Objective To study the imaging findings of the chest disease due to intravenous heroin abuse. Methods Twenty five cases of clinically confirmed chest disease due to intravenous heroin abuse were retrospectively analyzed. 25 cases had conventional X ray film, 6 cases had CT scanning, and 6 cases had echocardiography scanning. Results On X ray and CT, the following signs were found: lung making manifold ( n =5), small patchy shadow ( n =15), pneumatocele ( n =16), small cavity ( n =16), small node ( n =7), pleural effusion ( n =8), pneumothorax ( n =2), hydropneumothorax ( n =6), pulmonary edema ( n =2), megacardia ( n =11), multiple shaped lesion ( n =20). On echocardiography, tricuspid vegetation ( n =4) and tricuspid insufficiency ( n =4) were found. Conclusion The X ray and CT manifestations of chest inflammation due to intravenous heroin abuse are multiple. The multiple small cavities and pneumatoceles sign are of some value in the diagnosis of lung inflammation due to intravenous heroin abuse among young patients.

Chinese Journal of Practical Nursing 2009;25(14):16-19
Objective To calculate and analyze the grading nursing costs in a grade Ⅲ-A general hospital, and give suggestions to the government for rational pricing. Methods 6 wards were selected as the subjects, we calculated the total expenses by the ratio of income method, meanwhile, by sharing the cost we calculated the actual costs of grading nursing and let them underwent variance analysis. Results The actual cost of grade 1 nursing was (64.06±14.58) yuan, the actual cost of grade 2 nursing was (40.06±13.49) yuan, and the actual coat of grade 3 nursing was (31.11±5.81) yuan, by statistic analysis, the actual grad-ing nursing costs were significantly different from the current pricing system. Conclusions The grouping of nursing expense ia a difficult point in nursing cost study, and by calculating the actual grading nursing cost, we could develop measures to control the cost effectively. And the grading nursing total coat calculat-ing is the foundation of rational prices setting of grading nursing.

Objective To establish the capability level classification system and capability level staffing standard of ICUs in tertiary hospitals, in an effort to raise the efficiency of nursing resources while ensuring quality of care of patients. Methods Such methods as experts meetings and Delphi Method were called into play to establish the nurses capability level system in ICUs with the convenient sampling method, we made clinical observation and research in 5 ICUs of 3 tertiary hospitals in Shanghai. Results Nurses in ICUs were classified into four levels, so were their responsibilities and job description. This way the system of nursing staff capability levels takes shape, and the staffing standards for nurses in ICUs of tertiary hospitals come into being. Conclusion Such tiered staffing standards in tertiary hospitals enable clinical nursing managers to deploy and efficiently use nursing staff of all levels.

To investigate the nursing difference between abdominal aortic aneurysm patients undergone open surgery and endovascular graft exclusion, a comparative study was carried out in two groups of patients with infrarenal abdominal aortic aneurysms ( n =148), either by open surgical approach( n =96) or by endovascular stented graft technology( n =52). Four problems were compared between the two groups: (1) time of hospitalization, (2) dependency on nursing, (3)mobility after surgery, and (4) analgesic requirements. Data were obtained from a designated data sheet. The perioperation condition of the patients had no difference between the two groups, the time of hospitalization, dependency on nursing, mobility after surgery, and analgesic requirements in the endovascular stent repair group was shortened obviously. Analysis of the data can help us optimize the nursing process for patients undergoing open surgery and endovascular graft exclusion.

Objective To evaluate the decreased level of serum inhibin B(INHB)treated by gunadotropin releasing hormone agonist(GNRH-a)in predicting ovarian response and pregnancy in in vitro fertilization-embryo transfer(IVF-ET).Methods The prospective study enrolled 124 women given by GnRH-a+recombine follicle stimulating hormone(rFSH)+human chorionic gonadotrophin(hCG)long term stimulation protocol undergone their first cycle of IVF-ET treatment. The following predictive factors were collected and analyzed, such as age, basal level of follicle stimulating hormone(FSH),the ratio of FSH/luteinizing hormone(LH),the concentration of INHB after down-regulation,total number of antral follicle count(AFC)and mean ovarian volume. Ovarian response was evaluated by the number of oecytes obtained.A multiple regression analysis and logistic regression model were used for all possible prognostic variables to evaluate the value of difierent hormones in predicting ovariall response and pregnancy after IVF-ET.Receiver operating characteristic(ROC) analysis was used to evaluate the level of INHB in predicting the number of oocytes obtained. The sensitivity and specificity were calculated at the discriminating cut-off point Results The concentration of INHB after down-regulation showed a highly significant positive correlations with the number of oocytes obtained(r=0.435,P<0.01).The multiple regression analyses showed INHB was the most significant predictor of the number of retrieved oocytes, but INHB was not associated with IVF-ET outcome significantly(P>0.05).ROC analyses showed INHB after down-regulation had the largest area under curve(AUC)0.933(95%CI:0.878-0.988).When a threshold of 15 ng/L of INHB was established,95.5%sensitivity and 50.0% specificity in ovarian response were observed. Conclusions The level of INHB Was the best factor in predicting ovarian response in IVF-ET.Decreased level of INHB Was the early sign of ovarian reserve function failure, however, useless in predicting IVF-ET outcome.
