1.β-Catenin and The β-Catenin Destruction Complex: From Basic Science to Drug Design

The canonical Wnt/β-catenin signaling pathway plays critical roles in both embryonic development and tumorigenesis. Central to the pathway is the turnover of β-catenin, a protein that functions in both cell adhesion and transcription. In the absence of a Wnt signal, free cytosolic β-catenin is phosphorylated by a large protein complex called the "β-catenin destruction complex" that targets β-catenin for degradation by an ubiquitin ligase/proteasome system. In the presence of a Wnt signal, the binding of Wnt to its receptor Frizzled and co-receptor LRP leads to the inhibition of β-catenin phosphorylation in the β-catenin destruction complex through an unknown mechanism. Inhibition of the β-catenin destruction complex leads to the accumulation of nuclear β-catenin, which in turn forms a complex with Tcf and BCL9. Recent studies have provided important clues regarding the molecular mechanism of the β-catenin destruction complex as well as an explanation for how β-catenin switches between its roles in cell adhesion and transcription.
2.Clinical characteristics of neonatal severe purulent meningitis

Objective:To investigate the clinical characteristics of severe purulent meningitis in neonates.Methods:A retrospective study was conducted.One hundred and sixty-nine newborns with purulent meningitis diagnosed at the neonatal center of our hospital from January 2014 to December 2017 were selected.According to the severity of the disease, the cases were divided into severe group and mild group.The clinical data of all children were collected and analyzed, and the characteristics of severe purulent meningitis were summarized.Results:Among 169 cases of neonatal purulent meningitis, 43 cases(25.4%)were in severe group, and 126 cases(74.6%)were in mild group.Twenty-one cases were cured in severe group, 10 cases had complications, 9 cases abandoned and 3 cases died.Ninty-eight cases were cured in the mild group, 17 cases had complications and 11 cases were discharged automatically and 2 cases died.There were significant differences in respiratory failure requiring mechanical ventilation, convulsion, consciousness disorder, blood C-reactive protein, positive cerebrospinal fluid culture, severe abnormality of amplitude integrated electroencephalogram, cerebrospinal fluid/serum glucose ratio, the incidence rate of complications and mortality between two groups( P<0.05). Conclusion:Severe purulent meningitis not only has the manifestation of mild meningitis, but also often has the clinical characteristics of brain parenchymal damage and/or brain failure with more complications and higher mortality.
3.Effect of Insulin Injection on the Stability of Cefodizime Sodium for Injection

OBJECTIVE: To study the effect of insulin injection on the stability of cefodizime sodium for injection in 5% glucose injection.METHODS: The appearance and the pH value of the mixed solution in 12 h were monitored and the content of cefodizime sodium in the mixed solution was determined by HPLC.RESULTS: There were no evident changes for the mixture in appearance,pH value and the content of cefodizime sodium within 12 h at room temperature.CONCLUSION: Insulin injection has no effect on the stability of cefodizime sodium within 12 h at room temperature.
4.Application of amplitude-integrated electroencephalography in term neonates with brain injury

Objective To explore the value of amplitude-integrated electroencephalography(aEEG) in diagnosis and prognosis in term newborns with brain injury.Methods One hundred and thirty consecutive patients with brain injury admitted in NICU were prospectively enrolled in the study from Nov 2013to Apr 2015.The monitoring of aEEG was done at 1d,4d,7d,respectively.Clinical data were collected and com-pared with the result of aEEG.Results All the cases of newborns with abnormal aEEG background:discon-tinuous voltage(109cases),continuous low voltage(12cases),flat(4cases).The epileptic activity were re-corded in 33cases,and 15cases showed burst-suppression;sleep wake cycle:mature(32cases),immature (54cases),no sleep wake cycle(39cases).The judgment of abnormal level:70cases had mildly abnormal aEEG,severe abnormalities 60cases,and no significant difference in different types of diseases(x2=6.176, P=0.19).Recent prognosis:the death of mild and severe abnormalities were 1case,12cases,respectively, there were significant differences(x2=12.76,P﹤0.001).Developmental quotient(DQ)of these newborns were followed up for more than 6months,and there were significant differences in mild,severe abnormal aEEG in them with DQ≥85and DQ﹤85(x2=33.195,P﹤0.001).The sensitivity of aEEG in severe abnor-mal aEEG was 68.75%,the specificity was 78.68%,and the positive and negative predictive values of aEEG were 77.19%and 70.58%,respectively.The results of aEEG classification and sleep wake cycle were corre-lated with the prognosis of the patients(r=0.505,0.507,respectively,P﹤0.001).Conclusion aEEG can be used to monitor brain function,and it is helpful to evaluation of early diagnosis and prognosis.
5.Physical and mental health of children affected with AIDS and other children living in vulnerable families in a county of Yunnan province

Objective To understand physical and mental health status of children affected with AIDS and other children living in vulnerable families in a pilot county of the China /UNICEF AIDS Prevention and Care Project.Methods By random sampling,250 children of 0-17 years of age in a project county in Yunnan province were selected from 3 types of family:with family members affected with AIDS,with family members using illicit drugs,and poor family.Height were measured to all 250 children.Psychological questionnaire was used to interview 154 children over 6 years.Results There was no significant difference in height,but significant differences in psychological status among children from the 3 types of families.Among the children affected with AIDS,near 68% had difficulty or were indecisive in coping with problems in their daily life ;58% were worried about peer interaction;67% were relatively self-confident,which was lower than children affected by drug abuse,and the latter was 89%.Conclusion The mental status of children from the 3 types of family is different,and community care should be provided by considering the difference between them.
6.Successful practice of implementing community-based peer education in response to HIV/AIDS among out-of-school teenagers.

