1.Application of laparoscopic operations for acute or chronic pelvic inflammatory diseases

Objective To investigate the value of laparoscopic operations for the diagnosis and treatment of acute or chronic pelvic inflammatory diseases (PID). Methods A series of 83 patients with PID diagnosed and treated by laparoscopic technique from January 1999 to December 2003 were retrospectively analyzed. Results Conversions to open surgery were required in 2 patients because of serious pelvic adhesions. No complications were observed. The diagnostic accordance rate before and after the surgery was 63.9% (53/83): 74.3% (26/35) in patients with acute PID and 56.2% (27/48) in patients with chronic PID. Among 35 patients with acute PID, the duration of operation was 20~75 min (mean, 44.7 min) and the postoperative hospital stay was 3~12 d (mean, 6.2 d). Postoperative pyrexia occurred in 28 patients, whose temperatures returned to normal within 5 days. The blood routine examination results were restored to normal levels in 26 patients at 2 days after operation. Out of 48 patients with chronic PID, the length of operation was 20~130 min (mean, 75 min) and the postoperative hospital stay, 2~8 d (mean, 5.5 d). Postoperative pyrexia occurred in 5 patients and a complete recovery of temperature was observed within 3 days. The blood routine examinations revealed normal findings in 42 patients at 2 days after operation. Follow-up checkups for 3 months in all the patients found no relapse. Conclusions Prompt application of laparoscopic operations is of importance in increasing diagnostic accuracy, improving curative efficacy, and preventing possible complications.
2.Effects of laparoscopy in the early diagnosis of atypical tubal pregnancy

Objective To investigate effects of laparoscopy in the early diagnosis of atypical tubal pregnancy.Methods We retrospectively studied clinical records of 36 cases of "probable tubal pregnancy" from January 2003 to May 2005 in this hospital.All the patients were given transvaginal ultrasonography and serum ?-human chorionic gonadotropin examinations.Then laparoscopy was performed to clarify the diagnosis.The accordance rate before and after the laparoscopy was evaluated. Results After laparoscopy,26 patients were confirmed as having ectopic pregnancy(oviduct,25 patients;ovary,1 patient) and 3 patients were excluded for having ectopic pregnancy,the confirmative diagnosis rate being 80.5%(29/36).The diagnosis could not be identified with certainty in 7 patients,2 of whom were cured with radix trichosanthis and 5 of whom were afterwards clinically diagnosed as having tubal pregnancy(1 patient) or intrauterine early pregnancy(4 patients).Conclusions Laparoscopic management at proper time is the best choice in the diagnosis of ectopic pregnancy,especially in early-stage or atypical cases.
3.The protective effect of pyrrolidine dithiocarbamate on acute radiation injury in mice

Objective To study the protective effect of pyrrolidine dithiocarbamate (PDTC) on acute irradiated mice.Methods The 6-8 weeks old male ICR mice were randomly divided into five groups:irradiation alone group (IR),positive control group (amifostine WR-2721 250 mg/kg) and PDTC of 30,60 and 90 mg/kg dose groups.Each group had 10 mice and the drug was given at 0.5 h before whole body irradiation.At 30 d post-irradiation of 7.5 Gy 137 Cs γrays,the mice survival were observed.At 8 d post-irradiation of 5.0 Gy 137 Cs γ-rays,the peripheral blood,hematopoietic system and organ indexes were observed to evaluate the radiation protective effect of PDTC.Results PDTC increased the 30-day survival rates and 60 mg/kg dose had the most obvious effect by increase the survival to 60% (6/10).The survivals of irradiation alone group and the amifostine positive control group was 10% (1/10) and 70% (7/10),respectively.Compared with the irradiation alone group,60 mg/kg PDTC group had the significant difference in spleen index,WBC,HGB,PLT,bone marrow nucleated cells and colony forming unit of spleen (t =2.354,4.793,2.342,6.542,2.649,3.982,P < 0.05).Conclusions PDTC is effective in radiation protection with an optimum dose of 60 mg/kg.
4.Factors Inlfuencing Shear Wave Velocity by Using Elasiticity Imaging Technique

