable and effective for quality control of compound Jinpu tablets
3.Cholangiocarcinoma of the hepatic hilum: surgical management and prognosis

Objective To analyze the surgical management and prognosis of hilar cholangiocarcinoma(HCC).MethodClinical data of 72 patients with HCC were retrospectively analyzed.Results There was a significant difference in the 3- and 5- year survival rates and median survivals among the groups treated with different surgical methods(x2 =28.1,P< 0.01 ).Univariate analyses conducted for the 16 factors which might affect prognosis showed the surgical method,lymphatic metastasis,positive margin,as well as the type of pathology and staging were of statistically significant prognostic value (P<0.05).On a multivariate analysis using the Cox proportional hazards model,surgical management,lymphatic metastasis and the type of pathology and staging were independent prognostic factors (regression coefficient 1.4157,1.1072,1.6435).ConclusionRadical excision is an important prognostic factor for HCCa and extended resection improved the long-term survival rates of radical surgery.
4.Expression of IL-6 in indirectly injured pulpal tissue from firearm wound in dog

Objective: To study the role of IL-6 in the dental pulp indirect injury by gunshot. Methods: The mandible was wounded by gunshot at (961.18?8.94) m/s in 17 dogs.2 h,6 h,1 d,3 d,7 d after injury molars,caspids,and lateral incisors were sampled respectively for IL-6 detection by immunohistochemical method in dental pulp. Results: The results showed that positive staining of IL-6 was observed predominantly in infiltrating inflammatory cells, vascular endothelial cells and some fibroblasts,IL-6 expression was consistent with the recruitment and activation of inflammatory cells, and with the severity duration of pulpal tissue injury and response. IL-6 level was increased in the dental pulp 6 h~3 d after injury.Conclusion: IL-6 is involved in regulating physiologic responses of dental pulp injuries in maxillofacial trauma by a projectile.
5.The clinical significance of thymopeptides α1 in the treatment of severe sepsis

Objective To investigate the application values of thymosin α1 in severe sepsis.Methods Selected 94 patients with severe sepsis in our hospital from September 2014 to October 2016,were randomly divided into observation group(n=45)and control group(n=49),the control group was given routine treatment,the observation group was given thymosin α1 on the basis of conventional treatment,observed two groups before and after treatment T cell subsets and CD14+monocyte human leukocyte antigen(HLA-DR)and so on.Results The observation group the duration of ventilator use and ICU treatment were(12.51±3.82)d and(15.81±3.18)d,significantly shorter than the control group(P< 0.05).There was no significant difference in 28d mortality between the observation group and the control group; After treatment,the CD14+monocytes HLA-DR,CD3+and CD4+T cells in the observation group were significantly improved than before treatment(P<0.05);The HLA-DR,CD3+and CD4+T cells in the observation group after treatment were(36.04± 8.90)%,(58.93±8.74)%and(43.20±9.90)%,significantly higher than those in the control group(P<0.05);The observation group after treatment TNF-α,endotoxin and CRP respectively(56.40±11.78)ng/L,(27.83±9.98)ng/L and(53.20±9.73)g/mL,significantly lower than the control group(P< 0.05).Conclusion Application of thymosin α1 in the treatment of severe sepsis,which can improve the cellular immune function,adjust the state of inflammatory response,shorten the duration of mechanical ventilation and ICU stay.
6.Effect on growth of human hepatoma cells by antisense gene of type Ⅱ insulin-like growth factor receptor transfection

Objective To study the growth inhibition of human hepatoma cells tranfected with type Ⅱ insulin like growth factor receptor (IGF ⅡR) antisense gene. Methods We constructed the recombinant eucaryotic expression plasmids of IGF ⅡR sense and antisense genes, which were transfected into SMMC 7721 human hepatoma cell line using calcium phosphate coprecipitation, and selected in DMEM supplemented with 500 ?g/ml G418(Geneticin) for 2~3 weeks. Then the total number of colonies formed was counted. The effect of IGF ⅡR antisense gene transfection on endogenous IGF ⅡR mRNA levels was examined by Northern blot, and the growth rate and the ability of the transfected cells to form colonies in soft agar medium were also examined. Results The hepatoma cells transfected with IGF ⅡR antisense gene exhibited significant reduction in endogenous IGF ⅡR messenger RNA(mRNA) levels and loss of their ability for anchorage independent growth in soft agar. The IGF ⅡR antisense gene supressed the formation of colonies of SMMC 7721 cells resistant to G418 without alteration on cell growth curve. Conclusion The IGF ⅡR antisense gene expression effectively blocks IGF Ⅱ to IGF ⅡR autocrine and/or paracrine signal transduction pathway and reverses certain aspects of the cell malignant phenotype.

