1.The value of diagnosis and treatment of pulmonary function in children with refractory mycoplasma pneumoniae pneumonia

Objective To investigate the value of pulmonary function tests in the diagnosis and treatment of children with refractory mycoplasmapneumoniae pneumonia.Methods The clinical data of 58 children with refractory mycoplasmapneumoniae pneumonia (RMPP) from May 2015 to February 2016 were retrospectively analyzed in the Department of Paediawics, Shengjing Hospital of China Medical University.They have done bronchoscopy and interventional therapy in the acute phase.Pulmonary function test was performed 24 hours before bronchoscope examinations.The endoscopic manifestation and pulmonary function data were analysed in the study.According to whether the mucosa of endoscopic manifestation is complete, fifty-eight cases were divided into mild group and severe group, record the pulmonary function indexes of each of the children.Results Severe group decreased than mild group, both of large airway pulmonary function in children with acute severe MPP (FVC, FEV1, PEF) and small airway index (FEF25, FEF50, FEF75, FEF25-75%), and the difference was significant(P < 0.05).Conclusion In the acute phase, there may be different pulmonary function,such as, expressed as a normal, performance of the restrictive or obstructive ventilation dysfunction, performance ofrnixed ventilatory dysfunction, The most common one is the change of small airway function.The more severe of the endoscopic manifestation in RMPP acute phase, the more severe the same period of pulmonary function index decreased degree.The severity of RMPP in children can be predicted by the classification and severity of pulmonary function,it can be used as an important basis for the early identification of severe MPP.Pulmonary function indicators may further guide the need for bronchoscopy intervention treatment and to evaluate the curative effect.
2.Hemodynamic analysis of real-time enhanced-ultrasonography in diagnosis of focal liver lesions

Objective To evaluate the value of real-time enhanced-ultrasonography (EUS) in the diagnosis of hepatic tumor. Methods Twenty-nine patients with 34 hepatic solid lesions were examined by real-time EUS,including 16 primary liver malignant tumors,4 metastatic liver tumors,5 focal nodular hyperplasias (FNH),and 9 hemangiomas. All the cases were confirmed by histopathology. The ultrasound unit (Esaote Megas) with real-time EUS technique was used. The contrast agent used was SonoVue (Bracco,Italy). Results After injection of SonoVue,all the lesions were variously enhanced on real-time ultrasonogram. A holistic inhomogeneous enhanced pattern with quick disappearing was mostly detected by real-time EUS in hepatic malignant tumors,a peripheral and centripetal enhanced pattern was mostly shown in hamangiomas,and a radial centrifugal enhanced pattern with homogeneity and quickly was observed in FNHs. If the lesions with enhancement in arterial phase and disappearing in portal phase after injection of SonoVue were regarded as a sign for malignant liver tumor,the sensitivity,specificity and accuracy in the diagnosis of liver cancer with real-time EUS were respectively 95%(19/20),100%(14/14) and 97% (33/34). Conclusions The dynamic enhanced fashions and various enhanced features in various hepatic tumors will be reflected by real time EUS,which improves the accuracy of ultrasonography in the diagnosis of liver tumors.
3.Role of contrast-enhanced color Doppler ultrasonography in diagnosis of focal hepatic lesions

Objective To investigate the diagnosis of hepatic tumors by contrast-enhanced color Doppler ultrasonography. Methods Thirty-two patients with known focal liver lesions(diameter 11-112 mm,mean 43 mm) were studied with color Doppler flow imaging(CDFI) before and after injection of 2.4 ml SonoVue at the concentration of 5 mg/ml. The pathological diagnosis included 13 benign lesions and 19 malignant ones. Analysis was performed on signal intensity of intratumoral vascularity,time of enhancement duration,resistant index and maximum velocity of intratumoral arterial blood flow. The data were compared between benign and malignant lesions. Results Before injection of SonoVue,intratumoral blood flow could be detected among 53.8 %(7/13) benign lesions and 78.9 %(15/19) malignant tumors. However these detection rates went up to 76.9 %(10/13) and 100%(19/19) respectively after administration of SonoVue. Significant difference between benign and malignant tumors was observed in time of enhancement duration. Furthermore RI showed significant difference between benign and malignant lesions.Diagnostic accuracy of hepatic tumors increased from 75.0 %( 24/32 ) to 90.6 %(29/32) after enhancement. Conclusions The clinical results showed the ability of contrast-enhanced CDFI in reflecting the blood supply of the hepatic tumors,demonstrating the high sensitivity and specificity of the technique,which could be of great value to the diagnosis of hepatic tumors.
4.Association of hyperhomocysteinemia and methylenetetrahydrofolate reductase gene polymorphisms with ischemic stroke in Northwest Chinese population

