1.Progress of microglial efferocytosis in neonatal hypoxic ischemic encephalopathy
International Journal of Pediatrics 2023;50(12):815-819
Neonatal hypoxic ischemic encephalopathy(HIE)is a hypoxic ischemic brain injury caused by perinatal asphyxia. It is the major cause of neonatal death and neurological damage of infant and children, and its characteristic pathophysiological alteration is massive neuronal apoptosis.Microglia, one of the central nervous system phagocytes, play an important role in development of neonatal HIE.In recent years, efferocytosis has been discovered to be a specific phagocytosis of phagocytes. In neonatal HIE, microglia are able to remove apoptotic cells through efferocytosis, preventing further necrosis of apoptotic cells, which can trigger inflammatory responses and brain damage.Understanding the role of microglial efferocytosis in neonatal HIE may aid in elucidating its pathogenesis and exploring potential therapeutic approaches. This article is a review of the progress of microglial efferocytosis in neonatal HIE.

Objective To study efficacy and safety of cilazapril in treating patients with essential hypertension.Method 115 patients with hypertension were divided randomly into two groups:59 were treated with cilazapril and 56 were treated with enalopril. All the patients were washed out for one week, then took 2.5 mg cilazapril or 10 mg enalapril once a day in each group respectively for 8 weeks. If blood pressure was not reduced 2 weeks later, hydrochlorothiazide was added. Result The effective rate of cilazapril and enalapril was 89.8% and 91.1% respectively. The SBP and DBP reduced from 153±15 mmHg and 104±9 mmHg to 131±11 mmHg and 91±8 mmHg respectively in cilazapril group, and from 155±12 mmHg and 102±10 mmHg to 139±14 mmHg and 90±9 mmHg respectively in enalapril group. There were no significant differences in two groups. No side effects were observed in two groups. Conclusion Cilazapril is an effective and safe agent in treatment of mild and moderate essential hypertesion.

The impact factors were explored to determine the horizontal positional relationship between the umbilicus and the 2nd lumbar spinal process in adults and to verify the accuracy of the localization of Shenshu (BL 23) via the umbilicus. The position of the umbilicus and the 2nd lumbar spinal process was measured in 100 participants and the data were analyzed through SPSS 20.0 software. It was found that the umbilicus and the 2nd lumbar process were not positioned horizontally. The positional relationship of these two sites was not apparently correlated with gender, age, body weight, body height, BMI, waistline and discomfort of lumbar region. The umbilicus was commonly and posteriorly projected on the site between the 4th and 5th lumbar vertebra. It is explained that the localization of Shenshu (BL23) via the umbilicus is not accurate.

Objective To explore possible associations between host polymorphism of HLA classⅡgenotypes and advanced hepatosplenic schistosomiasis japonica. Methods 45 advanced schistosomiasis patients(experimental group) and 44 age,and sex, matched patients with chronic schistosomiasis(control group) from the same area were investigated for their HLA class II gene DRB genotypes by genotyping the alleles using microarray DNA chip. The correlation of allele frequencies to advanced hepatosplenic schistosomiasis was compared for the two groups. Results HLA,DRB1*04x exhibited markedly higher frequency in advanced patients than that in control group(P

The treatment status and progress in T cell lymphoma including epigenetic involved mutations that control DNA and histone methylation were reported and intensively discussed in 2014 international T cell lymphoma forum.According to the theory, treatment with HDAC inhibitor belinostat and romidepsin for peripheral T cell lymphoma (PTCL) can achieve 29 %-38 % overall response rate (ORR) and 13.6 months median relief time.NK/T cell lymphoma in southeast asia is a common malignant lymphoma,15 %-28 % of the NHL accounted in China and Japan for, which is significantly higher than that in the European and American countries, mainly related to EB virus widespread infection.L-asparaginase enzymes, such as SMILE and AspMetDex,can make the early cases with more than 70 % long-term survival rate, advanced cases with 40 % response rate. Some new drugs, such as pralatrexate, combined with romidepsin can be used in PTCL cases to improve the complete remission rate.

Objective To evaluate the efficacy and safety of the hyper-CVAD/MA regimen as an intensified treatment option for 28 T cell and aggressive/highly aggressive B cell NHL in Chinese patients. Methods Clinical data of 28 NHL patients treated with hyper-CVAD/MA regimen from Jan 2005 to Sep 2008 were retrospectively analyzed. Results 27 NHL patients were available for the efficacy analysis, with a response rate of 70.4 %. For the 13 B cell lymphoma cases, the response rate was 84.6 %. The main toxicity was Grade Ⅲ or Grade Ⅳ myelosuppression in all cases and 2 treatment related deaths. Conclusion Hyper-CVAD/MA regimen had a high response rate in T cell and aggressive /highly aggressive B cell NHL lymphoma, companied by significant toxicity when treating Chinese patients. Further clinical practices are needed to pick up a suitable dose which can balance efficacy and safety.

Depression is a common mental illness that seriously harms to the physical and mental health of human beings. Due to its biological-psychological-social interaction pathogenic feature, researchers hold different opinions for the pathogenesis of depression. Recently, accumulating the clinical and basic researches on the comorbidity of depression and diabetes make the roles of glucose metabolism, especially central glucose metabolism and insulin resistance in the pathogenesis of depression get much more attention. It has been believed that the "liver" and "spleen" are closely related to each other physiologically and pathologically. A large number of studies have confirmed that the core biological connotation of liver-spleen and pathology of traditional Chinese medicine is "emotion-energy network imbalance", which has similarities with the mechanism of glucose metabolism imbalance in depression. Therefore, it is necessary to combine the biological connotation of the liver and spleen with the modern medicine to conduct the further research, which will consequently provide scientific basis for further exploring the pathogenesis of depression and finding complementary and alternative treatment for depression.

Parkinson's disease with depression belongs to the category of "tremor syndrome" and "depression syndrome" of Traditional Chinese Medicine. Its syndrome characteristics and distribution are different from simple tremor syndrome and depression syndrome. Its neuroplastic mechanism involves dopamine, serotonin, norepinephrine and brain-derived neurotrophic factor. Parkinson's disease and depression have common neuro-biological mechanisms. Therefore, Chinese medicine treatment of Parkinson's disease with depression may have a dual regulation effect.

DNA methylation studies the natural modification of DNA that occurs when the nucleotide sequence is unchanged. At present, DNA methylation is used in the field of biomedicine to focus on the characteristics and regularity of disease-wide genome methylation and the DNA methylation biomarkers for disease. Based on the systematic review of DNA methylation research techniques, this paper summarizes and analyzes the current status of DNA methylation technology in basic research of TCM (traditional Chinese medicine) syndromes, and a suggestion to screen the TCM syndrome biomarkers from whole genome DNA methylation is proposed.