8.Evaluation of oral gastrointestinal ultrasound contrast agents on gastric motility in patients with anxiety disorder

Objective To apply the oral gastrointestinal ultrasound contrast agents for evaluating the gastric motility abnor‐mality in the patients with anxiety disorder .Methods Twenty patients with anxiety disorder complicating upper digestive tract symptoms without organic pathological changes were enrolled as the anxiety disorder group .Twenty healthy volunteers were en‐rolled as the control group in this study .The two groups orally took gastrointestinal ultrasound contrast agents .The antral contrac‐tion frequency ,antral contraction amplitude amd MI were measured at each time point .GER was calculated .The the gastric motility parameters in the patients with anxiety disorder were evaluated .Results The antral contraction frequency and MI at initial 2 min had no statistical difference between the anxiety group and the control group .The antral contraction amplitude ,antral contraction frequency amd MI at each time point during 5 -10 min after contrast in the anxiety disorder group were significantly decreased compared with the control group ,and the differences were statistically significant (P< 0 .05) .After 20 min ,GER in the control group was significantly higher than that in the anxiety group ,the difference was statistically significant (P<0 .05) .Conclusion O‐ral gastrointestinal ultrasound contrast agents is a economic ,strongly operable ,non‐invasive and highly repeatable method for evalu‐ating the gastric motility .
9.The methodology of symptom directing standardized intervention in therapeutic clinical trial design of traditional medicine

Double-blind randomized controlled trial design is the most recognized scheme of therapeutic experimental design.On the assessment of the efficacy of traditional medicine, randomized controlled clinical trial design also has the same value.Only when using the research method towards epidemiology to perform the rigorous design on the clinical trial of traditional medicine and make the objective evaluation of its effect, can it get the real understanding and approval from modern medicine.When making the therapeutic clinical trial under the traditional medicine, due to the characteristics of the subject, therapeutic measures usually need to be changed so they cannot be standardized, and the modification may result in the share of multiple intervention components, which leads to the inaccurateness of results under the effect of confounding bias.According to the characteristics of traditional medicine, a new method was proposed for therapeutic clinical trial.Firstly do the randomization with the inclusion criteria made by the same symptom arise in a certain stage of disease, secondly adopt the standardized intervention measures for processing.Eventually confirm whether there is significant statistical difference.The symptom directing standardized interventions have provided the experimental design of the traditional medical clinical therapy with a feasible scheme, which can solve several problems existing in this design.
10.Effect of emulsified isoflurane on myocardial apoptosis and expressions of bcl-2 and bax induced by hypoxia-reoxygenation

Objective To investigate the effects of emulsified isoflurane on myocardial apoptosis, and the expressions of Bcl-2 and Bax after hypoxia/reoxygenation in cultured myocardial cells of neonatal rats. Methods Myocardial hypoxia/reoxygenation model was established on cultured primary myocardial cells of neonatal rat. The myocardial cells were divided into four groups: the normal group, model group, fat milk group, and emulsified isoflurane group. The cellular morphologic changes and pulsatile frequency of each group were observed under inverted microscope and the ultrastructure of myocardial cells was observed under transmission electron microscope. The apoptotic rate of myocardial cells was determined by flow cytometry, the expression of bcl-2 mRNA and bax mRNA were detected by RT-PCR and the protein expressions of Bcl-2/Bax were observed by Western blot. Results The apoptotic rate were significantly decreased in both emulsified isoflurane and fat milk groups (P0.05). Conclusion Emulsified isoflurane and one of its components, fat milk,can inhibit myocardial apoptosis induced by hypoxia/reoxygenation in cultured myocardial cells. Emulsified isoflurane exerts its anti-apoptosis effect maybe through up-regulating Bcl-2 expressions and down-regulating Bax expressions.