BACKGROUND: It is proposed that elevated serum homocysteine is an important independent risk factor for ischemic stroke (IS), and 5, 10-methylenetetrahydrofolate reductase (MTHFR) is the key enzyme for homocysteine metabolism. The relationship between genetic mutation of MTHFR and IS remains controversial.OBJECTIVE: To examine the association of hyperhomocysteinemia and two MTHFR gene polymorphisms with IS in Northwest Chinese population.DESIGN: Case-control study.SETTING: Department of Neurology, First Hospital Affiliated to Jilin University, and Department of Neurology, Xijing Hospital, Fourth Military Medical University of Chinese PLA.PARTICIPANTS: Ninety-seven consecutive patients with ischemic stroke (71 males and 26 females) treated between November 2001 and May 2002were recruited, who were diagnosed by CT scan or MRI in the Department of Neurology, Xijing Hospital, Fourth Military Medical University of Chinese PLA. The control group consisted of 94 subjects (58 males and 36 females) without history of ischemic stroke. All the subjects were free of intracranial hemorrhage, cancer, renal dysfunction, and none used multivitamins or estrogen.METHODS: Serum homocysteine was measured by fluorescence polarization immunoassay. Polymerase chain reaction-restriction length polymorphism (PCR-RFLP) method was employed to detect the genotype at the two sites of C677T and A1298C in MTHFR gene.MAIN OUTCOME MEASURES: Serum homocysteine levels and the genotypic frequency frequencies of the two mutations of MTHFR.RESULTS: The 677T allele frequency was 59.3% in IS patients and 44.7% in the controls, showing significant differences (P=0.006), but no difference in 1298C allele frequency was detected between the two groups (22.7% vs 19.7%, P > 0.05). Homozygous 677TT genotype was closely associated with hyperhomocysteinemie (P < 0.01). In multivariate logistic regression analysis,677T gene mutation and hyperhomocysteinemie were all associated with the IS, with an OR of 1.870 and 1.031 (P< 0.05), respectively.CONCLUSION: Hyperhomocysteinemie is a risk factor of IS, and C677T mutation significantly increases homocysteine levels, and serves also as an independent genetic risk factor of IS.
5.Comparison of enhancement features of hepatic tumors on SonoVue-enhanced real-time gray-scale ultrasonography and contrast-enhanced helical CT

Objective To compare enhancement features of hepatic tumors on SonoVue-enhanced real-time gray-scale ultrasonography and contrast-enhanced helical CT.Methods SonoVue-enhanced real-time gray-scale ultrasonography and contrast-enhanced helical CT were performed on 49 patients with 54 focal liver lesions,including 29 primary liver carcinomas,4 metastatic liver tumors,11 hepatic carvernous hemangiomas,7 focal nodular hyperplasias,2 focal fatty sparing,1 cirrhotic.Results Contrast-enhanced ultrasonography and helical CT showed consistent results in demonstrating hemodynamics of hepatic tumors.In the differentiating of malignant tumors and benign tumors,the sensitivity,specificity and accuracy of contrast-enhanced ultrasonography were(90.9)%(30/33),(90.5)%(19/21) and(90.7)%((49/54)),respectively.Those of contrast-enhanced CT were(90.9)%(30/33),(95.2)%(20/21) and(92.6)%((50/54)),respectively.Conclusions Both of the two imaging methods are valuable for differential diagnosis of liver tumors.
6.Dynamic changes of liver cancer stem cell markers and inflammatory factors during the induction of liver cancer in rats

