1.Nursing effect of personalized traditional Chinese medicine nursing mode for patients with subtotal ;gastrectomy

Objective To explore the application effect of individualized traditional Chinese medicine nursing mode in the nursing of patients with subtotal gastrectomy. Methods A total of 60 eligible patients who met the standard were divided into the control group and the intervention group with 30 cases in each group according to the random number table method, the control group received only normal nursing while the intervention group was subjected to the individualized TCM nursing based on the normal nursing. The nursing effect was compared between two groups. Results Scores of negative mood of patients of the intervention group before surgery, electrogastrogram slow wave 3 h, the first day and the second day after the operation, first feeding time after operation, early ambulation time, average length of stay was (16.50 ± 3.20) points, 2.58 ± 0.46, 2.88 ± 0.08, 2.99 ± 0.54, (28.70 ± 4.43) h, (29.09 ± 4.55) h, (8.24 ± 1.38) d. The control group was(16.98 ± 2.61)points, 2.40 ± 0.53, 2.79 ± 0.14, 2.71 ± 0.30,(31.45 ± 3.80)h,(32.91±2.28)h,(10.40±1.99)d, there were significant differences between the two groups (t=2.53-25.19, P < 0.05). In the third day after operation, the electrogastrogram slow wave in the intervention group and the control group were 2.94 ± 0.53, 2.93 ± 0.12, there was no significant difference between the two groups (t=0.40, P>0.05). Conclusions Individualized TCM nursing can effectively alleviate the patients′ negative mood, shorten the recovery time of postoperative electrogastrogram slow wave, first feeding time and early ambulation time, to restore the gastrointestinal function of patients earlier, effectively promote the rehabilitation of patients;at the same time, shorten the hospitalization time.
2.Effect of electroacupuncture on postoperative cognitive function in elderly patients undergoing carot-id endarterectomy

Objective To evaluate the effect of electroacupuncture on postoperative cognitive func?tion in elderly patients undergoing carotid endarterectomy. Methods Fifty patients of both sexes, aged≥65 yr, of American Society of Anesthesiologists physical status Ⅱ orⅢ, with the educational level≥ the level of primary school, scheduled for elective unilateral carotid endarterectomy, were divided into 2 groups ( n=25 each) using a random number table: control group ( group C) and electroacupuncture group ( group EA). In group EA, electroacupuncture (frequency 2∕100 Hz, intensity 5-12 mA) of Baihui (GV20), Neiguan (PC6) and Zusanli (ST36) was performed throughout surgery starting from 30 min before opera?tion, and then general anesthesia was performed. General anesthesia was performed directly in group C. Before operation ( T0 ) , at the end of operation ( T1 ) and at 24 h after operation ( T2 ) , blood samples were collected from the internal jugular vein for determination of plasma S?100βprotein, tumor necrosis factor?αand brain?derived neurotrophic factor ( BDNF) concentrations by enzyme?linked immunosorbent assay. The cognitive function was assessed using Montreal Cognitive Assessment at T0,2 and at 3 and 7 days after opera?tion ( T3,4 ) . Results Compared with group C, the Montreal Cognitive Assessment scores were significantly increased at T2?4 , the plasma S?100β protein and tumor necrosis factor?α concentrations were significantly decreased at T1,2 , and the plasma BDNF concentrations were significantly increased at T2 in group EA ( P<0?05) . Conclusion Electroacupuncture can improve postoperative cognitive function in the elderly pa?tients undergoing carotid endarterectomy, which may be related to inhibition of inflammatory responses,
promotion of BDNF production and reduction of brain injury.

OBJECTIVETo explore the effects of moxibustion with seed-sized moxa cone at "Ganshu" (BL 18) on liver furiction and morphology in rat with precancerous lesion of hepatic cellular cancer MCC).
METHODSA total of 60 male Wistar rats were randomly divided into a normal group (10 rats), a model group (20 rats), a 20-day treatment group (15 rats) and a 40-day treatment group (15 rats). HCC model was established by intraperitoneal injection of diethylnitrosamine (DEN). Rats in the normal group received no treatment. Rats in the model group were treated with fixation. Rats in the 20-day treatment group and 40-day treatment group were treated by moxibustion with seed-sized moxa cone at "Ganshu" (BL 18), once every other day, for 20 days and 40 days, respectively. Blood sample in each group was collected 1 d before model establishment, 20 d, 40 d and 84 d after model establishment. Chemical method was applied to test the activity of ALT (alamine aminotransferase), AST (aspartate transaminase) and GGT (glutamyl transpeptidase); at the end of model establishment, all the rats were sacrificed to observe the liver morphology changes.
RESULTSAfter the first therapeutic course, the. content of ALT and AST in the 20-day treatment group was significantly lower than that in the model group (all P<0. 05); after the second therapeutic course, the content of ALT, AST and GGT in the 40-day treatment group was insignificantly lower than that in the model group (all P>0. 05). Under light microscope, the slice of liver tissue indicated that primary tumor was induced in the model group, and the tumor cells were stained and irregular; the cytoplasm in the 20-day treatment group was even, and the tumor cells were few with several nodules alone. In the 40-day treatment group the liver morphology was normal and the staining was even; the tumor cells were few without nodules or a few. Conclusion Moxibustion with seed-sized moxa cone at "Ganshu" (BL 18) could reduce the serum content of ALT, AST and GGT in rats with HCC, which could protect the liver and: delay the DEN-induced precancerous lesion on some levels.

