1.Genetic abnormalities and their relationship with the clinical features in pediatric Burkitt lymphoma

Objective To analyze the molecular cytogenetic abnormalities and pathogenesis of pediatric Burkitt lymphoma (BL) by array comparative genomic hybridization (aCGH).Methods First,immunophenotype,molecular genetics and EB virus (EBV) infection status were detected using immunohistochemistry and fluorescence in situ hybridization in 21 pediatric BL patients.Second,in addition to detecting genome-wide genetic gain/deletion status,aCGH results with EBV infection status were also correlated.Results aCGH results showed genetic alterations in 19 cases (90.5 %).Generally,frequency of chromosomal gain was higher than chromosomal deletion.The regions of frequently-occurring small DNA genomic fragment gains (≥40 % cases) were 3q21.1,5p13.2,19q13.32,12q23.1,14q32.33,6q27,20p13 and 20p11.21.Large DNA fragment gains and deletions could be detected in 42.9 % (9/21) cases in the 14q24.2 and 14q32.33 regions.There was no significant difference in genetic alterations between EBV (+) and EBV (-) BL cases (P≥0.05).Conclusion aCGH results show that BL cases have complex genetic alterations,which have no significant difference between EBV(+) and EBV(-) cases.Most BL cases show large DNA segment deletion or acquisition of 14q,indicating that 14q gene alteration plays an important role in the pathogenesis of BL.
2.Effects of biological intensity electric field on motility and CD9 expression of human epidermal cell line HaCaT and mouse epidermal cells

Objective:To investigate the regulation effect of biological intensity electric field (EF) on the motility and CD9 expression of human epidermal cell line HaCaT and mouse epidermal cells.Methods:The experimental research method was used. Human epidermal cell line HaCaT cells in logarithmic growth phase and primary mouse epidermal cells isolated from 16 BALB/c mice aged 1-3 days were used for the experiment. HaCaT cells were divided into EF group treated with EF in the intensity of 200 mV/mm and sham EF group treated with simulated operation. The cell migration (displacement velocity, trajectory velocity, and direction, with 46 samples in EF group and 34 samples in sham EF group) and arrangement were observed in the living cell workstation, and the distribution and expression of CD9 protein were detected by immunofluorescence method. Both HaCaT cells and mouse epidermal cells were divided into sham EF group (simulated operation) and 50 mV/mm group, 100 mV/mm group, 200 mV/mm group and 400 mV/mm group treated with EF in the corresponding intensity respectively. Both HaCaT cells and mouse epidermal cells were divided into blank control group without any treatment and 1 h group, 3 h group and 6 h group treated with EF in the intensity of 200 mV/mm for corresponding time respectively. The expression of CD9 protein was detected by Western blotting (n=3). Data were statistically analyzed with Mann-Whitney U test, one-way analysis of variance, t test and least significant difference test. Results:Within 3 hours of treatment, HaCaT cells in EF group tended to move towards the negative electrode obviously, while HaCaT cells in sham EF group moved randomly around the origin; compared with those of sham EF group, the directivity of HaCaT cells in EF group was significantly enhanced, and the displacement velocity and trajectory velocity were significantly increased (Z=-3.975, -6.052, -6.299, P<0.01). After 3 hours of treatment, the long axis of HaCaT cells in EF group was perpendicular to the direction of EF, while HaCaT cells in sham EF group arranged randomly. After 3 hours of treatment, the expression of CD9 protein in HaCaT cells in EF group was significantly down-regulated compared with sham EF group (t=4.527, P<0.01), although both expressed on cytomembrane. After 3 hours of treatment, the expression of CD9 protein in HaCaT cells and mouse epidermal cells in sham EF group, 50 mV/mm group, 100 mV/mm group, 200 mV/mm group and 400 mV/mm group were 0.332±0.021, 0.283±0.032, 0.254±0.020, 0.231±0.041, 0.212±0.031 and 0.565±0.021, 0.453±0.022, 0.389±0.020, 0.338±0.021, 0.233±0.011, respectively. For both types of cells, compared with that of sham EF group, the expression of CD9 protein in cells was significantly decreased in the four groups of EF treatment (P<0.01); compared with that of 50 mV/mm group, the expression of CD9 protein in cells was significantly decreased in the other three groups of EF treatment (P<0.01); compared with that of 100 mV/mm group, the expression of CD9 protein in cells was significantly decreased in 200 mV/mm group and 400 mV/mm group (P<0.01); compared with that of 200 mV/mm group, the expression of CD9 protein in cells was significantly decreased in 400 mV/mm group (P<0.01). The expression levels of CD9 protein in HaCaT cells and mouse epidermal cells in blank control group, 1 h group, 3 h group and 6 h group were 0.962±0.031, 0.784±0.020, 0.531±0.021, 0.409±0.011 and 0.963±0.031, 0.872±0.031, 0.778±0.040, 0.591±0.041, respectively. For both types of cells, compared with that of blank control group, the expression of CD9 protein in cells was significantly decreased in 1 h group, 3 h group, and 6 h group (P<0.01); compared with that of 1 h group, the expression of CD9 protein in cells was significantly decreased in 3 h group and 6 h group (P<0.05 or P<0.01); compared with that of 3 h group, the expression of CD9 protein in cells was significantly decreased in 6 h group (P<0.01).Conclusions:The biological intensity EF can induce the directional migration and arrangement of HaCaT cells and down regulate the expression of CD9 in HaCaT cells and mouse epidermal cells in a time-dependent and intensity-dependent manner.
3.Multi-disciplinary team on acute heart failure complicated with respiratory failure after allograft nephrectomy