BACKGROUND:Many liver cancer stem cel markers have been found in liver cancer tissues and cel lines such as CD133, acetaldehyde dehydrogenase (ALDH), CD90, CD44, EpcAM, CD13, OV6, K19, c-kit and ABCG2. Of them, CD133, CD90 and CD44 have been shown to be strongly associated with the recurrence and metastasis of liver cancer. OBJECTIVE:To explore the dynamic changes of liver cancer stem cel markers and inflammatory factors during the induction of liver cancer in rats and their correlation. METHODS:Diethyl nitrosamine solution was given to Sprague-Dawley rats for 24 hours to induce rat models of liver cancer. Rats that were given common water were considered as the healthy control group. RESULTS AND CONCLUSION: Immunohistochemical staining revealed that Kupffer cels-related ED2 expression showed a gradual increase in the model group. Compared with the healthy control group, ED2 expression was significantly higher at 12, 16, 20 and 24 weeks after induction in the model group (P < 0.05). Quantitative PCR demonstrated that CD90 showed a gradualy increased trend during induction (P < 0.05). Compared with healthy tissue, CD90 increased significantly in the liver cancer tissue (P < 0.05). CD133 showed an increased trend, but one-way analysis of variance did not show significant differences (P > 0.05). During induction, no significant change was found in other liver cancer stem cel markers (P> 0.05). During the induction, tumor necrosis factor α, transforming growth factor β, MCP-1 and interleukin-6 expression levels were significantly increased (P < 0.05). Compared with healthy tissue, transforming growth factor β, MCP-1 and interleukin-6 expression levels were significantly higher in the liver cancer tissue (P < 0.05). Other inflammatory factors did not exhibit significant alterations during the induction (P > 0.05). Pearson correlation analysis demonstrated that MCP-1, transforming growth factor βand interleukin-6 expression levels were significantly positively correlated with CD90 expression (P < 0.05). These findings suggest that partial inflammatory factors released from Kupffer cels have a certain correlation with liver cancer stem cels. Kupffer cels can promote the occurrence of liver cancer.
7.Expression and purification of asprosin and its effects on cardiac function in mice

Objective To purify asprosin protein expressed in Escherichia coli expression system and to study its effect on cardiac function.Methods Coding sequence of asprosin was obtained from GenBank.Codon optimization was performed according to the codon preference of E.coli.After gene synthesized, recombinant plasmid was made.Asprosin was then induced and purified by Ni-affinity purification.The mouse model of impaired cardiac function was established by ligating and relaxing the left anterior descending coronary artery.30 mice were randomly divided into 3 groups: sham operation group (sham), cardiac dysfunction group (MI/R) and cardiac dysfunction plus injection of recombinant asposin protein group (MI/R+rAsp).The left ventricular function was detected by echocardiography to determine the improving effect of recombinant asprosin protein on cardiac function.Results After prokaryotic expression and purification, the purity of the target protein was higher than 95%, and the endotoxin content was less than <0.1 EU/μg protein, which was suitable for cell and animal studies.After the recombinant asprosin protein was given, the left ventricular function of the mice was improved significantly (P<0.05).Conclusions Asprosin acts as a myocardial protective molecule to improve cardiac function.
8.Study on the effects of OPN on migration of MSCs and its molecular mechanism in mice

Objective To investigate the effects of exogenous osteopontin (OPN ) the migration of bone marrow mesenchymal stem cells(MSCs) in mice .Methods Using the OPN -/- and wild-type C57 mice ,the MSCs were isolated and cultured .These cells were analysed and sorted by flow cytometry .Then ,Transwell migration assay detected whether OPN can induce the migration of these MSCs .Finally ,the method of Western blot was used to detect the changes of expression of OPN ,CD44 and Integrinβ1 pro-teins .Results The morphology and features of MSCs were proved correctly on 3 passages ,which these cells expressed the proteins of CD44 and CD105 ,but not CD34 .The surface marker antigens were accorded with the general features of MSCs .Compared with OPN -/- mice ,the MSCs significantly increased cell migration in groups of 0 .5 μg/mL OPN and wild type C57 mice ,which was positively related to the times using OPN .Using recombinant OPN protein(0 .5μg/mL) ,expression of related proteins was signifi-cantly increased ,but still lower than the group of wild type mice (P<0 .01) .Compared with OPN -/- mice ,expression of OPN , CD44 ,and Integrinβ1 protein increased significantly different (P<0 .01) in groups of 0 .5 μg/mL OPN and wild type C57 mice . Conclusion OPN can up-regulate the expression of CD44 and Integrinβ1 proteins and promote MSCs migration .

Objective In order to explore the action of IL-1? involved in epileptogensis,the changes of Gs? expression were studied in hippocampus of epileptic rats induced by sodium glutamate. Methods Immunohistochemical staining and animal behaviours were observed during the experiments.Healthy adult SD rats were divided randomly into five groups:1.Control.2.GluNa group.3.IL-1?+GluNa group.4.rhIL-1ra+IL-1?+GluNa group.5.D-AP-5+IL-1?+GluNa group. Results The changes of behaviours showed that latent period of seizure in IL-1?+GluNa group was reduced remarkly compared with that of other groups;the degrees of seizure were more serious than that of other groups.The results of immuohistochemistry showed that immunoreaction of Gs? was obviously enhanced in hippocampal CA1,CA3 areas and dentate gyrus of IL-1?+GluNa group compared with other groups.Conclusion IL-1? may play an important role in medicating pathologenesis procession of epilepsy induced by glutamate and make effect by stimulating overexpression of Gs protein.