The chalcone synthase (CHS) superfamily of the type III polyketide synthases (PKSs) generates backbones of a variety of plant secondary metabolites. Benzalacetone synthase (BAS) catalyzes a condensation reaction of decarboxylation between the substrates of 4-coumaric coenzyme A and malonyl coenzyme A to generate benzylidene acetone, whose derivatives are series of compounds with various biological activities. A BAS gene Pcpks2 and a bifunctional CHS/BAS PcPKSI were isolated from medicinal plant P. cuspidatum. Crystallographic and structure-based mutagenesis studies indicate that the functional diversity of the CHS-superfamily enzymes is principally derived from small modifications of the active site architecture. In order to obtain an understanding of the biosynthesis of polyketides in P. cuspidatum, which has been poorly described, as well as of its activation mechanism, PcPKS2 was overexpressed in Escherichia coli as a C-terminally poly-His-tagged fusion protein, purified to homogeneity and crystallized, which is helpful for the clarification of the catalytic mechanism of the enzyme and lays the foundation for its genetic engineering manipulation.

6.Imaging features of split cord malformation associated with scoliosis and its correlation with neurologic symptoms

Objective To analyze the imaging features of congenital spinal deformity (CSD) associated with split cord malformation (SCM) and other intraspinal abnormalities, and to investigate the relationship to neurological symptoms. Methods 105 cases CSD with SCM were retrospectively studied. Analysis the imaging features of SCM (including type of SCM, location of SCM, location and apical vertebrae, symmetry of divided cord) and other intraspinal abnormalities. To investigate the relationship of the factors and neurological symptoms using Chi?square test of one factor and multiple factors logistic regression analysis. Re?sults 28 cases (26.7%) were formation failure, 33 cases (31.4%) were segmentation failure, and 44 cases (41.9%) were combina?tion of 2 disorders. 41 cases had neurological symptoms, 64 cases were asymptomatic. The distribution of SCM combined with spi?nal deformities:thoracic (11 cases), thoracolumbar (18 cases) and lumbar (20 cases) in type I SCM, thoracic (31 cases), thoracolum?bar (20 cases) and lumbar (5 cases) in type II, none was in cervical. The location of SCM upper than apical vertebrae 29 cases, on apical vertebrae 25 cases, lower than apical vertebrae 51 cases. Spinal cord was splitted symmetric 27 cases and asymmetric 78 cases. 66 cases combined with other intraspinal abnormalities, lower conus 42 cases, syringomyelia 38 cases, meningocele 10 cas?es and sakrale zyste 5 cases. Associated with intraspinal abnormalities, the rate of neural symptoms was different. According to Chi?square test of one factor and multiple factors logistic regression analysis, lumbar SCM, spinal cord asymmetric and lower conus were related with neurological symptoms. Conclusion The predilection spinal deformity of type I is combination, type II SCM is segmentation failure. When SCM patients associated with other intraspinal abnormalities, the incidence of neurologic symptoms is increased. The lumbar SCM, hemicords asymmetry and lower lying conus have significant relationship with neurologic symptoms.
7.Study on effect of self-made Guben Pingchuan Huatan decoction on curative effect and laboratory indexes of patients with chronic obstructive pulmonary disease

Objective To investigate the effect of self-made Guben Pingchuan Huatan decoction on curative effect and laboratory indexes of patients with chronic obstructive pulmonary disease .Methods 98 patients with chronic obstructive pulmonary disease (COPD) in stable stage in our hospital from September 2014 to September 2016 were selected and divided into observation group with 49 cases and control group with 49 cases according to the random number table method .The control group was given tiotropi-um bromide treatment ,while the observation group were given the basic treatment the same as the control group with the combina-tion of self-made Guben Pingchuan Huatan decoction .Two groups were treated for a period of 3 months .The therapeutic effects of the two groups were compared ,and the changes of pulmonary function ,6MWT and serum inflammatory factors before and after treatment were compared .Results The total effective rate in the observation group was higher than that in the control group (P<0 .05);FEV1 expected value ,PEF and FEV1/FVC value of the two groups after treatment were increased (P<0 .05);FEV1 expec-ted value ,PEF and FEV1/FVC value of the observation group were higher than those of the control group after treatment ,and the difference was statistically significant (P<0 .05);6MWT of two groups after treatment was increased (P<0 .05);6MWT of the observation group after treatment was higher than that of the control group ,and the difference was statistically significant (P<0 .05);IL-6 ,IL-8 and MMP-9 levels were decreased after treatment in two groups (P<0 .05);IL-6 ,IL-8 and MMP-9 levels of the observation group were lower than those of the control group ,and the difference was statistically significant (P<0 .05) .Conclusion The self-made Guben Pingchuan Huatan decoction has a remarkable curative effect on chronic obstructive pulmonary disease ,and can significantly improve the pulmonary function of patients ,and lower serum inflammatory cytokines IL-6 ,IL-8 and MMP-9 levels , thereby reducing the inflammatory state of patients .
8. Progress in neuromodulation therapy for Tourette′s syndrome