Purpose To investigate the influence on shear wave velocity (SWV) by different probe frequency, scanline angle and ROI depth. Materials and Methods The elasticity QA Phantoms was used in the study. First the SWV was measured by curve probe at depths of 1 cm, 2 cm, 3 cm, 4 cm, 5 cm, 6 cm, 7 cm and 8 cm. The scanline angle changed from 0°from midline, 1°-15°, 16°-30°and 31°-45°. Then the SWV was measured again by using line probe at depths of 1 cm, 2 cm, 3 cm and 4 cm. The measured data were analyzed statistically. Results The SWV showed difference by using the two probes (P<0.05). As depth increased, the measurement by curve probe changed from overestimate to underestimate, while the measurement by line probe always showed underestimate from standard value. When the curve probe was used, the angle and depth affected SWV interactively (F=85.87 and 35.96, P<0.01), and had interactive effects (F=17.63, P<0.01). When the scanline angle was within 15°, the angle had little inlfuence upon the measurement. When the scanline angle was 0 °, the measurement was the closest to the standard value at the depth of 4 cm, and the maximum deviation occurred at the depth of 5 cm. The inlfuence of depth upon measurement had signiifcant difference when the line probe was used (P<0.05). The measurement was the closest to the standard value at the depth of 1 cm, and the maximum deviation occurred at the depth of 4 cm. Conclusion Factors like probe frequency, scanline angle and ROI depth can affect the SWV. When the curve probe is used, the accurate measurement occurs at the depth of 4 cm when the scanline angle is within 15°. When the line probe is used, the accurate measurement occurs at the depth of 1 cm.
5.Complication of cesarean section: pregnancy on the cicatrix of a previous cesarean section.

OBJECTIVETo probe into the clinical manifestation, diagnosis, as well as treatment of pregnancy on the cicatrix of a previous cesarean section at the uterine isthmus in the first trimester.
METHODSAnalysis of 14 patients with pregnancy on the cicatrix of a previous cesarean section at the uterine isthmus in the first trimester was made after conservative treatment by drugs from January 1996 to December 1999.
RESULTSThe 14 patients with a pregnancy on the cicatrix of a previous cesarean section at the uterine isthmus in the first trimester were painless, had slight vaginal bleeding, and concurrently had increased serum beta-subunit human chorionic gonadotropin (beta-HCG). Doppler ultrasonic examination revealed an obvious enlargement of the previous cesarean section cicatrix in the uterine isthmus, and found a gestational sac or mixed mass attached to the cicatrice, with a very thin myometrium between the gestational sac and bladder walls. Among the 14 patients, 12 patients had crystalline trichosanthes injected into the cervix, mifepristone taken orally, or methotrexate in the form of intramuscular injection. Following this procedure, their serum beta-HCG dropped to normal. The other 2 patients had a total hysterectomy.
CONCLUSIONSPregnancy on the cicatrix of a previous cesarean section at the uterine isthmus in the first trimester is a complication of cesarean section. Early diagnosis and effective conservative treatment by drugs are instrumental in decreasing the potential occurrence of uterine rupture, which is also conducive to preserving the patient's future fertility.

6.Triterpenes from Kalimeris indica.

OBJECTIVETo study the constituents of Kalimeris indica.
METHODThe constituents were isolated by various chromatographic techniques and their structures were elucidated by their physicochemical properties and the spectral data analysis.
RESULTEleven compounds were isolated and identified as dammaradienyl acetate(1), friedelin(2), friedelinol(3), beta-amyrin(4), alpha-amyrin(5), lupenyl acetate(6), beta-sitosterol(7), alpha-spinasterol(8), alpha-spinasterone(9), stigmasterol(10), daucosterol(11).
CONCLUSIONCompounds 4-6, 8-11 were isolated from the genus Kalimeris for the first time.