Objective To evaluate the effect of multi-disciplinary team (MDT) on acute heart failure (AHF) complicated with respiratory failure after allograft nephrectomy. Methods MDT discussion was performed on a patient with hemorrhagic shock caused by sudden renal graft hemorrhage, who developed acute myocardial infarction (AMI) with AHF, acute pulmonary congestion, pulmonary infection and acute respiratory failure 2 weeks after allograft nephrectomy. And treatment plan was formulated and effect evaluation was conducted. Results Based on the opinions of MDT discussion, the patient was given nasal high-flow oxygen therapy, continuous veno-venous hemodiafiltration (CVVHDF) to reduce cardiac load, anticoagulant, dilating blood vessels, reducing myocardial oxygen consumption, improving myocardial remodeling, lipid regulation, anti-infection, nutritional support, and other comprehensive treatment. The clinical outcome of the patient was good and regular hemodialysis treatment was resumed. Conclusions Application of MDT pattern helps to formulate a comprehensive and effective individualized treatment plan for patients with AHF and respiratory failure after allograft nephrectomy, which can enhance clinical treatment effects and improve prognosis of patient.
4.Nursing of Methotrexate combined with Mifepristone in patients with ectopic gestation

Objective To explore the clinical nursing of Methotrexate combined with Mifepristone in patients with ectopic gestation .Methods Totals of 128 patients with ectopic gestation were treated by Methotrexate combined with Mifepristone , and blood β-HCG level and lump diameter size were measured .The patients were given comprehensive nursing including psychology , diet and daily activities , disease observation , drug adverse reaction and discharge guidance .Results The rate of successful treatment was 94.5%(121/128).The level of blood β-HCG and diameter of lump after 1 week were significantly smaller than those before the treatment (t=33.2,8.9, respectively;P<0.05).The level of blood β-HCG after 2,3,4 weeks were significantly lower than those after 1 week (t=64.2,78.3,105.9, respectively;P<0.05).And the diameter of lump after 2,3,4 weeks were significantly smaller than those after 1 week (t=6.4,7.5,8.3, respectively;P<0.05).Five patients (3.9%) had gastrointestinal reaction, three patients (2.3%) had slightly elevation of transaminase and leucopenia , two patients ( 1.6%) had oral ulcer and bone marrow suppression .Those symptoms were disappeared after treatment .Conclusions During the treatment of ectopic pregnancy , patients should be given positive psychological care , close clinical nursing and disease observation to promote the rehabilitation of patients .
5.Application effects of WeChat follow-up specialized management in esophageal cancer patients with radiotherapy

