The article is attempted to introduce PBL teaching in clinical biochemistry. The PBL teaching has improved some aspects of students' abilities in the self-study, accessing to information, solving the problem, teamwork and developing thinking. But in the course of PBL teaching, there are three problems: shortage of qualified teachers, teaching system and students' inability. This article puts forward the countermeasures to resolve these problems.

Objective To evaluate the efficacy and safety of micafungin in the treatment of invasive fungal infections (IFI) in patients with acute leukemia. Methods A total of 133 IFI patients with acute leukemia received micafungin 150 mg once daily for 14 days. The clinical and mycological efficacies were evaluated on (7±2) days and(14±2) days of treatment.Meanwhile, the adverse events were recorded. The normally distributed data was compared using analysis of variance and nonnormal distributed data was analyzed using Wilcoxon rank-sum test. Results Among 133 IFI patients with acute leukemia,116 finished the 14-day micafungin treatment. The total clinical efficacy was 94.8% and the total mycological efficacy was 75.0% at (14±2) days of treatment. The fungus eliminate rates were 82.9%,66.7% and 55.6% against Monilia,Aspergillus and others, respectively. The clinical and mycological efficacies of (14±2)-day treatment were both higher than those of (7±2)-day treatment(X2=6.060,34.416.both P<0.05).The clinical efficacy was not related with age, sex,IFI diagnose, types of leukemia and combinative drugs (X2=26.541,P<0.05).The incidence of drug-related adverse events of micafungin was 3%among 133 patients, which included skin rash in 3 eases, diarrhea in 1 case. Only one case was discontinued because of severe skin rash and micafungin was well tolerant in other patients. Conclusion Treatment of micafungin 150 mg daily for 14 days is effective and safe in IFI patients with acute leukemia.

OBJECTIVETo explore evaluation strategies for middle ear dysfunction in cleft palate patients, to optimize the diagnosis and treatment of this dysfunction, and ultimately to improve the comprehensive treatment of cleft palate.
METHODSThe relationship among abnormal tympanic types (B, C, and Anomaly), effusion rate, tympanic pressure, and hearing loss were analyzed. We collected relevant information on 469 ears of cleft palate patients and traced one-year longitudinal changes in the tympana of 124 ears from 62 patients with both cleft lip and cleft palate.
RESULTSThe effusion rates of cleft palate patients with type B, type C, and type Anomaly were 50.3% (97/193), 34.8% (8/23), and 20.9% (53/253), respectively. The tympanic pressure of the ears with and without effusion showed no significant difference (P>0.05). The hearing loss in type B cleft palate patients with middle ear effusion was worse than that in patients without effusion (P=0.001). However, the hearing loss in type Anomaly showed no difference (P>0.05). The constituent ratio of each tympanic type remained constant during the period between cheiloplasty and palatoplasty for cleft lip and palate patients (P>0.05).
CONCLUSIONCleft palate patients of all tympanic types may all suffer from middle ear effusion at different rates. Examination by centesis is suggested for ears with abnormal tympanic types. Early aggressive therapy is essential for type B cleft palate patients with middle ear effusion to avoid hearing loss. However, catheterization may be not necessary for type Anomaly patients, and conservative observation should be performed instead. Myringotomy with grommet insertion during palatoplasty does not delay treatment timing for patients with both cleft lip and cleft palateg.

In recent years, researches about liquid biopsies which contain circulating tumor cells (CTC), circulating tumor DNA (ctDNA) and exosomes in the body fluids of patients with solid tumor have caused great concern. Compared with traditional biopsies, liquid biopsies have many advantages as they are real time, noninvasive and comprehensive. With the progress in the detecting techniques of CTC, ctDNA and exosomes, liquid biopsies can be applied to the early screening, diagnosis, treatment, prognostic evaluation of tumor. This review focuses on the recent advances in detection methods and clinical applications of CTC, ctDNA and exosomes.(

Journal of International Oncology 2016;43(9):706-710
Long noncoding RNA (lncRNA) is a class of RNA molecules that longer than 200 nucleotides.lncRNA is lack of functional open reading frames and has no protein coding ability.Recently, accumulating evidences indicate that lncRNA plays active roles in tumor carcinogenesis and progression. The aberrant expression of lncRNA is significantly correlated with the growth, metastasis, and therapy-resistance of digestive system tumors. Thus, lncRNA may be served as new targets for the diagnosis, treatment, and progno-sis of patients with digestive system tumors.

AIM: To explore the mechanism of Nao-re-qing oral liquid (NRQ) decreasing endotoxin (ET)-induced fever in rabbits. METHODS: (1) The ET-induced fever model was established in rabbits. Febrile response of rabbits was observed. (2) The arginine vasopressin (AVP) content in the ventral septal area (VSA),and cAMP content in hypothalarmus (HP) and CSF were determined by radioimmunoassay.RESULTS: (1) In ET group, the maximal increment in body temperature (?T) [(1.80?0.16) ℃],6 h thermal respone index (TRI_6)(11.31?0.20),the cAMP content in the HP [(1.35?0.21)nmol/g],the cAMP content in CSF [(66.69?1.82) nmol/L] and AVP content in the VSA [(30.80?9.59)ng/g ] were significantly higher than those in NRQ+ET group[?T(0.82?0.08) ℃,TRI_6(5.73?0.09),HP: cAMP(0.70?0.50)nmol/g,CSF: cAMP(56.86?1.34),AVP:(11.91?3.47)ng/g]( P

Objective To explore the clinical significance of expression of soluble mesothelin relatedprotein(SMRP)and carbo-hydrate antigen (CA125)in the serum and tumor tissues of the patients with ovarian cancer. Methods The preoperative and post-operative levels of SMRP and CA125 in serum and ovarian cancer tissues were detected in 82 patients with ovarian cancer (group A),76 cases of benign ovarian tumor (group B)and 53 healthy women (group C)by using ELISA and the immunohistochemical method respectively.Serum levels of SMRP and CA125 in the ovarian cancer patients were measured after one year by ELISA.The correlation among the various statistical indexes was analyzed. Results The positive expression rates of SMRP and CA125 in the group A were significantly higher than those in the group B(P <0.05);compared with the group B and C,the preoperative serum level of CA125 and SMRP in the group A was significantly increased (P <0.001);the preoperative serum CA 125 level in the group B was higher than that in the group C;compared with the stage Ⅰ and Ⅱ,the serum CA125 and SMRP in the stage Ⅲ and Ⅳ of o-varian cancer were significantly increased(P <0.05 );compared with before operation, the postoperative SMRP and CA125 levels were significantly decreased(P <0.05).After 1 year of discharge from hospital, compared with the basically stable patients, serum CA125 and SMRP levels in the patients with ovarian cancer recurrence were significantly increased(P <0.05).The sensitivity and specificity for diagnosing ovarian cancer, any single detection was inferior to the combination detection of CA125 and SMRP.Conclu-sion The combination detection of CA125 and SMRP has an important value for increasing the sensitivity and specificity of ovarian cancer diagnosis, early diagnosis, illness condition monitoring and effect evaluation.