Objective To observe the regulation of Panx1 on ATP/IP3 signaling pathway and its mechanism during cisplatin-induced apoptosis of lung adenocarcinoma cells. 8.Quality and reporting standards appraisal of guidelines and consensuses on ketogenic diet therapy in children with refractory epilepsy

Objective:To evaluate the quality and reporting standards of the ketogenic diet guidelines and consensuses in children with refractory epilepsy at home and abroad.Methods:A systematic search was conducted using the English and Chinese keywords for screening relevant guidelines and expert consensus on the Guidelines International Network, National Guideline Clearinghouse, Scottish Intercollegiate Guidelines Network, National Institute for Health and Clinical Excellence, the Joanna Briggs Institute (JBI) Center for Evidence-Based Health Care in Australia, DynaMed Evidence-Based Medicine Database, American Academy of Neurology, American Epilepsy Society, International League Against Epilepsy, Child Neurology Society, European Paediatric Neurology Society, International Ketogenic Diet Research Group, Cochrane Library, PubMed, CNKI, Wanfang Database, VIP, China Biology Medicine disc, etc. The search period was from establishment of databases to August 13, 2021. The quality of the guidelines was evaluated using the The Appraisal of Guidelines for Research and EvaluationⅡ, and the Reporting Items for Practice Guidelines in healthcare was used to evaluate the reporting standards of guidelines. The JBI Center for Evidence-based Health Care Quality Evaluation tool was used to evaluate the quality of expert consensuses, and the National Institutes of Health Reporting Standards was used to evaluate the reporting standards of expert consensuses.Results:A total of 1 864 literatures were searched initially. According to the inclusion and exclusion criteria and reading the literature content, 13 literatures were selected, and after manual search, 1 literature was supplemented. Finally, a total of 14 literatures were obtained, including 7 guidelines and 7 expert consensuses. The quality evaluation results showed that 2 guidelines were grade A, 2 guidelines were grade B and 3 guidelines were grade C. 5 expert consensuses were rated "Yes" in the six items of quality evaluation. The results of the evaluation of reporting norms showed that the seven expert consensus reports were of poor quality; the higher the methodological quality rating of the guidelines, the better their reporting quality.Conclusions:The overall quality of the 14 guidelines and expert consensuses included is high, and attention should be paid to research quality in terms of application and participants. The reporting standards need to be further improved, and in the future, researchers can pay more attention to the reporting standards in the process of formulating guidelines and expert consensus, achieving a more rigorous research process and better applying evidence to clinical practice.
9.Role of intestinal stem cells in prevention of radiation-induced intestinal injury

At present, there is no effective treatment method of radiation-induced intestinal injury. Clinically, only symptomatic remission therapy can be used. Intestinal stem cells (ISCs) have strong renewal ability, which is an important part of intestinal structure regeneration. To promote the recovery of the number and function of ISCs has been a research hotspot in the treatment of radiation gastrointestinal syndrome. This article introduces the important role of ISCs in the protection of radiation-induced intestinal injury and its clinical application significance from the classification and regulatory mechanism of ISCs, and drugs that regulate the damage, apoptosis and repair of stem cells, so as to provide reference for the future research on the role of intestinal stem cells in radiation protection.

Nasopharyngeal carcinoma (NPC) is the third most common malignancy with a high recurrence and metastasis rate in South China. Natural compounds extracted from traditional Chinese herbal medicines have been developed and utilized for the treatment of a variety of cancers with modest properties and slight side effects. Maackiain (MA) is a type of flavonoid that was first isolated from leguminous plants, and it has been reported to relieve various nervous system disorders and exert anti-allergic as well as anti-inflammatory effects. In this study, we demonstrated that MA inhibited proliferation, arrested cell cycle and induced apoptosis in nasopharyngeal carcinoma CNE1 and CNE2 cells in vitro and in vivo. The expression of the related proteins associated with these processes were consistent with the above effects. Moreover, transcriptome sequencing and subsequent Western blot experiments revealed that inhibition of the MAPK/Ras pathway may be responsible to the anti-tumor effect of MA on NPC cells. Therefore, the effects of MA and an activator of this pathway, tertiary butylhydroquinone (TBHQ), alone or combination, were investigated. The results showed TBHQ neutralized the inhibitory effects of MA. These data suggest that MA exerts its anti-tumor effect by inhibiting the MAPK/Ras signaling pathway and it has the potential to become a treatment for patients with NPC.