Objective To explore the application effects of WeChat follow-up specialized management in esophageal cancer patients with radiotherapy.Methods A total of 92 esophageal cancer patients with radiotherapy from Yancheng Third People's Hospital of Jiangsu Province were selected as subjects from January 2015 to May 2016. They were divided into observation group (n=46, follow-up specialized management with WeChat) and control group (n=46, conventional follow-up management) with the random number table. The self-care ability, treatment compliance, quality of life and follow-up satisfaction was compared between two groups 6 months after intervention.Results The scores of self-concept, level of health knowledge, nursing skills, sense of self-responsibility and the total score of observation group after intervention were all higher than those of control group with significant differences (t=6.342, 2.635, 3.124, 2.365, 3.258;P<0.05). The scores of 7 dimensions including taking medicine on time, review on time, radiotherapy on time, prevention of complications, nutritional diet, reasonable rest, rehabilitation exercise and the total score in observation group after implementing WeChat follow-up specialized management were all significantly higher than those in control group (P<0.01). There was no statistically significant difference in the changing degree of cognitive function, pain, anhelation, financial hardship between observation group and control group along with the changing degree of other 11 dimensions in observation group better than those in control group with significant differences (P<0.01). Patients' satisfactions with forms, contents and effects of follow-up in observation group were significantly higher than those in control group (x2=8.364, 5.705, 7.731;P<0.05).Conclusions The implementation of WeChat follow-up specialized management for esophageal cancer patients with radiotherapy can obviously improve the self-care ability, treatment compliance, quality of life and follow-up satisfaction of patients.
6.Clinical research on changes of mineral and bone metabolism before and after renal transplantation

Objective To explore the changes of mineral and bone metabolism before and after renal transplantation as well as the effect of preoperative parathyroid hormone (PTH) level on postoperative mineral and bone metabolism.Methods In this retrospective analysis,we recruited 82 cases of renal transplant recipients with normal renal function and receiving kidney transplantation in our hospital from January 2011 to January 2015.All of these patients had intact PTH (iPTH) level >300 pg/mL.We chose 26 cases of recipients whose preoperative iPTH was more than or equal to 800 pg/mL as very high PTH group,and 56 cases of recipients whose preoperative iPTH was between 301-799 pg/mL as high PTH group.We monitored and performed analysis of the total serum calcium (Ca),serum inorganic phosphorus (P),25-(hydroxyl) vitamin D3 (25 OHD),serum alkaline phosphatase (ALP),Beta C-terminal telopeptide (β-CTX),N-terminal/midregion (N-MID) pre-and 1 month,4 months,1 year,2 years,3 years post-kidney transplantation.Results Serum total calcium in the two groups was gradually increased,returned to normal range 1 month post-transplantation and reached the plateau 4 months post-transplantation.The incidence of hypercalcemia in very high PTH group was statistically significantly higher than in high PTH group.Serum phosphorus in the two groups showed a trend of gradual decline after renal transplantation,and returned to the normal range 1 month post-transplantation.The serum phosphorus level in very high PTH group reached the plateau 4 months post-transplantation,and that in high PTH group 1 month post-transplantation.Compared with high PTH group,very high PTH group has greater The incidence of long-term hypophosphatemia after renal transplantation was significantly higher in very high PTH group then in high PTH group.iPTH,ALP,β-CTX and N-MID in the two groups showed a downward trend after renal transplantation.At first month post-transplantation,iPTH,ALP,β-CTX and N-MID levels were reduced most significantly.The average levels of the three mentioned indicators in very high PTH group were higher than in high PTH group at every time point after surgery with the difference being significant during the early post-transplantation period.The anomalies of iPTH and β-CTX levels persisted to long term after transplantation in very high PTH group.25-OHD levels in these two groups showed rising trend after renal transplantation,reached the plateau 4 months posttransplantation,but failed to achieve the ideal reference level,and no significant difference was found between two groups at any time point monitored.Conclusion The anomalies of mineral and bone metabolism after renal transplantation could persist a long time.Conclusion hyperparathyroidism in the renal transplantation plays an important role in mineral and bone metabolism.Preoperative severe HPT could continue to post-transplantation period and increase the incidence of hyperphosphatemia and hypocalcemia long term after transplantation,which may aggravate bone turnover and this effect can last a long time after transplantation.
7.Analysis of 17 cases of Vaterian system adenomyomatous hyperplasia

