2.Practice of the Pediatric Pharmaceutical Services Based on WeChat Platform

OBJECTIVE:To investigate the effects of WeChat platform online service on the safety of pediatric family medica-tion and the quality of pharmaceutical care. METHODS:The online consultation records of"Qs and As for pediatric medications"were collected and summarized from the WeChat platform(WeChat ID:etyywd)during Jun.1st,2014-Dec.31st,2015 in our hos-pital. Statistical analysis was performed on the consulting time periods,age of pediatric patients with consultation demands,and the systems and mistake types involved in medication consultations. RESULTS:A total of 29 850 dialogue records on WeChat were collected,and 13 315 of these were related to medications(44.61%). The peak time of medication consultation was at 9:00-10:00 in the morning,with an accumulative consultation number of 1 840 cases(13.82%);while the least consultation time pe-riod was at 4:00-5:00 early in the morning,with only 13 cases(0.10%).Pediatric patients in need of consultation were more with-in the age of 1-6 years old,and most of them were preschool children(31.20%)and toddlers(26.32%). The systems consulted mainly involved the respiratory system,immune system and digestive system;in which respiratory system had the most cases (60.28%). There were 1 508 Qs and As records for medication mistakes that were occured or may occure and were corrected in time(11.33%),in which the mistake types were mainly the unsuitable use of antipyretics(3.91%),antibiotics abuse(3.14%), careless use of combination medication and excessive medication(3.04%),excessively large or small doses(0.69%),improper use of nebulized inhalant after discharge(0.41%),and excessive use of vitamins(0.14%). CONCLUSIONS:The WeChat plat-form expands the service scope of clinical pharmacists.As an extension of hospital pharmaceutical care,it reduces the risk of fami-ly medication mistakes due to a patient's or members'wrong recognition to a certain degree,reinforces communication between doctors and patients,and improves the experience of medical services.
3.Pharmaceutical Care for Children Based on WeChat

Objective To improve the quality of pharmaceutical care in specialized children ' s hospital, provide convenient on-line medical consultation to parents, create the professional brand of whole-ranged pharmaceutical care by clinical pharmacist. Methods The First WeChat platform named " drugs consultation of Shanghai Children ' s Hospital" was inaugurated in Shanghai. Parents of children could search WeChat code or scan the QR code to join in WeChat friends, and asked questions online. Clinical pharmacists took turns on duty to answer questions online every day. Results During June 2014 to January 2016, number of WeChat friends was more than 8000, total number of medication consultation was 13315, average number of daily effective medication consultation was 26 (the peak was more than 60), 20 original medical articles of popular science were released, 100 other medical articles of popular science were reposted. Peak date of consulting was Tuesday, trough date of consulting was Saturday. Peak time of consulting was 9:00-10:00, while trough time of consulting was 4:00-5:00. For consultation, top ten diseases or symptoms was cough, fever, runny nose, sore throat, coughing phlegm, diarrhea, infection, pneumonia, skin eczema, adverse drug reactions; Top ten drugs was ibuprofen suspension or suspension drops, ambroxol hydrochloride and clenbuterol hydrochloride oral solution, paracetemol, pseudoephedrine hyrochleride, dextromethorphan hyorobromode and chlorpheniramine maleate suspension, pediatric pseudoephedrine hydrochloride and dextromethorphan hydrobromide drops, azithromycin tablets or suspension, vitamin, combined bacillus subtilis and enterococcus faecium granules with multivitamines, live, ambroxol hydrochloride oral solution, montmorillonite powder, loratadine tablets or syrup. Conclusion New mode of internet+pharmaceutical care is recognized by public and official. Clinical pharmacist can ensure the safety of pharmacy in children inside and outside of hospital effectively through WeChat real-time online interaction with parents. Professional brand of full process pharmaceutical care has been made by clinical pharmacists successfully.
4.Application of magnetic resonance imaging in nerve fiber tract analysis in a mouse model of stroke

Objective:To evaluate the cerebral infarct volume and the nerve fiber connectivity between cortical and neurogenesis-related regions in the mouse model of reperfusion after middle cerebral artery occlusion (MCAO) by 11.7 Tesla(11.7 T) magnetic resonance imaging (MRI).Methods:MCAO models were established in SPF grade adult male C57BL/6 mice using the suture-occluded method.MRI scans were performed at 3 days before and 1 day after modeling.Infarct volumes were calculated, and nerve fiber tracking was performed on specific brain regions to analyze the nerve fiber number and the parameters of fractional anisotropy(FA), mean diffusivity(MD), axial diffusivity (AD)and radial diffusivity(RD). SPSS 26.0 was used for statistical analysis, and paired t test was used to compare the data before and after modeling. Results:(1) After MCAO-induced ischemia, the infarct volume was up to (35.11±17.57)mm 3, and the FA value of the infarct area was significantly reduced compared with that of before modeling( t=4.73, P<0.01). (2) At the anterior-posterior(AP): + 1.2 mm section, the results of fiber tracking showed that compared with before modeling, the number of fiber bundles originating from the dorsal horn of the lateral sub-ventricle zone(SVZ)to the cortex reduced ((92 584.20±14 751.00) vs (59 815.60±6 752.46), t=4.87, P<0.01), and the number of fiber bundles projected to the infarcted area reduced ((107 671.40±10 497.57) vs (61 658.60±10 178.21), t=6.43, P<0.01). FA, AD, MD, and RD values were all decreased in different degrees( t=3.38-6.43, all P<0.05). (3) At the AP: -3.8 mm section, the number of fiber bundles originating from the dorsal horn of the SVZ to the cortex decreased (after modeling(96 944.00±18 331.09), before modeling(58 767.80±16 445.25), t=2.99, P<0.05), and the values of FA, AD, MD and RD decreased after ischemia ( t=7.30, 5.05, 6.74, 4.13, all P<0.05). Conclusion:The ultra-high field strength of 11.7 T MRI can accurately detect the following results that the number of nerve fiber bundles from the SVZ to the cortex or infarct area are both significantly reduced, and diffusion tensor parameters are consistently changed in mice after 1 day of ischemia-reperfusion.
