1.Epidemiology of Drug Resistance Genotypes of Pseudomonas aeruginosa in Burn Ward
Chinese Journal of Nosocomiology 2006;0(12):-
OBJECTIVE To evaluate the epidemiology of Pseudomonas aeruginosa(PAE) nosocomial infection and analyze ?-lactamase in multi-resistant strains and the enzymes produced by PAE,and to detect the correlation of the PAE in patients without nosocomial infection and from the hospital environment. METHODS The strains were identified by VITEK32,dynamically the strains of multi-resistant PAE were selected with K-B susceptibility method, then the three-dimensional method, and metal-?-lactamase determined by Etest. The isolated PAE strains from the patients or the environment were analyzed according to the detected time, environmental site as which part of the patient by randomly amplified polymorphism DNA(RAPD). RESULTS Totally 352 strains of PAE were isolated from 4246 specimens, the isolated rate was 8%.Study on the drugfast pathogens and RAPD pathogens indicated that there was no obvious correlation between the PAE from inpatients and from hospital environment. CONCLUSIONS It indicates that PAE exists in the burn ward extensively and hospital environment and exogenous infection are unimportant in nosocomial infection. The most of PAE are derived from the patients themselves.

OBJECTIVE:To discuss the necessity of clearing the pharmaceutical butyl rubber stoppers for infusion bottles before use.METHODS:Pharmaceutical butyl rubber stoppers for infusion bottles produced by manufacturer A and B were se-lected and washed at least once before a preliminary test was performed. Then product A was chosen for further test. The stoppers of two different batch numbers by manufacturer A were selected, one washed with injecting water and another not washed, all of which were used in the production of all kinds of transfusions in the hospital where the authors worked, then infusible motes in these products were measured.RESULTS:The infusible motes in the products with unwashed stoppers was not in accordance with the requirements in China Pharmacopoeia, while those with washed ones was in accordance.CONCLU_ SION:The pharmaceutical butyl rubber stoppers for infusion bottles must be washed by injecting water at least once before use to ensure the quality of the products.

OBJECTIVE:To study the cost-effectiveness of4dose regimens in treating chronic prostatitis/chronic pelvic cavity pain syndrome(CP/CPPS).METHODS:A total of140patients with CP/CPPS were randomly divided into4groups:Baiyanjing tablets and tamsulosin hydrochloride modified release capsules were administered for Group A;minocycline micro pills and alfuzosin retard tablets for Group B;rexithromycin tablets and doxazosin mesylate controlled release tablets for Group C;and levofloxacin tablets and terazosin tablets for Group D.The cost-effectiveness analysis was performed using pharma?coeconomics.RESULTS:The costs for the Group A,B,C and D were268.8yuan,641.8yuan,660.8yuan and666.4yuan,respectively.The effective rates were71.4%,91.4%,88.6%and91.4%,respectively.The cost-effectiveness ratios were3.76,7.02,7.46and7.29,respectively.The incremental cost-effectiveness ratios of Group B,C and D were18.65,22.79and19.88,respectively as against Group A.CONCLUSION:The treatment cost of CP/CPPS was composed of2parts:an?tibiotics? 1 -receptor blockers.Doctors should make an optimum choice based on patients'previous medication, economic status and physical condition, etc.

Objective To explore the mechanism of combined treatment with methotrexate (MTX) and Ga-Al-As laser irradiation for rheumatoid arthritis (RA) and to assess the effectiveness of Ga-Al-As laser therapy for RA. Methods Twenty-two patients with RA were randomly and evenly divided into two groups: the treatment group treated with Ga-Al-As laser irradiation combined with MTX and the control group treated with MTX only. Ten age-matched normal subjects were observed as normal controls. The amount of CD4 + CD25 + regulatory T cells in peripheral blood (PB) of the normal controls and that in the PB and synovial fluid (SF) of the 22 patients before and after therapy were counted by flow cytometry. Meanwhile, the amount of prostaglandin E2 (PGE2) in synovial fluid of the patients was measured before and after treatment by enzyme-linked immunosorbent assay(ELISA). Results After combined treatment the clinical symptoms of the patients were improved significantly, and the amount of PGE2in SF decreased significantly. The count of CD4 + CD25 + regulatory T cells in PB of RA patients was ( 3.84 ±3.20) % , compared to ( 10.05 ± 7.04) % in healthy individuals. The count of CD4 + CD25 + regulatory T cells in SF of RA patients was ( 14.89 ± 12.30) % , much higher than that in PB. The count of CD4 + CD25 + regulatory T cells in SF decreased significantly in treatment group compared to control group (P <0.05). Conclusion Ga-Al-As laser irradiation eombined with MTX can effectively improve the clinical symptoms of RA patients. It may be related to the decrease of amount of PGE2 and count of CD4 + CD25 + regulatory T cell in PB and SF.

Objective To explore the effect of bilateral hand function training on the hand function of stroke survivors. Methods Thirty-one patients with unilateral hand dysfunction after stroke were randomly divided into an experimental group of 16 and a control group of 15.Both groups were given 60 minutes of hand function training of their affected hands daily, five days a week for 4 weeks. The experimental group was additionally provided with 40 minutes of bilateral hand training on the basis of the aforementioned intervention. Before the treatment and after the 4 weeks, integrated surface electromyography (iEMG) signals were recorded from both hands' flexor and extensor muscles. Both groups were also assessed using the Fugl-Meyer motor assessment for the upper extremities (FMA-UE).Results After the treatment, the average iEMG of the flexor and extensor muscles during flexion and extension were significantly better than before the treatment in both groups. There was no significant difference between the two groups in their average iEMGs before the treatment, but afterward significant differences were observed in all the iEMG measurements. After the treatment the average total FMA-UE score of the experimental group was significantly higher than that of the control group. Conclusion Bilateral hand function training is significantly more effective than unilateral training in promoting the recovery of flexor and extensor muscle function after a stroke.
