1.Two considerations about the current National Standard of Laboratory Animals

According the current National standard for laboratory animal, herpes B virus antibody must be tested and negative for all the breed/tested macaques,howere,in fact the positive rate is around 60-70% in breeding colony. Being the National Standard, if it is too far from the reality of the filed, weighing and changing should be made.Hardness/crispness degree of laboratory animal diet never been considered as a listed value in the National Standard for Laboratory Animal to identify the quality of diet.In fact, this index affects a lot about daily diet intake and physiological status of laboratory animals.Hereby, the authors suggest to take the index into the National Standard, to assess the quality of laboratory animal diet in a more comprehensive way.
2.Genetic analysis and prenatal diagnosis of a Chinese pedigree affected with microphthalmia/coloboma and skeletal dysplasia syndrome due to variant of MAB21L2 gene.

OBJECTIVE:
To explore the genetic basis for a Chinese pedigree affected with microphthalmia.
METHODS:
Clinical data of the proband was collected. Whole exome sequencing (WES) was carried out to screen potential pathogenic variants in the proband. Candidate variant was verified by Sanger sequencing of the proband and his family members. Pathogenicity of the variant was predicted by searching the PubMed database and bioinformatic analysis. Sanger sequencing of amniotic fluid sample was carried out for prenatal diagnosis.
RESULTS:
The proband and his father were found to harbor a heterozygous c.151C>G (p.R51G) variant of the MAB21L2 gene. The same variant was not found in his mother and grandparents. Based on the guidelines of American College of Medical Genetics, the c.151C>G (p.R51G) variant was predicted as likely pathogenic.
CONCLUSION
The c.151C>G (p.R51G) variant of the MAB21L2 gene probably underlay the microphthalmia in the proband. Above finding has facilitated prenatal diagnosis for this pedigree.

3. Clinical effects of microsurgery in spinal cord anaplastic astrocytoma

Objective:
To investigate the surgical outcomes and prognosis of spinal cord anaplastic astrocytoma (AA).
Methods:
A total of 27 consecutive patients diagnosed as spinal cord AA between January 2008 and May 2015 in Department of Neurosurgery of Beijing Tiantan Hospital were retrospectively reviewed. There were 18 males and 9 females, the mean age was (30.7±13.0) years (ranging from 5 to 52 years). The lesions were located at cervical level in 8 patients, at thoracic level in 9 patients, at cervicothoracic level in 3 patients, and at thoracolumbar level in 7 patients, the average number of vertebral was 3.3±1.3.The median time from onset of symptom to surgery was 4 months, ranging from 3 days to 48 months. The clinical presentations were weakness (23 cases), paresthesia (22 cases), pain (20 cases), sphincter disorder (15 cases) and paralysis (7 cases). The preoperative modified McCormick scale was as follows: grade Ⅱ for 6 cases, grade Ⅲ for 7 cases, grade Ⅳ for 7 cases, grade Ⅴ for 7 cases. The tumors were surgically removed via posterior median approach with the monitoring of the somatosensory-evoked potentials to minimize the neurological injury. All of the patients were recommonded to receive adjuvant chemotherapy and radiotherapy postoperatively after pothological verified and followed up by clinic interview or telephone postoperatively. Log-rank test was used to calculate the survival rate.
Results:
Gross total resection and subtotal resection were achieved in 18 patients and partial resection in 9. Twenty patients received adjuvant chemotherapy and (or) radiotherapy, 7 patients did not received chemoradiation postoperatively. Nineteen patients died and 8 were alive at the last follow-up. The median survival time was 23 months with 1 and 2-year survival rates of 85.2% and 50.0%.There was no statistical significance between subtotal resection group and partial resection group(χ2=0.089,
4.Design and implementation of surgical medical waste management system based on B/S structure

Objective:To explore the application effect of medical waste management system based on B/S architecture in medical waste disposal.Methods:Select a third-class hospital in Yantai for continuous quality improvement of medical waste disposal, and verify the feasibility of the medical waste management system through key indicators such as medical waste related theoretical knowledge, medical waste disposal efficiency, and medical waste disposal management process standardization rate. Through monitoring indicators and result feedback evaluation, it is judged whether the medical waste management system has positive significance for the improvement of medical waste disposal.Results:Taking the adoption of medical waste management system as the node, a five-month comparison before and after was conducted to monitor key indicators such as medical waste disposal efficiency and standardized rate of medical waste disposal management processes. The daily error rate of medical waste classification was reduced from 6.00% to 1.33%, the daily confusion rate of working procedures was reduced from 6.67% to 0.67%, the time delay rate of medical waste disposal was reduced from 7.33% to 2.00%, and the standardized rate of medical waste disposal management processes was increased from 93.33% to 100%. All the data were statistically significant ( P<0.05). Conclusions:Since the operation of the medical waste management system, through the top-down management and control of the administrator and with its unique time retrieval function, the time delay problem of the previous manual management has been greatly improved. The classification management function of medical waste solves the criticism of unclear and chaotic classification of medical waste. The department retrospective function enables the division of responsibilities for medical waste work to be clear, and improves the work efficiency. The construction of medical waste management system based on B/S architecture provided a new information approach for the scientific management of medical waste and a reference for the scientific and effective management of clinical medical waste.
5.Risk factors for senile patients with osteoporosis