Tourette′s syndrome (TS) is a common neurodevelopmental disorder that affects the learning and life of children. So far, its pathogenesis remains unclear and its treatment effect is limited. As an important means of non-pharmacological treatment, neuromodulation technology has some curative effects on TS. This article describes the progress in neuromodulation therapy of TS, so as to facilitate the development of new technologies for the treatment of such patients.
9.The effects of low calcium dialysate on bone mineral density in old patients under maintenance hemodialysis with low turnover renal osteodystrophy

Objective To study the effects of low calcium dialysate on bone mineral density (BMD) in old patients with low turnover renal osteodystrophy under maintenance hemodialysis.Methods Totally 72 elderly patients aged≥ 60 years under MHD for 6 months or more with parathyroid hormone(iPTH)<100 ng/L were selected and randomly divided into treatment group(n=36,calcium 1.25 mmol/L in dialysate)and a control group(n =36,calcium 1.5 mmol/L in dialysate),for 12 months of treatment.The changes of albumin-corrected calcium and phosphorus,calciumphosphorus product,iPTH level,bone mineral density,and other indicators as well as related adverse events were observed before and 12 months after the study.Results There was no statistically significant difference in general conditions and the correlated laboratory examinations between the two groups before and after treatment (P > 0.05).After dialytic treatments with dialysate containing calcium 1.25 mmol/L for 12 months,the therapy group versus pre-therapy and control group showed statistically significant decrease in parameters of mean arterial pressure(MAP) [(88.6 ± 9.2) vs.(92.6±10.4)and(93.7±8.8)mmHg],serum calcium[(2.4±0.1)vs.(2.6±0.3)and(2.6±0.2)mmol/L,t =5.061,5.074],phosphorus[(2.0±0.2)vs.(2.1 ±0.2)and(2.1±0.3)mmol/L,t=2.276,2.271],calcium-phosphorus product[(4.7 ± 0.5) vs.(5.3 ± 0.6) and (5.4 ± 0.7) mmol2 / L2,t =4.682,4.627](all P<0.05),and showed statistically significant increase in parameters of iPTH levels[(132.6 ±37.8) vs.(71.3 ± 11.48) and (69.7 ± 16.0) ng/L;t value 8.824 and 9.048,respectively],bone mineral density values(Lumbar:0.8±0.9 vs.-1.2±1.1 and-1.2±1.1;t value 2.170 and 2.170,respectivly.Femoral neck:-0.8± 1.0 vs.1.3±1.2 and-1.3±1.3;t value 2.258 and 2.243,respectively) (all P<0.05).In the control group after 12 months of treatment with calcium 1.5mmol/L dialysate,there was no significant difference in related parameters (P> 0.05).There was no significant difference in the adverse reactions between study and control groups (P > 0.05).Conclusions The low calcium dialysate(calcium 1.25 mmol/L)used in elderly patients with iPTH<100 ng/L under MHD can effectively improve the excessive depression of parathyroid function and the decreased BMD,and better control the albumin corrected calcium,phosphorus,calcium-phosphorus product level,and has a good security.
10.Effect of the multidisciplinary diagnosis and treatment nursing on postoperative psychological state and quality of life in patients with lateral skull base tumor

Objective:To explore the application effect of the multidisciplinary diagnosis and treatment nursing in improving postoperative anxiety and depression and quality of life in patients with lateral skull base tumor, so as to provide a constructive template for the multidisciplinary diagnosis and treatment nursing.Methods:This was a prospective cohort study design. A total of 100 patients who underwent lateral skull base tumor surgery in Shandong Second Provincial General Hospital from January 2021 to April 2022 were selected as the research objects. The patients were divided into observation group and control group by random digits table method with 50 cases in each group. The control group was given routine nursing care after operation of lateral skull base tumor and routine follow-up management. The observation group received the multidisciplinary diagnosis and treatment nursing on the basis of the control group. The intervention time was 3 months. The psychological state and quality of life of the two groups before and after intervention were evaluated by Self-rating Anxiety Scale (SAS), Self-rating Depression Scale (SDS), Short Form Health Questionnaire (SF-36).Results:There was no significant difference in SAS, SDS, and SF-36 scores before intervention between the two groups ( P>0.05). The scores of SAS, SDS and SF-36 after the intervention were (44.58 ± 5.61), (41.66 ± 8.48), (75.66 ± 13.65) points in the observation group, and (50.86 ± 4.91), (45.80 ± 9.32), (68.43 ± 14.34) points in the control group, there were statistically significant differences between the two groups( t values were -5.95, -2.32, 2.58, all P<0.05). Conclusions:The multidisciplinary diagnosis and treatment nursing can significantly improve anxiety, depression and quality of life of patients with lateral skull base tumor after surgery.