Objective To summarize the experience of HIV/AIDS preventive interventions among out-of-school teenagers and provide scientific evidence for the development of large-scale interventions in the future. Method Using both non-random snowball and cluster sampling approach,a total of 5,000 participants were selected from 7 provinces (9 provincial sites ) and a cross-sectional survey was conducted by employing a semi-open-ended self-administrative interview schedule to evaluate their behavioral change before and after the interventions. Results The rate of HIV related knowledge and condom use increased significantly among the participants. 7.Acetylized Racemization Process and Kinetics of D-penicillamine

OBJECTIVE:To study the process of acetylized racemization of D-penicillamine in the acidic condition. METHODS:The acetylized racemic mixture of D-penicillamine was prepared by racemizing acetyl chloride in acetic acid solu_tion with D-penicillamine as feedstock.The preparation process was optimized with the quantities of solvent and acetyl chloride,the reaction temperature,the reaction time etc.as parameters.The influence of reaction temperature,reaction time on the specific optical rotation in the acetylized process was determined and the kinetic equation of acylation process was computed. RESULTS:The optimal condition for racemization was the following,the quantity ratios of D-penicillamine-acetic acid -acetyl chloride were 1∶7∶2,the reaction temperature was 80℃ and the reaction time was 5h.The kinetic equation of acylation process fitted first order linear relation. CONCLUSION:This preparation process is mild and simple,and it offers direct feedstock for the preparation of D,L-penicillamine.
8.Allogeneic hematopoietic stem cell transplantation followed by donor derived lymphocyte and EBV specif-ic cytotoxic T-lymphocyte immunotherapy for hydroa vacciniforme-like lymphoma:A case report and literature review

The hydroa vacciniforme -like lymphoma is clinically manifested by rash ,fever,hepatospleno-megaly and lymphadenectasis .Pathological character showed that lymphocytes infiltrated into dermis and there were some heterocysts.Immunohistochemistory showed that Bcl -6,Bcl -2,CD3,CD8,TIA,CD4,granzyme, EBER were positive,CD56,CD20 all tested negative.The rearrangement of TCR of bone marrow cell was positive . The hydroa vacciniforme-like lymphoma is uncommon clinically ,and it is easy to misdiagnosed .The effect of tra-ditional therapy is poor ,while the allogeneic hematopoietic stem cell transplantation followed by lymphocyte and donor-derived EBV-specific cytotoxic T -lymphocyte immunotherapy is effective ,which may offer an curable therapy for HVLL.
9.Comparison of efficacy between emergency ESWL and elective ESWL in management of ureteral stones

Objective To compare the efficacy of the emergency ESWL within 24 hours after the onset of renal colic and elective ESWL at 5 - 7 days after the onset of renal colic in the treatment for ureteral stones.Method From January 2008 to October 2009, 365 patients in the Second Hospital of Nanchang, with ureteral stones were admitted to our department for renal colic. They underwent emergency ESWL or elective ESWL with HK. ESWL- 108 energy electromagnetic shock wave lithotripter. Success rate of fragmentation, stone-free rate after 2 weeks of operation, pain recurrence rate, and complications were evaluated. The data were analyzed using SPSS 15.0. Results The success rate of fragmentation and the stone-free rate after 2 weeks of emergency ESWL were 96.76% and96.21%, respectively, which were higher than those (88.33% and 87.22%, respectively) of elective ESWL ( P < 0. 05). The postoperative pain recurrence rate of emergency ESWL ( 4.32 % ) was lower than that (9.44%) of elective ESWL ( P <0.05). There was no difference of total complications between emergency ESWL (3.24%) and elective ESWL (2.78%) group (P >0.05). Conclusions Emergency ESWL is superior to selective ESWL and is the safe and effective method for the management of ureteral stones.
10.Expression profiles of microRNA molecules in different stages of chronic hepatitis B virus infection and its clinical significance

Objective To explore the expression profiles and their clinical significance of microRNA (miRNA) molecules in different stages of chronic hepatitis B virus (HBV) infection.Methods The miRNA molecule expressions of 12 patients with chronic hepatitis B,12 chronic HBV carriers,12 inactive hepatitis B surface antigen (HBsAg) carriers,and 9 healthy controls were screened using miRNA microarray.The miRNA profiles were validated by the real time fluorescence quantitative polymerase chain reaction (PCR).The t-test was used for comparison between twogroups,whereas one-way ANOVA and SNK-q tests were used for multiple comparisons.Mann-Whitney and Kruskal Wallis H tests were used for comparison of two or more groups of data with skewed distribution.The receiver operation characteristic (ROC) curve was constructed to evaluate the diagnostic significance of miRNA molecules.Results Compared with the healthy controls,significant differences in the expression profiles of miRNA molecules were found in peripheral blood mononuclear cells of chronic HBV carriers (2 molecules up-regulated,and 18 down regulated) and chronic hepatitis B patients (33 molecules up-regulated and 19 down-regulated).No significant difference was found between inactive HBsAg carriers (2 molecules up regulated,and 3 down-regulated) and controls.The results of six miRNA molecules detected by real-time fluorescence quantitative PCR were basically consistent with the results detected by microarray.The area under ROC curve of the six miRNA molecules of hsa miR-4711-3p,hsa-miR-3191 5p,hsa-miR-5704-5p,hsa-miR 548ah-5p,hsa-miR-146a-5p and hsa-miR-29b-3p in distinguishing immune tolerance and clearance of chronic HBV infection were 0.994,0.984,0.967,1.000,1.000 and 0.996,respectively.Conclusions The different expression profiles of miRNA molecules could be used to distinguish the different stages of chronic HBV infection,and are closely related with the immune tolerance and activation in chronic HBV infection.