Objective:To study the features of adenomyomatous hyperplasia (AH) of the Vaterian system (common bile duct and ampulla of Vater) to help in the diagnosis and management of this disease.Methods:A retrospective analysis on the data of 17 patients who had a postoperative pathological diagnosis of AH of the Vaterian system treated from January 2005 to December 2021 at the First Medical Center of the PLA General Hospital was carried out with 12 males and 5 females, aged (58.4±11.3) years. The clinical presentations, treatment and postoperative pathology of these patients were analyzed. Patients with dysplasia of the tubular mucosal epithelium in the non-cancerous area around the AH under microscopy were included in the AH with dysplasia group ( n=8), and those without dysplasia were included in the control group ( n=9). The clinical characteristics of the two groups were compared. Results:The main clinical symptoms were abdominal pain in 8 patients, jaundice in 7 patients and fever in 2 patients. Preoperative imaging showed 10 cases of occupying lesions and 6 cases of abnormally dilated intrahepatic and extrahepatic bile ducts without obvious lesions or stones or biliary tract injury stenosis. Sixteen patients underwent radical pancreaticoduodenectomy, and 1 patient underwent extrahepatic biliary resection combined with choledochojejunostomy for bile duct obstruction due to biliary stones, 3 patients had combined malignant tumors, 1 patient had a carcinoma of AH origin at the ampulla of Vater, and the other 2 patients had neoplastic lesions in the mucosal epithelium adjacent to the AH (cholangiocarcinoma and ampullary carcinoma, respectively). There were no significant differences in age, gender, bile duct stones, cholangitis, combined carcinoma and liver function indexes between the two groups of patients with AH of the Vaterian system (all P>0.05). Conclusion:Adenomyomatous hyperplasia of the Vaterian system was difficult to distinguish preoperatively from malignant tumors basing on its clinical presentations or imaging findings. Such patients are recommended to be treated surgically.
8.Effects of self-relaxation training on self-efficacy and sleep quality of patients undergoing radiotherapy for head and neck tumors

Objective? To explore the effects of self-relaxation training on the self-efficacy and sleep quality of patients undergoing radiotherapy for head and neck tumors. Methods? A total of 76 patients with head and neck tumor who received radiotherapy in our department from February 2016 to February 2018 were selected. All the patients were randomly divided into the intervention group and the control group, with 38 cases in each group. The control group received routine nursing care of radiotherapy for head and neck tumor. Beyond that, the intervention group also received self-relaxation training for 3 months. The differences of self-efficacy and sleep quality between the two groups were observed and compared. Results? After the intervention, the self-efficacy score of the intervention group was (79.14±6.12), significantly higher than that of the control group (71.76±6.27) (t=2.263, P=0.039). The difference in self-efficacy scores of the intervention group before and after the intervention was statistically significant (t=2.267, P=0.038). There was no significant difference in the self-efficacy scores in the control group before and after the intervention (t=1.792, P=0.069). After intervention, the total score and dimension scores (sleep quality, sleep latency, sleep time, sleep efficiency, sleep disorder, hypnotic drugs, and daytime dysfunction) of the Pittsburgh sleep quality index table were all lower in the intervention group than the control group, and the difference was statistically significant (t=-7.604,-3.183, -2.666, -4.320, -2.776, -3.772, -2.572,-5.085; P<0.05). Conclusions? Self-relaxation training can improve the self-efficacy and sleep quality of patients undergoing radiotherapy for head and neck tumors.
9.The impact of KRAS gene status on clinical parameters and long-term prognosis of patients with colorectal cancer liver metastases

Objective:To study the relationship between KRAS gene mutation and clinical parameters and prognosis in patients with colorectal cancer liver metastases (CRLM).Methods:To retrospectively study the impact of different KRAS status on the clinical parameters parameters and prognosis of 1 248 patients with CRLM treated from January 2005 to December 2019 at the First Medical Center, Chinese PLA General Hospital. There were 880 male and 368 female, age ranged from 21 to 88, median 56. The single factor and multi-factor logistic regression analyses were used to identify factors relating to KRAS mutation. Survival was analyzed by the Kaplan-Meier method, and survival rate by the log-rank test.Results:There were 729 KRAS gene wild-type patients and 519 mutant patients. The mutation rate was 41.6%(519/1 248). Primary site of tumor in 11 patients were located in the bilateral colon. The KRAS gene mutation rates between the male and female CRLM patients whose CA19-9 level were ≥38 g/L and <38 g/L, with or without diabetes, and whose primary sites were on the right (52.1%, 160/307) or the left colon (38.2%, 355/930) was significantly different (all P<0.05). A single factor logistic regression analysis showed that gender, CA19-9 levels, diabetes and the primary site were associated with KRAS mutations, with significant difference ( P<0.05). Multivariate logistic regression analysis showed that the primary site of tumor was an independent influencing factor of KRAS mutation ( OR=0.557, 95% CI: 0.423-0.733, P<0.05). The overall survival rates of KRAS wild-type patients was significantly higher than mutant patients ( P<0.05). Conclusion:Among patients with CRLM, KRAS mutation was more frequently appeared in those patients with right sided colonic cancer. The long-term prognosis of patients with KRAS mutant was significantly worse.