Objective To explore the risk factors and nursing measures for senile patients with osteoporosis. Methods All 160 cases of senile osteoporosis patients in January 2012 to August 2014 in our hospital and 160 cases of non senile osteoporosis patients at the same period were as the research object,the osteoporosis patients were as the observation group,the non osteoporosis patients were as the control group,general information of two groups were compared,includ-ing age,height,weight and BMI,and analyzed the influence factors of osteoporosis,including risk factors Logistic re-gression in senile osteoporosis risk factors by univariate Logistic regression of senile osteoporosis and multiple factors,and to explore the nursing measures. Results①The average height of the observation group was(165.21±4.89)cm,the control group (168.44±6.23)cm, with statistical difference (t=4.2069, P<0.05); the average weight of the observation group was(64.21±10.01)kg,the control group was(68.37±10.24)kg,with statistical difference(t=3.3607,P<0.05);②Single factor Logistic regression analysis showed that the factors associated with senile osteoporosis including lie in bed,height, exercise, calcium agent and drink milk; ③Logistic multi factor regression analysis showed taller, sport and drinking milk were protective factors of senile osteoporosis,and lie in bed was the risk factor of senile osteoporosis. Conclusion The taller,sport and drinking milk are protective factors of senile osteoporosis,and lie in bed is risk factor for senile osteoporosis.
6.Growth Inhibitory and Pro-Apoptotic Effects of Hirsuteine in Chronic Myeloid Leukemia Cells through Targeting Sphingosine Kinase 1

Chronic myeloid leukemia (CML) is a slowly progressing hematopoietic cell disorder. Sphingosine kinase 1 (SPHK1) plays established roles in tumor initiation, progression, and chemotherapy resistance in a wide range of cancers, including leukemia.However, small-molecule inhibitors targeting SPHK1 in CML still need to be developed. This study revealed the role of SPHK1 in CML and investigated the potential anti-leukemic activity of hirsuteine (HST), an indole alkaloid obtained from the oriental plant Uncaria rhynchophylla, in CML cells. These results suggest that SPHK1 is highly expressed in CML cells and that overexpression of SPHK1 represents poor clinical outcomes in CML patients. HST exposure led to G2/M phase arrest, cellular apoptosis, and downregulation of Cyclin B1 and CDC2 and cleavage of Caspase 3 and PARP in CML cells. HST shifted sphingolipid rheostat from sphingosine 1-phosphate (S1P) towards the ceramide coupled with a marked inhibition of SPHK1. Mechanistically, HST significantly blocked SPHK1/S1P/S1PR1 and BCR-ABL/PI3K/Akt pathways. In addition, HST can be docked with residues of SPHK1 and shifts the SPHK1 melting curve, indicating the potential protein-ligand interactions between SPHK1 and HST in both CML cells. SPHK1 overexpression impaired apoptosis and proliferation of CML cells induced by HST alone. These results suggest that HST, which may serve as a novel and specific SPHK1 inhibitor, exerts anti-leukemic activity by inhibiting the SPHK1/S1P/ S1PR1 and BCR-ABL/PI3K/Akt pathways in CML cells, thus conferring HST as a promising anti-leukemic drug for CML therapy in the future.
7.Diagnosis and treatment of primary intraspinal melanocytoma.

OBJECTIVETo investigate the clinical features, diagnostic and therapeutic strategy of primary intraspinal melanocytoma.
METHODThe clinical data of primary intraspinal melanocytoma patients who underwent surgical operations from June 2011 to December 2014 in Department of Neurosurgery, Beijing Tiantan Hospital, Capital Medical University were retrospectively reviewed.
RESULTSSix cases of intraspinal melanocytoma, identified with pathology were collected, including 5 male and 1 female patients with the average age of 39.8 years (range 23-50 years). Limbs anesthesia and weakness were present in 2 cases. Neck and back pain were present in 4 cases. All patients performed MRI examination before operation, and only 1 case was diagnosed as primary melanocytoma, and the others were confirmed by postoperative pathology. All cases were confirmed by the pathological examination. Total resection was achieved in 2 cases, subtotal resection was achieved in 2 cases, 2 disseminated cases only achieved partial resection. The average period of postoperative follow-up process was 25.7 months(range 9-52 months). One case recurred in 9 months after surgery and died in 16 months, one case recurred in 1 year after surgery, and the other 4 cases had no recurrence. The recurrenced 2 patients had been not treated with radiotherapy, 2 patients of the 4 that not recurrenced had received radiotherapy. All patients didn't receive chemotherapy. The other patients were in good condition after surgery.
CONCLUSIONSThe intraspinal primary melanocytoma is less likely to recurrence and metastasis if total resection is achieved. Diagnosis relies on pathology. Surgery is the first choice and early total resection is strongly recommend. Appropriate radiotherapy for partial resection cases can delay tumor recurrence.
